BACKGROUND:Depression is a common mental illness that has become a major economic burden worldwide. Recently, increasing evidence has highlighted the inflammatory mechanism of depression. In order to understand the research status of this field, this study used the bibliometric analysis method to overview the research content and progress, as well as analyze the development trend and limitations. METHODS:In this study, articles and reviews were included in the specific search strategy. The matched papers were exported from the Web of Science database. CiteSpace 6.3 R1 and Bibliometrix (R package) were utilized to generate bibliometric and knowledge maps. RESULTS:A total of 25,063 articles were included in this study. The number of publications in this field has gradually increased, especially in recent years. These papers come from 156 countries, led by the United States and China mainland. The leading research institution is the University of Toronto (Canada). Brain Behavior and Immunity is the journal with the most publications and the most frequently co-cited journals. Among 91,100 authors, Maes M has the most publications and co-citations. According to the keywords burst and co-cited reference analysis, the hotspots in the field in recent years include kynurenine, c-reactive protein, neuroinflammation, and gut microbiota. CONCLUSION:Although abundant academic achievements have been made on the inflammatory mechanism of depression, there is still a long way to go before these research results can be applied to clinical practice. Strengthening international academic exchanges and cooperation may promote breakthroughs in this field.
Insulin resistance (IR) has been proposed as a contributing factor to major depressive disorder (MDD), with previous studies reporting a positive correlation between triglyceride-glucose (TyG) a proxy indicator of IR and MDD. However, limited information is available regarding their longitudinal association. This study aimed to clarify the connection between TyG levels and depression risk, as well as explore its predictive potential. A total of 3021 participants without a prior history of depression were recruited from the China Health and Retirement Longitudinal Study and followed for seven years. Participants were categorized into tertiles based on their TyG levels. The cumulative hazard of depression was analyzed using Kaplan-Meier curves, while cox regression analyses and multivariable-adjusted restricted cubic spline (RCS) curves were employed to assess the relationship between TyG levels and depression risk. Stratified analyses across various subgroups were also conducted to confirm the robustness of the conclusions. Over the follow-up period, 1782 participants (58.9%) developed depression, with incidence rates of 30.2%, 34.0%, and 35.8% in tertiles 1, 2, and 3, respectively. After adjusting for confounding factors, each 1-unit increase in TyG was associated with a significantly higher risk of depression. RCS curve analysis revealed a compelling dose-response relationship between TyG levels and depression susceptibility. These findings indicate that elevated TyG levels are strongly associated with an increased risk of depression and could serve as a reliable biomarker for assessing depression risk. These insights provide valuable guidance for developing more effective strategies for the prevention and treatment of depressive disorders.
目的 评估海军水面舰艇部队官兵的人格特质、抑郁、焦虑、压力和心理弹性,探讨心理弹性在大五人格和抑郁、焦虑、压力之间的中介作用.方法 2021年8月采用整群抽样法抽取海军某部1 530名官兵进行问卷调查,采用大五人格简式量表、CONNOR-DAVIDSON心理弹性量表中文版和短式抑郁、焦虑和压力量表简体中文版来评估部队官兵的大五人格各因子、心理弹性各因子、抑郁、焦虑和压力水平,并对数据进行分析.结果 不同性别部队官兵的神经质、外向性、责任性、抑郁、压力、坚韧性和力量性因子得分比较,差异均有统计学意义(均P<0.05);独生子女和非独生子女的宜人性和乐观性两个因子得分比较,差异均有统计学意义(均P<0.05);已婚官兵和未婚官兵的开放性、责任心和压力性3个因子得分比较,差异均有统计学意义(均P<0.05).官兵大五人格各特质、心理弹性各因子与抑郁、焦虑和压力之间均呈显著相关(均P<0.01).大五人格通过心理弹性对抑郁、焦虑与压力的间接影响有显著低程度影响(中介效应占直接效应的比率为15.24%),同时发现心理弹性在人格特质对抑郁、焦虑和压力的影响中有遮掩效应.结论 海军某部水面舰艇部队官兵心理健康状况不容乐观,心理弹性在大五人格和抑郁、焦虑、压力之间起到部分中介作用.探索适合水面舰艇部队官兵的心理弹性训练方案,通过提高心理弹性,来调节抑郁、焦虑和压力.
