Objective:To explore the intervention effect of sensorimotor training on cognitive function, balance and fall risk of elderly patients with schizophrenia.Methods:The 60 hospitalized elderly schizophrenic patients in Wuxi Mental Health Center were randomly divided into experimental group and control group from June to September 2021, with 30 cases in each group. The control group implemented traditional exercise on the basis of routine nursing. The experimental group received 8-week and 24 times sensorimotor training on the basis of the routine nursing. The Chinese version of the Mini Mental State Examination (MMSE) was used to assess the cognitive level of patients, the Modified Morse Fall Scale (MMFS) was used to assess the risk of falls, and the Berg Balance Scale (BBS) was used to assess the balance function.Results:After 8 weeks of intervention, the total score of MMSE was (26.72 ± 1.71), higher than the total score of the control group (22.36 ± 1.93), the factor scores of temporal orientation, spatial orientation, recollection, attention and calculation, memory and language ability in the experimental group were (4.90 ± 0.31), (4.72 ± 0.53), (2.86 ± 0.35), (4.66 ± 0.61), (2.83 ± 0.47), (6.76 ± 1.09), higher than scores of the control group (4.36 ± 0.56), (3.93 ± 0.66), (2.57 ± 0.50), (3.64 ± 1.03), (2.18 ± 0.67), (5.68 ± 1.02), and the difference was statistically significant( t values were 2.53-9.06, all P< 0.05). The total score of MMFS in the experimental group was (5.93 ± 1.49), which was lower than that in the control group (8.75 ± 2.30). The difference was statistically significant ( t=-5.51, P<0.01). The total score of BBS in the experimental group was (38.83 ± 1.71), which was higher than that in the control group (30.89 ± 1.93). The difference was statistically significant ( t=16.43, P<0.01). Conclusions:Sensorimotor training can improve the level of balance, cognitive function and reduce the risk of falls in elderly patients with schizophrenia.
目的 探讨感觉运动训练对老年精神分裂症患者认知水平、自我效能和日常生活能力的干预效果.方法 选择2020年1月—12月在无锡市某精神卫生中心老年精神科住院的患者作为研究对象,将60例患者以住院号尾号分组,双号为试验组(30例),单号为对照组(30例),两组住院期间均予以常规训练和健康教育,试验组在对照组基础上进行感觉运动训练,共4个训练模块,训练10周、共30次.采用中文版简易智能状态检查表评定患者认知水平,采用自我效能量表评定患者自我效能水平,采用护士用住院患者观察量表评定患者病情和行为障碍,采用改良Barthel指数评定患者日常生活活动能力.结果 干预后10周,试验组中文版简易智能状态检查表总分高于对照组,其中定向力、记忆力、注意力和计算力评分明显高于干预前,自我效能水平高于对照组,护士用住院患者观察量表因子分社会能力、社会兴趣、个人整洁高于对照组,而护士用住院患者观察量表因子分激惹、精神病性表现、退缩和抑郁低于对照组,改良Barthel指数评分除大便评分无明显改变,其余评分均高于对照组,两组比较差异均具有统计学意义(P<0.05).结论 感觉运动训练能提高老年精神分裂症患者认知水平、自我效能和日常生活能力.
目的 观察改良容积粘度测试在神经康复科吞咽障碍患者吞咽功能评估中的应用效果.方法 选取神经康复科的80例吞咽障碍患者,按随机数字表分为观察组和对照组各40例.对照组采用进食评估问卷调查工具-10和洼田饮水试验进行吞咽评估,并给予常规饮食护理和指导.观察组在对照组的基础上采用改良容积粘度测试进行评估,并根据测试结果给予饮食训练和指导.结果 观察组吸入性肺炎发生率、留置胃管率均低于对照组(P=0.028;P=0.022),差异有统计学意义.结论 采用改良容积粘度测试能降低吞咽障碍患者吸入性肺炎的发生率,降低留置胃管率.
