L'hypertension pulmonaire (HTP) chez les patients atteints de bronchopneumopathie obstructive chronique (BPCO) est associée à un mauvais pronostic. Hormis l'oxygénothérapie de longue durée, aucun traitement spécifique de l'hypertension artérielle pulmonaire (HTAP) n'a été validé dans cette population et une prise en charge individualisée en centre expert est recommandée si l'HTP est sévère. L'objectif de notre étude est de décrire la cohorte strasbourgeoise (suivie au sein du centre de compétence Pulmotension des Hôpitaux Universitaires de Strasbourg) de patients atteints d'HTP sévère associée à une BPCO ayant bénéficié de traitements spécifiques de l'HTAP et d'évaluer des facteurs prédictifs de mortalité et de réponse à ces traitements. Notre étude monocentrique a été menée sur labase des données rétrospectives de 196 cathétérismes cardiaques droits (KTD) effectués chez des patients atteints de BPCO entre janvier 2015 et décembre 2022. Trente-cinq patients présentaient les critères d'inclusion, à savoir une HTP pré-capillaire sévère associée à une BPCO post-tabagique ayant reçu un traitement spécifique de l'HTAP et ayant bénéficié d'une réévaluation hémodynamique par KTD 3 à 6 mois après l'initiation du traitement (V1). Les données fonctionnelles respiratoires, cliniques, hémodynamiques et de l'imagerie thoracique ont été recueillies. Notre étude confirme les caractéristiques particulières de cette population; majoritairement masculine avec un âge médian de 71 ans (intervalles interquartiles (IQ), 69–74), aux antécédents de consommation tabagique importante et aux nombreuses comorbidités cardiovasculaires. Ils présentaient une obstruction modérée des voies aériennes (VEMS médian de 67%; IQ, 48–82) et une DLCO très abaissée (médiane à 29,5%; IQ, 22–40), sans toutefois présenter de distension thoracique (CPT médiane à 93%; IQ, 90–106) ou d'emphysème (index d'emphysème total médian à 16%; IQ, 3,5–31) importants. Tous présentaient une dyspnée marquée (classe fonctionnelle III ou IV de la NYHA) associée à une hypoxémie sévère (PaO2médiane 50 mmHg; IQ 44–56) et à une hypocapnie (PaCO2médiane 34 mmHg; IQ, 28–38). En raison d'une hémodynamique plus sévère au diagnostic, 48,5% des patients ont bénéficié d'une bithérapie initiale tandis que 51,5% d'une monothérapie. Une amélioration hémodynamique significative à V1 a été observée, néanmoins plus importante chez les patients traités par bithérapie: diminution de 49% des résistances vasculaires pulmonaires (RVP) vs 30% chez les patients sous monothérapie. Quatorze patients ont obtenu des RVP ≤ 5UW à V1 (p < 0,0001). Le score selon le tableau de risque simplifié à quatre strates utilisées dans l'HTAP [1] était également significativement amélioré puisque 60% des patients ont amélioré d'au moins une catégorie ce score à V1 avec 57% des patients à risque faible ou intermédiaire faible à V1 contre 14% avant mise en place du traitement. Les deux paramètres les plus discriminants étaient la classe fonctionnelle de la NYHA et la valeur du BNP. Autre fait intéressant, aucune modification de l'hématose n'a été retrouvé sous traitement. L'analyse de survie a confirmé le mauvais pronostic de cette population avec une médiane de survie globale de 4,1 ans (IQ 1,7–7,7) et le caractère discriminant de l'hypoxémie sévère définit par une PaO2 < 50 mmHg (Hazard ratio à 4,82 avec IC à 95% de 1,27 à 18,24; p = 0,02 en analyse multivariée). Les analyses univariées ont également mis en évidence une réduction de la survie chez les patients qui aggravaient leur score de risque simplifié à V1 (HR 2,88 (IC à 95% de 1,16 à 7,15); p = 0,02). Nos données confirment le mauvais pronostic des patients atteints de BPCO avec HTP sévère pour lesquels l'utilisation de la stratification du risque à quatre strates se révèle pertinente. La prescription des traitements spécifiques de l'HTAP pourrait être considérée chez ces patients avec un intérêt prometteur pour la bithérapie qui semble bien tolérée avec une meilleure réponse hémodynamique et clinique par rapport à la monothérapie.