目的 了解和探讨海军某部官兵心理应激、应对方式与负性情绪的关系,为军人情绪管理提供科学依据.方法 在海军某部整群抽取 1092 名军人,采用军人应激自评问卷(PSET)、简易应对方式问卷(SCSQ)、焦虑自评量表(SAS)和抑郁自评量表(SDS)进行心理测评,分析受试者心理应激、应对方式与焦虑、抑郁情绪之间的关系.结果 心理应激与负性情绪呈正相关,积极应对方式与负性情绪呈负相关,消极应对方式与负性情绪呈正相关,差异均有统计学意义(P<0.05).积极应对方式在心理应激对抑郁、焦虑情绪的影响中起调节作用.结论 心理应激能够影响军人的负性情绪,个体积极的应对方式是缓解心理应激反应造成情绪困扰的保护因素;在军队训练和教育中,需加强军事应激调控及心理咨询和干预,提高应对能力,改善情绪.
Objective To determine the effects of a psychological training program based on problem-solving therapy on the problem-solving ability and psychological resilience of naval personnel in a certain unit, and to evaluate the effectiveness and applicability of the psychological training program. Methods A total of 92 military personnel from a surface fleet were randomly sampled and divided into a training group (n=46) and a control group (n=46). The training group received a 6-week psychological training program based on problem-solving therapy once a week, while the control group received no such psychological intervention. Results A significant interaction effect was observed in the results of the problem-solving questionnaire (F=4.366, P < 0.05) and the psychological resilience scale (F=6.017, P < 0.05) before (T1) and after (T2) the training between the training and control group. Compared to the scores at T1, the problem-solving ability and psychological resilience were significantly improved in the training group at T2. Conclusion The psychological training based on problem-solving therapy could effectively improve the problem-solving ability and the psychological resilience of military personnel in the studied fleet.
Objective To explore the loneliness and its influencing factors in naval officers and soldiers.Methods With simple cluster sampling, the University of California at Los Angeles(UCLA) loneliness scale, perceived social support scale, interpersonal comprehensive diagnostic scale, self-esteem scale, general self-efficacy scale, general anxiety disorder scale 7, patient health questionnaire scale 9 and self-designed demography questionnaire were used to investigate the officers and soldiers in Apr. 2022. Correlations between general demographic variables and psychological variables were analyzed by Spearman correlation analysis and Pearson correlation analysis. Simple linear regression analysis and multiple linear regression analysis were used to explore the relationships between social support, interpersonal relationship,self-esteem, self-efficacy, anxiety, depression and loneliness. Results A total of 3 338 questionnaires were distributed and 3 105 valid questionnaires were collected, with an effective rate of 93.02%. Among the survey subjects, 3 032(97.65%)were male and 73(2.35%) were female, aged 18-55(25.85±5.30) years old. The loneliness score of the officers and soldiers was 20-69(34.04±9.79). There were significant differences in loneliness in different gender and age groups(both P<0.05), among which loneliness of the females was significantly higher than that of the males and loneliness of those aged 26-33 was significantly higher than that of those aged 18-25(both P<0.05). The loneliness of officers and soldiers was positively correlated with interpersonal relationship score and anxiety and depression levels(all P<0.01), and was negatively correlated with perceived social support, self-esteem and self-efficacy(all P<0.01). Interpersonal relationship score and anxiety and depression levels had significant positive predictive effects on loneliness(all P<0.01), while the levels of perceived social support, self-esteem and self-efficacy had significant negative predictive effects on loneliness(all P<0.01).Conclusion Social support, interpersonal relationship, self-esteem, self-efficacy, anxiety and depression have significant impacts on loneliness in naval officers and soldiers.
