BackgroundBulevirtide (BLV) received EMA approval for treatment of chronic compensated hepatitis due to Delta virus (HDV) infection, however real-life data in large cohorts of patients with cirrhosis are lacking.MethodsConsecutive HDV patients with cirrhosis starting BLV 2 mg/day since September 2019 were included in a retrospective multicenter real-life European study (SAVE-D). Patients’ characteristics before and during BLV treatment were collected. Virological, biochemical, combined responses, adverse events and liver-related events (HCC, decompensation, liver transplant) were assessed.Results215 patients with HDV compensated (CPT-A) cirrhosis receiving BLV monotherapy for a median of 72 (24-120) weeks were included: at BLV start, age was 49 (18-81) years, 60% men, ALT 78 (23-1,074) U/L, liver stiffness measurement (LSM) 18.3 (6.4-75.0) kPa, platelets 91 (17-454) x 103/mm3, 54% with varices, 8% HIV-positive, 6% with previous ascites, 6% with active HCC, 91% on NUC, median HDV RNA 5.5 (1.5-8.5) log IU/mL and HBsAg 3.7 (0.8-4.7) log IU/mL. Virological responses and HDV-RNA undetectability at W24, W48, W72, W96 were 54%, 68%, 68%, 77% and 18%, 32%, 41% and 46%, respectively. Biochemical and combined responses were 54%, 63%, 60%, 57%, and 34%, 48%, 48%, and 50%, respectively. Patients with <1 log HDV-RNA decline vs. baseline declined from 21% at W24 to 8% at W96. AST, GGT, albumin, IgG and LSM values significantly improved throughout treatment. Bile acids significantly increased, 9% patients reported mild and transient pruritus, 3% injection site reactions. The W96 cumulative risks of de-novo HCC (n=7) and decompensation (n=6) were 3.8% (95% CI 2-7%) and 3.2% (95% CI 1-6%), respectively. 7 patients underwent liver transplantation and 4 died of BLV-unrelated causes.ConclusionsBLV 2 mg/day monotherapy up to 96 weeks was safe and effective in patients with HDV-related compensated cirrhosis. Virological and clinical responses increased over time and liver-related events were few.
Les progrès en transplantation hépatique et les changements épidémiologiques récents ont conduit à une augmentation du nombre de receveurs âgés de plus de 65 ans, et ce malgré un contexte de pénurie d’organes. Nous avons étudié l’impact de l’âge avancé des receveurs sur la mortalité à cinq et dix ans d’une transplantation hépatique. Les patients majeurs transplantés pour la première fois entre 2007 et 2017, enregistrés dans la base nationale de l’Agence de la biomédecine française étaient inclus. Des modèles flexibles de survie observée et de survie nette ont été utilisés pour tenir compte des effets dépendants du temps et/ou non log-linéaires et de la mortalité attendue pour chaque patient compte tenu de son sexe et de son âge. Au total, 7610 patients ont été inclus (âge moyen à la greffe de 55 ans [± 10,5]). Plus les receveurs étaient âgés, meilleure était leur fonction hépatique. Les analyses de survie observée comme de survie nette ont montré une association significative entre l’augmentation de l’âge du receveur et le risque de décès. Cependant si cet effet péjoratif était majeur dans la première année suivant la greffe, il diminuait progressivement au cours du suivi. Une différence absolue de 8 % (50 % versus 42 %) de survie nette persiste malgré tout à 10 ans entre des receveurs de 28 et 71 ans. Malgré une sélection des candidats âgés, l’âge à la greffe était associé à une augmentation du risque de décès, particulièrement marquée dans la première année suivant la transplantation. Cet effet diminuait ensuite mais restait présent à cinq et dix ans de la greffe, y compris après prise en compte de la mortalité attendue par âge et par sexe dans la population française.