L’atteinte respiratoire est décrite dans les maladies inflammatoires du tube digestif, restant exceptionnelle dans la maladie de Crohn (MC) alors qu’elle est plus fréquente lors de la rectocolite hémorragique [1]. Une patiente de 46 ans consultait pour une toux sèche, associée à des sueurs nocturnes, un amaigrissement et une fébricule depuis 4 semaines, sans efficacité des bronchodilatateurs, des corticoïdes inhalés et des macrolides prescrits. Elle était fébrile à 38,8 °C, non hypoxique, sans dyspnée et l’auscultation pulmonaire était normale. Le syndrome inflammatoire était modéré (CRP 35 mg/L), sans hyperleucocytose. Les hémocultures étaient négatives, tout comme la recherche de germes atypiques et la PCR SARS-COV-2. Cette patiente présentait comme antécédent notable une MC totalement quiescente sur le plan digestif depuis 2016 et non traitée, avec cependant des manifestations extra-digestives actives comme en témoignaient un épisode d’érythème noueux deux ans auparavant et une récente douleur de hanche finalement attribuée à une coxite, ayant bénéficié d’une infiltration et d’une corticothérapie dégressive interrompue depuis 2 mois, et pour laquelle elle devait débuter un traitement par méthotrexate prescrit par son rhumatologue. Un TDM thoracique montrait uniquement un épaississement trachéal diffus associé à une infiltration péritrachéale s’étendant sur chaque bronche souche, pouvant évoquer une trachéo-bronchite. La muqueuse trachéobronchique était très inflammatoire en endoscopie (réalisée 1 semaine après mise en place d’une corticothérapie), surtout au niveau de la trachée, des bronches souches et de la lobaire supérieure droite. Les biopsies de discrètes structures arrondies trachéales mettaient en évidence une inflammation lymphocytaire avec d’exceptionnelles cellules macrophagiques géantes multinucléées sans granulome organisé. L’analyse microbiologique du LBA ne montrait aucune bactérie ou mycobactérie, virus, champignon ou parasite pathogène. Devant l’absence d’arguments pour une autre cause et l’aspect des biopsies, la trachéo-bronchite a été considérée comme une atteinte extra-digestive de la MC. L’évolution était favorable sous corticothérapie à 1 m/kg/j rapidement dégressive. Un traitement par anti-TNF a été débuté compte tenu de la coxite associée. L’atteinte respiratoire de la MC est le plus souvent de type trachéo-bronchite, mais peut également concerner le parenchyme. Elle apparaît le plus souvent lorsque la maladie digestive est inactive (parfois même après colectomie dans le cas de la recto-colite hémorragique). La physiopathologie est hypothétique : poumon et intestin ont la même origine embryologique, ont tous deux un tissu lymphoïde sous-muqueux qui joue un rôle crucial dans la défense contre les antigènes exogènes [2]. D’autres pensent que ce sont des médiateurs inflammatoires ou des antigènes libérés par la rupture de la muqueuse intestinale qui provoquent les manifestations extradigestives [1], [2]. Il faut savoir évoquer les diagnostics différentiels et notamment une sarcoïdose, des infections banales ou opportunistes survenant sous immunosuppresseurs, des complications autres induites par les traitements (atteintes secondaires à la sulfasalazine, à la mésalazine ou aux thiopurines à type de pneumonie à éosinophiles, alvéolite fibrosante et pneumopathies interstitielles ou encore pneumopathie d’hypersensibilité au méthotrexate). Les manifestations trachéobronchiques répondent généralement bien aux corticoïdes topiques ou généraux. Le pronostic des atteintes trachéales est plus réservé. Il faut savoir évoquer ce diagnostic rare chez un patient atteint de MC, même à distance de toute poussée de la maladie.