Depression is a prevalent psychiatric disorder with elusive pathogenesis. Studies have proposed that enhancement and persistence of aseptic inflammation in the central nervous system (CNS) may be closely associated with the development of depressive disorder. High mobility group box 1 (HMGB1) has obtained significant attention as an evoking and regulating factor in various inflammation-related diseases. It is a non-histone DNA-binding protein that can be released as a pro-inflammatory cytokine by glial cells and neurons in the CNS. Microglia, as the immune cell of the brain, interacts with HMGB1 and induces neuroinflammation and neurodegeneration in the CNS. Therefore, in the current review, we aim to investigate the role of microglial HMGB1 in the pathogenetic process of depression.
Centering on the strategic transformation of Chinese Navy and focusing on military medical education in battlefield, troops, and the future, this article explores the actual combat-based innovation and reconstruction of the course of Military Nautical Psychology in Navy Medical University. This article analyzes the existing problems in combat-oriented teaching in the original course and then proposes to deepen the actual combat-oriented teaching reform of the new course. The new course should adjust teaching contents according to actual combat and place more emphasis on the teaching of combat stress and mental health service support, so as to form the teaching mode of "self-study+theoretical lectures+practical teaching+naval exercises". Moreover, the course should create an actual combat-oriented teaching environment relying on cutting-edge technology and establish an actual combat-oriented teaching team to realize the complex transformation of teachers to theory, skills, and experience, in order to improve the post competencies of peacetime mental health maintenance, psychological injury treatment on battlefield, and wartime mental health service support among cadets.
目的 探讨军人的神经质人格与主观幸福感的关系,及心理弹性与负性情绪在其中的作用机制.方法 2020年10月整群抽取海军某部军人进行问卷调查,包括简式大五人格量表神经质分量表、主观幸福感量表、CONNOR-DAVIDSON心理弹性量表和抑郁-焦虑-压力量表,并对数据进行分析.结果 共回收有效问卷3 883份.海军某部军人平均神经质人格得分为(28.21±9.24)分,平均主观幸福感得分为(23.66±5.55)分,平均心理弹性得分为(68.05±16.22)分,平均抑郁、焦虑和压力得分分别为(4.72±5.99)、(5.51±5.08)和(8.58±6.72)分.神经质人格是军人主观幸福感的危险因素(β=-0.410,P<0.01);心理弹性对神经质人格与主观幸福感关系的调节效应显著(β=0.149,P<0.01),且调节效应部分以负性情绪为中介(β=-0.262~-0.120,P<0.01).结论神经质人格、主观幸福感、心理弹性、负性情绪4个变量之间的关系是有中介的调节效应.建议通过训练提升军人的心理弹性水平以缓冲其神经质人格导致的不利影响.
Objective:To develop a family resilience assessment tool for officers and sailors deployed on surface ships.Methods:The data of 4 654 officers and sailors deployed on surface ships were analyzed by exploratory factor analysis,confirmatory factor analysis,criterion validity,and reliability analysis.Results:Seven factors were extracted by the exploratory factor analysis,and the cumulative variance explanation rate was 63.923%. The confirmatory factor analysis showed that the factor model fitted well( χ2/Df=4.214,RMSEA=0.031,CFI=0.929,NFI=0.909,GFI=0.915,AGFI=0.905). The total score of the family resilience assessment scale was moderately correlated with each dimension( r=0.505-0.688, P<0.01),and there was a low correlation among each dimension( r=0.190-0.310, P<0.01). The Cronbach’s α coefficient of the total scale was 0.904. The Cronbach’s α coefficients of the seven sub-scales ranged from 0.775 to 0.864. The criterion correlation validity showed that the total score of the scale had a significant positive correlation with the family adaptability and cohesion scale and the Connor-Davidson resilience scale( r1 =0.681, r2 =0.637, P<0.01). Conclusion:The family resilience assessment scale developed in this study has good reliability and validity,which qualifies as an effective instrument to evaluate the family resilience of officers and sailors deployed on surface ships.
