Introduction. Glucocorticoid-induced osteoporosis (GIOP) is a bone metabolic disease that develops as a result of glucocorticoid (GC) use, is one of the most frequent causes of secondary osteoporosis, leads to an increase in the risk of low-energy fractures, disability and mortality of GC users. Despite the few studies conducted in Ukraine on the epidemiology and treatment of GIOP, there have been no national recommendations for its management until now, which has become the background for their creation. The purpose was to develop national guidelines for the diagnosis, prevention, and treatment of GIOP based on the analytical analysis of modern literary sources to improve the awareness of the medical community of Ukraine, improve management, and reduce the socio-economic burden of the disease. Materials and methods. A group of 18 experts, leading Ukrainian scientists in various fields, was created to develop the guideline. An analysis of current literary sources on epidemiology, risk factors, diagnosis, prevention and treatment of GIOP, and monitoring of the effectiveness and safety of its treatment was carried out. Evidence was synthesized using the GRADE system, and a critical assessment of the quality of the recommendations was carried out using the AGREE II tool. Results. The guideline consists of 12 provisions on screening, diagnosis, prevention, and treatment of GIOP. The authors emphasized the need to increase the awareness of the medical community of Ukraine regarding the problem, as well as patients regarding the possible side effects of GC. They presented the features of the clinical assessment of the osteoporotic fracture risk in GC users, as well as modern and available in Ukraine methods of diagnosis and treatment of GIOP. Conclusion. The first Ukrainian guideline for screening, diagnosis, prevention, and treatment of GIOP is an important national tool for its management, which is recommended by the Board of the Ukrainian Association of Osteoporosis for use in practical health care by doctors of various specialties.
У статті узагальнено та проаналізовано матеріал, присвячений вивченню патогенетичних ланок розвитку ожиріння для удосконалення методів ранньої профілактики та лікування. Мета роботи — оптимізувати виявлення факторів ризику розвитку ожиріння у хворих для запобігання його прогресуванню та розробки методів профілактики та лікування. Об’єкт: ожиріння, цукровий діабет 2-го типу (ЦД2), інсулінорезистентність, діабетична ретинопатія, рівень лептину, ретинолзв’язувального білка-4 (retinol binding protein-4, RBP-4), глю- кагоноподібного пептиду-1 (glucagon-like peptide, GLP-1), пептид тирозин-тирозин (peptide YY, PYY), ступінь дефіциту 25-OH-D. Матеріал та методи: біохімічні, імуноферментні, інструментальні, статистичні. Результати. Проводилося вивчення рівнів та взаємозв’язків інкретинів, а саме GLP-1, адипокінів лептину та RBP-4, нейромедіаторів (PYY) у хворих із надлишковою масою тіла на тлі порушень вуглеводного обміну. Для пацієнтів, хворих на ЦД2 з підвищеною масою тіла та ожирінням встановлено переважання абдомінального типу роз- поділу жирової тканини (ЖТ). Доведено, що для цієї категорії обстежених характерним є підвищення рівня лептину та зниження 25-OH-D на тлі підвищених показників інсулінорезистентності незалежно від ступеня ожиріння. Доведено, що рівень лептину та вміст 25-OH-D у сироватці крові залежать від компенсації основного захворювання; ці показники вірогідно знижуються у хворих із декомпенсованим ЦД — при рівні HbA1c більше ніж 8,5%. Для пацієнтів із ЦД2 на тлі ожиріння встановлено зниження рівня PYY та GLP-1, що може бути прогностично негативним чинником порушення харчової поведінки та збільшення маси тіла. Встановлено взаємозв’язок показників 25-OH-D та PYY (r=0,67; p<0,001). При