After rice is processed into parboiled rice, it becomes a food with low glycemic index.Due to the effect of parboiled rice-preparing process on the rice, the changed starch structure and digestibility have induced a reduced hyperglycemic response to rice.After changes in starch structure, direct chain starch content increases with the result in digestive rate decrease.Furthermore, the proportion of the slowly digesting-starch(SDS)in the starch, and the content of the resistant starch(RS)go up, which induces an decreased digestive rate.In addition, parboiled rice also confers greater protection against oxidative stress.It also contains high fat and protein with strong sense of satiety, which reduces the level of blood glucose after eating.The parboiled rice is suitable for patients with diabetes and for the individuals with high risk for diabetes, and can also be applied to patients with hypertension, obesity and other metabolic diseases.
目的 探讨老年重症高血压性脑出血患者辅助应用颅内压( ICP)监测对二次伤害的预防价值.方法 选取成功实施颅内血肿清除术治疗的重症高血压性脑出血老年病患54例,行前瞻性观察研究,依据病患术后监测方案差异分组,对照组27例,术后常规多系统监测,观察组27例,在多系统监测方案上辅助应用ICP监测,评估记录两组术后归转及二次伤害状况.结果 两组肺部感染发生率差异无统计学意义(P>0. 05);观察组电解质代谢紊乱、急性肾衰竭、应激性溃疡发生率均显著低于对照组(P<0. 05);观察组二次手术率显著低于对照组(P<0. 05);观察组术后归转结局显著优于对照组(P<0. 05).结论 老年重症高血压性脑出血者实施手术干预后辅助应用ICP监测,可实时掌握患者颅内环境变化,预测病情发展及归转状态,指导临床用药,减少二次伤害.
目的:观察微创针刀镜治疗老年性类风湿关节炎(elderly onset rheumatoid arthritis,EORA)的临床疗效.方法:52例EORA患者按随机数字表法分为治疗组和对照组各26例,对照组给予口服甲氨蝶呤10 mg/w+美洛昔康7.5 mg/d,治疗组在对照组方案基础上联合应用微创针刀镜治疗,分别于治疗前和治疗后4周观察2组患者Lysholm膝关节评分、疼痛程度评估、患者的躯体活动功能(health assessment questionnaire,HAQ)、28关节病情活动度评分(Disease Activity Score,DAS28)、血沉(Erythroeyte Sedimentation Rate,ESR)、C-反应蛋白(C-reactive Protein,CRP)和类风湿因子(Rheumatoid Factor,RF)以及2组治疗后ACR20、ACR50和ACR70的改善率.结果:2组患者治疗后与同组治疗前比较,2组患者Lysholm膝关节评分、VAS疼痛评分(visual analogue scale,VAS)、HAQ、DAS28均有明显改善,治疗组治疗后患者Lysholm膝关节评分、疼痛程度评估、HAQ比对照组患者均有明显改善,治疗组患者ACR20改善率(76.42%)明显高于对照组患者(53.28%).结论:微创针刀镜治疗EORA能显著提高临床疗效,改善患者的生活质量.

Objective To study the effect of high temperature and humidity environment on sweat gland function and its possible mechanism.Methods Fourteen Sprague-Dawley (SD) rats were fully randomized by randomized digital tables randomly and divided into normal temperature and humidity group (control group) and high temperature and humidity group (test group) (n =7 in each group).The test group was subjected to treat by heat stress after feeding for three days.The rats were exposed to the environment of 40 ℃ and relative humidity of 65% and removed after 2 h,and the samples were taken out after 72 h.The animals in the control group were placed at room temperature without any treatment,and the samples were obtained after 72 h.Results Under the light microscope,the vacuoles of the epithelium of the sweat secretion region were decreased in the test group,and the apoptosis of cells was increased.The apoptosis index was (70.7 ± 14.0)% in the test group,significantly higher than that in the control group [(58.6 ± 11.7) %] (t =2.478,P =0.021).Western blotting showed that the relative expression of M3 receptor in the test group was (1.21 ± 0.16)%,significantly higher than that in the control group [(1.81 ±0.41)%,t =2.711,P =0.035].The expression of serum acetylcholine in the test group was significantly lower than that in the control group.The superoxide dismutase (SOD) activity [(13.65 ± 5.14) U/mg] in the test group was significantly lower than that in the control group [(25.89 ± 11.05) U/mg,t =2.633,P =0.023].Conclusion The damage of sweat gland cells in the high temperature and humidity condition is mainly due to the decrease of tissue antioxidant.