目的 评估军队人员的睡眠质量、自杀意念与心理弹性水平,探讨心理弹性在睡眠质量和自杀意念之间的调节作用.方法 于2020年5月整群抽取某部官兵进行问卷调查,采用匹兹堡睡眠质量指数(PSQI)量表、自杀意念自评量表(SIOSS)和Connor-Davidson心理弹性量表评估官兵的睡眠质量、自杀意念和心理弹性水平.结果 共回收问卷13831份,其中有效问卷10367份,有效率为74.95%.某部官兵的PSQI量表总分为(5.04±3.60)分,睡眠质量差检出率为21.13%(2191/10367);SIOSS总分为(2.84±3.33)分,自杀意念检出率为3.30%(342/10367);心理弹性量表总分为(70.54±15.32)分.不同性别、学历的官兵睡眠质量、自杀意念和心理弹性水平差异均有统计学意义(P均<0.05).控制社会人口学变量后,睡眠质量(β=0.46,P<0.01)、心理弹性(β=-0.31,P<0.01)及两者交互项(β=-0.20,P<0.01)均对自杀意念有显著的预测作用.结论 心理弹性在睡眠质量与自杀意念之间起到负向调节作用.建议开发适合军队的心理弹性训练,通过训练提升官兵的心理弹性来缓冲睡眠质量差等危险性因素对官兵的不利影响.
军人职业认同是军人职业健康水平和军事适应能力的重要影响因素.我们综述了职业认同的概念和研究历程,重点回顾了国内外关于军人职业认同的内涵、形成过程和研究现状的理论文献,对军人职业认同的概念进行了界定,并介绍了国内外关于军人职业认同的测量工具,同时总结了军人职业认同的影响因素和影响结果.从理论和实践研究的角度,对未来军人职业认同的研究方向进行了展望,为我军深入开展职业认同的研究、提供军人职业化的对策建议,以及将研究成果应用于军队职业教育和军事人员晋升选拔提供了有益参考.
Objective:To survey the health condition and job burnout level of the officers and sailors on the hospital ship Peach Ark during the "Mission Harmony 2018".Methods:All types of officers and sailors of the "Mission Harmony 2018" on the hospital ship were surveyed by the physical and mental state questionnaire and the job burnout scale.Results:Among all, 96.7% of the officers and sailors deemed the Mission was of great significance and voluntarily joined the Mission. More than 70% of the officers and sailors had adapted themselves well to the environment on board. There were significant differences in the level of job burnout among the officers and sailors of different ages, identities, and academic qualifications ( P <0.05). In terms of job burnout level, there was no significant difference between the officers and sailors who are the only child in his/her family and those who are not, nor was there any significant difference due to different marital status ( P>0.05). However, the officers and sailors aged 30-49 had a higher level of job burnout than those aged 20-29; the officers and sailors with a bachelor’s degree or above had a higher level of job burnout than those with a high school degree or below; and the medical staff had a higher level of job burnout than the sailors, and the above differences were statistically significant ( P<0.05). During the "Mission Harmony 2018", family situation, physical health, and adaptability on board were the factors related to the job burnout level of officers and sailors ( P<0.05). The officers and sailors with more family worries, heavy family burdens, and poor health tended to have higher levels of job burnout. Conclusion:The physical and mental health of the officers and sailors on Mission Harmony should be paid close attention to, and their family troubles and difficulties should be known about in time. In particular, those who join long oceangoing mission for the first time and have difficult in adapting themselves to the environment on board should be provided regular psychological counseling service. Meanwhile, the medical staff should also be given more attention. A reasonable task assignment emphasizing the proper balance between work and rest can be helpful to reduce their job burnout level.
During "Mission Harmony 2018", the crew on the hospital ship were prone to various physiological and psychological reactions that affected their health. Based on the characteristics of the Mission, our own work experience on board, and the requirements for mental health support during the Mission, we summarized and reflected on our mental health support for the crew and the mental health service for the local people of the host countries. We believe that it is vital to support the mental health of the crew during a long oceangoing mission by formulating and carrying out psychological interventions and training according to the on-board environment and different legs of the voyage. We hope that this paper can provide a reference for the mental health service in future long oceangoing missions.