дослідженні кишкової мікробіоти показано, що в пацієнтів з ожирінням відмічалася менша різноманітність мікробіоти, зниження співвідношення Firmicutes/Bacteroidetes, спостерігалось збільшення анаеробної флори. Отже, зміни мікробіоти можуть бути важливим фактором розвитку і прогресування ожиріння. Встановлено наявність високої розповсюдженості діабетасоційованого остеоартриту з домінантною локалізацією у верхніх кінцівках: при цукровому діабеті 1-го типу (ЦД1) — у 74,5% хворих переважно у формі олігоартриту, при ЦД2 — у 78,2% у вигляді поліартриту. Виділено та обгрунтовано чинники ризику розвитку діабетасоційованих остеоартритів (ДО): вік пацієнта, рівень глікованого гемоглобіну (HbA1c), індекс маси тіла (ІМТ) та тривалість основного захворювання. Висновки. Таким чином, ожиріння у хворих на ЦД2 має свої особливості, а саме встановлено переважання абдомінального типу розподілу ЖТ з підвищенням рівня лептину та зниженням 25-OH-D на тлі підвищених показників інсуліно-резистентності незалежно від ступеня ожиріння. Зниження рівня PYY та значне зниження GLP-1, що може бути прогностично негативним чинником порушення харчової поведінки та збільшення маси тіла. Встановлено наявність високої розповсюдженості ДО в пацієнтів із ЦД. Модифікуючими факторами розвитку артропатії для хворих на ЦД обох типів є рівень HbA1c та ІМТ, отже заходи профілактики ДО повинні базуватися на підтримці оптимального для компенсації ЦД рівня HbA1c та ІМТ. Проведене дослідження показало, що метаболічні порушення є значущим фактором, що негативно впливає на перебіг інфекції COVID-19 і її можливі ускладнення. Особливо небезпечними є наслідки для осіб із вперше виявленим і нелікованим діабетом, надмірною вагою та ожирінням. Встановлено, що ЦД2 можна вважати фактором ризику виникнення постковідного синдрому в пацієнтів, які одужали від COVID-19. Отже, підвищення маси тіла у хворих на ЦД є важливим обтяжувальним фактором перебігу основного захворювання та його ускладнень потребує особливого підходу в комплексному лікуванні таких пацієнтів.
Поширеність дефіциту вітаміну D (віт D) серед дитячого населення щороку збільшується, паралельно із цим зростає і захворюваність на різноманітну ендокринну патологію в дітей та підлітків. І вже наприкінці ХХ століття почалося активне вивчення позакісткових функцій віт D. Тоді було виявлено, що віт D, а саме його активна форма 1,25-дигідрокси-вітаміну за своїм впливом на організм схожа більше на гормон, ніж на класичні вітаміни. Опосередковуючи свій вплив на клітини через рецептор віт D (Vitamin D receptor, VDR), віт D впливає не тільки на гомеостаз кальцію та фосфору, а й на баланс імунної системи, на рівні певних гормонів (зокрема, паратиреоїдний гормон (ПТГ)) і протизапальних цитокінів. Через те що ендокринна система загалом пов’язана і являє собою один механізм, то і дефіцит віт D може викликати певні порушення її ланок. Дедалі більше з’являється досліджень про можливий тригерний вплив дефіциту віт D на розвиток цукрового діабету 1-го типу (ЦД1) та інших автоімунних захворювань. Були проведені дослідження, які продемонстрували протекторний вплив приймання препаратів віт D вагітними та малюками до року на можливий розвиток ЦД1. Повідомляється також про дослідження, які демонструють, що приймання віт D у пацієнтів із вже виявленим ЦД1 та ЦД 2-го типу (ЦД2) покращувало глікемічні показники й зменшувало ймовірність появи ускладнень. Також є численні дані про безпосередній вплив зниження рівнів віт D на розвиток ожиріння й збільшення інсулінорезистентності в дітей та підлітків. При цьому існують деякі дослідження і метааналізи, які не знайшли вищезгаданої ролі віт D у розвитку ЦД1, ЦД2 й ожиріння серед дітей та підлітків. Надалі потрібне більше досліджень для розуміння ролі віт D у розвитку ожиріння й ЦД у дітей та підлітків.