Introduction: Pulmonary hypertension in COPD (COPD-PH) is a common complication generally proportionate to the severity of bronchial obstruction. Severe PH is less common (1 to 10% of COPD patients) but has a major impact on morbi-mortality. Computed tomography (CT) and histopathological features of COPD-PH are still incompletely known. We aimed to describe CT and histopathological lesions in patients with end-stage COPD. Methods: 59 patients who received lung transplantation for terminal COPD were retrospectively included. We divided the population into 3 PH severity groups based on the 6th World Symposium on PH classification. We performed CT-Scan quantification of emphysema and measured pulmonary artery to ascending aorta ratio (PA/Ao). We analyzed the following histopathological characteristics in explanted lungs after transplantation: small pulmonary arteries layers, neo-muscularization of pre-capillary vessels and capillary density. Results: In the severe PH group compared to the moderate PH group, the PA/Ao ratio was significantly higher (p<0.00002); the media layer of small pulmonary arteries and the neo-muscularization of pre-capillary vessels were significantly increased (p=0.0004 and p<0.0001 respectively), and the capillary density was lower (p=0.01). No lesions suggestive of primary pulmonary arterial hypertension were found. Conclusion: The PA/Ao ratio could be an interesting noninvasive tool to detect severe COPD-PH. Severe COPD-PH is associated with microvascular remodeling and capillary pruning.
Rationale and Objectives Studies suggest an association between chest CT findings assessed with semiquantitative CT score and gravity of Covid-19. The objective of this work is to analyze potential correlation between visual quantification of lesion severity at initial chest CT and clinical outcome in confirmed Covid-19 patients. Materials and Methods From March 5 th to March 21 st , 2020, all consecutive patients that underwent chest CT for clinical suspicion of Covid-19 at a single tertiary center were retrospectively evaluated for inclusion. Patients with lung parenchyma lesions compatible with Covid- 19 and positive RT-PCR were included. Global extensiveness of abnormal lung parenchyma was visually estimated and classified independently by 2 readers, following the European Society of Thoracic Imaging Guidelines. Death and/or mechanical ventilation within 30 days following the initial chest CT was chosen as the primary hard endpoint. Results 216 patients (124 men, 62 years-old, range 22 ; 94 yo) corresponding to 216 chest CT were included. Correlation between lesion severity and percentage of patients that met the primary endpoint was high, with a coefficient p; of 0.87 (p = 0.05). A greater than 25% involvement was significantly associated with a higher risk of mechanical ventilation or death at 30 days, with a Risk Ratio of 5.00 (95% CI [3.59- 6.99]). Conclusion This retrospective cohort confirms a correlation between visual evaluation of lesions severity at initial chest CT and the 30 days clinical outcome of Covid-19 patients and suggests using a threshold of greater than 25% involvement to identify patients at risk.
Background. - Scant data are currently available about a potential link between comorbid chronic lung diseases (CLD) and the risk and severity of the coronavirus disease 2019 (COVID-19) infection. Methods. - To describe the clinical characteristics of and outcomes for patients with COVID-19 infection, including patients with comorbid respiratory diseases, who have been primarily hospitalized in the pulmonology department of Strasbourg University Hospital, France. In this retrospective, single-center study, we included all confirmed cases of COVID-19 from March 3 to April 15, 2020. We then compared the symptoms, biological and radiological findings, and outcomes for patients with and without CLD. Results. - Of the 124 patients that were enrolled, the median age was 62 years, and 75 patients (60%) were male. Overall, 40% of patients (n = 50) had preexisting CLD, including chronic obstructive pulmonary disease (COPD) (n = 15, 12%) and asthma (n = 19, 15%). Twenty-eight patients were transferred to the intensive care unit (ICU), and six patients died in our unit. CLD were not predictive of ICU hospitalization, but a significantly higher total mortality was observed (17.6% vs. 5.5%, P < 0.05) in these patients. Conclusions. - Our results suggest the lack of an over-representation of CLD in COVID-19, representing 40% of patients in this cohort and even within a pulmonology department. CLD were not a risk factor for ICU management. However, a tendency to higher global mortality was observed in COVID-19 patients with CLD. Further studies are warranted to determine the risk of COVID-19 for patients with comorbid CLD.
