目的 探讨葡萄糖易化转运蛋白9(SLC2A 9)基因rs938552位点多态性与肾结石的关系.方法 选择华东地区高尿酸血症合并肾结石患者(病例组)及健康对照组各300例,用聚合酶链式反应(PCR)扩增SLC2A 9基因SNP位点rs938552序列,Sanger法测序扩增产物;同时检测病例组血清、尿液的生化代谢指标;采用红外光谱分析法检测病例组结石成分.分析病例组与对照组中基因型分布差异;分析病例组中不同基因型携带者的血、尿电解质、尿酸等代谢水平的差异;分析病例组中结石成分的差异.结果 SLC2A 9基因rs938552位点基因型与等位基因频率在病例组和健康对照组差异均有统计学意义(P均<0.001),携带杂合型(CT)和突变型(TT)的人群患肾结石的风险是携带野生型(CC)人群的2.933倍(OR=2.933,95%CI:1.632~5.264,P<0.001);病例组中CC基因型者血液尿酸、尿液尿酸和尿钙均低于CT+TT基因型者,前者尿pH值高于后者(P均<0.05),其他代谢指标差异均无统计学意义(P均>0.05);病例组中携带CC基因型者与携带CT+TT基因型者比较,后者结石成分草酸钙和尿酸的比例高于前者(P均<0.05).结论 SLC2A 9基因rs938552位点多态性与华东地区汉族人群患肾结石的风险有关联.
目的:探讨葡萄糖易化转运蛋白9(SLC2 A9)基因SNP位点多态性与机体生化代谢及肾结石的相关性和可能机制.方法:选择2015年1月~2017年12月在我院就诊的华东地区汉族人群肾结石患者300例(结石组)和健康志愿者300例(健康对照组),用聚合酶链式反应(PCR)扩增SLC2A9基因3个SNP位点(rs938552、rs1014290和rs4311316)序列,Sanger法测序扩增产物;同时检测结石组血清、尿液中钙、磷、尿酸等生化代谢指标.分析结石组与健康对照组中基因型分布差异以及结石组中不同基因携带者的血、尿电解质、尿酸等代谢水平的差异.结果:SLC2 A9基因SNP位点rs938552存在多态性,其基因型分布在结石组和健康对照组之间比较差异有统计学意义(OR =0.341,95% CI:0.206~0.563,P<0.01),携带有杂合型(C/T)和突变型(T/T)的人群患肾结石的风险是携带野生型(C/C)人群的2.933倍;结石组中C/C基因型者血尿酸、尿尿酸、尿钙均低于C/T+T/T基因型者,C/C基因型者尿pH值高于C/T+T/T基因型者,差异均有统计学意义(P<0.05),其他代谢指标无统计学意义;C/C基因型与C/T+T/T基因型比较,后者结石的主要成分草酸钙和尿酸的比例明显高于前者(P<0.05).位点rs1014290和rs4311316在结石组和健康对照组基因型分布比较差异无统计学意义(OR=0.608,95%CI:0.364~1.014,P>0.05;OR=0.641,95%CI:0.407~1.008,P>0.05),其C/C基因型与C/T+T/T基因型比较,血尿生化及结石主要成分无统计学意义(P>0.05).结论:SLC2 A9基因rs938552位点多态性与华东地区汉族人群患肾结石的风险有关联,SLC2A9基因rs938552位点可能通过调节尿酸和钙的代谢来影响结石的形成.
目的 探讨泌尿系结石患者的结石成分及其与肾功能损伤的关系.方法 收集该医院诊断和治疗的泌尿系结石患者641例,经手术、体外冲击波碎石、自行排出等方法获得结石,运用红外光谱分析法进行结石成分测定,并对患者进行发射单光子计算机断层扫描仪(ECT)肾功能检测.结果 641例泌尿系结石患者中,男、女性别比为2.02:1.发病年龄18~86岁,45~54岁年龄段构成比占35.1%.在641例结石样本中,以草酸钙为主的结石占80.03%,磷酸钙为主的结石占16.07%,尿酸为主的结石占2.03%,六水磷酸镁铵为主的结石占1.87%.ECT肾功能检测结果可见,肾小球滤过率(glomerular filtration rate,GFR)正常者42例(占6.55%),轻中度降低者453例(占70.67%),有146例患者明显降低(占22.78%),尿酸为主要结石成分的患者GFR值均低于其他三种成分者(P均<0.05).结论 上海浦东地区泌尿系结石患者结石成分以草酸钙为主,以尿酸为主的结石对患者肾功能损伤较重.