Background. Vitamin D deficiency (VDD) is widespread in the world; its proportion varies considerably in different populations and depends on many causes. Up to now, there were no National recommendations for the diagnosis, prevention, and treatment of VDD in adults in Ukraine. Their creation became the purpose of this work. Methodology. Consensus was created using the Delphi method, voting was conducted using the SurveyMonkey® platform. After approval of the composition of the Consensus Group, agreement on the order of formation and structure of the Consensus, creation and correction of the main statements, and two voting rounds, the main Consensus statements were formed and were successfully voted on. The 15 authors of the article are 15 experts who participated in the voting. The final 14 Consensus statements are presented in this article. Each statement is preceded by a justification based on high-quality evidence available in the current literature. Results. Despite the reduction of VDD in the Ukrainian population in recent years, experts have recommended increasing the awareness of the medical community and the Ukrainian population about the problem and ways to overcome it, with a screening of the total serum level of 25-hydroxyvitamin D (25(OH)D) in subjects from the groups of risk to achieve the target concentration of 30–50 ng/ml (75–125 nmol/l). To ensure it, we recommend the individual selection of a prophylactic dose of vitamin D (800–2000 IU/d for young healthy persons and 3000–5000 IU/d for patients with diseases and conditions that affect the metabolism of vitamin D). For the treatment of VDD, we recommend short-term intake of higher doses (4000–10,000 IU/d) of vitamin D with control of the 25(OH)D level after 4–12 weeks of treatment and subsequent use of maintenance doses. Also, we recommend the determination of serum 25(OH)D level before the initiation of antiosteoporotic therapy in patients with osteoporosis and its complications to prevent its ineffectiveness and increase the safety profile.
Background. The research deals with the provision of calcium, magnesium and zinc in patients with type 2 diabetes mellitus (T2DM). The purpose was to investigate the serum content of calcium, magnesium and zinc in patients with type 2 DM. Materials and methods. The open-label controlled study included 27 patients with T2DM. The control group consisted of 12 people without endocrine and somatic diseases. 70 % of patients had newly diagnosed DM; in others, the disease duration was up to 5 years. Most of the examined were of mature age, on average 61.83 ± 5.52 years for patients and 55.25 ± 5.52 years for controls. In the group with T2DM, women accounted for 62.96 %, among controls — 63.64 %. Anthropometric measurements and glucose indicators were determined according to a standard procedure. Concentrations of calcium, magnesium, zinc in serum were evaluated by atomic optical emission spectrometry with inductively coupled plasma (PerkinElmer Optima 2100 DV ICP-OES, USA) according to the original procedure approved by the Kundiiev Institute of Occupational Health of the NAMS of Ukraine. Results. The level of fasting blood sugar ranged from 7.1 to 17.2 mmol/l and indicated the presence of T2DM, glycated hemoglobin was from 7.1 to 11.2 %. According to anthropometric data, the group of patients almost completely corresponded to the control group. Although a slight increase in body weight and waist circumference was registered already at the stage of initial diabetes, the level of triglycerides increased significantly, and the level of high-density lipoprotein decreased. A significant reduction in the levels of calcium and magnesium in the blood serum of patients with T2DM was found. The zinc content was at the level of the lower edge of the reference value in 44 % of patients, in the control group — only in 16.6 % of cases. There were no significant differences in this indicator in the observation groups. A high correlation was found between the studied parameters in the blood, as well as an inverse correlation between the fasting glucose level and the content of calcium and magnesium. Conclusions. A significant decrease in the level of calcium and magnesium in the blood of Ukrainian patients with type 2 diabetes mellitus was found. There is a high correlation between serum calcium, magnesium and zinc levels in patients with T2DM. Significant risks of diabetes were revealed when the serum concentration of calcium and magnesium decreased.
