Objective To evaluate the efficacy and safety of new epalrestat on diabetic peripheral neuropathy (DPN) compared with Tang Lin.Methods A total of 235 patients with DPN were enrolled in our study.They were randomly divided into two groups:the new epalrestat group (n=117) and the Tang Lin group (n=118).Their clinical,biochemistry,electrocardiogram,clinical symptoms and physical examinations,by using Michigan Neuropathy Screening Instrument (MNSI),and electrophysiological assessments were performed.Results The mean changes of MNSI scores both decreased compared with baseline in two groups after treatment for 12 weeks (P<0.05) and nerve conduction velocity improved in the two groups (P<0.05),but there were no difference of MNSI scores and nerve conduction velocity between the two groups.There was no difference of adverse event,blood pressure,heart rate,blood and urine routine examinations,liver and renal function between the two groups.Conclusion The new epalrestat is effective and safe as Tang Lin in treatment of DPN.
复旦大学附属华山医院的内分泌代谢专科多年以来,从无到有,由小到大,科研临床水平领先的发展历史,离不开一位先驱--钟学礼的引领和奠基铺路。<br> 立足内分泌研究领域<br> 钟学礼,1941年毕业于国立上海医学院(复旦大学上海医学院前身),曾积极参加抗日救国运动和建设新中国事业。1950年后,为了内科教学和临床需要,他在林兆耆教授的分配和指导下,开始钻研内分泌-代谢病学。当时,这是一门比较新而难的专业,理论深奥而且化验繁复,在我国临床病例少,缺乏前人指导。然而钟学礼知难而上,在既无机会去国内外进修学习,又乏先辈辅导指引的条件下,靠自学、讨论和临床实践,为复旦大学上海医学院创立了此专业。
认识BMI指数 其实,有一个指数可以用来判断一个人是否肥胖.目前,国际上常用BMI指数来衡量人体胖瘦程度以及是否健康.BMI是英文BodyMass Index的缩写,即身体质量指数,简称体质指数,又称体重指数.其公式为: BMI指数=体重(千克)/身高的平方(米2) BMI是与体内脂肪总量密切相关的指数,考虑了体重和身高两个因素.根据统计结果,当一个人的BMI指数为18.5~24时属正常.如果一个人的身高为1.52米,体重为59千克,则其BMI指数为25.5.由此断定,他偏胖了.
Plasma nesfatin-1 levels were assayed by ELISA in patients with impaired glucose regulation (IGR),type 2 diabetes mellitus( T2DM ),and healthy subjects with normal glucose tolerance( control group).The results showed that plasma nesfatin-1 levels in IGR and T2DM groups were significantly higher than that in control group [ ( 1.91 ±0.79 and 1.80±0.80 vs 1.41 ±0.58) μg/L,P<0.01 ].Plasma nesfatin-1 levels were positively correlated with body mass index(BMI),HbA1c,waist hip ratio,fasting plasma glucose,fasting insulin,and homoestasis model assessment insulin resistant index( HOMA-IR,P<0.05 or P<0.01 ).Multiple regression analysis showed that BMI and HOMA-IR were independent related factors in influencing plasma nesfatin-1 levels (both P<0.01 ).These results suggest that nesfatin-1 may partially contribute to the pathogenesis of insulin resistance and T2DM. 糖尿病肾病主要的病变部位在肾小球,早期可见肾小球毛细血管基底膜增厚,继之以血管狭窄、阻塞,流经肾脏的血流量明显减少,甚至出现"短路",血液中的废物不能完全排出,遂在血中堆积,达一定程度,即为尿毒症.
众所周知,糖尿病患者应对血糖水平进行全天候的控制,将餐前和餐后的血糖水平都控制在正常的范围之内。但是,目前临床上采用的口服降糖药疗法及皮下注射胰岛素疗法容易受到患者进餐的时间和进餐量等因素的影响,使患者的血糖水平波动较大,易患各种并发症。
血糖的测定对糖尿病患者有着重要的意义,它不但是诊断糖尿病的主要依据,而且是反映糖尿病治疗效果的重要指标.最近,我在与几位"资深"的糖尿病病友谈及有关血糖监测的问题时发现,他们只知道测定空腹时的血糖,不知道应该全天候地监测血糖,更不知道测定餐后、睡前和夜间血糖的特殊意义.现将与这些糖尿病病友的淡话内容记录如下,供读者参考.