目的 探讨银杏叶提取物(EGb)对体外冲击波碎石术(ESWL)所致的早期肾损伤是否有保护作用.方法 80例行ESWL术的单侧肾结石病人按随机数字表法分成观察组和对照组,每组40例,观察组于ESWL术前0.5 h开始,给予银杏叶提取物20 mL加入生理盐水250 mL静脉滴注,每12小时1次,给药5 d;对照组同时间节点给予生理盐水250 mL静脉滴注.两组分别收集ESWL术前2 h、ESWL术后2 h、6 h、12 h、24 h、1周尿液标本10 mL,测定尿液中的中性粒细胞明胶酶相关载脂蛋白(uNGAL)和尿胱抑素C(uCys-C)水平.结果 uNGAL在两组中于ESWL术后2 h开始升高,12 h达到高峰,1周时恢复到术前水平.观察组uNGAL于ESWL术后12h为(17.56±3.94)ng/mL,低于对照组(22.68±4.82)ng/mL,其差异有统计学意义(t=3.56,P=0.025).uCys-C在两组中于ESWL术后6 h开始升高,12 h达到高峰,24 h有所下降,1周时仍高于术前水平.观察组uCys-C于ESWL术后12 h达(35.61±6.24)ng/mL,低于对照组(41.28±5.57)ng/mL,其差异有统计学意义(t=4.47,P=0.017).结论 EGb能有效防护ESWL所致的早期肾小球和肾小管的损伤,其机制尚需进一步研究.
目的 探讨不同阶梯式增能方式对体外冲击波碎石(ESWL)后肾损伤的保护作用.方法 120例7~15 mm单发肾结石患者被随机分为3组:常规组(常规阶梯式增能)、间歇组(间歇阶梯式增能)和缓慢组(缓慢阶梯式增能).在ESWL前及ESWL后2h、24 h、48 h、1周和2周分别检测患者的尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)和胱抑素C(Cys C).同时比较ESWL前后视觉模拟疼痛评分(VAS).2周后统计排净率及并发症率等.结果 3组患者的基线VAS评分及尿NGAL、Cys C值、2周排净率及并发症率均无显著性差异(P>0.05).间歇组、缓慢组各时点NGAL及Cys C升高幅度显著低于常规组(P<0.001).缓慢组术后VAS评分显著低于常规组、间歇组(P <0.001).结论 ESWL治疗肾结石时,间歇阶梯式增能及缓慢阶梯式增能在保持良好的碎石效果的同时均可以起到减轻肾损伤的作用;并且后者的疼痛感更轻,患者接受度更好.
目的 对比体外冲击波碎石(ESWL)和经输尿管镜钬激光碎石(URL)治疗输尿管上段结石的疗效.方法 回顾性收集2016年1至2017年12月在我院接受治疗的输尿管上段结石患者共982例,选择体外冲击波碎石患者580例,选择输尿管镜钬激光碎石患者402例,将两组碎石成功率、并发症进行比较研究.结果 结石直径>1 cm,URL组的总体碎石成功率高于ESWL组(85.4%vs 60.7%),并发症率低于ESWL组(6.1%vs 15.4%),差异具有统计学意义(P<0.05);结石直径≤1 cm,URL组的总体碎石成功率与术后发症率与ESWL组相似,差异无统计学意义(P>0.05).结论 对于输尿管上段大于1 cm的结石,URL较ESWL更安全、有效;而对于输尿管上段小于等于1 cm的结石,两种治疗方法疗效相似.一水草酸钙、磷酸钙结石质地较硬,更适宜选择行URL.