Background. The development of diabetes-associated osteoarthritis is a complex and multifactorial process that depends on the type and compensation of diabetes mellitus (DM). Currently, the data about the role of various risk factors in the development of joint damage among the patients with DM are extremely contradictory. The purpose of the research was to study the clinical characteristics and risk factors of diabetes-associated osteoarthritis in the patients with type 1 DM (T1DM) and type 2DM (T2DM). Materials and methods. 556subjects were examined, who were divided into groups according to the type of DM, the presence, and the degree of severity of diabetes-associated osteoarthritis. Results. In the group of the subjects with T1DM, diabetes-associated osteoarthritis was diagnosed in 185 (74.5%) patients, and in patients with T1DM, joint damage was diagnosed in 241 (78.2%) persons. It was established that in the vast majority of the patients, diabetes-associated osteoarthritis was localized in the joints of the upper limbs. Oligoarthritis was frequent of the patients with T1DM, while polyarthritis is more common in patients with T2DM. It has been established that the probability of diabetes-associated osteoarthritis developing in patients with T1DM probably increases after the age of 35, with a body mass index (BMI) of more than 25.0kg/m2, a level of glycated hemoglobin (HbA1c) of more than 8.0%, and duration of DM of more than 28 years, and in the patients with T1DM, it develops more often among the patients after the age of 61 years, with HbA1c higher than 8.2%, BMI 27.9 kg/m2 and higher, and duration of DM more than 14 years. Conclusions. Measures to prevent diabetes-associated osteoarthritis in patients with DM should be based on long-term compensation of the underlying disease and maintenance of the optimal body mass.
This chapter reviews the issue of overweight management, which is one of the major challenges faced by most countries today. The causes of obesity include genetic and epigenetic factors, a lack of physical activity, eating disorders, and gut microbiota status. Physical exercise is the main means of prevention and management of overweight and obesity. The effectiveness of exercise programs for obese people typically varies around 80%, but it can be increased by taking into account biochemical, genetic, epigenetic, and microbiome markers, which allows choosing the most appropriate type of exercise according to individual characteristics. The pathogenetic preconditions for reducing exercise tolerance were examined based on the existing imbalance of adipokines, cytokines, and incretins. The association between genotype and weight loss induced by different diets and types of exercise was discussed along with obesity epigenetic markers. The effects of dietary choice on the microbiome composition and its contribution to the development of systemic inflammation in obese people were assessed. The weight management exercise program for middle-aged women was presented. The structure and value of the factors that determine the physical condition of overweight middle-aged women were described. These data provide the basis for designing a sound exercise program for weight management.
Актуальність. Метформін — один із цукрознижувальних засобів першої лінії, що найчастіше призначається для лікування хворих на цукровий діабет 2-го типу (ЦД2). Однак механізм його лікувальної дії ще недостатньо вивчений. У той же час ЦД2 вважається захворюванням запальної природи, при якому порушені різні імунні реакції. Однак вивченню ролі імунної системи в механізмі цього захворювання присвячені лише одиничні роботи. Мета дослідження: висвітлення питання про те, якою мірою різні види лейкоцитів, імунофенотип лімфоцитів та низка цитокінів беруть участь у механізмі терапевтичної дії метформіну. Матеріали та методи. Обстежені група хворих обох статей з уперше виявленим ЦД2 з індексом маси тіла 33,1 ± 1,3 кг/м2, яких ще не лікували цукрознижувальними засобами, та група нормоглікемічних здорових осіб того ж віку та статі. Кількість лейкоцитів у периферичній крові (ПК) визначали за допомогою гематологічних аналізаторів, а лейкоцитарний склад — у мазках, пофарбованих за Папенгеймом. Імунофенотип лімфоцитів (CD3+ T, CD4+ T, CD8+ T, CD56+) визначали методом проточної цитометрії за допомогою цитофлуориметра FACStar plus. Вміст різних цитокінів (ІЛ-1b, ФНП-a та ІЛ-10) — імуноферментним методом ELISA. Результати. Терапія метформіном хворих з уперше виявленим ЦД2 та ознаками ожиріння приводить до нормалізації підвищеної кількості лейкоцитів, нейтрофілів та моноцитів, а також зниження вмісту CD4+Т клітин у ПК, особливо у хворих із високими показниками індексу маси тіла. Характерною особливістю терапії є різке зниження рівня прозапальних цитокінів (ІЛ-1b та ФНП-a), підвищеного до лікування. Отримані дані вказують на те, що при ЦД2 відзначаються порушення природженого й адаптивного імунітету, та підтверджують гіпотезу про запальну природу цього захворювання. Висновки. Сприятлива лікувальна дія метформіну при ЦД2, особливо ускладненого ожирінням, багато в чому зумовлена нормалізацією показників запалення та імунітету.
