ObjectiveThe incidence of coronary heart disease (CHD) is progressively increasing on an annual basis. Dyslipidemia constitutes a significant pathogenic factor in CHD, exerting a substantial influence on its onset and progression. Consequently, precise and effective lipid management is crucial for the prevention and cure of CHD. This study aims to examine the risk factors, therapeutic approaches, current research status and emerging trends in lipid management related to CHD.MethodWe searched for publications on lipid management of coronary heart disease from 01/01/2014 to 12/31/2023 in Web of Science and performed bibliometrics using CiteSpace, VOSviewer, Scimago Graphica, Gephi and R Studio.ResultsA total of 6,027 related articles were retrieved from the Web of Science database. After screening, 5,954 articles were included. Journal of Clinical Lipidology and Atherosclerosis were the journals with the most publications and citations, respectively. In this discipline, the United States has the largest number of publications, research institutions, citations, and collaborative partnerships. The burst keywords include sex difference, heterozygote familial hypercholesterolemia, PCSK9, lipid peroxidation, fish oil, monoclonal antibody, insulin sensitivity, and gene, etc.ConclusionResearch on CHD indicates that risk factors influencing lipid levels encompass sex, genetics, PCSK9, and lipid peroxidation. There has been a growing trend in investigating the underlying mechanisms of these factors. Recent research hotspots have concentrated on disease prevention, prognosis, specific treatments for CHD, the development of new pharmaceuticals, and the molecular mechanisms of action. Future research is likely to continue focusing on more precise treatment protocols and the exploration of novel mechanisms.
BackgroundCoronary microvascular disease (CMVD), particularly ischemia with non-obstructive coronary arteries (INOCA)-related CMVD, is associated with recurrent angina, impaired quality of life, and an increased risk of adverse cardiovascular outcomes. Although recognition of this condition is increasing, treatment options remain limited, and many patients continue to have symptoms despite conventional medical therapy. Acupuncture has shown potential as an adjunctive therapy for angina-related symptoms and other cardiovascular conditions. However, high-quality evidence for INOCA-related CMVD remains scarce. This trial aims to evaluate the efficacy and safety of acupuncture as an adjunct to conventional medical therapy in patients with INOCA-related CMVD.MethodsThis prospective, single-center, randomized, sham-controlled, parallel-group pilot trial will recruit 60 participants with INOCA-related CMVD at Guang'anmen Hospital, China Academy of Chinese Medical Sciences, Beijing, China. Eligible participants will be randomly assigned in a 1:1 ratio to an acupuncture group or a sham acupuncture group. Both groups will continue guideline-directed conventional medical therapy. The acupuncture group will receive true acupuncture at Dabao (SP21), Neiguan (PC6), Danzhong (CV17), Xinshu (BL15), and Geshu (BL17), while the sham group will receive non-penetrating sham acupuncture at the same acupoints with the same schedule. Treatment will be given three times weekly for 4 consecutive weeks, followed by a 4-week follow-up. The primary outcome is the response rate at week 4, defined as the proportion of participants with an increase of at least 5 points in the Seattle Angina Questionnaire summary score from baseline. Secondary outcomes include changes in SAQ scores, angina attack frequency, EuroQol 5-Dimension 5-Level questionnaire, traditional Chinese medicine syndrome score, nitric oxide, endothelin-1, and high-sensitivity C-reactive protein. Additional assessments include Clinical Global Impression–Improvement, expectancy and credibility assessments, and blinding assessment. Safety evaluation includes monitoring adverse events and serious adverse events throughout the trial.DiscussionThis pilot trial will provide preliminary evidence on the safety and potential efficacy of acupuncture for INOCA-related CMVD and may help clarify its effects on angina-related health status, quality of life, and selected endothelial and inflammation-related markers. The findings are expected to inform future larger multicenter confirmatory trials.Clinical Trial RegistrationInternational Traditional Medicine Clinical Trial Registry (http://itmctr.ccebtcm.org.cn/), Identifier ITMCTR2026000515.