Purpose: Anastomotic biliary strictures (ABS) remain one of the most frequent complications after liver transplantation (LT). This study aims to identify ABS risk factors. Additionally, ABS management and impact on survival were assessed. Methods: All patients who underwent liver transplantation with duct-to-duct anastomosis between 2010 and 2019 were included. All patients who died within 90 days post-LT after LT were excluded. Results: Among 240 included patients, 65 (27.1%) developed ABS. Median time to ABS was 142 days (range, 13-1265). Upon multivariable analysis, donor BMI (p=0.026), post-LT CMV primoinfection or reactivation (p<0.001) and the occurrence of post-LT anastomotic biliary fistula(p=0.014) were independent predictive factors of ABS. Anastomotic technical difficulty did not independently impact the risk of ABS (p=0.066). First-line ABS treatment was systematically endoscopic (100%), requiring a median of 2 (range, 1-11) procedures per patient. Twelve patients (18.5%) eventually developed ABS recurrence, of which nine were managed endoscopically, two percutaneously and one surgically. Repeat LT was not required in patients with ABS. The occurrence of ABS was associated with a shorter overall survival after LT (55 vs. 65 months; p=0.019). Conclusion: The risk of developing ABS after LT is multifactorial but seems mostly driven by the occurrence of postoperative events such as CMV infection especially primoinfection and anastomotic biliary fistula. Albeit generally managed endoscopically, ABS did impact negatively survival after LT.
Le rôle de l’endotoxémie liée à la translocation bactérienne sur l’aggravation de la cirrhose fait l’objet de nombreuses études, mais son impact sur la mortalité au cours de la cirrhose est mal connu. Le but de ce travail était d’étudier l’impact de la concentration sanguine de 3-hydroxymyristate (3-HM) total (composant spécifique des lipopolysaccharides, endotoxine), sur le risque de décès et de complications des malades cirrhotiques. Au total, 593 patients atteints de cirrhose alcoolique ou virale, sans carcinome hépatocellulaire ont été inclus entre 2008 et 2012 dans six centres hospitalo-universitaires français. Le dosage de 3-HM total, libre et lié aux lipoprotéines a été réalisé en l’absence de complication aiguë de la cirrhose par une méthode innovante et brevetée de dosage massique des LPS. Le suivi annuel pendant cinq ans a permis de relever la survenue de complications ou du décès. L’analyse de survie a été réalisée par un modèle de Cox multivarié avec stratification sur le centre et la sévérité de la cirrhose. Les patients avec une concentration de 3-HM élevée avait une cirrhose plus sévère que les autres selon les scores de Child-Pugh et de Meld (p < 0,001). Après cinq ans de suivi, 271 (45,7 %) patients sont décédés. Une concentration élevée de 3-HM total était significativement associée au risque de décès pour les patients Child A (HRTertile 3 vs Tertile 1 = 3,86, p < 0,001). Cette association n’était pas retrouvée pour les patients Child B out C. La concentration élevée de 3-HM total était associée à la survenue d’une première complication (p < 0,0001), ascite (p = 0,01) ou encéphalopathie (p = 0,008) mais pas à la survenue d’hémorragie digestive. Une concentration élevée de 3-HM, marqueur spécifique de l’endotoxémie, est associée à un sur-risque de décès à cinq ans chez les patients avec cirrhose alcoolique ou virale compensée (Child Pugh A) suggérant un rôle majeur de la translocation bactérienne dans l’aggravation de la cirrhose.
Les dermatoses à élimination transépidermique (dermatoses perforantes) sont définies par la migration transépithéliale de structures dermiques. Nous rapportons ici le cas d’une forme survenue sous sorafénib (inhibiteur multikinase). Un homme de 51 ans consultait pour l’apparition en mars 2019 de nodules douloureux des membres inférieurs et des fesses, d’évolution nécrotique. Il était traité depuis août 2018 par sorafénib pour un carcinome hépatocellulaire infiltrant avec thrombose porte tumorale. Ses autres antécédents comportaient un diabète de type 2 sous insuline, une maladie de Basedow traitée par carbimazole et lévothyroxine et un psoriasis en plaque des membres supérieurs. Le patient était apyrétique. L’éruption était faite de papules et nodules violacées, au centre nécrotique, hyperalgiques. Certaines lésions hyperkératosiques pouvaient faire évoquer un psoriasis. La biopsie d’une lésion mettait en évidence en coloration HES un cratère endophytique avec invagination de l’épiderme surmonté d’un clou hyperkératosique. La coloration à l’orcéine distinguait par endroit une verticalisation des fibres élastiques, posant le diagnostic de dermatose à élimination