Diabetic foot syndrome with long-term unhealed wounds is the most common complication and cause of limb amputation in diabetes. The search for effective therapeutic agents and their inclusion in treatment protocols is a priority due to the increase in the number of cases of this socially significant disease and disability among the working population every year. The aim of the study is to evaluate the effectiveness of cryopreserved human amniotic membrane in the treatment of long-term non-healing wounds of the lower extremities in diabetic foot syndrome. Materials and methods. The pilot clinical study described 4 clinical cases of treatment of patients with diabetes mellitus type I and II (1 woman and 3 men aged 52 to 68 years) with long-term unhealed wounds of the limbs under standard therapy. After previous wound sanation the applications of the cryopreserved human amniotic membrane were performed. Once a week after the application, the dynamics of wound healing was assessed. Blood glucose levels were determined before amniotic membrane treatment and two hours after the procedure. Results. As a results of weekly applications of human amniotic membrane there was a gradual decrease in the area of the wound from the original size and increase the rate of healing. Thus, at the time of the second visit (after 7 days) the reduction in the area of the ulcer from the initial size in patient 1 was 33 %, patient 2 – 25 %, patient 3 – 33 % on the sole and patient 4 – 3 %, and the healing rate – 4.7 %, 3.6 %, 4.7 % and 0.43 % per day, respectively. The use of human amniotic membrane did not affect blood glucose levels when comparing values before application and two hours after the procedure. Regular follow-up visits of patients 3, 6, 9 and 12 months after the start of the study showed no recurrence of ulcers. Conclusions. It has been shown that the use of cryopreserved human amniotic membrane in patients with diabetes mellitus and diabetic foot syndrome with long-term unhealed wounds results in complete healing of ulcers with stable remission during the year of observation.
The paper analyzes the current literature data and the results of our own researches concerning the state of the cytokine network: pro- and anti-inflammatory cytokines (interleukin (IL) 1α, IL-1β, IL-4, IL-6, IL-10, IL-17 and tumor necrosis factor (TNF) α), α- and β-chemokines, including IL-8 and IL-16, as well as adipokines (leptin and adiponectin) in the peripheral blood of patients with type 2 diabetes (T2D) with normal and increased body weight/obesity. It has been shown that patients with T2D are characterized by an increased content of proinflammatory cytokines (IL-1, IL-6, IL-17, TNFα), α- and β-chemokines in the peripheral blood, including IL-8 and IL-16, as well as leptin with a decrease in adiponectin content. In lean patients (with body mass index (BMI) < 25.5 kg/m2) compared to lean normoglycemic individuals from the control group (BMI < 25.5 kg/m2), there is a small but significant increase in IL-1β, IL-6, IL-17, TNFα and leptin, which, as BMI increases, significantly increases in severe obesity (BMI > 30.0 kg/m2), especially in obese women (BMI > 35.0 kg/m2). Similarly, an increase in proinflammatory cytokines is observed in normoglycemic people, but not as significant as in T2D. Less clear data were obtained when during determination of the anti-inflammatory cytokines IL-4 and IL-10, which is explained by a significant polymorphism of their genes, and both protective and compensatory effects on pro-inflammatory cytokine rise. In T2D patients, especially those with obesity, there is an increase in the leptin level and a decrease in the adiponectin content. The severity of the course and the percentage of mortality are closely associated with the BMI of patients. The effectiveness of the fight against an increase in the incidence of T2D should be primarily aimed at preventing obesity, and in case of already developed T2D — at reducing concomitant obesity. The analysis of the data presented also suggests that a sharp increase in the content of pro-inflammatory cytokines (so called cytokine storm) observed in patients with T2D and obesity infected with COVID-19, is a consequence of the summation and potentiation of already existing inflammatory process.
