目的 探讨心肌酶谱与酒依赖患者成瘾严重程度的相关性.方法 采用全自动生物化学分析仪对95例酒依赖患者(观察组)心肌酶谱,包括谷草转氨酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、羟丁酸脱氢酶(HBDH)的活性进行检测,并与同期46例体检健康者(对照组)进行比较.采用密西根酒精依赖调查表(MAST)评分对酒依赖患者的严重程度进行评估,采用Spearman相关分析对心肌酶谱评分与酒依赖患者成瘾严重程度的相关性进行分析.结果 观察组患者血清AST、CK、CK-MB、HBDH水平显著高于对照组(P<0.01),而2组患者血清LDH水平比较差异无统计学意义(P>0.05).MAST评分轻度15例,中度24例,较重35例,重度21例;MAST评分越高,血清AST、CK、CK-MB和HBDH水平也越高(P<0.01);Spearman相关分析结果显示,观察组患者血清AST、CK、CK-MB、HBDH水平与MAST评分呈正相关(r=0.746、0.813、0.523、0.701,P<0.01).结论 心肌酶谱与酒依赖患者的MAST评分密切相关,并且其水平与病情程度呈正相关.
Objective To survey the changes of gonadal hormone levels of male alcohol dependence (AD) patients to provide basis for clinical diagnosis and treatment. Methods Fifty-eight male AD patients were assigned to dependence group and 58 normal healthy people to control group,serum pituitary prolactin (PRL),testosterone (T) ,progesterone (PROG),follicle stimulating hormone (FSH),luteinizing hormone (LH) and estradiol (E2) levels detected using electrochemiluminescence,and detection results of 2 groups contrastively analyzed. Results Serum PRL,FSH,LH and E2 levels were significantly higher (P<0.05 or 0.01) and RROG lower (P<0.01) in dependency than in control group,group difference in T wasn't significant (P>0.05). Serum PRL,FSH,LH and E2 levels were positively related to ages in dependence group,but which had no statistical significance (P>0.05);PROG level was negatively related to ages,but which had no statistical significance (P>0.05);T level was positively related to ages,which had statistical significance (P<0.05). Conclusion Gonad hormone levels of male AD patients are abnormal,gonad axis functions imbalance,and the detection of gonadal hormone and treatment should be clinically enhanced.
Objective To explore the effects of specimens with different temperatures on monocytes detec‐ted by LH750 hematology analyzer .Methods Three specimens were collected from each patients of 20 ones and respectively kept at constant 5 ℃ ,15 ℃ ,25 ℃ for 30 minutes ,monocyte percentages of EDTA‐K2 anticoagulation whole blood specimens detected using LH 750 hematology analyzer ,and at the same time blood smears made and detected by 3 skilled workers of cell morphology .Results The lower the spec‐imen temperature ,the higher the percentage of monocytes detected by LH750 ,temperature was negatively correlated with detection results . The percentage of monocytes detected by LH750 at 5 ℃ and 15 ℃LH750 was significantly higher compared with manual microscope detection results (P<0 .01) ,that at 2 5 ℃ had no significant difference from manual microscope detection results (P>0 .05) .Conclusion Speci‐mens with different temperatures a significant impact on the percentage of monocytes detected by LH 750 , the percentage of monocytes detected by LH750 at 25 ℃ is more accurate .
Objective:To investigate patients with first -episode schizophrenia to contrast between sensory gating P 50, P300 and Wis-consin Card Sorting test .Methods:Paired auditory conditions , testing stimulus paradigm and Wisconsin Card Sorting Test in 60 first-epi-sodeschizophrenic patients and 60 healthy people were detected , analyzed and compared significant .Results:In the patient group auditory sensory gating P50 inhibition, higher than that of the control group (P<0.01), P300 was significantly lower than that in the control group (P<0.01);Wisconsin Card Sorting test results show , in patients withcomplete classification number and the number of correct clas-sifications were significantly lower than those of controlgroup , number of errors and perseverative errorssignificantly higher than that of the control group (P<0.01);patients with auditory sensory gating P50 inhibition,P300 and Wisconsin Card Sorting Test Index withoutcon-trast.Conclusion:Patients with first-episode schizophrenia cognitive function impairment and frontal dysfunction , reflect may exist differ-ent neural mechanism .