目的:分析专科护士联合健康教育在精神分裂症伴发糖尿病患者中的应用效果.方法:选择2016年1月—12月我院符合精神分裂症伴糖尿病诊断标准的63例患者,随机分为教育组(31例)和对照组(32例),教育组采用由精神科专科护士和糖尿病专科护士组成联合健康教育小组对患者进行联合健康教育,对照组采用传统方法进行健康教育,采用中文版糖尿病患者知识量表评定患者糖尿病知识知晓率;自拟治疗依从性调查表,比较两组患者治疗依从性.结果:出院时教育组患者糖尿病疾病知识知晓率88.7%高于对照组的62.3%,教育组治疗依从率93.6%显著高于对照组的78.1%,差异均有统计学意义(P<0.05).结论:专科护士联合健康教育能提高精神分裂症伴发糖尿病患者糖尿病知识知晓率,提高治疗依从性.
目的:探讨基于回馈理论以病人为主导的小讲堂式健康教育模式对稳定期精神分裂症病人自我效能和病耻感的影响.方法:选取2020年1月—2020年6月我院精神科收治的稳定期精神分裂症病人80例,按信封法分为对照组和观察组,各40例.对照组行常规健康教育,观察组在常规健康教育基础上行以回馈理论为基础的病人主导式健康宣教小讲堂.干预前后比较两组病人的病耻感和自我效能.结果:干预第8周和第12周,观察组自我效能感得分[(25.25±1.42)分、(28.62±3.42)分]高于对照组[(21.74±3.15)分、(24.52±2.78)分],差异有统计学意义(P<0.05);干预第12周,观察组病耻感得分[(34.62±4.31)分]低于对照组[(42.05±2.46)分],差异有统计学意义(P<0.05).结论:基于回馈理论的健康教育可提高稳定期精神分裂症病人自我效能,降低病耻感.
Objective To understand the learning needs of psychiatric nurses during the training period, and to provide reference and basis for formulating more perfect training programs.Methods Collective focus interviews were conducted with 12 provincial psychiatric nurses. Results In terms of theoretical teaching, two topics were extracted from the feedback, including theoretical course design and content, theory and practice time allocation. Three themes were extracted from the feedback of clinical rotation practice, including the diversity of teaching methods, the sense of approval of multi-point practice, and the updating of knowledge. In terms of training gains and suggestions, two themes were extracted, including the helpfulness of paper writing and increasing opportunities for sharing and communication. Conclusions Psychiatric nurses in this province have a strong willingness to train skills related to nursing scientific re-search, cultivate critical thinking ability, and practice in many places.
[目的]探讨多学科合作延续护理对抑郁症伴糖尿病病人自我护理能力和生存质量的影响. [方法]将58例抑郁症伴糖尿病病人按住院号单双号分组,单号为干预组(29例) ,双号为对照组(29例) .两组病人住院期间均予以常规护理和健康教育,出院时给予健康指导,出院后干预组给予基于团队的延续护理,两组病人均随访6个月.采用自我护理能力测定量表(ESCA)、WHO生存质量测定量表简表(WHOQOL-BREF)评定病人的自我护理能力和生活质量,用全自动生化分析仪测定病人空腹血糖(FPG)、餐后2 h血糖(2 hPBG)控制状况. [结果]出院后6个月末,干预组ESCA总分和4个因子得分均高于对照组(P<0 .05) ,WHOQOL-BREF中生理领域、心理领域、社会关系领域和环境领域评分均高于对照组(P<0 .05) ,FPG和2 hPBG低于对照组(P<0 .01). [结论]多学科合作延续护理可提高抑郁症伴糖尿病病人自我护理能力,改善生存质量.
