Right Ventricular (RV) involvement during inferior acute myocardial infarction (AMI) was known to be associated with poor outcome, but might have been mitigated by recent therapeutics. The aim of study was to determine the characteristics and outcomes of patients with RVAMI compared to isolated inferior AMI. This is an observational study that enrolled consecutive patients with RVAMI; age-sex matched patients with isolated inferior AMI served as controls. A total of 51 patients with RVAMI were studied (male 39, age 63 ± 16) and compared to 39 age-sex matched patients with isolated inferior AMI. Atherosclerosis risk factors, previous MI and treatment at presentation were similar in both groups. Primary coronary angioplasty was successful in > 90% in both groups. When compared to patients with isolated inferior AMI, patients with RVAMI had more frequent cardiogenic shock at presentation (35% versus 0%, P < 0.01), and in-hospital mortality (18% versus 0%, P < 0.01). Associated factors with in-hospital mortality included age, sex, RV involvement, delay from onset to management, angioplasty, anti GPIIb-IIIa treatment, ejection fraction, creatinine level but not the severity at presentation (including the presence of cardiogenic shock). After discharge from hospital and during a mean 200 weeks follow-up period, mortality increased similarly in both groups. RVAMI is still associated with high in-hospital mortality. The severity of initial presentation is not a prognostic factor. Mortality rates after discharge increased very slowly and similarly in both groups. L’impact de la revascularisation précoce sur la mortalité des infarctus aigus du myocarde inférieur étendu au VD (IDMVD) est peu connu. Le but de cette étude est d’évaluer les caractéristiques et le pronostic des patients ayant un IDMVD en les comparant à des patients ayant un IDM inférieur isolé (IDMI). Il s’agit d’une étude observationnelle ayant évaluée les patients admis consécutivement pour IDMIVD. Ces patients ont été appariés avec l’âge et le sexe aux patients admis pendant la même période pour IDMI (groupe témoin). Au total, 51 patients (39 males, âge moyen 63 ± 16 ans) ayant développé un IDMIVD ont été comparés à 39 patients avec un IDMI. Les caractéristiques de base des patients étaient comparables dans les deux groupes. Comparés aux patients avec un IDMI, les patients avec IDMIVD présentaient plus fréquemment un choc cardiogénique à la prise en charge (35 % versus 0 %, p < 0,01). La mortalité intra-hospitalière était plus élevé dans le groupe IDMIVD (18 % versus 0 %, p < 0,01). Les facteurs associés à la mortalité intra-hospitalière étaient l’âge, le sexe, l’atteinte du VD, une angioplastie retardée, un traitement par anti-GPIIb-IIIa, la fraction d’éjection et la créatinine plasmatique. La présence d’un choc cardiogénique à l’admission n’était pas associée à la mortalité. Après la sortie de l’hôpital et pendant un suivi moyen de 200 semaines, la survie était similaire dans les deux groupes. L’atteinte du VD dans l’IDM inférieur reste associée à une mortalité intra-hospitalière élevée. La sévérité clinique initiale ne semble pas être un facteur pronostique.
PURPOSE:To develop French recommendations about the management of vaccinations, the screening of cervical cancer and the prevention of pneumocystis pneumonia in systemic lupus erythematosus (SLE). METHODS:Thirty-seven experts qualified in internal medicine, rheumatology, dermatology, nephrology and pediatrics have selected recommendations from a list of proposition based on available data from the literature. For each recommendation, the level of evidence and the level of agreement among the experts were specified. RESULTS:Inactivated vaccines do not cause significant harm in SLE patients. Experts recommend that lupus patient should receive vaccinations accordingly to the recommendations and the schedules for the general public. Pneumococcal vaccination is recommended for all SLE patients. Influenza vaccination is recommended for immunosuppressed SLE patients. Live attenuated vaccines should be avoided in immunosuppressed patients. Yet, recent works suggest that they can be considered in mildly immunosuppressed patients. Experts have recommended a cervical cytology every year for immunosuppressed patients. No consensus was obtained for the prevention of pneumocystis pneumonia. CONCLUSION:These recommendations can be expected to improve clinical practice uniformity and, in the longer term, to optimize the management of SLE patients.