AIM:To assess the value and efficacy of real-time shear-wave elastography (SWE) of normal testicular parenchyma and various common testicular diseases in clinical practice. MATERIALS AND METHODS:SWE was undertaken in 338 patients (mean age: 43.2±17.2 years, range 17-78 years) comprising normal testicles (n = 358), testicular microlithiasis (n = 40), and various testicular diseases (n = 208) and the stiffness was recorded. The final diagnosis was correlated with the clinical context, long-term follow-up, or histopathology. Statistical evaluation was performed to provide a stiffness threshold for pathological diagnosis. RESULTS:The mean size of testicular lesions was 2.6±1.5 cm (range: 10-42 mm). The mean Young's modulus value for normal testis was recorded at 4.55±2.54 kPa. Whatever the stage of microlithiasis, a higher statistically significant stiffness value was recorded. For acute orchitis, the mean stiffness value was slightly higher, but not statistically significantly. The testicular tumoural processes presented a median stiffness value of 21.02 kPa with a cut-off of 16.1 kPa. Fibrosis presented the highest median stiffness value of 30.03 kPa with a cut-off of 26.3 kPa. By analysing the distribution of the different pathological groups, the difference was statistically significant between fibrosis and tumoural processes (p = 0.001). CONCLUSION:SWE is a feasible technique in the exploration of the testicular parenchyma. SWE values can be used to differentiate testicular fibrosis from a tumoural process with confidence.
BACKGROUND:The main complication after hypofractionated radiotherapy for lung carcinoma is radiation-induced lung toxicity, which can be divided into radiation pneumonitis (acute toxicity, occurring within 6 months) and lung fibrosis (late toxicity, occurring after 6 months). The literature describes several predictive factors related to the patient, to the tumor (volume, central location), to the dosimetry and to biological factors. MATERIALS AND METHODS:This study is a retrospective analysis of 90 patients treated with stereotactic body irradiation for stage I non-small-cell lung carcinoma between December 2010 and May 2015. RESULTS:Radiation pneumonitis was observed in 61.5% of the patients who were mainly asymptomatic (34%). Chronic obstructive pulmonary disease was not predictive of radiation pneumonitis, whereas active smoking was protective. Centrally located tumors were not more likely to result in this complication if the radiation schedule utilized adapted fractionation. In our study, no predictive factor was identified. Whereas the mean lung dose was a predictive factor in 3D radiotherapy, the lung volume irradiated at high doses seemed to be involved in the pathogenesis after hypofractionated radiotherapy. CONCLUSION:The discovery of predictive factors for radiation pneumonitis is difficult due to the rarity of this complication, especially with an 8×7.5Gy schedule. Radiation pneumonitis seems to be correlated with the volume irradiated at high doses, which is in contrast to the known knowledge about the organs in parallel. This finding leads us to raise the hypothesis that vessel damage, organs in series, occurring during hypofractionated radiotherapy could be responsible for this toxicity.