BACKGROUND:A growing body of clinical evidence points to an association between hypertension and anxiety, but the mechanisms by which the two occur are unclear. This article aims to explore possible common influences and associations between hypertension and anxiety. METHODS:We searched for publications on hypertension and anxiety from January 01, 2004 to December 31, 2022 in Web of Science and performed bibliometrics using CiteSpace, VOSviewer, Scimago Graphica and Gephi. RESULTS:A total of 3216 related articles were retrieved from the Web of Science database. After screening, 3051 articles were included. The number of published articles has increased over the past 19 years. The United States has more researches in this area and has strong collaborative relationships with other countries, which gives it some credibility and authority. The words that appear in the burst keywords are gender, age, obesity, depression, panic disorder, pregnancy induced hypertension, coronary heart disease, chronic kidney disease, and pituitary adrenal axi, which are co-related with hypertension and anxiety. CONCLUSION:There is a link between hypertension and anxiety, and the 2 influence each other, usually in a positive way. Common influences on hypertension and anxiety include age, gender, obesity, depression, panic attacks, pregnancy, coronary heart disease and chronic kidney disease. Recent research hotspots have focused on population aging and comorbidities. Future research hotspots are likely continue to focus on influencing factors, clinical research and prognosis.
The metabolic score for visceral fat (METS-VF), which integrates measures of visceral adiposity, insulin resistance, and demographic variables, serves as a composite indicator of metabolic health. However, the association between METS-VF and atrial fibrillation (AF) risk is unclear. This study aimed to investigate the prospective association between METS-VF and incident AF within the UK Biobank cohort. A total of 400,668 participants from the UK Biobank were included in the analysis. Flexible parametric survival models were applied to estimate hazard ratios (HRs) and 95
OBJECTIVE:To screen the target population with symptomatic bradyarrhythmia for pharmacotherapy. METHODS:This study use database from clinical trial that use Yuanjiang decoction, a traditional Chinese medicinal prescription. Eligible participants were recruited and treated with Yuanjiang decoction (composed of 6 Chinese herbal medicines), 200 mL twice daily. Cross-contingency analysis, logistic regression analysis, interaction analysis, discriminant analysis and 10-fold cross validation methods were used to establish discriminant model. RESULTS:This study concluded that the clinical treatment of bradyarrhythmia has a clear effect. Low minimum heart rate, high maximum heart rate are risk factors that affect the efficacy. Patients with only one comorbid disease did not significantly affect the efficacy, but patient with two or more diseases of coronary heart disease, hypertension, paroxysmal atrial fibrillation, premature ventricular contraction and premature atrial contraction at the same time did not have a good effect. Using the discriminant analysis method to establish a efficacy prediction model, y = 0.07 X 1 + 0.16 X 2 - 0.65 X 3 - 1.12 X 4 - 0.71 X 5 - 0.75 X 6 - 0.91 X 7 (X1 = 24 h mean heart rate, X2 = minimum heart rate, X3= coronary heart disease, X4 = paroxysmal atrial fibrillation, X5 = premature ventricular contraction, X6= sinus block, X7 = atrioventricular block). CONCLUSION:Our model based on the clinical features of patients with bradyarrhythmia. Should be useful aid for predicting pharmacotherapy response and could screen the optimal pharmacotherapy target.
Background:Autophagy is essential for the homeostasis and function of the cardiovascular system. Citespace is a visual analysis software developed in the context of scientometrics and data visualization. The purpose of this study is to use Citespace software to conduct bibliometric and visual analysis of the research on autophagy in cardiovascular diseases, identify the current status, hot spots and trends in this field, help researchers clarify the future research focus and direction of autophagy in cardiovascular diseases, and provide more positive and broader ideas for the treatment and drug development of cardiovascular diseases. Methods:In the Web of Science Core Collection database to download the data from 2004 to 2022 regarding autophagy in cardiovascular research. CitespaceV was used to collect the research status, hotspots and development trends for visual analysis. Results:The 3568 articles were published by 547 authors from 397 institutions in 75 countries. From 2004 to 2021, the annual publications increased over time. The top 3 productive nations were China, the United States, and Germany. The leading institution was China's Fudan University. The most cited paper is Guidelines for the use and interpretation of assays for monitoring autophagy (3rd edition). The research hotpots include monitoring methods for autophagy activity, changes in autophagy levels in different types of cardiovascular diseases, autophagy signal transduction mechanism in cardiovascular diseases, etc. Conclusion:Bibliometric analysis provided valuable information for autophagy research in cardiovascular disease, which is full of opportunities and challenges. The research of autophagy in the field of cardiovascular diseases is still worthy of in-depth exploration. A challenge with autophagy-targeted therapies is their dichotomy in which the goal is to target maladaptive autophagy while maintaining a baseline level of cell survival to optimize a beneficial outcome. It is necessary for scientists to develop new methods to evaluate the level of autophagy from basic application to human body and reveal the signaling mechanism of autophagy in different types of cardiovascular diseases.