锂盐(Li+)是精神科常用的抗躁狂药物,但因其作用的特殊性,临床使用易引起中毒,严重的可能导致死亡.因而血Li+浓度检测是临床精神科专业实验室常设或必须要监测的药物之一.同时Li+也和其他项目一起,常作为急诊检测项目用于危重患者的诊疗.在临床检验实际工作中,采用血清样本测定时,由于受到血液凝固时间的限制,血清的分离需要较长时间,有不少患者,特别是急诊患者,尤其在冬季的预温处理更为不便.
我国是乙型肝炎病毒(HBV)感染率较高的国家,大部分感染者为无临床症状的HBV携带者,而HBV与肝硬化、原发型肝癌有密切关系.加之精神病医院的特殊管理模式,精神病患者由于受精神症状支配,因意外发生皮肤破损的情况较常见,因而患者易发生HBV感染,严重威胁患者的健康.为了解住院精神病患者HBV感染状况,作者对在我院住院精神病患者HBV五项指标进行了检测分析,现将结果报告如下.
Objective To investigate the production and distribution of extended spectrum β-lactamase(ESBLs)in Enterobactericaeae,and to provide references for the clinical application of drugs.MethodsESBLs were detected by disc diffusion confirmatory test.Results743 strains of Enterobacteriaceae were indentified and among them 341 ESBLs strains were isolated,176 ESBLs strains were isolated from sputum specimens;13 kinds of Enterobacteriaceae produced ESBLs strains. The constituent ratio of E.coli,Klebsiella spp and E.cloacae in ESBLs positive strains were 42.8%、41.6% and 6.5% respectively,the ESBLs isolating ratea of which were 48.0%、56.3% and 37.9% respectively.ConclusionThe detection rates of ESBLs in enterobacteriaceae are high,and the categories of producing ESBLs in Enterobacteriaceae are many. It is necessary for clinical laboratory to carry out routine detection on ESBLs in clinical isolates of Enterobacteriaceae.
[Objective] To compare the differences of biochemical metabolism between male and female nonalcoholic fatty liver patients and explore preventive countermeasures.[Methods] Detection was made on the levels of TP,ALB,GLO,AST,ALT,TBIL,DBIL,IBIL,GLU,T CHO,TG,HDL C,LDL C,apoA1 and apoB 100 among 973 people who undertook physical examination; calculation was made on non HDL C and the ratio of apoB 100/apoA1,LDL C/HDL C, apoB 100/HDL C and TC/HDLC;comparison was made on the result between 59 nonalcoholic fatty liver patients and 202 healthy people in the control.[Results] The total morbidity of fatty liver was 9.05%,the rate of the male patients was 12.53%,which was significantly higher than that of the female ones(6.0%)(P0.05);significant difference was found among the 59 male patients in the levels of ALT,IBIL,TG,HDL C,LDL C,apoA1,non HDL C and the ratio of apoB 100/apoA1,LDL C/HDL C,apoB 100/HDL C and TC/HDLC(P0.05,P0.01);significant difference was also found among the 30 female patients in the levels of AST,TBIL,DBIL,IBIL,GLU,T CHO,TG,LDL C,apoA 1,non HDL C,and the ratios of apoB100/apoA1,LDL C/HD C,apoB 100/HDLC and TC/HDLC(P0.05, P0.01);non HDL C was obviously correlated with TC,LDLC,TG,HDLC,apoA1, apoB100,apoB100/apoA1 and TC/FIDLC(r=0.4113~0.9893,P0.01).[Conclusion] Metabolism disorder and liver damage are the common features of nonalcoholic fatty liver and there are significant differences between males and females;effective measure should be taken for the prevention and interference.
[Objective]To test the change of CHE of the hobbledehoy schizophrenics treated by clozapine,and find out the its function mechanism and its influence on liver.[Methods]70 hobbledehoys who suffered from schizophrenia for the 1st time and had never been treated were sellected. The CHE of them were measured before and after 2,3,4,5,6 and 7 weeks of treatment,and the curative effect and side reaction were evaluated using BPRS and TESS after 2,4,6,8 weeks of treatment.[Results]Their was significant difference between the evaluation results of BPRS and TESS before and after treatment(P﹤0.05) and the results among the ones treated for 2 weaks or not(P﹤0.05). CHE was a little higher than that before treatment,but there was no statistical difference(P﹥0.05).[Conclusion]It is certain that clozapine has an effect on hobbledehoy schizophrenics,but it may enhance the ability of lipoprotein synthesis by liver,which results in the disorder of fat metabolism.