Objective To compare the clinical efficacy and safety of wide-focus high energy lithotriptor with narrow-focus low energy lithotriptor treating renal calculi. Methods A prospective study was conducted to compare both modalities for the management of renal calculi.Stone formers were randomly enrolled into two groups. Group A was managed with wide-focus high energy lithotriptor,while group B was managed with the other. Urine samples were collected to detect neutrophil gelatinase-associated lipocalin(NGAL)and α1-microglobulin(α1-MG)levels before and after SWL. Results 60 were randomized to the group A and 60 to group B. There was no significant difference between two groups in stone free rate and complication rate.For stones more than 10 mm,re-treat rate(11.1%vs 39.1%,P=0.021)was lower and complication rate(25.9%vs 4.3%,P=0.038)was higher in group A. There had a larger increase of NGAL in group A(P < 0.001)after SWL. Conclusion SWL with both lithotriptors are effective and safe for renal stones.Wide-focus high energy lithotripsy was associated with lower re-treat rate and higher complication rate for stones more than 10 mm.NGAL may play a potential role in the evalua-tion of SWL induced early renal injury.
Objectives To investigate early postoperative renal function in the shock wave lithotripsy (SWL) and retrograde intrarenal surgery(RIRS).Methods From June 2014 to June 2015,a prospective study was conducted to compare SWL and RIRS in the treatment of renal calculi which were 1.0-1.5cm in diameter.Seventy patients were randomly divided into the observation group (SWL) and the comrol group(RIRS).Voided urine samples were collected to detect NGAL level before and 2,6,12,24,48 hours and 1,2 weeks after SWL and RIRS.Results Mean NGAL levels 6,12,24,48 hours and 1 week after SWL and RIRS were significantly increased,reached peak at 12 hours,began to decline after 24 hours.Mean NGAL levels were significantly increased post-SWL when compared with baselinehe baseline within 1 week [(5.96 ± 1.5) μg/L vs.(4.84 ± 0.9) μg/L,P =0.0025],and returned to baseline within 2 weeks post-SWL [(5.01 ± 0.8) μ g/L vs.(4.84 ± 0.9) μg/L,P =0.346],as well as mean NGAL levels were returned to baseline within 1 weeks post-RIBS [(5.05 ±0.8) μg/L vs.(4.84 ±0.9) μg/L,P =0.445].Mean NGAL levels 6,12,24,48 hours and 1 week after SWL were significantly increased when compared with post-RIRS (P < 0.05).Conclusions SWL and RIRS are both effective on early postoperative renal function.SWL has significantly actute kidney injury when compared with RIRS,and the RIRS time of actute kidney injury recovery is earlier than SWL.
Objective:To investigate the relationship between SLC2A9 gene polymorphism in rs938552 and nephrolithiasis.Method:SLC2A9 gene polymorphism in rs938552 was investigated in 140 patients with renal stone formation (case group) and 140 age-matched healthy volunteers without history of renal stone formation (control group),using polymerase chain reaction and Sanger sequence detection,and the genotype distribution was compared between the two groups.The levels of calcium (Ca),phosphorus (P),uric acid (UA) in the blood and urine were measured in each nephrolithiasis subject,and were compared among different genotypes.Result:For SLC2A9 gene rs938552 polymorphism,the statistical results of its genotype were odds ratio (OR) =3.106,95% confidence interval (CI):1.441-6.696,x2 =9.000,P=0.003.The statistical results of the frequencies of C and T were OR=2.935,95% CI:1.440-5.981,x2=9.500,P=0.002.In the case group,there was significant difference (P<0.05) in serum UA level,urine UA level and urinary PH value level between CC wild type and CT + TT genotype carriers.Conclusion:The rs938552 polymorphism of SLC2A9 gene was associated with the risk of nephrolithiasis in East China,and it may lead to the occurrence of kidney stones through the metabolic disorder of serum uric acid.
Objective By comparing the efficiency and safety of Over - table position extracorporeal shock wave lithotripsy(ESWL), Prone position extracorporeal shock wave lithotripsy for ureteral calculi,to determine which modality is more cost - effective. Method A prospective study was conducted to compare two modalities for the management of 120 patients with lower ureteral calculi which were 6 ~ 15 mm in diameter. Stone formers were random enrolled into three groups by random number table:the OT position ESWL group,Prone position ESWL group,every group had 60 cases. The stone fragments were collected for composition analysis by Fourier transform infrared spectros-copy,and counting patients general condition,stones coposition,stone row rate,stone free rate,complication rate,to compare the efficiency and safety of two modalities. Results No statistically significant difference was found in the patients′ demographics and calculis′size,compo-sition for three groups(P > 0. 05). The stone row rate and stone free rate of postoperative 3 days of the OT ESWL group and the Prone ES-WL group were not statistically significant(P > 0. 05);the stone row rate of postoperative 2 weeks and 1month of the OT ESWL group,the Prone ESWL group were not statistically significant(P > 0. 05),but the stone free rate of postoperative 2 weeks and 1 month of the OT ES-WL group,the Prone ESWL group and the URL group were statistically significant(P < 0. 05). The complication rate of the Prone ESWL group was higher than the OT ESWL group,but the differences were not statistically significant(P > 0. 05). Conclusion OT ESWL as an e-merging therapy was superior than the prone position,the forward stone row rate and stone free rate were close to URL,was cost - effective surgical method.