Le scanner thoracique a montré de bonnes performances diagnostiques dans la maladie à coronavirus 2019 (COVID-19), avec une sensibilité supérieure à la PCR. L'objectif principal de ce travail était d'évaluer la valeur pronostique du scanner thoracique avec quantification de l'atteinte pulmonaire réalisé à l'admission à l'hôpital des patients atteints de COVID-19. Nous avons étudié prospectivement 572 patients avec un diagnostic de COVID-19 confirmé par PCR et ayant bénéficié d'un scanner thoracique systématique à l'admission à l'hôpital. La quantification des lésions pulmonaires, opacités en verre dépoli et/ou condensations, a été réalisée de manière visuelle par deux radiologues pour permettre de classer les patients selon le degré de l'atteinte : scanner normal, atteinte minime (0–10 %), modérée (11–25 %), importante (26–50 %), sévère (51–75 %) ou critique (> 75 %). Le critère de jugement principal était la survenue d'une forme sévère de la maladie, définie comme un décès ou un transfert en réanimation dans les 7 jours suivants l'admission. La moyenne d'âge était de 66 ± 16 ans et 343 (60 %) patients étaient des hommes. Le scanner a retrouvé une atteinte minime, modérée, importante, sévère et critique chez 68 (11,9 %), 224 (39,2 %), 171 (29,9 %), 82 (14,3 %), et 13 (2,3 %) patients, respectivement. La majorité des patients avait une atteinte bilatérale (524/572, 91,6 %) et des lésions en verre dépoli (540/572, 94,4 %). Au total, 206 (36 %) patients ont présenté une forme sévère de COVID-19, dont 55 (9,6 %) sont décédés. L'extension des lésions au scanner était corrélée au pronostic, avec 66/95 (69,5 %) patients ayant une atteinte > 50 % qui ont développé une forme sévère, contre 70/306 (22,9 %) patients ayant une atteinte ≤ 25 % (odds ratio = 7,6 ; intervalle de confiance 95 % [4,5–13,3] ; p < 0,01). Il n'y avait pas de différence en termes d'âge ou de comorbidités selon le degré d'atteinte au scanner. Les patients ayant une atteinte > 50 % avaient une CRP moyenne significativement plus élevée que ceux avec des lésions ≤ 25 % (164 mg/L versus 80 mg/L, p < 0,01). Une atteinte bilatérale et la présence de condensations au scanner étaient associées à un mauvais pronostic (p < 0,01 et p < 0,01, respectivement). Aucun des 14 patients ayant un scanner normal n'a développé de forme sévère. Le délai moyen entre le début des symptômes et la réalisation du scanner était de 6,6 ± 3,4 jours. Le scanner avait une valeur pronostique quelle que soit la durée d'évolution de la maladie, mais il était plus discriminant chez les patients présentant des symptômes depuis 5 à 10 jours. Le scanner thoracique, en plus d'être un outil diagnostique, aurait un intérêt pronostique grâce à la quantification visuelle de l'atteinte pulmonaire qui semble corrélée à une atteinte sévère précoce. Le délai entre l'apparition des symptômes et sa réalisation doit être pris en compte dans son interprétation.
Abstract Aim: Stereotactic body radiation therapy for lung tumours can expose patients to radiation pneumonitis (RP) (<6 months after irradiation) and lung fibrosis (beyond 6 months). The aim of this study was to describe post-irradiation radiographics appearances. Materials and methods: This retrospective study of 90 patients with a stage I non-small cell lung carcinoma reports a detailed description of the computed tomography (CT) or positron emission tomography/CT changes that can be observed after treatment, according to modified Kimura score for RP and Koenig’s classification for fibrosis. This evaluation was realised at 1 month and then every 3–4 months, with a median follow-up of 35 months. Results: The most common radiological RP pattern was diffuse consolidation. It appears in a mean time of 4 months and reaches its maximum at 9 months after radiotherapy. Seventy-three per cent of the RP evolved to fibrosis. Most of these findings were encompassed in the 35 Gy isodose. Findings: Radiological parenchymal changes are frequent in the treatment region, which renders the tumour response monitoring by tumour size, particularly by response evaluation criteria in solid tumours, unsuitable.
L’ischémie aiguë de membre est une urgence dont le diagnostic est clinique, basé sur une douleur intense, un membre froid et pâle et une abolition des pouls. L’imagerie a pour objectif d’en établir la cause et de guider la stratégie de revascularisation. Elle est recommandée dans les ischémies aiguës sans déficit neurologique, et ne devra pas retarder la prise en charge lorsqu’un déficit est présent. Dans le cadre de l’urgence, l’angioscanner est actuellement l’examen offrant les informations de meilleure qualité. Son usage peut donc être préconisé dès lors qu’une cause évidente (embolie sur artères saines, thrombose aiguë d’un pontage récent) n’est pas certaine. Il devra être réalisé au plus vite, et dans tous les cas sans délai dès qu’il s’agit d’une ischémie aiguë avec déficit neurologique.