Objective To explore the effects of self-role recognition nursing on the agitation behavior and social interaction in schizophrenic patients with diabetes mellitus. Methods A total of 98 cases of schizophrenia patients with diabetes mellitus treated in Wuxi Mental Health Center of Jiangsu Province from October 2014 to October 2016 were selected as the research objects. The patients were randomly divided into control group and study group by random number table method. The control group was given routine nursing intervention, while the study group was given self-role recognition nursing intervention for 6 months. The level of fasting blood glucose and 2-hour postprandial blood glucose were compared between the two groups. Patients' agitation behavior was evaluated by Cohen-Mansfield agitation inventory (CMAI); social communication ability was evaluated by social skills checklist (SSC) and social adaptive functioning evaluation (SAFE); mental status was evaluated by positive and negative syndrome scale (PANSS). The data of this study were statistically analyzed with SPSS 19.0 statistical software, in which the measurement data were tested by t-test, and the enumeration data were tested by χ2test. Results Before intervention, there was no significant difference in fasting blood glucose and 2-hour postprandial blood glucose between the two groups (P> 0.05). After intervention, the fasting blood glucose and 2-hour postprandial blood glucose level in the study group were significantly lower than those in the control group, and the differences were statistically significant (P< 0.01). The scores of physically aggressive agitation, physically non-aggressive agitation, verbal agitation and the total score of CMAI in the control group were (16.4±3.7), (31.5±4.9), (25.2±4.5), (73.1±4.5) respectively after intervention, and the scores were (11.8±3.2), (24.1±5.3), (17.5±4.2), (53.4±4.7) in the study group. The differences between the two groups after intervention were statistically significant (t=7.268, 8.617, 8.561, 15.341;P<0.01). The results of SSC scale and SAFE scale in both groups decreased than before intervention, and the scores of the study group were all lower than those of the control group, with statistical significance (P<0.01). After intervention, the scores of positive symptoms, negative symptoms, negative symptoms and the total score of PANSS scale of the study group were significantly lower than those of the control group, and the differences were statistically significant (P<0.01). Conclusions Self-role recognition nursing intervention can be used to improve the patients' social interaction ability and mental status in the nursing process of schizophrenia patients with diabetes mellitus, and it is worthy of clinical promotion.
Objective To understand the psychological experience and coping styles of psychiatric nurses suffered from workplace violence.Methods The phenomenological method was used to conduct a semistructured interview in 15 clinical nurses in a tertiary psychiatric hospital from April to May 2016.Results The psychological feelings of psychiatric nurses suffered from workplace violence included anger and confusion,helpless and hopeless,anxiety and fear,self-blame and guilt.The coping styles included positive response,negative response,low self-worth and turnover intention,urgent needs for supporting system.Conclusion The psychiatric nurses suffered from workplace violence present complex and intense psychological responses,which seriously affect their job security and career development.The hospital managers should not only pay a close attention to the nurses' mental health,but also actively create a safe working environment to improve the nurses' professional sense of belonging and happiness.
Objective To observe the effect of medication compliance and self-efficacy of schizophrenia patients with diabetes by continuing self-management education. Methods A total of 60 patients with schizophrenia with diabetes diagnostic criteria were divided into experimental group (30 cases) and control group (30 cases) by the admission number, 2 groups received conventional drug treatment, the control group received general preaching health education pattern, were each issued a health education manual, with schizophrenia and diabetes health knowledge division, the test group on the basis of the continuation of the implementation of self-management education, followed up 12 months after discharge, compared 2 groups of patients with medication compliance and recurrence rate, with General Perceived Self-Efficacy Scale (GSES) assessment level of 2 groups of patients with self-efficacy, fasting blood glucose (FPG) was determined by automatic biochemical analyzer, 2 hours postprandial blood glucose (2 hPBG). Results Six months after discharge, 82.1%(23/28) cases were complete compliance, 14.3%(4/28) cases of partial compliance, 3.6%(1/28) cases of non-compliance, 55.2%(16/29)cases of the control group complete compliance, 27.6%(8/29) cases of partial compliance, 17.2%(5/29) cases of non-compliance, the differences between two groups were statistically significant (χ2=37.41, P<0.05). The 12 month test group of full compliance discharged 75.0%(21/28) cases, 14.3%(4/28) cases of partial compliance, 10.7%(3/28) cases of non-compliance, 31.0%(9/29) cases of the control group complete compliance, 31.0%(9/29) cases of partial compliance, 37.9%(11/29) cases of non-compliance, the test group compliance was higher than the control group (χ2=11.27, P<0.05). The score after 6 months and 12 months respectively for the test group general self-efficacy scored 35.32 ± 2.02, 34.18 ± 1.77, control group scored 24.03 ± 3.04, 22.48 ± 2.01, the differences between two groups were statistically significant (t=16.445, 23.304, P<0.01). After 6 months in test group FPG and 2 hPBG were (5.71 ± 1.05), (9.46 ± 1.37) mmol/L, control group were (7.79 ± 1.05),(12.28 ± 2.07) mmol/L, the differences between two groups were statistically significant (t=7.483, 6.023, P<0.01). 12 months after discharge in test group FPG and 2 hPBG respectively (5.89±1.13), (9.68±1.09) mmol/L, control group (9.14±1.19), (13.17±1.42) mmol/L, test group FPG and 2 hPBG were lower than the control group (t=10.550, 7.431, P<0.01). After 6 months and 12 months, the recurrence rate of experimental group were 7.1% (2/28) and 24.1% (7/29) respectively, and control group were 14.3%(4/28) and 44.8%(13/29), the recurrence rate of experimental group was lower than the control group, there was significant difference between 2 groups (χ2=4.326, 6.225, P< 0.05 or 0.01). Conclusions Self-management education can improve the medication compliance and self-efficacy, improve mental symptoms and sugar metabolism, and reduce the recurrence rate.