L'objectif de ce travail était de proposer des recommandations relatives aux modalités des vaccinations, de la prévention de la pneumocystose et du dépistage du cancer du col de l'utérus dans le lupus systémique (LS) chez l'adulte et l'adolescent. Trente-sept experts français du LS ont pris part à un vote visant à retenir des recommandations préparées par un groupe de travail à partir d'une revue de la littérature. Les vaccins inactivés ou sous-unitaires peuvent être administrés sans danger car le risque de déclencher une poussée de LS après une vaccination n'a jamais été démontré. Les patients lupiques doivent suivre les recommandations de vaccination et le calendrier vaccinal de la population générale. Il est également recommandé que tous les patients lupiques soient vaccinés contre le pneumocoque et que les patients lupiques immunodéprimés soient vaccinés annuellement contre la grippe saisonnière (vaccin inactivé). De façon générale, les patients lupiques immunodéprimés ne doivent pas recevoir de vaccin vivant. Cependant, ces vaccins peuvent être envisagés dans certaines situations où le niveau d'immunosuppression n'est pas trop prononcé (par exemple, les patients traités par une posologie ≤ 10 mg/j d'équivalent-prednisone sans immunosuppresseur ni biothérapie) et où le bénéfice de la vaccination est plus important que le risque de la maladie infectieuse qui doit être prévenue. Les experts recommandent que le dépistage du cancer du col de l'utérus chez les patientes lupiques immunodéprimées soit réalisé annuellement par frottis cervico-utérins. Ces recommandations devraient aider à homogénéiser la pratique des différents médecins français impliqués dans la prise en charge du LS. La survenue d’une mononeuropathie multiple (MM) au cours du lupus systémique (LS) est rare et peu de cas ont été rapportés dans la littérature [1], [2], [3]. L’objectif de cette étude était de décrire les caractéristiques cliniques de patients présentant une MM associée au LS, les traitements utilisés dans ce contexte et leur efficacité. Cette étude rétrospective multicentrique collectait les cas de patients présentant une MM associée au LS, provenant de 3 centres français spécialisés en médecine interne ou en rhumatologie. Le diagnostic de LS était basé sur les critères ACR, l’activité du LS était mesurée par le Systemic Lupus Erythematosus Disease Activity Index (SLEDAI). Le diagnostic de MM était documenté par électroneuromyogramme et/ou biopsie neuromusculaire dans certains cas. Dix patients ont été inclus (8 femmes et 2 hommes d’âge médian au diagnostic de LS 28,5 ans [de 17 à 53 ans]). Le LS était diagnostiqué avant la MM dans 9 cas/10 avec un délai médian de survenue de la MM de 8,2 ans (de 1 mois à 22 ans). Lors du diagnostic de MM, le SLEDAI était ≥ 6 pour 6 patients/9 (SLEDAI médian = 9, de 2 à 17). Les symptômes révélant la survenue d’une MM étaient : douleur neuropathique (8/10) ou hypoesthésie (5/10) affectant des territoires asymétriques, touchant les membres supérieurs (3/10) et/ou inférieurs (10/10). Tous les patients présentaient au moins l’atteinte du nerf sciatique poplité externe unilatérale ou bilatérale et/ou du nerf sciatique poplité interne. Les analyses biologiques montraient : une baisse du C4 définie par C4 ≤ 0,08 g/L chez 8 patients/8, une baisse du C3 définie par C3 ≤ 0,75 g/L chez 5 patients/9, un Facteur Rhumatoïde positif chez 5 patients/7, et un test de Farr augmenté chez 5 patients/6. La recherche de cryoglobulinémie était positive chez 5 patients/9 (de type II dans 4 cas/5 et III chez un patient). Les anticorps anticardiolipine étaient positifs chez 7 patients/10. Trois patients/10 présentaient un syndrome des anticorps antiphospholipides diagnostiqué avant la survenue de la MM. Les anticorps anti-SSA étaient positifs chez 7 patients/9. Une biopsie neuromusculaire était réalisée chez 5 patients, montrant une vascularite chez 2 et des infiltrats inflammatoires lymphocytaires péri-vasculaires chez 2 autres. Tous les patients ont reçu des glucocorticoïdes (intraveineux relayés par 0,5 à 1 mg/kg pour 6 patients et corticoïdes sans bolus initial entre 10 mg/j et 1 mg/kg/j pour 4 patients) combinés en première ligne avec du cyclophosphamide (6/10), du rituximab (3/10) ou du mycophénolate mofétil (1/10). Quatre patients avaient déjà reçu du cyclophosphamide avant le diagnostic de MM pour traiter une atteinte rénale lupique. Après une durée médiane de suivi de 5 ans (moyenne = 5,65 ± 4,865), les symptômes cliniques de la MM étaient améliorés chez 7 patients/10 (cyclophosphamide = 4, rituximab = 2, mycophénolate mofétil = 1), stabilisés dans 2 cas/10 (cyclophosphamide = 1, rituximab = 1) et aggravés chez un patient. Ce dernier avait initialement été traité par cyclophosphamide, la rechute était survenue 8 ans après l’épisode initial, secondairement à un défaut d’observance et avait alors été traitée par rituximab. Cliniquement, un patient avait des séquelles motrices, 2 patients avaient des séquelles sensitivomotrices, aucun n’avait de séquelle sensitive pure. L’électroneuromyogramme montrait des séquelles sensitivomotrices pour 4 patients et sensitives pures pour 1 patient. La survenue d’une MM au cours du LS peut être secondaire à une vascularite, avec présence d’une cryoglobulinémie et d’un C4 bas dans une majorité de cas. Le pronostic est globalement bon avec un traitement associant corticoïdes et immunosuppresseurs en première ligne. À l’avenir, une meilleure compréhension des mécanismes physiopathologiques conduisant à la lésion du nerf permettrait de cibler les choix thérapeutiques.