目的 通过比较OT位体外冲击波碎石术(ESWL)和输尿管镜钬激光碎石术(URL)治疗输尿管下段结石的结石排净率和卫生经济学,研究经济有效的治疗手段.方法 将120例输尿管下段结石患者分为2组,OT位ESWL组与URL组,ESWL组60例患者采用OT位体外冲击波碎石术,URL组60例患者采用输尿管镜钬激光碎石术.结果 术后3d的排石率、排净率,URL组明显高于OT位ESWL组,2组差异有统计学意义(P<0.05);术后2周、3个月的排石率、排净率,URL组高于OT位ESWL组,但2组差异无统计学意义(P>0.05);手术时间、效率商(EQ)URL组明显低于OT位ESWL组,差异有统计学意义(P<0.05);住院时间、住院花费:URL组明显高于OT位ESWL组,2组差异有统计学意义(P<0.05);并发症率URL组低于OT位ESWL组,2组差异无统计学意义(P>0.05).结论 OT位ESWL、URL均是输尿管下段结石高效安全的治疗方法,OT位ESWL结石的排净率低于URL,但URL治疗周期及住院花费高于OT位ESWL.
Objective The aim of this work was to determine possible acute kidney injury with new marker neutrophil gelatinase-associated lipocalin (NGAL) in urine and to show the relationship between changes of this marker and conventional markersα1-microglobulin (α1-MG) in patients undergoing Shocknare lithotripsy (SWL). Methods From Jul. 2012 to Dec. 2013, a prospective study was conducted to compare SWL and fURL for the management of solitary radiopaque renal calculi which were 10-15 mm in diameter. Stone formers were random enrolled into two groups by random number table. Observation group was managed with SWL (Siemens MODUALRIS Variostar lithotriptor), while control group was managed using flexible ureteroscopy lithotripsy (fURL). Voided urine samples were collected to detect NGAL and α1-MG levels before and 2, 6, 12, 24 and 48 hours after SWL and fURL. In addition, further urinary specimens were collected 7days and 2 weeks post-treatment. Results Of 60 patiens, 30 were randomized to the SWL group and 30 to fURL group. All patients had a single stone in the kidney with an average size of 11.5 mm (range 10-15 mm). The gender, age, stone location, stone size, urine NGAL [(3.98 ± 1.3)μg/L vs. (3.94 ± 0.8)μg/L] and α1-MG levels [(17.21 ± 2.4)mg/L vs. (17.96 ± 3.6)mg/L] were comparable in both groups (P>0.05). Mean NGAL levels were significantly increased 2 hours post-SWL when compared with baseline [(5.03±1.7)μg/L vs. (3.98±1.3)μg/L, P=0.008], reached peak at 6 hours, began to decline after 12 hours, close to the baseline within 1 week (P=0.048), and returned to baseline within 2 weeks post-SWL [(4.03±0.9)μg/L vs. (3.98±1.3)μg/L, P=0.851], as well as meanα1-MG levels were slightly increased 12 hours post-SWL [(18.62±1.7)mg/L vs. (17.21±2.4)mg/L, P=0.012], and returned to baseline within 24 hours [(17.96 ± 3.6)mg/L vs. (17.21 ± 2.4)mg/L, P=0.34].There was no significant change in urinary NGAL and α1-MG levels before and after fURL (P>0.05). Conclusion NGAL levels significantly increased 2 hours post-SWL treatment suggesting a potential role for this urinary marker in evaluating SWL-induced acute renal injury.