Численні дослідження показали високу ефективність баріатричної хірургії у пацієнтів, які страждають на ожиріння і цукровий діабет (ЦД) 2-го типу. В даний час хірургічне лікування ожиріння є одним із найбільш дієвих способів зниження і довгострокового підтримання маси тіла й управління ЦД 2-го типу. При цьому особливий інтерес викликає виражений позитивний вплив баріатричних оперативних втручань на порушення вуглеводного обміну у прооперованих хворих, що призводить не тільки до поліпшення перебігу ЦД 2-го типу, а й у значній кількості випадків — до його ремісії. У цьому огляді висвітлені механізми, пов’язані з поліпшенням глікемічного контролю у хворих на ожиріння після баріатричних оперативних втручань, проведено порівняльний аналіз різних баріатричних оперативних втручань при ЦД 2-го типу, наведені причини постбаріатричних гіпоглікемій, а також предиктори післяопераційного прогнозу ефективності баріатричних операцій щодо метаболічного контролю у пацієнтів з ожирінням і ЦД 2-го типу. Наведено дані, які стосуються хірургічних і метаболічних ускладнень баріатричних оперативних втручань, що визначає необхідність виваженого підходу до вибору оперативної стратегії лікування.
Актуальність. Ураження суглобів у хворих на цукровий діабет належить до поширених ускладнень і пов’язане з виникненням дeгeнepативно-дистpофічних змін у навколосуглобових структурах. Метою нашої роботи було вивчити рівень гормонів інсуліну і лептину в пацієнтів із діабетичним остеоартритом і дослідити їх можливий вплив на розвиток цього ускладнення. Матеріали та методи. Обстежені 77 пацієнтів, які були розподілені на групи за типом цукрового діабету, наявністю та ступенем тяжкості діабетичної артропатії. Вміст інсуліну і лептину в сироватці крові визначали імуноферментним методом. Результати. Серед обстежених пацієнтів діабетична артропатія діагностована більше ніж у 70 % осіб. У пацієнтів із діабетичною артропатією вірогідно підвищені рівні інсуліну (при цукровому діабеті 1-го типу — на 38,5 %, при цукровому діабеті 2-го типу — на 55,6 %) і лептину (при цукровому діабеті 1-го типу — на 43,8 %, при цукровому діабеті 2-го типу — на 53,7 %). Спостерігається прямий кореляційний зв’язок між ступенем тяжкості ураження суглобів і рівнем інсуліну і лептину. Ступінь тяжкості артопатії в пацієнтів із цукровим діабетом 2-го типу прямо корелює з показниками інсулінорезистентності. Висновки. Шанси на виявлення артропатії при цукровому діабеті 1-го типу при підвищенні рівня інсуліну збільшуються в 3,8 раза, при підвищенні лептину — в 1,3 раза, у хворих на цукровий діабет 2-го типу — у 2,6 і у 1,2 раза відповідно. Для даної вибірки встановлено, что розвиток артропатії не залежить від типу цукрового діабету. У жінок із цукровим діабетом 2-го типу шанси розвитку артропатії вищі в 6,4 раза, ніж у чоловіків. Таким чином, підвищення рівня інсуліну і лептину може бути маркерами наявності і прогресування артропатії у хворих на цукровий діабет.