Background The independent causality of periodontal disease on atrial fibrillation (AF) and the modifiable risk factors mediating the pathogenesis remain unclear. Method First, to confirm the independent causality, this study underwent the bidirectional and multivariable Mendelian Randomization (MR) underlying the genome-wide association studies (GWAS) of periodontal disease (87497 cases/259234 controls) from the FinnGen and AF (55114 cases/482295 controls) from AFGen. Then, the two-step Mendelian randomization evaluated the mediating role and proportions of 25 candidate factors. Result Periodontal disease was associated with a higher risk of AF with an odds ratio of 1.16 ([95% CI, 1.027–1.314], p = 0.017), independently of dental caries and pulp and periapical disease. Reversely, there is no causation of AF on periodontal disease status. Using the two-step MR, we identified 5 out of 25 candidate mediators as statistically significant; sorted by the mediation proportion, including weight (30.3%), IL-17 (17.2%), TNF (14.08%), coronary atherosclerosis (13.4%) and hypertension (11.6%). Conclusion Our findings preliminarily demonstrated the genetic causality between periodontal disease and atrial fibrillation. For patients suffering from periodontal disease status, it is noted that maintaining oral hygiene, accepting standardized periodontal treatment, and managing body weight are crucial targets for controlling the disease progression towards AF.
In recent years, spatial transcriptomics (ST) research has become a popular field of study and has shown great potential in medicine. However, there are few bibliometric analyses in this field. Thus, in this study, we aimed to find and analyze the frontiers and trends of this medical research field based on the available literature. A computerized search was applied to the WoSCC (Web of Science Core Collection) Database for literature published from 2006 to 2023. Complete records of all literature and cited references were extracted and screened. The bibliometric analysis and visualization were performed using CiteSpace, VOSviewer, Bibliometrix R Package software, and Scimago Graphica. A total of 1467 papers and reviews were included. The analysis revealed that the ST publication and citation results have shown a rapid upward trend over the last 3 years. Nature Communications and Nature were the most productive and most co-cited journals, respectively. In the comprehensive global collaborative network, the United States is the country with the most organizations and publications, followed closely by China and the United Kingdom. The author Joakim Lundeberg published the most cited paper, while Patrik L. Ståhl ranked first among co-cited authors. The hot topics in ST are tissue recognition, cancer, heterogeneity, immunotherapy, differentiation, and models. ST technologies have greatly contributed to in-depth research in medical fields such as oncology and neuroscience, opening up new possibilities for the diagnosis and treatment of diseases. Moreover, artificial intelligence and big data drive additional development in ST fields. Spatial transcriptomics (ST) technologies and application prospects. (1) Imaging-based approaches, including in situ sequencing (ISS)—where transcripts are amplified and sequenced in tissue—and ISH-based approaches—where imaging probes are sequentially hybridized in tissue. (2) NGS-based techniques, in which positional information is encoded onto transcripts prior to NGS sequencing.
A growing body of research indicates the promising potential of vaccines in both preventing and treating atherosclerosis (AS). To gain a comprehensive understanding of the current research landscape and emerging trends in this field, this study conducted a bibliometric analysis of publications on AS vaccines using the Web of Science Core Collection (WoSCC) database, based on the “bibliometric” R package and VOSviewer software. From 1991 to 2024, a total of 462 publications were identified in the WoSCC. The United States appeared as the leading contributor in terms of both total publications and citations. The Vaccine journal exhibited the highest publications output. Nilsson J from the Lund University in Sweden was the author with the most published articles, total citations and Hirsch index (H-index). Keywords analysis and thematic maps analysis revealed the passive immunotherapy (AS protective antibodies vaccines) was a hot mature theme, the active immunotherapy (AS antigens vaccines) was an emerging and booming theme, while the efficacy and safety of AS vaccines was a niche and well-developed theme. These findings offered valuable insights into the AS vaccination and provided guidance for future research in this domain.
The vascular endothelial growth factor (VEGF) family plays a major role in tumors and ophthalmic diseases. However, increasingly more data reported its potential in regulating lipids. With its biological functions mainly expressed in lymphatic vessels, some factors in the families, like VEGF-A and VEGF-C, have been proved to regulate intestinal absorption of lipids by affecting chylous ducts. Other effects, including regulating lipoprotein lipase (LPL), endothelial lipase (EL), and recombinant syndecan 1 (SDC1), have also been confirmed. However, given the scant-related studies, further research should be conducted to examine the concrete mechanisms and provide pragmatic ways to apply them in the clinic. The VEGF family may treat dyslipidemia in specific ways that are different from common methods and concurrently contribute to the treatment of other metabolic diseases, like diabetes and obesity.