PURPOSE:Hepatocyte growth factor activator inhibitor type-1 (HAI-1) is an integral-membrane proteinase inhibitor. Some studies have shown that HAI-1 as a matriptase inhibitor that plays a significant role in regulating cancer progression and metastasis. In this study, we attempted to clarify whether the levels of HAI-1 could be a useful marker in patients with prostate cancer (Pca).METHODS:HAI-1 protein was evaluated by immunohistochemistry (IHC) and HAI-1 mRNA was evaluated by reverse transcription-polymerase chain reaction (RT-PCR) in 48 patients with Pca and 20 patients with benign prostate hyperplasia (BPH). The association between HAI-1 and clinicopathological features and survival were analyzed.RESULTS:A high level of HAI-1 protein and mRNA expression was detected in BPH compared to Pca specimens. The HAI-1 expression inversely correlated with Gleason score and pathological stage (p<0.05). It was significantly stronger in N0M0 tumors than in N+ or M+ tumors (p<0.05). Furthermore, low HAI-1 expression was a significant predictor for poor prognosis when compared with high HAI-1 expression (disease-free survival/DFS rate, p=0.0487; overall survival/ OS rate; p=0.0492).CONCLUSION:The results of the present study identified HAI-1 as a favorable prognostic marker for Pca and may indicate that HAI-1 could be a therapeutic target for the treatment of this malignancy.
Objective:To study the feasibility of the standardized treatment of upper urinary tract stone in clinical applications. Method:One thousand and seventy-one patients with upper urinary tract stone were received standardized treatment(experimental group),and 774 cases with conventional treatment(control group).To compared the success rate of lithotripsy,the stone-free rate,complications,hospital costs,hospital stay with the experimental group and control group. Result:The overall success rate of lithotripsy in experimental group was higher than in control group(kidney,Upper ureteral calculi 87.2% vs 75.5%,Middle and lower ureteral stones:98.6% vs 84.8%);and the stone-free rate of the experimental group was higher than in control group(kidney,Upper ureteral calculi:82.0% vs 63.1%,Middle and lower ureteral stones:94.8% vs 83.6%).When the stones were in the kidney or Upper ureteral calculi with diameter ≤ 20 mm,the complications rate of experimental group were significantly reduced than the control group(3.6%vs7.7%,P=0.005).Of middle and lower ureteral stones,the complications rate of experimental group and the control group had no significant difference(P>0.05).The satisfaction of patients in the experimental group was higher than the control group(P<0.05). Conclusion:The standardized treatment of upper urinary tract stones is more safety,effective,and economic than the conventional treatment.
Objective To explore the pathophysiology and the method to prevent recurrence by analyzing the urinary stone composition.Methods Fourier transform infrared spectroscopy(FT-IR) was used to analyze the stone compsition and the relationship with gender and age of the patients and position of stone in 1 415 cases of urinary stone.Results FT-IR evaluation showed that 545 cases(38.5%) were pure stone,in which calcium oxalate was the most common type,and 870(61.5%) were mixed stone,in which the most common type was the mixed stone of calcium oxalate and calcium phosphate.There was no significant difference in stone conposition between males and females(P>0.05).In males aged over 70 years,the proportion of calcium oxalate was lower than that in the other age groups(P<0.01),the proportion of calcium phosphate was higher than the patients aged over 30 to 40 years and over 40 to 50 years(P<0.05),and the proportion of uric acid was higher than that in the other age groups(P<0.01).There was no significant difference in the stone composition in different age group in females(P>0.05).The patients' age of lower urinary tract stone was older than that of upper urinary tract stone(P<0.05),and the proportion of uric acid and calcium phosphate were higher and the proportion of calcium oxalate was lower in lower urinary tract than that in upper urinary tract(P<0.05).Conclusion FT-IR is an efficient method of stone analysis to reflect the stone composition and content.The gender and age of patients and stone position may influence stone composition.