Objective To examine the effects of ATⅡ and ATⅡ receptor subtype antagonist on expression of pro α1(Ⅰ) collagen mRNA and protein of cardiac fibroblasts in diabetic rat. (Methods) Myocardial fibroblasts were isolated for primary culture from three-month-old, male, (streptozotocin-)induced diabetic rats and control Sprague-Dawley rats. The fibroblasts were identified with (immuno-)histochemical method. Myocardial fibroblasts were treated with different concentrations of ATⅡ (10-9~10-7 mmol/L) or ATⅡ (10-7 mmol/L) plus losartan (10-7~10-5 mmol/L) for 48 h (separately.) The expression of pro α1 (Ⅰ) collagen was examined by semi- quantitative RT-PCR and Western blot analyses in different concentration of these agents. Results In diabetic group, the (expression) of pro α1 (Ⅰ) collagen mRNA and protein of these cells rose gradually according to the (concentrations) of ATⅡ. But this effect could be blocked partly or completely when ATⅡ (10-7) were plused with serial concentrations of losartan. These results suggested that ATⅡ can enhance and losartan can block the expression of pro α1(Ⅰ) collagen mRNA and protein in myocardial fibroblasts. Conclusions ATⅡ might take a great part in cardiac fibrosis and cardiomyopathy in diabetic heart. Such function may be blocked by ATⅡ receptor antagonist.
近年来,胰岛素品种、输注器具及方法不断创新,临床疗效得以显著提高.糖尿病控制和并发症研究(DCCT)充分表明强化治疗对控制和改善1型糖尿病(T1DM)并发症的重要性[1];英国前瞻性糖尿病研究(UKPDS)[2]和日本的Kumamoto研究[3]则观察了强化治疗对2型糖尿病(T2DM)的作用,发现糖化血红蛋白(HbA1c)每降低1%,微血管和大血管并发症发生和进展的危险性减少约20%.因此,对于T1DM和T2DM强调强化治疗以使血糖控制达标具有重要意义.
1 肥胖与糖尿病的相关性 临床流行病学研究表明,肥胖与糖尿病(尤其是2型糖尿病)常合并存在,尤其在早期观察2型糖尿病患者的自然病程,相当多数的病人有肥胖的历史.肥胖者常表现为胰岛素抵抗、高胰岛素血症和糖耐量降低.肥胖作为一个独立的病因能否引起糖尿病目前虽无十分确切的证据,但至少是其诱发和加重因素.对肥胖型糖尿病进行减肥是此型糖尿病重要的治疗目标,特别在早期常常可以收到显著乃至根治的疗效.尽管肥胖者多存有胰岛素抵抗,肥胖者也并不一定发展为糖尿病,但肥胖显然是2型糖尿病一大重要危险因素.
1引言 近年来,糖尿病已成为全世界共同面临的主要公共健康问题之一,也是危害人类健康和导致死亡的主要原因之一.中国目前有近3 000万例被确诊的糖尿病患者,并且在以每日3 000例新患者的速度增加,因此,糖尿病的治疗问题日趋突出.近几年来,在糖尿病的治疗方面取得了许多新的进展,对原有的治疗观念进行了补充和提高,在此一并进行回顾.糖尿病急、慢性并发症不在此范畴.
1引言 糖尿病是一组由遗传和环境因素相互作用,胰岛素分泌缺陷以及靶组织对胰岛素敏感性降低(胰岛素抵抗)引起的以高血糖为特征的、慢性、全身性代谢性疾病.近年来糖尿病患病率逐年上升.其病死率仅次于癌症、冠状动脉粥样硬化性心脏病、高血压,严重威胁人们的健康.因此,熟悉了解糖尿病的诊断和分型加强防治工作极其重要.
研究表明,肾脏存在丰富的瘦素受体的转录表达[1],近来报道给予大鼠瘦素后刺激肾小球内皮细胞增殖并刺激内皮细胞合成转化生长因子(TGF)β,引起肾小球系膜基质的扩张和尿蛋白排泄率增加[2],提示瘦素在肾脏的致病作用.目前对于糖尿病动物模型肾脏瘦素受体的分布、类型了解甚少,本研究旨在观察瘦素受体在链脲佐菌素(STZ) 糖尿病大鼠肾脏中的转录表达及在肾脏的免疫组化定位.