Objectives To summarize the measures and rules of Chinese medicine (CM) and provide reference for clinical application in the prevention and treatment of coronavirus disease (COVID-19). Methods The data source was from CM COVID-19 prevention and treatment programs on government websites and official media websites of the different provinces and cities. The search lasted from December 8, 2019 to March 10, 2020. Main variables were medication frequency and combinations of medicines. Cluster analysis and complex network analysis were used by prevention and treatment stage and by area. Results Among 27 CM diagnosis and treatment plans, 203 therapeutic prescriptions were enrolled, of which the top 4 herbs were: Radix glycyrrhizae, Semen armeniacae amarum, Herba ephedrae, and Herba agastachis, respectively. The core combinations were Herba ephedrae and Semen armeniacae amarum. Forty-eight preventive formulae were identified. Ten herbs, including Radix Astragali seu hedysari, Radix glycyrrhizae, Radix saposhnikoviae, Flos lonicerae, etc. were most frequently used. The core prescription of CM compatibility was Radix astragali seu hedysari, Radix glycyrrhizae, and Radix saposhnikoviae, which is the main component of Yu Ping Feng San. There were 45 prevention and treatment prescriptions in East China; the most used CM was Radix glycyrrhizae, Herba agastachis, Pericarpium citri reticulatae, and Gypsum fibrosum. Fifty prescriptions were identified in North China. According to CM analyses, Herba agastachis, Semen armeniacae amarum, Herba ephedrae, and Poria were most frequently used. Conclusions CM for COVID-19 prevention mainly focuses on improving human immunity; for treatment, prescription focuses on clearing the lungs and removing dampness. Prescriptions vary with regions, perhaps due to climatic and environmental differences, which help clinicians to quickly make CM plans and treat patients according to clinical status, further minimizing resource wastage.
Takotsubo cardiomyopathy (TTC) is often acute with a high mortality rate and is subject to relapse. Meanwhile, its complex pathogenesis has attracted increasing attention. To learn more about TTC, CiteSpace V.5.7 R5W was used in this study to analyze the research status, hot spots, and trends in TTC before 2020. The keywords, co-citation references, as well as country and institution distribution were explored. A total of 2,349 papers were reviewed. The United States, Italy, and Germany were the main countries studying TTC and had good cooperation relationships. The Mayo Clinic topped the institution list, but the rate of inter-institutional cooperation was not high. Research hotspots include disease features, auxiliary diagnostic methods, epidemiology, and pathophysiological mechanisms, and the latest ones are complications related to prognosis, such as cardiovascular abnormalities caused by myocardial infarction and normal or non-obstructive coronary arteries (MINOCA), atrial fibrillation, stroke, cancer, and COVID-19. In conclusion, the research of TTC is in a hot development period. Our research will help clinicians and researchers to better understand TTC and its research status by providing a foundation for research objectives. In doing this, our research will help to provide better scientific management, diagnosis, and treatment for patients with TTC, which will in turn improve the prognosis of this condition.

Abstract Objective: Andrographis paniculata (Burm.f.) Nees is a medicinal plant that has been traditionally used as an anti-inflammatory and antibacterial remedy for several conditions. Andrographolide (AG), the active constituent of A. paniculata (Burm.f.) Nees, has anti-lipidic and anti-inflammatory properties as well as cardiovascular protective effects. The present study aimed to explore the effects of AG on the progression of atherosclerosis and to investigate related mechanisms via network pharmacology. Materials and methods: Compound-related information was obtained from the PubChem database. Potential target genes were identified using STITCH, SwissTargetPrediction, Bioinformatics Analysis Tool for Molecular mechANism of Traditional Chinese Medicine, and Comparative Toxicogenomics Database. Genes involved in atherosclerosis were obtained from DisGeNet and compared with AG target genes to obtain an overlapping set. Protein–protein interactions were determined by STRING. Gene ontology (GO) analysis was performed at WebGestalt, and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment was analyzed using Metascape. The final network showing the relationship between compounds, targets, and pathways was constructed using Cytoscape. After that, oxLDL-induced RAW264.7 cells were used to further validate a part of the network pharmacology results. Result: Eighty-one potential AG target genes were identified. PPI, GO, and KEGG enrichment revealed genes closely related to tumor progression, lipid transport, inflammation, and related pathways. AG improves the reverse cholesterol transport (RCT) through NF-κB/CEBPB/PPARG signaling in oxLDL-induced RAW264.7 cells. Conclusion: We successfully predict AG’s potential targets and pathways in atherosclerosis and illustrate the mechanism of action. AG may regulate NF-κB/CEBPB/PPARG signaling to alleviate atherosclerosis.