目的 调查老年男性骨密度及25-羟维生素D(25-OHD),Ⅰ型前胶原氨基端肽(PⅠNP),β胶原降解产物(β-Crosslaps)水平,为老年男性骨质疏松的诊治提供依据.方法 记录129名60 ~ 90岁受试者的年龄、身高、体质量、BMI,测量其腰椎1-4、左侧股骨颈、Wards三角、大粗隆、左侧股骨上端的骨密度(BMD),检测空腹血清25-OHD、PⅠNP,β-Crosslaps水平.根据骨密度水平将受试者分为骨量正常组(n=22)、骨量低下组(n=73)和骨质疏松组(n=34).结果 骨量低下组及骨质疏松组的BMD均明显低于正常骨量组,骨质疏松组又明显低于骨量低下组(P<0.05).骨量低下组及骨质疏松组PⅠNP、β-Crosslaps均明显高于正常骨量组,骨质疏松组β-Crosslaps又明显高于骨量低下组(P<0.05).25-OHD水平在骨质疏松组有降低趋势,但差异无统计学意义(P>0.05).结论 大部分老年男性存在骨量丢失及维生素D不足,其主要原因为高骨转换状态.
目的:分析XB130蛋白在人体肿瘤组织中的分布特点及相关性。方法应用免疫组化法和组织芯片技术检测 XB130蛋白在人体比较常见的恶性肿瘤组织中的分布。结果 XB130蛋白在15种正常人组织中,即胃黏膜、肾皮质组织、甲状腺组织中高表达,在肺泡上皮、子宫内膜中未见XB130蛋白阳性表达,其余组织均为中-低表达。常见18种肿瘤组织及生殖系统肿瘤中,除子宫颈鳞状细胞癌XB130为阴性,其余肿瘤胰腺导管腺癌、前列腺腺癌、乳腺浸润性导管癌、乳腺浸润性小叶癌、乳腺导管原位癌、卵巢间变型无性细胞瘤等肿瘤细胞XB130蛋白高表达;甲状腺乳头状癌、胃腺癌、结肠腺癌、直肠腺癌、肝细胞性肝癌、肾透明细胞型肾癌、肺鳞状上皮癌、肺腺癌、食管鳞状细胞癌、卵巢浆液性乳头状腺癌、卵巢囊腺癌等低表达。结论 XB130蛋白在多种肿瘤组织中高表达,首次报道在前列腺癌中高表达,表明XB130可能在促进前列腺癌发生发展起到一定作用。
Objective To determine whether high intensity aerobic training-induced profuse sweating could affect the metabolism of some substances of human in humid heat environment.Methods Fifty-six soldiers were randomly selected.Potassium,sodium,chloride,calcium,phosphorus,urea nitrogen,creatinine and uric acid concentrations in blood and urine samples collected before and after exercise and sweat samples collected during training were measured.Results Potassium (from 3.85 mmol/L to 4.10 mmol/L),sodium (from 142.96 mmol/L to 143.82 mmol/L),creatinine (from 61.25 μ mol/L to 67.65 μmol/L) and uric acid (from 358.61 μmol/L to 399.63 μmol/L) concentrations in blood were increased,and chloride (from 100.24 mmol/L to 99.31 mmol/L) concentration was declined;urea nitrogen (from 244.52 mmol/L to 214.93 mmol/L),calcium (from 3.99 mmol/L to 2.71 mmol/L),sodium (from 197.00 mmol/L to 139.96 mmol/L),chloride (from 207.35 mmol/L to 138.75 mmol/L) and uric acid (from 2 608.15 μmol/L to 2 072.05 μ mol/L) concentrations in urine were reduced,and potassium (from 38.82 mmol/L to 55.66 mmol/L) and phosphorus (from 9.97 mmol/L to 12.62 mmol/L) concentrations were increased (P < 0.01).The changes of urine creatinine(P <0.01) and calcium (P < 0.05)concentrations before and after sweating were associated with sweat composition.Conclusion Under humid heat environment,high intensity aerobic training results in inorganic and organic ingredients metabolism.Sweating plays an important role in the process.