Background: Recently, abundant evidence indicated proinflammatory cytokines might play a crucial role in pathophysiology and treatment of depression. According to our preclinical research, we propose glycyrrhizic acid (GZA) for an adjunctive treatment owing to its safety, economical and anti-inflammatory profile. Methods: Eligible participants were recruited and randomly allocated into independent treatment groups of SSRI + GZA (n = 30) and SSRI + PBO (placebo, n = 26). Depressive symptoms and specific serum biomarkers were detected during the 4-week treatment course. Afterward, the relationships between biomarkers and clinical effects were explored. Results: Depressive symptoms relieved more in SSRI + GZA than SSRI + PBO, both at week 2 (P = 0.003) and week 4 (P = 0.016). Meanwhile, at week 4, both response rate (P = 0.035) and remission rate (P = 0.031) acutely became higher in SSRI + GZA compared with SSRI + PBO. Mediation analysis further demonstrated that TNF-alpha reduction mediated the association between GZA treatment and clinical improvement, the indirect effect lay between 0.124 and 3.514 (95% CI). The exploratory analysis also suggested that the symptomatic improvement existed in patients with high-inflammation (baseline CRP > 3 mg/L) rather than those with low-inflammation (baseline CRP <= 3 mg/L). Limitations: The sample size in this study was not large enough and the follow-up duration was relatively short. Conclusions: This study offers a novel strategy for the diagnosis, categorization, individualization and prognosis regarding upgrading traditional antidepressant therapy, which is from biomarkers to diagnostic indicator and therapeutic target. Patients are necessary to be classified according to the inflammatory state, those with high levels of baseline inflammation should receive combined treatment with anti-inflammatory agents like GZA.
Background: Our previous study reports the causal role of high mobility group box 1 (HMGB1) in the development of depression; and we find glycyrrhizic acid (GZA) can be a potential treatment for major depressive disorder (MDD) considering its inhibition of HMGB1 activity. This study aims to further explore the exact cell types that release HMGB1 in the hippocampus. Methods: We detected the effects of microglia conditioned medium on primary astrocytes and neurons. The effects of minocycline on depressive-like behaviors were tested in BABLB/c mice after four weeks of chronic unpredictable mild stress (CUMS) exposure. Furthermore, the immunofluorescence (IF) assays, hematoxylineosin (HE) and TUNEL staining were used to observe hippocampal slices to evaluate the release of HMGB1. The cytoplasmic translocations of HMGB1 protein were assayed by western-blot. Results: Exposure to CUMS caused an active release of HMGB1 from microglia and neurons in the hippocampus. After minocycline administration for inhibiting the activation of microglia, both microglia and neurons reduced the release of HMGB1 and the protein level of central and peripheral HMGB1 recovered accordingly. Along with blocking the release of HMGB1, behavioral and cognitive deficits induced by CUMS were improved significantly by minocycline. In addition, the supernatant of primary microglia stimulated the secretion of HMGB1 in primary neurons, not in astrocytes, at 24 h after 4 h-LPS treatment. Conclusion: All the evidence supported our hypotheses that microglia and neurons are the main cell sources of HMGB1 release under CUMS condition, and that the release of HMGB1 by microglia may play an important role in the development of depressive-like behavior.