The optimal duration of oral anticoagulant therapy after a first "unprovoked" venous thromboembolism (VTE) is uncertain. Oral anticoagulation therapy is very efficient in reducing the risk of recurrence but increases the risk of hemorrhage. The balance between the risk of recurrent VTE and bleeding complication is difficult to predict on an individual basis. D-dimer levels during and after anticoagulation cessation have been evaluated in several cohort studies to potentially identify patients at high or low risk of recurrent event. Overall, these studies showed that D-dimer measurement had a high negative predictive value. According to the recent PROLONG trial and a meta-analysis, elevated D-dimer level may help identifying patients with a higher risk for recurrent VTE. Nevertheless, D-dimers should not be used as a stand-alone test to determine whether to stop or continue anticoagulation in patients with a first unprovoked VTE.La durée du traitement anticoagulant au décours d'un premier événement thromboembolique veineux idiopathique ne fait pas l'unanimité. L'anticoagulation, bien que très efficace dans la prévention de la récidive, s'accompagne d'un risque hémorragique non négligeable. Cette balance risque–bénéfice est difficile à évaluer chez une personne donnée. Depuis quelques années, l'évolution du taux des D-dimères sous anticoagulants et à distance de l'arrêt de ces derniers a été étudiée dans l'idée de pouvoir identifier les patients à faible ou à haut risque de récidive. Plusieurs études de cohorte utilisant des seuils et des méthodes de dosage différents ont montré que ce test avait une haute valeur prédictive négative. Une étude interventionnelle ainsi qu'une récente méta-analyse ont rapporté qu'en absence de traitement anticoagulant, les patients avec D-dimères élevés étaient à risque augmenté de récidive. Toutefois, malgré ces résultats encourageants, il semble encore prématuré de recommander l'utilisation des D-dimères pour se déterminer sur la durée du traitement anticoagulant chez les patients ayant présenté un premier événement thromboembolique veineux idiopathique.
La production et l’usage des médicaments ont généré une pollution par certains de leurs principes actifs, des métabolites et autres produits de dégradation. Certaines eaux de consommation sont contaminées par des résidus médicamenteux. Les risques environnementaux sont avérés alors que les risques sanitaires pour l’humain sont a priori moins significatifs. La gestion de ce problème s’inscrit dans une stratégie globale de développement durable qui ne doit en aucun cas nuire à la santé publique.The production and use of pharmaceuticals have generated pollution by active ingredients, metabolites and other degradation products. Some drinking water is contaminated with pharmaceuticals residues. The environmental risks are proven while the health risks for humans are seemingly less significant. The management of this problem is part of a global strategy of sustainable development which must in no way harm public health.
Optimal management of coronary artery disease in very old patients – over eighty years of age – remains unclear due to the absence of large randomized trials in this high-risk subgroup and the benefit/risk ratio of percutaneous coronary intervention (PCI) is difficult to assess in clinical practice.