Objective:To probe into the application effect of self management education in recovery of diabetic pa‐tients with depression .Methods:A total of 71 diabetic patients with depression were selected and were divide in‐to the patients in intervention group and control group .The patients in control group received the routine health education ,while the intervention group received the self management education .Then determined and reviewed the patients by the disease know ledge master status questionnaire ,depression self rating scale (SDS) and Hamil‐ton Depression Scale(HAMD) and quality of life for the determination of the amount and short form (WHO QOL BREF) respectively on admission and 6 months and 12 months after discharged ,and measured the pa‐tients’fasting blood glucose and postprandial 2 h blood glucose by the automatic biochemistry analyzer .Results:6 months and 12 months after discharge ,the patients’knowledge mastery score in the intervention group were higher than that in the control group ,the SDS score and HAMD score were lower than those of the control group ,the fasting blood glucose and postprandial 2 h blood glucose were lower than those of the control group . 12 months after discharge ,the patients’quality of life in the intervention group was higher than that in the con‐trol group ,the difference was all statistically significant (P<0 .05) .Conclusion:The continuing self manage‐ment education can help patients master the health knowledge ,improve glucose metabolism and depression ,and improve the quality of life .
Objective:To explore the effect of "Handover Sheet of Checking and Treatment outside"in reducing adverse events in psychiatric patients.Methods:174 patients were splitted into control group and intervention group by their admitting time.January-June 2013 was control group(83 cases) ,July-December 2013 the intervention group(91 cases).For control group,the verbal handover was used.For intervention group,on the base of oral handover, the "Handover Sheet of Checking and Treatment out-side"was used,in which escorting by someone was written.The rate of adverse event was compared be-tween the two groups.Results:There were six adverse cases(6.6%) in the intervention group,17 cases (20.5%)in the control group.There was a significant difference between the two groups( P<0.01). Conclusion:"Handover Sheet of Checking and Treatment outside"can effectively reduce the occurrence of adverse events.
Objective To investigate the effect of music therapy on anxiety and depression in patients with spinal cord injury. Methods Forty anxious and depressive patients with spinal cord injury were selected as the control group from December 2012 to October 2013, and another 40 anxious and depressive patients with spinal cord injury as the observation group from November 2013 to September 2014. Patients in the observation group were given the following metheds after admiting for 3 days besides psychological nursing; music therapy once a day, 1 hours one time for 4 weeks. The two groups were compared in terms of the scores by Hamilton anxiety scale (HAMA) scores and Hamilton depression scale (HAMD) before and after treatment. Results There were insignificant differences in HAMA scores and HAMD scores between the two groups before music therapy (P>0.05), but the scores by HAMA and HAMD in the observation group were both significantly lower than those in the control group. Conclusion Music therapy can alleviate anxiety and depression of patients with spinal cord injury, and promote the functional recovery.