Objective To test several kinds of biochemical indexes in hobbledehoy schizophrenics who were treated by clozapine dynamically and to explore the damage and the adaptability of the liver function according the change of these indexes.Methods A total of 70 untreated hobbledehoys with first-episode schizophrenia were enrolled this study.The enzyme levels were measured before and 2~7 weeks after treatment which including ALT、AST、ALP、LDH、γ-GT、CHE、TBA、T-Bil、D-Bil、I-Bil、TP、ALB、GLO、P-ALB.The curative effect and side reaction were evaluated with BPRS and TESS.Results The scores of BPRS and TESS had significant differences between before and after treatment(P0.05).There had significant differences between the first 2 weeks’ treatment and the other weeks(P0.05).The trend pattern of ALT、AST、ALP and γ-GT changed obviously,they rose up to the peak after two weeks’ treatment,dropped to the same as before treatment,even a little lower in the third week,rose again in the forth week,dropped gradually after the fifth week,and kept the level a little higher than before treatment.LDH began rise after the second week,reached the peak in the forth week,and then dropped gradually,and kept the level a little higher than before treatment after the sixth week.The levels of T-Bil and I-Bil at 2-6 week treatment were significantly lower than before treatment (P0.05~P0.01) ,and dropped to the same as before treatment after 7 weeks(P0.05).The levels of D-Bil after 2 and 4 weeks treatment were significantly lower than that before treatment(P0.05).The levels of P-ALB at 3nd,4th,6th,7th and 8th week were significantly higher than before treatment(P0.05).The levels of CHE、TBA、TP、ALB、GLO had no significant change after treatment(P0.05).The level of ALT at third week and other weeks has significant differences(P0.01).The other items had no significant change after treatment.Conclusion Clozapine is effective for hobbledehoy schizophrenics,the liver damage has regularity and is light.The damage can be adapt gradually with treatment.
Objective To test six familiar enzymes of hobbledehoy schizoprenics who were treated by clozapine dynamlyical,according the change of these indexes to find out the damage and the adaptability of the liver function. Methods Choose 70 hobbledehoys who were the first time suffer schizophrenia and had never been treated,measure their enzyme levels before and after 2、3、4、5、6 and 7 weeks of treated,the enzyme including ALT、AST、ALP、LDH、γ-GT、CHE、TBA、T-Bil、D-Bil、I-Bil、TP、ALB、GLO、P-ALB,and then measured the curative effect and side reaction using BPRS and TESS after 2,4,6,8weeks of treated. Results The BPRS and TESS of sufferer had significant difference between before treatment and after treatment(P<0.05).It had significant difference between the first 2 weeks treatment and the other weeks(P<0.05).ALT and γ-GT risen up significantly during the second week(P<0.05~P<0.01).The significant difference dropped to the same as before treatment in the third week,during the fourth week,it rose up primarily,and had higher significant difference than before treatment(P<0.01),and then drop gradually to the same as before treatment afterwards(P>0.05).during second week to fifth week,the significant difference of LDH is higher than before treatment(P<0.01),it drop gradually to the same as before treatment after the sixth week(P>0.05).Although the AST and ALP had the trends of heighten first and then drop to a little higher than before treatment,it had no significant difference(P>0.05).The CHE always had no significant difference compared to it of before treatment(P>0.05). Compare the third week to the other weeks,ALT has significant difference(P<0.01),but the other items has no significant difference(P>0.05). Conclusion It is certain that clozapine has an effect on hobbledehoy schizophrenics,The liver damage it caused had regularity,and was light,God's image can be adapt to the situation gradually with treat deeply.
目的:探讨女性精神分裂症患者氯氮平治疗前后免疫球蛋白及补体水平变化.方法:对55例女性精神分裂症患者和52例女性健康者血清IgG、IgA、IgM及C3、C4水平进行检测.结果:女性精神分裂症患者治疗前IgG、IgM、C3水平显著高于对照组,经氯氮平治疗3、7、15、30天后血清IgG、IgM、C3均有不同程度下降,15天时IgG已恢复到正常水平,1个月后C3也恢复到正常水平;与治疗前比较,IgG、C3水平显著降低.IgM虽有下降趋势,但仍高于正常对照,且治疗前后差异无显著性.IgA、C4治疗前后比较及与对照组比较差异均无显著性.结论:氯氮平在改善患者精神症状的同时,也使其免疫功能得到改善.