Objective To investigate the application of professional rescue techniques in the diagnosis and treatment of critically ill elderly. Methods A retrospective analysis was performed on the critically ill elderly patients from Department of Geriatric Critical Care Medicine from June 2007 to June 2014. Total number of residents in geriatric critical care medicine, severity of illness, application of rescue techniques, success rate, annual mortality, average length of hospital stay and medical expenses were observed and analyzed. Results The total number of residents rose from 295 in the year of 2007 to 347 in the year of 2014. Their age was increased from (80.3±6.9) years to (87.4±7.6) years (P<0.001), and APACHEⅡ was elevated from (13.6±3.9) to (20.6±3.9) (P<0.001). What’ more, the percentage of patients with APACHEⅡ score over 15 were increased from 25.2% to 40.6% (P<0.05). During 2007 to 2014, the employment of professional rescue techniques as fiberbronchoscopy, mechanical ventilation and continuous veno-venous hemofiltration were dramatically decreased, while the success rate of rescue was remarkably elevated from 52.2% to 72.3%. The average length of hospital stay was also shortened from 42.5d to 16.3d, and thus the medical expenses were declined by more than 25%. Conclusion Though the age, amount and severity of the critically ill elderly are increased, specialized professional treatment greatly decreases the employment of professional rescue techniques, increases the success rate, and significantly reduces the ICU stay time and medical expenses. All these indicate that professional rescue techniques are of great helpfulness for the diagnosis, treatment and prognosis of the elderly in geriatric critical care medicine.
Objective To observe the effect of genistein on benign prostate hyperplasia (BPH)in mice and human prostatic epithelial cells (HPEC)in vitro.Methods Thirty-two Kunming mice were injected with testosterone to establish BPH models and then were divided into four groups.In three treatment groups, mice were given with different doses of genistein (1 0,30,90 mg/(kg·d))for four weeks and the mice in the control group were not treated.Then,the mice were sacrificed,the prostates were weighed and the prostate index was calculated.HPECs isolated from prostatectomy specimens of BPH patients were inoculated with cul-ture media containing 0.1 ,0.5,1 .0,2.5,5.0 μmol/L of genistein for 1 0 days,trypsinized and then counted with a hemocytometer.Results The weights of prostate in mice treated with genistein were lower than that of control group (P <0.05).Prostate indexes of middle and high dose group were lower than that of control group (P <0.05).The number of HPECs treaded with 0,0.1 ,0.5,1 .0,2.5,5.0 μmol/L of genistein was (4.71 ±0.78) ×1 04、(4.46 ±1 .1 5) ×1 04、(3.84 ±1 .33) ×1 04 ,(2.01 ±0.31 ) ×1 04 ,(0.58 ±0.28) × 1 04 and (0.20 ±0.1 2) ×1 04.Trypan blue staning revealed that the percentage of living cells was 99.3%, 98.7%,96.7%,93.3%,90.7% and 88.7%.Conclusion Genistein can suppress the growth of HEPCs and mitigate BPH induced by testosterone.
Objective Tto evaluate the change in intestinal barrier function in patients with severe sepsis during HVHF therapy .Methods Fifty -four patients were randomly divided into the control group ( n =31 ) and the HVHF group ( n =23 ) .Peripheral blood from sepsis patients were routinely collected on day 0, 2, 3, 7 after admission.The levels of IFABP, DAO and D-lactate were measured, and death rate of patients in 28 days were recorded.Results There was no difference between the two groups at baseline .However , compared with the conventional treatment group , the plasma levels of IFABP , DAO and D-lactate of HVHF group after HVHF were significantly decreased except for IFABP on day 3 (P<0.05), and the 28-day mortality was significantly reduced (39.1%vs 80.6%, P <0.05).Conclusion HVHF can protect intestinal barrier function, and reduce the mortality as well as improve clinical outcome in elderly patients with sepsis .