На сьогодні пандемія COVID‑19 триває вже близько двох років. Хоча знання фахівців значно покращились у питаннях профілактики та лікування важких форм захворювання, патогенез та лікування синдрому пост-COVID‑19 залишаються актуальними питаннями в медичній та науковій спільнотах. Актуальність проблеми полягає в значному поширенні цього стану серед перехворілих, зниженні якості життя пацієнтів, нестачі знань про частоту, механізми перебігу та причини віддалених наслідків, нечітких уявленнях стосовно підходів до діагностики та лікування, а також відсутності нормативних документів щодо ведення таких пацієнтів. У серпні 2020 року британськими дослідниками вперше було запропоновано термін пост-COVID‑19. Наявні дані свідчать про значні відмінності в епідеміологічних оцінках поширеності синдрому пост-COVID‑19 через відмінності в методах відбору, періодах спостереження та розмірах вибірок. Частота синдрому пост-COVID‑19 оцінюється в 10-35%, тоді як для госпіталізованих пацієнтів вона може досягати й 85%. Втомлюваність є найпоширенішим симптомом, про який повідомляється в 17,5-72,0% випадків після перенесеного COVID‑19, потім услід за задишкою, частота якої становить 10-40%, виникають психічні проблеми, біль у грудях, нюхова і смакова дисфункція, що зустрічаються відповідно до 26, 22 та 11% реконвалесцентів. Більше однієї третини пацієнтів із синдромом пост-COVID‑19 мають вже наявні супутні захворювання, найчастіше зустрічаються гіпертонія та цукровий діабет. Опубліковані на сьогодні дані свідчать про те, що більшість пацієнтів із синдромом пост-COVID‑19 мають хороший прогноз без подальших ускладнень та летальних наслідків. Більшість досліджень досі зосереджувалися на симптомах, пов’язаних із синдромом пост-COVID‑19, а не на дисфункції органів. В огляді представлено аналіз досліджень щодо визначення синдрому пост-COVID‑19, вивчення впливу перенесеної інфекції на різні системи органів та надано основні потенційні механізми розвитку ускладнень. Особлива увага приділена наслідкам перенесеного COVID‑19 з боку ендокринних органів. Запропоновані рекомендації щодо обстеження і ведення пацієнтів із синдромом пост-COVID‑19.
The aim of study was to investigate markers of connective tissue metabolism (COMP level) and indicators that reflect the synthetic and catabolic phases of the metabolism of the main components of connective tissue - collagen and glycosaminoglycans in patients with diabetic arthropathy.The study involved 87 patients with diabetes. Patients were examined using a visual analogue scale of the Leken index and WOMAK. Cartilage oligomeric matrix Protein (COMP) was determined using enzyme immunoassay.Results. The presence of diabetic arthropathy was detected in 78% of patients with type 1 diabetes and in 80% of patients with type 2 diabetes. In the vast majority of patients, the joints of the upper extremities were involved in the pathological process. A direct correlation was established between the presence of arthropathy and the COMP level (r = 0.76, p = 0.001), the Leken index (r = 0.76, p = 0.001), YOUR scale (r = 0.88 , p = 0.001) and WOMAK (R = 0.88, p = 0.001). Patients with type 1 diabetes with joint damage are characterized by a compensatory increase in the synthesis of subchondral bone in response to increased collagen breakdown. For patients with type 1 diabetes with joint damage is characterized by a compensatory increase in subchondral bone synthesis in response to increased collagen breakdown.Conclusions: The results suggest that arthropathy in patients with type 1 and type 2 diabetes is characterized by an increase in the cartilage degradation marker in direct proportion to the severity and number of affected joints.