Objective To explore the effect of extended care on the social function of schizophrenia patients in community rehabilitation.Methods Eighty four cases of schizophrenia were randomly divided into intervention group (Beitang District,40 cases) and control group( Chong’an District, 41cases).All patients received regular drug treatment and health education, and got guildance when discharged.Patients in the intervention group were recived follow-up intervention and telephone interview for 12 months, and bulided the extended care data.Social Disability Screening Schedule ( SDSS ) was used to evalue the social function of patients, and relapse rate and readmission rate were compared between the two groups.Results When discharged 6 months and 12 months, medication adherence and medication management form of the patients in the intervention group were better than that in the control group, the relapse rates in the intervention group were 12.5%, 27.5%, and readmission rates were 7.5%, 17.5%,which lower than 34.1%, 58.5%,26.8%, 48.8%in control group, and the difference was significant (P<0.05).Total scores and partial factors scores of SDSS were lower in the intervention group when dischared 12 months (P<0.05).Conclusions Extended nursing intervention could improve the social function of patients, and reduce relapse rate and readmission rate.
目的 探讨护理延伸服务在酒精所致精神障碍患者中的临床应用效果.方法 80例患者按所居住的社区街道入组,北塘区为干预组(41例),崇安区为对照组(39例).两组患者住院期间均予以常规护理和健康教育,出院时予健康指导.干预组于出院转入社区后,予持续进行护理延伸服务,并建立延伸服务档案.两组患者随访12个月,采用社会支持评定量表(SSRS)评定社会支持程度和戒酒情况,并计算复饮率.结果 随访12个月末时,干预组患者主观支持、客观支持和对支持的利用度得分显著高于对照组(P<0.01);戒酒效果好于对照组(x2=10.722),复饮率低于对照组(x2=8.816),两组差异具有统计学意义(P<0.01).结论 护理延伸服务能提高患者社会支持水平,提高戒酒效果,降低复饮率.
Objective To explore the effect of the extended nursing services model on coping style and life quality of patients with depression .Methods Sixty patients were admitted by the community street living into the group, the patients in Beitang district were assigned to the observation group (n=30), and the patients in Nanchang district were assigned to the control group ( n=30 ) .The patients received health education and rehabilitation during hospitalization , health guidance in the discharge , and were followed up for 12 months.The patients in the observation group were transferred to the research system on the integration of community -based rehabilitation , were given the extended nursing services , and were established the service extension file .The coping style and life quality of patients were assessed by the simplified coping style assessment scale ( SCSQ ) and WHO quality of life BREF ( WHOQOL-BREF ) , and the relapse rate was analyzed between two groups . Results No significant differences were found in the scores of SCSQ and the life quality before intervention (P>0.05).The scores of positive and negative response factors were respectively ( 1.79 ±0.37 ), (1.38 ±0.35) in the observation group 12 months after follow-up, and were respectively (1.25 ±0.42), (1.69 ±0.38) in the control group, and the differences were statistically significant ( t =3.53, 3.16, respectively;P <0.01).The scores of physical, psychological, social relations and environmental fields in WHOQOL-BREF scale were respectively (67.58 ±5.76), (75.57 ±5.32), (57.48 ±6.06), (62.26 ± 6.23) in the observation group12 months after follow-up, and were significantly higher than (64.52 ±7.51), (68.61 ±6.43), (53.59 ±6.12), (59.35 ±7.27) in the control group, and the differences were statistically significant (t=2.88, 3.67, 2.78, 2.36, respectively;P<0.01).The relapse rate was 10.7%(3 cases) in the observation group 12 months after follow-up, and was 43.3% (13 cases) in the control group, and the difference was statistically significant (χ2 =7.714, P<0.01).Conclusions The extended nursing services model is helpful to improve the patients ’ coping style , reduce the relapse rate and increase the quality of life .