目的 通过对比研究探讨潜艇环境对潜艇艇员领悟力、记忆力和作业能力的影响.方法 采取整群抽样的方法,抽取某潜艇支队141名艇员作为研究对象,以某基地潜艇岸勤人员159名作为对照.运用韦氏成人智力量表中国修订版中的领悟力测验分量表测量两组人员的领悟力,用修订韦氏记忆量表中的图片回忆能力测验分量表测量两组人员的记忆力.用双手调节器和手指灵活性测试仪测量两组人员的作业能力,并对两组人员的测量结果进行对比分析.结果 潜艇艇员多期士官(兵龄>5年)领悟力低于潜艇岸勤人员多期士官(t=4.67,P<0.01).潜艇艇员记忆力与潜艇岸勤人员无明显差异(P>0.05).双手协调性测量中,潜艇艇员一期士官(兵龄为3~5年)和多期士官脱轨次数分别多于潜艇岸勤人员一期士官(Z=-2.20,P<0.05)和多期士官(Z=-2.21,P<0.05).手指灵活性测试中,潜艇艇员一期士官和多期士官的针掉次数分别多于潜艇岸勤人员一期士官(Z=-3.08,P<0.01)和多期士官(Z=-4.52,P<0.01).结论潜艇艇员领悟力和作业能力低于潜艇岸勤人员.潜艇艇员多期士官的领悟力较潜艇岸勤人员多期士官差,潜艇艇员一期士官和多期士官的作业能力分别较潜艇岸勤人员一期士官和多期士官差.潜艇环境对潜艇艇员记忆力的影响不明显.
目的 探究热门影视教学法在发展心理学教学中的应用价值.方法 2016年12月—2017年12月,选取我校2015级和2016级医学心理学本科生两个班次,人数均20人.2015级班次为实验班,采用热门影视教学法;2016级班次为对照班,采用传统教学法授课;两个班次学时、授课内容一样.观察并比较两组的临床教学效果.结果 实验组理论成绩(87.4±4.4)分、学生满意度均优于对照组,差异均具有统计学意义(P<0.05).结论 利用热门影视案例教学有利于构建有效课堂,激发学生的学习热情和提高学生的学习能力.
Our previous study implied the role of central high mobility group box 1 (HMGB1) in lipopolysaccharide (LPS)-induced depressive-like behaviors that could partially abrogate by glycyrrhizic acid (GZA). Here, we considered the potential mechanism underlying GZA ameliorating chronic stress-induced depression both in vivo and in vitro. Depression model was established with the 4-week chronic unpredictable mild stress (CUMS) mice. Sucrose preference test, tail suspension test and open field test were performed to reflect depressive-like behaviors. Enzyme activity of indoleamine-2,3-dioxygenase (IDO) was recorded with the ratio of kynurenine (KYN) / tryptophan (Trp). Transcription of gene was evaluated by RT-PCR. Along with depressive-like behaviors, IDO, the rate-limiting enzyme of the kynurenine pathway (KP), was upregulated at the level of mRNA expression, and enzyme activity was also elevated in stressed hippocampi and LPS/HMGB1-treated hippocampus slices. Treatment of mice with GZA, the inhibitor of HMGB1, prevented the activated enzymes in KP and the development of depressive-like behaviors. These experiments demonstrate that GZA may restrain HMGB1 thus improving chronic stress-induced depressive behavior through regulating KP.
Objective To study the change pattern of cognition of the ship crew at different stages during prolonged sea deployment,so as to provide scientific evidence for the screening of the ship crew,development of the protocols for the prevention,treatment and improvement of cognition impairment that occurred among the ship crew with a view to reduce the rate of accidents during prolonged deployment at sea.Methods A retrospective study was made on the 185 naval ship personnel who were involved in the 3-month prolonged sea deployment.Detection,comparison,and analysis were performed by using the occupational fatigue scale,the on-line test (A,B) and the continuous performance test (CPT).Results During the 3-month prolonged sea deployment,the fatigue level of the ship crew at day 35 was significantly greater than the fatigue level one week before and after the prolonged deployment,and statistical significance could be seen,when comparisons were made between them (P < 0.05).The length of time spent on the on-line test A and B was longer than that one week before and after the prolonged deployment,and statistical significance could also be seen,when comparisons were made between them (P < 0.05).The accumulated scores of the continuous performance test were all lower than those one week before and after the prolonged deployment,and statistical significance was also noticed,when comparisons were made between them (P < 0.05).Conclusions Prolonged sea deployment could affect such cognitive capabilities as information processing,reasoning,attention and alert of the ship crew.