Background. At present, data regarding the role of various risk factors for the development of joint damage in patients with diabetes mellitus (DM) are extremely controversial. Particularly many questions remain regarding the clinical manifestations of this complication, since the development of diabetic arthropathies is a complex and multifaceted process that depends on the type of DM. The purpose of our study was to investigate the clinical features of diabetic arthropathy and to identify plausible risk factors for joint damage in patients with type 1 and 2 DM. Materials and methods. Five hundred and fifty-six patients were divided into groups according to the type of DM, the presence and severity of diabetic arthropathy. Diagnosis of arthropathies was performed using the radiographic method and ultrasound of the joints. Results. In the group of patients with type 1 DM, arthropathy was diagnosed in 185 (74.5 %) cases, and in type 2 DM, joint lesions were detected in 241 (78.2 %) individuals. It was found that in the vast majority of patients with DM of both types, arthropathy was localized in the joints of the upper extremities. Most often, small distal or proximal joints of the hand and one of the large joints were involved in the pathological process. Oligoarthritis is characteristic of patients with type 1 DM, whereas polyarthritis is more common in patients with type 2 DM (t = 4.88, p ≤ 0.001). It was found that the risk of developing arthropathy in patients with type 1 DM increases significantly after 35 years, with body mass index (BMI) greater than 25.0 kg/m2, HbA1c levels higher than 8.0 %, and DM duration over 28 years. In patients with type 2 DM, the likelihood of developing arthropathy is increased in patients aged 61 years and older, with HbA1c levels greater than 8.2 %, BMI of 27.9 kg/m2 and above and the duration of diabetes more than 14 years. Conclusions. Preventive measures for diabetic arthropathy should be based on the maintenance of target HbA1c levels of less than 8 % for patients with type 1 DM and 8.2 % for people with type 2 DM, and BMI not higher than 25.0 kg/m2 for individuals with type 1 DM and 27.8 kg/m2 for those with type 2 DM.

Joint damage in patients with diabetes mellitus (DM) is a common complication and is associated with the induction of metabolic inflammation against the background of increased catabolic processes in various joint structures. The aim of our study was a study of the levels of proinflammatory cytokines in the serum of patients with diabetes-associated osteoarthritis. Materials and methods. We examined 118 patients, who were divided into groups according to the type of diabetes, the presence and severity of diabetic arthropathy. The content of IL-1α, IL-6, S IL-6-R receptors, TNF-α, osteoprotegerin (soluble TNF-α receptor) in blood serum was determined by enzyme-linked immunosorbent assay. Results Among the examined patients, diabetic arthropathy was diagnosed in more than 70% of patients with diabetes of both types. In patients with diabetic arthropathy, levels of TNF-α (44.5% in type 1 diabetes, 42.9% in type 2 diabetes) and its soluble osteoprotegerin receptor (74.1% in type 1 diabetes) were significantly increased. Type 2 diabetes by 52.9%, as well as IL-6 (with type 1 diabetes by 52.1%, with type 2 diabetes by 64.4%) There is a direct correlation between the severity of joint damage and the level of TNF-α , osteoprotegerin and IL-6. We examined 77 patients who were divided into groups by type of diabetes, the presence and severity of diabetic arthropathy. The content of insulin and leptin, osteocalcin in the blood serum was determined by the enzyme immunoassay, the level of glycosaminoglycans, hydroxyproline, hyaluronidase, collagenase according to traditional biochemical methods. Among the examined patients, diabetic arthropathy was diagnosed in more than 70%. Patients with diabetic arthropathy significantly increased levels of insulin (with type 1 diabetes by 38.5%, with type 2 diabetes by 55.6%) and leptin (with type 1 diabetes by 43.8%, with type 2 diabetes by 53.7,%), the level of osteocalcin (only with type 1 diabetes by 53.9%) There is a direct correlation between the severity of joint damage and the level of insulin and leptin. The severity of arthopathy in patients with type 2 diabetes is directly correlated with indicators of insulin resistance. In patients with diabetes-associated osteoarthritis, indicators that characterize catabolic processes in the connective tissue (hydroxyproline free and collagenase (p<0.001) are increased. The chances of detecting arthropathy with type 1 diabetes increase 3.8 times with an increase in insulin levels, with an increase in leptin 1.3 times, in patients with type 2 diabetes, 2.6 and 1.2 times, respectively. For this sample, it was found that the development of arthropathy does not depend on the type of diabetes. In women with type 2 diabetes, the chances of developing arthropathy are six times higher. 4 times than men.