Drug-eluting sterns (DES) are known to dramatically reduce restenosis. However, they are more expansive than bare-metal stents (BMS) and they require prolonged dual antiplatelet therapy. In France, the French Society of Cardiology and the "Haute Autorite de sante" have defined recommendations for the use of DES (restricted to patients in high-risk group). The aim of this work was to evaluate our practice (whether these recommendations were well respected or not in our center). Between November 2007 and January 2008 then November 2008 and January 2009 we evaluated all Percutaneous Coronary Interventions (PCI). Two hundred and sixteen (216) patients (mean age 65 +/- 13 years, 164 (76 %) were males and, 41 (19 %) were diabetics) had a PCI for stable angina or silent ischemia (47 %), unstable angina or acute coronary syndrome (ACS) ST-(26 %), ACS ST+ <48 hours (24 %) or ACS ST+ >48 hours-1 month (3 %). Two hundred and seventy six (276) stents were used, including 35 % of DES. The recommendations were well respected in 82 % of cases. However, 27 % of BMS were implanted in patients in whom DES were indicated. The French recommendations for DES are a reference to help practitioners, but they require to be adapted to each patient, depending on clinical state and their ability to be treated with prolonged dual antiplatelet therapy. Bivalirudin, with provisional GP IIb/IIIa inhibitor use allows the same protection against ischemic complications while reducing the hemorrhagic complications compared with the systematic association of a GP IIb/IIIa inhibitor plus heparin (The Randomized Evaluation in PCI Linking Angiomax to Reduced Clinical Events-2 [Replace-2]). In clinical practice, the use of heparin is not systematically associated with a GP IIb/IIIa inhibitor. That's why we studied the clinical and economic interest of bivalirudin only versus heparin (UFH) only. Opened pragmatic monocentric study carried out in 2007. We made a chronological matching: for each patient treated with bivalirudin, we included the next patient with the same clinical presentation treated with unfractionated heparin. Ninety-two patients were included (46 in each group). The need for a GP IIb/IIIa inhibitor during the PCI was not significantly different between the two groups (p=0.11). No major hemorrhagic complications were observed in the two groups. Prevalence of ecchymosis was not significantly different: 22 % in the UFH group versus 13 % in the bivalirudin group (p=0.27). The average troponin level the next day was significantly higher in the bivalirudin group (p=0,049), although the change in troponin levels before and after the procedure was similar in the two groups. The average cost by patient of anticoagulation by bivalirudin and HNF is very different, respectively 473+/-150 and 51+/-146 euro (p=0.0001). Bivalirudin can be an interesting alternative for patients with a high risk of having complications. But considering its cost this therapy must be used only for selected patients.
Prescribing recommended medications is associated with improved outcomes in coronary artery disease patients. Likewise, adherence to prescribed medications, which frequently appears far from optimal, is significantly correlated with improved survival in "real world" observational databases. The present article reviews currently available data on the impact of medication adherence on outcomes in patients with coronary artery disease.
La prescription des traitements recommandés s’accompagne d’une amélioration du pronostic des patients coronariens. De même, l’adhérence aux traitements prescrits, souvent moins bonne que les médecins ne le pensent, est significativement corrélée au devenir des patients. Cet article passe en revue les données récentes sur l’observance thérapeutique et son impact pronostique chez les coronariens.
Les recommandations britanniques du NICE (National institute for health and clinical excellence) en 2006, ecartant les betabloquants du traitement de premiere ligne de l'hypertension arterielle (HTA), amenent a s'interroger sur l'utilisation de cette classe therapeutique, notamment chez les patients diabetiques. En fait, les betabloquants constituent une classe therapeutique de premiere importance dans le post-infarctus et dans l'insuffisance cardiaque, ou ils reduisent dans des proportions considerables la mortalite, chez les diabetiques comme chez les non diabetiques. Sauf contre-indication authentique, ils doivent donc etre prescrits systematiquement dans ces cas. Mais la plupart des betabloquants, et notamment l'atenolol, interferent avec le metabolisme glucidique. Il parait donc logique, lorsque l'indication de privilegier cette classe n'est pas imperative, comme c'est le cas dans le traitement de premiere intention de l'HTA, de privilegier d'autres medicaments, en particulier chez les diabetiques ou les patients avec une intolerance au glucose.
In acute coronary syndromes, risk stratification is essential, particularly in those without ST-elevation, and is based upon clinical and biological markers. Among them, recent and repeated anginal attacks, ST-segment modifications on admission electrocardiogram, and increased markers of myonecrosis (particularly increased troponine levels) are strong predictors of untoward outcome. These variables can be used to construct risk scores, among which the TIMI and GRACE scores are the most widely used. These scores may prove helpful to define the optimal diagnostic and therapeutic management of the patients.