Objective To investigate the correlation between benign prostatic hyperplasia (BPH) and blood pressure variability (BPV) in the elderly patients with H type hypertension.Methods 561 elderly patients with H type hypertension from 2010 to 2012 were enrolled in this study,including 436 BPH patients and 125 non-BPH (NBPH) patients.All patients underwent ambulatory blood pressure monitoring (ABPM) and ultrasound examination of prostate.The 24-hour systolic blood pressure variation (24 h SBPV),24 hour diastolic blood pressure variation (24 h DBPV),daytime systolic blood pressure variation (dSBPV),daytime diastolic blood pressure variation (dDBPV),nocturnal systolic and diastolic blood pressure variations were recorded and compared.The differences in blood pressure variation between the two groups were compared.Correlation between BPV and prostatic volume (PV) was analyzed.Results There were no statistical differences in variabilities of 24 h SBPV,nSBPV and dSBPV between BPH group and NBPH group(all P>0.05).24 h DBPV,nDBPV and dDBPV were significantly higher in BPH group than inNBPH group [(18.5±3.3) mmHg vs.(14.1±3.7) mmHg,(17.1±3.4) mmHg vs.(14.9 ±3.4) mmHg,(16.6±4.1) mmHg vs.(12.8±5.1) mmHg,respectively,all P<0.05].There were correlations of 24 h DBPV,nDBPV and dDBPV with PV (r =0.137,0.032,0.079,respectively,all P<0.05).Conclusions The diastolic blood pressure variability may associate with the degree of prostatic hyperplasia in the elderly patients with H type hypertension.
本研究通过对军队医院非战争军事行动卫勤保障预案内容框架的探析,结合演练实际,采用专家咨询、层次分析等方法建立评价指标体系,在此基础上建立模糊综合评价模型,并对部分医院预案进行评估,发现存在问题,并指导修订.
目的 调查广州地区青年男性维生素D、PTH状态及其与区域骨密度之间的关系.方法 随机选取2011年5月-2012年8月在广州军区广州总医院门诊部体检的60名广州青年男性志愿受试者,年龄18~44岁.评估血清25(OH)D、PTH水平,并分析血清25(OH)D、PTH与骨转换指标和区域骨密度(BMD)的相关性.结果 46.7%的青年男性血清25(OH)D水平>30 ng/mL,53.3%低于30 ng/mL,5%低于20 ng/mL.血清25(OH)D水平与年龄分布无统计学差异(P>0.05).双变量分析显示血清25(OH)D水平与PTH负相关(r=-0.264,P=0.042),与β-Crosslaps负相关(r=-0.257,P=0.047).血清25(OH)D水平、PTH与L1-4以及单个腰椎L1、L2、L3、L4的BMD无相关性.股骨大转子BMD与血清25(OH)D正相关(r=0.271,P=0.040),校正年龄、BMI和骨生化指标后,正相关性更加明显(r =0.319,P=0.020).多元线性回归分析显示BMI、ALP与全腰、L1、L3、L4椎BMD相关,BMI、PINP与L2椎BMD相关,BMI、25(OH)D与全髋、大转子BMD相关.结论 青年男性广泛存在血清25(OH)D不足状态,血清25(OH)D水平与股骨大转子BMD明显正相关,适当补充维生素D和钙剂,对维持大转子区域的骨量至关重要.
BACKGROUND: Perspiration can remove some metabolites dissolved in the sweat out of the body, playing a supporting role in the excretion. Modern medicine cannot restore the sweat glands of severe trauma or burn patients, and how to solve the regeneration of the skin and sweat glands and culture tissue-engineered skin functional appendage has become a hot spot in today’s medical research. OBJECTIVE: To explore the body’s metabolism and metabolic way of the sweat, thereby analyzing the functional sweat gland regeneration of tissue-engineered skin. METHODS: Studies addressing the tissue-engineered skin functional appendage regeneration effects on perspiration and metabolism were col ected to analyze the body’s metabolism and excretion ways, sweat metabolites and metabolism methods, as wel as the regeneration of tissue-engineered skin appendages. RESULTS AND CONCLUSION: Sweat as an important member in the excretory system cannot be ignored in maintaining the dynamic equilibrium of the body and preventing the body against metabolite damages. Ideal tissue-engineered skin functional appendages have abilities of sweat secretion, excretion of waste, regulating body temperature and maintaining the dynamic equilibrium of the skin, thereby playing a vital role in skin wound repair and reconstruction after extensive burns. Studies have shown that stem cells and sweat gland cells were co-cultured, and then transplanted into the scar wound for sweat gland reconstruction, which has achieved some success.
