Objective:To explore the correlation between early pupillary light reflex reduction and delirium in mechanically ventilated patients receiving sedation in the Intensive Care Unit (ICU) .Methods:From December 2020 to March 2021, convenience sampling was used to select 97 ICU patients with mechanical ventilation who received sedation in the China-Japan Union Hospital of Jilin University as the research subject. On the second to fourth days of the patient's stay in the ICU, the quantitative pupillary light reflex (q-PLR) and pupillary contraction velocity (CV) of the patients after photostimulation were measured by an automatic infrared pupillometer with a frequency of two times a day. Patients were assessed using the Richmond Agitation-Sedation Scale (RASS) . When the RASS score was≥-2, the patient was screened for delirium using the Confusion Assessment Method for Intensive Care Unit (CAM-ICU) with a frequency of two times a day. According to the screening results, patients were divided into delirium group and non-delirium group.Results:The incidence of delirium in 97 ICU mechanically ventilated patients who received sedation was 58.76% (57/97) . Binomial Logistic regression showed that q-PLR on the second day was an influencing factor for delirium in mechanically ventilated ICU patients receiving sedation ( P<0.05) . Conclusions:Early pupillary light reflex reduction is associated with delirium in mechanically ventilated ICU patients receiving sedation.

Objective:To explore the effect of pulmonary rehabilitation training based on Chronic Obstructive Pulmonary Disease (GOLD) classification on self-efficacy and rehabilitation effect in patients with acute exacerbation of chronic obstructive pulmonary disease.Methods:From May 2019 to April 2021, a total of 85 patients with acute exacerbation of chronic obstructive pulmonary disease who were admitted to China-Japan Union Hospital of Jilin University were selected and divided into the observation group (43 cases) and the control group (42 cases) using the random number table method. The control group received routine pulmonary rehabilitation training, and the observation group conducted pulmonary rehabilitation training based on GOLD classification. Two weeks after intervention, the self-efficacy, pulmonary function, 6-Minute Walking Test (6MWT), quality of life, and the condition of adverse reactions were compared between the two groups.Results:Two weeks after intervention, the scores of dyspnea management, emotion, physical activity, safety behavior, and total score of self-efficacy of the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.05). The forced exhalation volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC and 6MWT of the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.05). The scores of respiratory symptoms, activity limitation, disease impact on life and total score of St. George's Respiratory Questionnaire (SGRQ) of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05). There were no serious adverse reactions in two groups. Conclusions:Pulmonary rehabilitation training based on GOLD classification can help patients' promote the recovery of lung function, improve their self-efficacy and the quality of life, which has good security.

The Cisplatin and its analogues are scarce for the second leading human health problem, cancer. Therefore, we have effectively engineered the PEGlyated carbon nanodots loaded with cisplatin PEG-CDs@Cis-Pt(IV) for targeted lung cancer therapy. The PEG-CDs@Cis-Pt(IV) confirmed by the electroscopic and spectroscopic methods. The in vitro cytotoxicity of the nanoparticles evaluated two lung cancers (A549 and HEL-299), and one non-cancerous cell line (HUVECs) were used in this study by using MTT assay. The nanoparticles are effectively killing cancer cells without affecting the nano-cancerous cells. Further, dual AO-EB fluorescent staining assay established the programmed cell death through cell morphological changes. Further, flow cytometry confirmed the induction of apoptosis by S phase arrest of the cancer cell cycle by the complexes. The results of our investigations also bear witness to CDs@Cis-Pt(IV)+NIR-NPs promising scope and potency care management for precise cancer therapy beyond platinum drugs.
目的:研究与探讨喘息性支气管肺炎患者临床护理措施及其效果.方法:选取2017年5月~2019年1月期间在本院接受治疗的喘息性支气管肺炎患者50例作为研究对象,随机分为对照组和观察组各25例,在药物治疗后对照组给予常规护理措施,而观察组给予综合性护理措施,比较两组患者的临床疗效.结果:本次研究,对照组患者恢复正常体温时间为4.15±0.7天,观察组为2.13±0.5天,对照组明显高于观察组,差异具有统计学意义;对照组患者总有效率为80.00%,观察组总有效率为96.00%,观察组总有效率显著优于对照组(P<0.05),二组比较具有统计学意义.结论:综合性护理措施能够有效改善喘息性支气管肺炎患者临床症状,炎症明显缓解,且恢复正常体温快,值得临床研究和借鉴.
目的 探讨PCI术后3个月发生支气管扩张大咯血,采用支气管动脉栓塞止血的护理.方法 术前护理的关键是对患者进行正确评估,做好大咯血的处理、急救准备及术前准备,术后护理的重点是病情观察及饮食护理,暂停抗凝,应用抗血小板策略;咯血停止后恢复双抗治疗.结果 本病例救治成功,术后无不良事件发生,患者住院15 d,痊愈出院.结论 支气管动脉栓塞止血的护理方案安全、有效.
