Резюме. Боковой амиотрофический склероз (БАС) – фатальное прогрессирующее заболевание центральной нервной системы с поражением верхнего и нижнего мотонейронов. Изучение особенностей течения и распространения нейродегенеративного процесса при БАС имеют большое значение, поскольку до настоящего времени эффективные методы лечения заболевания не разработаны. В клинической практике отсутствуют объективные биомаркеры поражения верхнего мотонейрона и экстрамоторных регионов головного мозга, несмотря на очевидные доказательства мультисистемности поражения головного мозга при БАС. В последние годы большую роль в изучении БАС играют методы структурной и функциональной нейровизуализации, такие как МР-морфометрия, диффузионно-тензорная МРТ, МР-спектроскопия, фунциональная МРТ, позитронно-эмиссионная томография (ПЭТ) и другие. В обзоре анализируются результаты нейровизуализационных исследований в контексте их применения для диагностики, прогнозирования и мониторирования течения БАС. Для диагностики заболевания наиболее чувствительными и специфичными являются диффузионно-тензорная МРТ, МР-спектроскопия, ПЭТ, комбинация нескольких методов нейровизуализации и их сочетание с транскраниальной магнитной стимуляцией. Диффузионно-тензорная МРТ и МР-спектроскопия могут использоваться для мониторинга и прогнозирования течения заболевания. Обсуждаются основные ограничения и недостатки проведённых исследований, а также возможные перспективы применения нейровизуализации при БАС.
Objective: to generalize the Russian experience with perampanel (PER) in routine clinical practice; to do this, the results of its use as an adjuvant partial epilepsy medication were retrospectively assessed. The study is still in progress now; therefore, the paper gives its preliminary results. Patients and methods. The investigation included 52 patients with drug-resistant partial epilepsy. Their mean age was 28.92±14.02 years (a small number of the patients had not attained the age of 12 years); the proportion of men was 56%; the disease duration was over 10 years (69.2%); symptomatic epilepsy was in 76.9% with an epileptic focus being in the frontal (46.2%) and temporal (44.2%) regions. PER was prescribed to the majority (71.2%) of patients after three previous therapy lines. The baseline monthly rates of all types of seizures were 127.29±82.29; those of generalized seizures were 6.72±1.90. Results and discussion. After addition of PER to therapy just within the first month, there was a significant reduction in the frequency of all types of seizures to 52.06±29.26 per month (Sign test; p = 0.00001) and in that of secondary generalized seizures to 3.71±1.71 (Sign test; p=0.00001). The duration of PER administration was more than 6 months in the overwhelming majority of cases. In 58% of the patients, the frequency of seizures decreased by more than 50% (respondents). The lack of all types of seizures was noted in 8%; that of only secondary generalized seizures was in 31%. Adverse events were observed in 30.1% of the patients (aggression in 11.5% and somnolence in 9.6%; others were seen more rarely). The dose of PER was decreased because of side effects in 7 (13.5%) patients; the drug was discontinued in 4 (7.7%). The mean dose of PER for adults was as high as 6 mg.
Transcranial magnetic stimulation (TMS) is a new technique that results in neuron excitation using alternating magnetic fields. According to recent reports, high-frequency TMS could be an effective treatment for drug-resistant central post-stroke pain (CPSP). The present case report describes the clinical observation of two patients with CPSP with different lesion localisations and volumes. High frequency (10 Hz) TMS of the primary motor cortex of the affected hemisphere was used in each patient. After stimulation, we recorded a significant reduction in pain on a visual analogue scale (VAS). However, after the end of treatment with TMS, pain gradually returned to previous levels after 3–4 months.
FIELD: medicine. SUBSTANCE: patient receives a task of kinesthetic imagination of limb movements, patterns of the patient's brain activity resulting from the imaginary movement are analyzed. Data are to a computer for allocation of signals responsible for movement imagination. The patient is presented the task recognition results via the visual feedback as a mark on the screen. A mark re-position indicates task accuracy. At that, the results of task recognition associated with kinesthetic imagination of paretic limb movement are further presented via the tactile and proprioceptive feedback through the exoskeleton worn on the paretic limb In case of proper task recognition, the exoskeleton moves the limb in the imaginary movement direction, and in case of a wrong result - in the opposite direction. Одно из важнейших мест в системе реабилитационного лечения занимают методы, позволяющие, с одной стороны, обеспечивать мобильность пациентов независимо от степени обездвиженности, с другой, активизировать афферентные потоки. Исследования последних десятилетий показали, что структурной составляющей восстановления при поражениях центральной нервной системы (ЦНС) является пластичность головного мозга. Анатомической основой пластичности является реорганизация кортикальных отделов, увеличение эффективности функционирования сохранившихся структур и активное использование альтернативных восходящих путей, например, интенсификация афферентной информации, поступающее в ЦНС. Однако использование методов сенсорной коррекции в отрыве от функционального состояния больного может приводить к формированию патологических симптомов, таких как спастичность, повышенная рефлекторная активность и т.д. Именно поэтому приоритетной задачей является изучение возможности адекватного управления нейропластическими процессами, что невозможно без разработки оригинальных и валидных методов изучения механизмов нейропластических процессов с использованием современных высокотехнологичных средств нейровизуализации. В данном исследовании был разработан принципиально новый подход для изучения механизмов кортикальной пластичности, ответственных за локомоцию, представляющий собой комплексное применение метода функциональной магнитно-резонансной томографии (фМРТ) и метода навигационной транскраниальной магнитной стимуляции (нТМС). Показано, что при фМРТ с использованием сенсомоторной пассивной парадигмы, имитирующей опорную нагрузку при ходьбе, которая создается механическим стимулятором опорных зон стоп “Корвит”, выявляется обширная активация первичной и вторичной сенсомоторной коры, участвующей в контроле локомоции. Последующее применение нТМС в этих зонах активации позволяет более точно локализовать область моторного представительства мышц, участвующих в локомоции. Предполагается, что новый подход позволит в дальнейшем использовать его как для изучения нейропластичности, связанной с локомоцией, так и для оценки ее изменений в ходе реабилитационных мероприятий, направленных на восстановление и коррекцию ходьбы.
Results of basic studies in space medicine revealed the mechanism of motor disorders and the importance of support input in regulation of the tone and posture system of the mammals. Progress in functional magnetic resonance tomography (fMRT) enabled in vivo estimation of activity of various parts of the brain during stimulation of support afferent pathways. The goal of our study was to detect specific activation of the cortex during stimulation of support input in healthy subjects and in patients with lesions of CNS. The study included 19 healthy volunteers (mean age of 38 ± 15, 13 years) and 23 patients with cortical-subcortical ischemic stroke (mean age 53 ± 9.07); all subjects underwent fMRT. During scanning of each subject, support areas of the foot soles were stimulated to imitate slow walking using the block design. In healthy volunteers, primary somatosensory cortex, premotor and dorsolateral cortex, and insula were significantly activated (corrected <0.05 at cluster level). In patients with stroke, pattern of activation of the supraspinal systems of locomotion control clearly depended on the stage of the disease. In patients with cortical-subcortical stroke undergoing motility rehabilitation, the sensomotor locomotion module was predominantly activated as the contralateral pattern.