Objective To discuss the effect of “six preventions” risk assessment form in psychiatry to reduce the accidents occurrence among inpatients .Methods By the time of admission , 233 inpatients were divided into control and intervention group ,from May 2011 to December 2012 as the control group (116 cases), from March 2012 to December 2012 as intervention group ( 117 cases ) .After the patients were admitted to hospital in the intervention group , psychiatric nursesusedself-designed “six preventions” risk assessment form ( impulsion prevention ,running-outprevention , hiding medicine prevention , self-abandonprevention , falling down prevention , and choking prevention ) to assess the risk , divided into high , medium and low risk level according to the rating results , and then implemented tiered nursing intervention , while the control group received routine treatment and nursing .Results Comparing the number and rate of accidents occurrence in the two groups ,the intervention group had three cases with a rate of 2.56%, and the control group had 24 cases with a rate of 20.69%.The difference was statistically significant (χ2 =18.67, P<0.01).For the intervention group, the comparison of the rate of accidents between different types and durations showed no statistical significance ( P>0.05).For the control group, the comparison of the rate of accidents between different types and durations showed a statistical significance (χ2 =14.63,7.79,respectively;P<0.05).Schizophrenia and duration less than 1 year contributed the higher rate of accidents occurrence with a statistical significance (χ2 =23.62, 23.45, respectively;P<0.01).Conclusions Adopting psychiatric risk assessment form of “six prevention” to stratify inpatients can effectively reduce the rate of accidents occurrence .
Objective:To probe into influence of cognitive behavioral intervention on coping style and quality of life of patients with depression.Methods:A total of 60depression patients were randomly divided into experimental group(30cases)and control group(30cases),based on the drug treatment,the patients in experimental group involved in the cognitive behavioral intervention for 8weeks,the patients of control group received traditional care.Zung self-rating depression scale(SDS),the Hamilton depression Scale(HAMD)and simplified coping style questionnaire(SCSQ)were used to assess depression and coping style of patients,BREF(WHOQOL-BREF)was used to assess the quality of life satisfaction degree.Results:SDS,HAMD scores of the experimental group were significantly lower than that of control group(P<0.01),positive coping score was significantly higher than that of control group,negative coping score was lower;in addition to the environment,WHOQOL-BREF score in the three areas of psychological field,physiological and social relations were significantly higher than that of control group(P<0.05or P<0.01).Conclusion:Cognitive behavioral intervention can improve patients' coping styles and depression state,and improve their quality of life.
Objective To probe into the clinical application effect of the transitional care model on alcohol-induced mental disorders.Methods One hundred and two patients with alcohol-induced mental disorders were divided into the intervention group (n =50) and the control group (n =52) patients in group according to block randomization method.The control group received routine care interventions; the patients in the intervention group discharged to home in open wards,received transtional care model intervention and established services file on the basis of routine care.The patients were followed up for 6 months,and were surveyed by medical coping modes questionnaire (MCMQ),social dysfunction screening scale (SDSS) and social support rating scale (SSRS).The complex drink rate and satisfaction were analyzed in patients.Results The differences in the social support,social functioning,coping style and complex drink rate were not statistically significant between two groups in the discharge.The scores of the social support,social functioning,coping style and complex drink rate were respectively (12.68 ± 3.45),(24.68 ± 5.02),(9.88 ± 2.41) in the intervention group six months after discharge,and were better than (10.98 ± 3.47),(20.77 ± 5.90),(6.78 ±2.13) in the control group,and the differences were statistically significant (t =5.63,5.55,14.07,respectively; P <0.01).The scores of face,avoid and yielding were respectively (19.86 ±3.68),(10.84 ±1.89),(8.58 ± 2.46) in the intervention group six months after discharge,and were better than (15.84 ±3.65),(16.54 ± 2.88),(13.78 ± 2.47) in the control group,and the differences were statistically significant (t =7.89,9.12,15.75,respectively; P<0.05).The score of SDSS was (8.21 ±1.91) in the intervention group six months after discharge,and was better than (10.84 ± 2.14) in the control group,and the difference was statistically significant (t =3.87,P < 0.05).The total abstinence rate,the limited drinking rate,the satisfaction of patients were respectively 52.00%,18.00%,98.00% in the intervention group six months after discharge,and were better than 19.23%,15.38%,84.61% in the control group,and the differences were statistically significant (x2 =14.504,5.676,respectively; P <0.05).Conclusions Transtional care model intervention can improve the patients' social support,social function,coping styles and abstinence rate,and improve the satisfaction of patients and their families.