Objective To investigate the effects of salvia miltiorrhiza and ligustrazine combined with antibiotics on the in-flammatory factors in older patients with severe pneumonia. Methods 162 older patients with severe pneumonia between January 2015 and December 2018 were enrolled. The patients were randomly divided into the observation group and the control group. The ob-servation group was treated with salvia miltiorrhiza and ligustrazine combined with antibiotics, while the control group was treated with antibiotics alone. The levels of arterial partial pressure of oxygen (PaO2), c-reactive protein ( CRP), interleukin-6 ( IL-6), interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) in blood were measured. Results After treatment, the level of PaO2 in the observation group was higher than that in the control group (P<0.05). The levels of CRP, IL-6, IL-8 and TNF-α were lower than those in the control group ( P<0.05). Conclusion Salvia miltiorrhiza and ligustrazine combined with antibiotics can improve the oxygenation, promote the absorption of inflammation, reduce the release of inflammatory mediators, shorten the course of disease in older patient with severe pneumonia.
Developing noble metal-free hydrogen evolution catalysts with excellent performance is of great significance but still remains a considerable challenge for electrocatalytic water splitting. This paper reports the synthesis of three-dimensionally ordered macroporous iron phosphide self-supported structure on carbon cloth (3DOM-FeP/CC) through an electrostatic self-assembly method along with heat-treatment and phosphidation. The resultant 3DOM-FeP/CC catalyst combined the advantages of the interconnected ordered macropores, the self-supported structure and the highly-conductive carbon substrate, which endowed the catalyst with substantial accessible active sites, abundant mass transport channels for electrolyte/products, and convenient electron transfer pathways. As a result, the 3DOM-FeP/CC catalyst exhibited outstanding catalytic activity for the hydrogen evolution reaction in acid electrolyte, providing a current density of 10 mA cm(-2) at an overpotential of 68 mV with a small Tafel slope of 42 mV dec(-1). 目的:研究与探讨综合排痰护理在老年支气管哮喘急性发作期的临床效果.方法:选择2017年10月-2019年1月期间我院收治的30例老年支气管哮喘急性发作患者作为研究对象,将其随机分为两组,分别为对照组和观察组,均为15例,对照组患者给予常规护理,观察组在其基础上给予综合排痰护理,对比两组患者的护理效果及肺功能变化情况.结果:观察组患者总有效率为93.3%,对照组总有效率为66.7%,比较差异显著(P<0.05).且经过对比发现观察组患者肺功能明显优于对照组患者(P<0.05).结论:给予老年支气管哮喘(急性发作期)患者以综合排痰护理方式护理,临床护理效果显著,可充分提升治疗有效率,值得临床广泛应用.
目的:研究与探讨慢性支气管炎应用健康教育的护理效果.方法:选取2016年8月至2018年10月期间本院收治的30例慢性气支管炎患者作为研究对象,采用随机分组方式,将其分为研究组与对照组各15例.对照组,给予常规护理,研究组,在常规护理基础上,进行健康教育,观察并对比两种护理方式在慢性支气管炎患者护理中的应用效果.结果:研究组并发症发生率为6.7%,对照组为26.7%,研究组明显低于对照组,差异显著,具有统计学意义(P<0.05).研究组护理满意度为93.3%,对照组为73.3%,研究组明显高于对照组,差异显著,具有统计学意义(P<0.05).结论:对慢性支气管炎患者在常规护理的基础上予以健康教育,能够提高患者的护理满意度,具有临床推广价值.