La fibrose rétropéritonale est une affection rare caractérisée par le développement de tissu fibreux au niveau du rétropéritoine enserrant parfois les uretères aboutissant à une uropathie obstructive. La forme bénigne idiopathique isolée ou associée à d’autres localisations s’intègre actuellement dans la cadre d’une maladie immunologique appelée « maladie des IgG4 ». Son diagnostic repose sur l’élévation sérique des IgG4, le contexte clinique, l’aspect en imagerie en coupes et dans certains cas le recours à l’analyse anatomopathologique. Des localisations isolées atypiques sont possibles. Le diagnostic différentiel comprend principalement le lymphome et la carcinose rétropéritonale.

Purpose.-To date, no correlation has been found between clinical and radiological efficacy after irradiation of skull base meningiomas. However, the evaluation of the radiological response was most often made by questionable methods that may have underestimated the radiological effectiveness of radiotherapy. The objective of this work is to verify this hypothesis by quantitative volumetric analysis. Material and methods.-Data from 35 patients treated with either helical tomotherapy (45.7%) or fractionated stereotactic radiotherapy (54.3%) were retrospectively analysed. These were mainly women (94%) aged 59 (43-81) with lesions mainly of the cavernous sinus (60%). There was a median of 2 (1-4) symptoms and the main symptoms were visual impairment (39%), cranial nerve deficits (23.4%) and headaches (17.2%). Results.-Median tumour volume decreased significantly (P<0.05) from 9.6 mL (0.3-36.6) to 6.8 mL (0.1-26.5) after median follow-up of 44 months (24-77). Sixty-three percent of patients had an improvement of at least one symptom. In univariate analysis, clinical efficacy (P<0.05), radiotherapy technique (P<0.05), tumor topography (P<0.05) and initial tumor volume (P<0.05) were predictive factors for radiological response. In multivariate analysis, only the inverse correlation between radiological response and initial tumor volume remained significant (rho: -0.47 95% CI -3.2 to 5.7; P<0.05). Conclusion.-The quantitative volumetric monitoring demonstrates a major radiological efficiency of radiotherapy. However, no clear correlation between clinical and radiological efficacy was found. To report frequency, etiology and severity of left ventricle (LV) myocardial calcifications (MC) and its impact on prognosis. We retrospectively analyzed 47,042 CT scanners between 2009 and 2017, 3D multiplanar reconstructions used to determine the location and extension of MC. Detailed clinical, echocardiogram and coronary angiography data were recorded. Clinical follow up was obtained at a mean of 91 months. 44 (0.09%) patients had MC, aged 67 ± 15 y; 25 (56.8%) had previous history of MI (I-MC) and 19 (43.2%) non-ischemic origin (NI-MC): 3 renal failure, 3 valvular cardiomyopathy, 1 traumatic, 2 post-cardiac surgery, 1 post-radiotherapy, 2 sarcoidosis, 1 myocarditis, 1 hydatid cyst, 1 endomyocardial fibrosis and 3 idiopathic MC. Apical wall and sub-endocardial topography of MC was more frequent I-MC compared to NI-MC (16 vs. 3, P = 0.002; 10 vs. 2, P = 0.03). None of I-MC were associated with cardiac extra-LV MC (0 vs. 6, P = 0.003). I-MC had lower LVEF, higher LV volume and LV fat metaplasia than NI-MC (20 vs. 6, P = 0.001; 10 vs. 1, P = 0.02; 10 vs. 0, P = 0.001). At follow-up, the widespread of LV MC was associated cardiac death and ventricular arrhythmia (r = 0.58, P = 0.0001; r = 0.35, P = 0.01). Altered LVEF was more frequent in the I-MC vs. NI-MC (80% vs. 31%, P = 0.001). Left ventricle MC is very rare. The dystrophic MC from ischemic origin is the most frequent. At long-term follow-up, 59% of MC had altered LVEF, it is more frequent (80%) in I-MC. The extent of LV MC was associated with high risk of cardiac death and ventricular arrhythmia.