Objective To study urinary stone components in urinary stone patients with different body mass index(BMI).Methods In 850 urinary stone patients,245 were normal weight group(BMI24),327 were overweight group(BMI 24~27),and 278 were obese group(BMI≥28) according to Chinese guideline.Physical analysis of stone components were carried out by the Fourier transform infrared spectroscopy(FT-IR).Results In 850 patients,the FT-IR evaluation showed that 645 cases(75.9%) were calcium oxalate stones,followed by calcium phosphate stones 128 cases(15.1%),uric acid stones 52 cases(6.1%) and struvite 25 cases(2.9%).Uric acid stone formers were more elder than with other components(P0.05).Uric acid was the major components more frequent in men(P0.05),while struvite was more frequent in women(P0.01).The prevalence of calcium oxalate stones in the normal weight,overweight and obese groups were 70.2%,78.0% and 78.4%,respectively(P0.05);the prevalence of uric acid stones in the different groups was 2.9%,7.0%,and 7.9%,respectively(P0.05),and that of struvite was 7.3%,1.2% and 1.1%,respectively(P0.01).There was no significant deference in prevalence of calcium phosphate stone formers with different BMI(P0.05).Conclusions The prevalence of urinary stone disease is found to be associated with BMI.The percentages of uric acid and calcium oxalate stones are higher in overweight and obese group than in normal one.Uric acid and calcium oxalate stone patients are suggested to keep body weight in normal range.
Objective:To study the relative factors of urinary stones between patients of urinary stone and the health people.Methods:The composition of the stone,entire blood biochemistry and 24 h urine of the 300 patients were evaluated,and then compared with the result of the health people.Results:Calcium oxalate stones were found in 232 cases(77.3%),phosphate stones in 50 cases(16.7%).infection stones in 9 cases(3%),uric stones in 9 cases (3%).The urinary stone patients significantly greater blood magnesium,calcium,phosphorus and urine chlorine, calcium,magnesium,phosphorus,uric acid than the healthy people(P<0.05);and significantly less blood Potassium,and urine citrate than the healthy people(P<0.05).Conclusions:Urinary stone recurrence is closely related with more than one kinds of metabolic disturbances.Stone composition analysis and metabolic evaluation are very important in finding the causes and in treating and preventing urinary stones.
目的:对尿酸结石的组成成分进行分析,为进一步探讨其成因提供依据。方法:从2007年7月至2009年6月,对837例尿石症患者的结石标本用傅立叶红外光谱法进行成分分析。探讨其中64例含尿酸成分的结石与性别、年龄及结石位置之间的关系。结果:6例(9.4%)为纯尿酸结石,尿酸混合结石58例(90.6%),多以尿酸与草酸钙混合(59.4%)。其中18例(28.1%)尿酸的比例在50%及以上(即为尿酸结石),46例(71.9%)少于50%(非尿酸结石)。单因素分析显示尿酸结石全部来自于男性(P=0.008),且患者平均年龄显著高于非尿酸结石(54.7岁vs46.6岁,P=0.039)。尿酸结石多来自于下尿路(P=0.007)。结论:红外光谱分析法是一种研究结石成分比较理想的分析方法。性别和年龄对尿酸结石形成有一定的影响,此项研究能够为以后更深入的研究尿酸结石的病因提供线索。
Urolithiasis is a common urologic disease whose prevalence is about 1-20% and increasing throughout the world. The recurrence rate after treatment is more than 50%. Urinary stone analysis is important in determining the possible etiology and the pathophysiology of stone formation. A better understanding of the stone composition may help prevent urinary stone formation. From March 2007 to December 2008, physical analysis of urolithiasis in patients who lived in eastern China for more than 5 years and underwent surgery or shock wave lithotripsy in our hospital or passed their stones spontaneously was carried out using the Fourier transform infrared spectroscopy (FT-IR). Clinical and demographic findings were evaluated and compared with the stone components. Stone analysis was performed in 625 patients. The FT-IR evaluation showed that 234 (37.4%) were pure, and the most frequent was calcium oxalate (33.9%), followed by calcium phosphate (2.7%), and uric acid (0.8%). 391 (62.6%) were mixed stone, calcium oxalate (43.2%) was the most commonly major component, followed by calcium phosphate (16.3%), cystine (1.3%), uric acid (1.1%), and struvite (0.6%). Uric acid (p = 0.029) was the major component found more frequently in men, while struvite (p = 0.037) was more frequent in women. Uric acid (p = 0.031) was more common in lower urinary tract stones, and its formers with the mean age of 55 years were older than those with other components (p = 0.039). In eastern China, the most commonly found pure stone was calcium oxalate, while the most frequent mixed stone was calcium oxalate and calcium phosphate mixture. Stone location, gender, and age may influence stone component.