Ubiquitin specific protease 4 (USP4) is a member of the USPs family, which catalyzes the cleavage of ubiquitin from a series of protein substrates, thereby modulating a number of cellular signaling pathways. In this study, we aimed to explore the expression profile of USP4 in lung adenocarcinoma (LUAD) using large patient cohorts in the Cancer Genome Atlas and the International Cancer Genome Consortium and to investigate its prognostic value and the possible mechanisms of its dysregulation. Results showed that USP4 was significantly downregulated in LUAD tissues (N = 514) compared with the normal controls (N = 59). The high USP4 expression group had significantly better overall survival (OS) and recurrence‐free survival (RFS). Multivariate analysis showed that preserved USP4 expression was an independent prognostic factor of favorable OS (HR: 0.574, 95%CI: 0.427–0.771, P < 0.001) and RFS (HR: 0.625, 95%CI: 0.444–0.880, P = 0.007) in LUAD. In comparison, although USP4 was downregulated in lung squamous cell carcinoma, its expression had no prognostic value in term of OS and RFS. By examining USP4 DNA copy number alterations (CNAs) (N = 511) and DNA methylation (N = 453) in LUAD, we found that DNA shallow deletion was frequent (–1, N = 239, 46.8%) and was associated with significantly decreased USP4 expression compared with the copy‐neutral (0) cases. The methylation status of some CpG sites in USP4 DNA was negatively correlated with USP4 expression. Based on these findings, we infer that USP4 expression might be a favorable biomarker in terms of OS and RFS in LUAD patients. DNA shallow deletion and hypermethylation might be two important mechanisms of decreased USP4 in these patients. © 2018 IUBMB Life, 70(7):670–677, 2018
Objective To explore the quality of life (QOL) of asthma inpatients and analyze its influencing factors. Methods From July 2016 to August 2017, 163 asthma inpatients of Department of Respiratory in China-Japan Union Hospital of Jilin University were selected by convenience sampling. All of the patients were investigated with the Asthma Quality of Life Questionnaire (AQLQ) , Medical Coping Modes Questionnaire (MCMQ) , Herth Hope Index (HHI) . Simple correlation and multivariate regression analysis was used to analyze patients' influencing factors of QOL. Results The total mean score of QOL of asthma inpatients was (3.41±0.46) . A total of 60.1% of patients mainly took confrontation coping mode. There was no patient with the low hope level, 33.1% of patients with the intermediate hope level and 66.9% with the high hope level. There was statistical difference in total score of QOL of patients with different ages, occupations, education, times of being admitted to hospital due to asthma, courses of disease, disease condition, types of drug using, hope level, coping modes (t=4.593, 3.315, 6.698, 4.826, 5.637, 2.686, 6.637, 19.752, 26.330; P<0.05) . Multivariate regression analysis showed that avoidance and confrontation coping mode, drug combination by various ways, whether with basic disease or not, marital status as well as overall hope level were main influencing factors of QOL of asthma inpatients. Conclusions The total QOL of asthma inpatients is below intermediate level. Most patients take confrontation medical coping mode. The overall hope level is high. The main influencing factors of QOL of asthma inpatients include types of drug combination, whether with basic disease or not, marital status, avoidance and confrontation coping mode as well as overall hope level. Resistin, secreted by macrophages in tumor microenvironment, has never been investigated in pancreatic cancer models, despite a vibrant tumor microenvironment around pancreatic tumors. We evaluated serum resistin levels in healthy individuals versus pancreatic cancer patients representing different tumor grades. In vitro mechanistic analysis involved MiaPaCa-2 and SW1990 cells. Resistin signaling depends on binding of resistin to its cognitive receptors. Therefore, we silenced adenylyl cyclase-associated protein 1 (CAP1) and toll-like receptor 4 (TLR4), its two known receptors, individually as well as in combination, by short hairpin RNA (shRNA). Effect of resistin on cell proliferation, migration, invasion, cell cycle, and sensitivity to gemcitabine was studied without or with silencing of resistin receptors CAP1 and/or TLR4. The results were also confirmed in vivo in mice xenografted with MiaPaCa-2 cells without or with receptor silencing. We report high resistin levels in pancreatic cancer patients which correlate positively with tumor grades. We observed a marked reduction in the resistin-induced proliferation, migration, invasion, and cell cycle of pancreatic cancer cells MiaPaCa-2 and SW1990 when the receptors were silenced. The results were confirmed in vivo wherein resistin effects were significantly attenuated in MiaPaCa-2 xenografts with silenced receptors. The combined silencing of CAP1 and TLR4 was found to be most effective in vitro and in vivo. We found activation of STAT3 by resistin in vivo and in vitro which was dependent on the presence of CAP1 and TLR4. Further, resistin was found to induce resistance to gemcitabine through its receptors. Our results describe novel functional roles of resistin with implications toward a better understanding of pancreatic tumor microenvironment.
目的 探讨集束化护理管理在老年失禁性皮炎中的临床意义.方法 将2014年3月-2015年3月吉林大学中日联谊医院呼吸内科入院的60例患有老年失禁性皮炎的患者,按入院时间先后顺序分为2组:2014年3-9月入院的30例为对照组,采用大小便失禁常规护理管理;2014年10月-2015年3月入院的30例为观察组,在常规大小便失禁护理的基础上,采用集束化护理管理.结果 观察组总有效率100%,对照组总有效率90.0%,2组比较,差异有统计学意义(P<0.05);失禁性皮炎愈合时间观察组明显少于对照组.结论 应用集束化护理管理能有效提高老年失禁性皮炎的治愈率,缩短愈合时间.