Purpose: To evaluate the best collimation used in wide volume (WV) mode to cover the abdomen in computed tomography (CT) urography in terms of radiation dose and image quality. Materials and methods: This study was performed on a 320 x 0.5 mm detector row CT unit. The first part identified the lowest volume CT dose index (CTDIvol) by using the topograms data of 25 medium size patients (13 men and 12 women; mean age: 52+9 [SD] years; age range: 46-68 years) using different collimations on WV from 6 cm to 16 cm and the one of the helical mode for the same coverage length. The second part consisted of a clinical evaluation result including 45 medium size patients (32 men and 13 women; mean age: 68 +/- 14 [SD] years; age range: 45-72 years). The qualitative evaluation included several items based on a 5-point Likert scale. Results: The first part of the study indicated that a collimation of 10cm (200 x 0.5mm) in WV mode with 5 volumes had the lowest CTDIvol (2.78 +/- 0.35 mGy; range: 2.35-3.21 mGy) compared to helical mode (4.38 +/- 0.48 mGy, range: 3.75-4.95 mGy). In the second part, the mean radiation dose reduction by comparison with helical mode was 44.03% +/- 0.36% (P< 0.001) and 51.16% +/- 1.22% (P< 0.005) for CTDIvol and DLP, respectively. Conclusion: Wide volume mode of the abdomen can be performed with a significant radiation dose reduction with a collimation of 10cm (200 x 0.5 mm) and five volumes. Skull base meningioma leads to functional disturbances, which can significantly alter the quality of life. The optimal management of these lesions, whose goals are neurological preservation and tumour local control, is not yet clearly established. It is widely recognized that the goal of a radical excision should be abandoned despite the advances in the field of microsurgery of skull base lesions. Although less morbid, partial tumour excision would be associated with increased risk of local tumour recurrence. Although discussed both exclusive and adjuvant have proven to be highly successful in terms of clinical improvement and local control. Various radiation techniques have demonstrated their efficacy in the management of this pathology. However, high rates of clinical improvement are in contrast with low rates of radiological improvement. The notion of clinical and radiological dissociation appeared. However, in most of these studies, the analysis of the radiological response could be subject of legitimate criticism. This work proposes to review the local control, the efficacy and the clinical tolerance and the radiological response of the various radiation techniques for the meningioma of the base of the skull and to demonstrate the interest of quantitative volumetric analyses in the follow-up of meningioma after radiotherapy. PURPOSE:To qualitatively and quantitatively compare unenhanced ultra-low-dose chest computed tomography (ULD-CT) acquired at 80kVp and 135kVp. MATERIALS AND METHODS:Fifty-one patients referred for unenhanced chest CT were prospectively included. There were 29 men and 22 women, with a mean age of 64.7±11.6 (SD) years (range: 35-91 years) and a mean body mass index of 26.2±6.3 (SD) (range: 17-54.9). All patients underwent two different ULD-CT protocols (80kVp-40mA and 135kVp-10mA). Image quality of both ULD-CT examinations using a 5-level scale as well as assessability of 6 predetermined lung parenchyma lesions were blindly evaluated by three radiologists and compared using a logistic regression model. Image noise of the two protocols was compared with Wilcoxon signed-rank test. RESULTS:The mean dose-length product at 80kVp and at 135kVp were 14.7±1.8 (SD) mGy.cm and 15.6±1.9 (SD) mGy.cm, respectively (P<0.001). Image noise was significantly lower at 135kVp (58.9±12.4) than at 80kVp (74.7±14.5) (P<0.001). For all readers and for all examinations, the 135kVp protocol yielded better image quality than 80kVp protocol, with a mean qualitative score of 4.5±0.7 versus 3.9±0.8 (P<0.001). The 135kVp protocol was significantly more often of diagnostic quality than the 80kvp protocol (92.3% versus 77.8%, respectively) (P<0.001) and was less prone to image quality deterioration in obese patients. Parenchymal lesions were never better depicted on the 80kVp protocol than with the 135kVp protocol. CONCLUSION:Unenhanced chest ULD-CT should be acquired at a high kilovoltage and low current, such as 135kVp-10mA, over a low kilovoltage and high current protocol.