transépidermique. Un arrêt de 1 mois du sorafénib permettait la régression des nodules. La reprise du sorafénib à pleine dose entraînait une réaggravation rapide de l’éruption. Le patient souhaitait stopper de nouveau ce traitement car l’éruption était trop douloureuse et l’arrêt était validé de manière collégiale avec les gastro-entérologues. Le matériel dermique expulsé est souvent polymorphe (fibres élastiques, collagène…). Une association pathologique avec des maladies chroniques est classique (86,4 %) : insuffisance rénale chronique (72,7 %), diabète (50 %). On note aussi une association avec certains cancers (carcinome hépatocellulaire, carcinome rénal), lymphomes et certaines maladies infectieuses. Dans la littérature, le rôle des traumatismes comme facteurs favorisant la survenue des lésions a été mis en évidence chez 3 patients traités par sorafénib. Le sorafénib, inhibiteur des métalloprotéases, entraîne une diminution de la dégradation des fibres élastiques, à l’origine de la formation des lésions. Il n’existe pas de consensus concernant la prise en charge des dermatoses perforantes et les traitements topiques (dermocorticoïdes, rétinoïdes) sont souvent inefficaces. Un cas de dermatose perforante a été traité par isotrétinoïne, permettant en 5 mois une régression des lésions avec un maintien du traitement par sorafénib. La dermatose perforante induite par le sorafénib est rare. Sa prise en charge est mal codifiée, l’arrêt du traitement inducteur permet la guérison mais ne doit pas être systématique en raison du bénéfice oncologique. Dans notre cas, même si le traitement par sorafénib était efficace sur le carcinome hépatocellulaire multifocal il a été décidé avec le patient de ne pas le reprendre à cause des effets indésirables cutanés.
BACKGROUND:Patients with primary biliary cholangitis who have an inadequate response to therapy with ursodeoxycholic acid are at high risk for disease progression. Fibrates, which are agonists of peroxisome proliferator-activated receptors, in combination with ursodeoxycholic acid, have shown potential benefit in patients with this condition. METHODS:In this 24-month, double-blind, placebo-controlled, phase 3 trial, we randomly assigned 100 patients who had had an inadequate response to ursodeoxycholic acid according to the Paris 2 criteria to receive bezafibrate at a daily dose of 400 mg (50 patients), or placebo (50 patients), in addition to continued treatment with ursodeoxycholic acid. The primary outcome was a complete biochemical response, which was defined as normal levels of total bilirubin, alkaline phosphatase, aminotransferases, and albumin, as well as a normal prothrombin index (a derived measure of prothrombin time), at 24 months. RESULTS:The primary outcome occurred in 31% of the patients assigned to bezafibrate and in 0% assigned to placebo (difference, 31 percentage points; 95% confidence interval, 10 to 50; P<0.001). Normal levels of alkaline phosphatase were observed in 67% of the patients in the bezafibrate group and in 2% in the placebo group. Results regarding changes in pruritus, fatigue, and noninvasive measures of liver fibrosis, including liver stiffness and Enhanced Liver Fibrosis score, were consistent with the results of the primary outcome. Two patients in each group had complications from end-stage liver disease. The creatinine level increased 5% from baseline in the bezafibrate group and decreased 3% in the placebo group. Myalgia occurred in 20% of the patients in the bezafibrate group and in 10% in the placebo group. CONCLUSIONS:Among patients with primary biliary cholangitis who had had an inadequate response to ursodeoxycholic acid alone, treatment with bezafibrate in addition to ursodeoxycholic acid resulted in a rate of complete biochemical response that was significantly higher than the rate with placebo and ursodeoxycholic acid therapy. Les personnes atteintes de cirrhose sont les plus à risque de développer un carcinome hépatocellulaire (CHC), cancer au pronostic sombre. Le foie joue un rôle central dans le métabolisme des nutriments, en particulier des lipides. Pourtant, les liens entre consommation de lipides, métabolisme lipidique et risque de CHC en cas de cirrhose ne sont pas encore bien définis. L’objectif de ce travail était d’étudier le profil en acides gras (AG) de la membrane érythrocytaire (reflet des apports alimentaires et du métabolisme) chez des personnes atteintes de cirrhose, selon la présence ou non d’un CHC. L’étude multicentrique CiRCE a inclus 1178 patients cirrhotiques sans CHC à l’inclusion (témoins) ou avec CHC histologiquement confirmé (cas). La composition en AG de la membrane des érythrocytes a été déterminée par chromatographie en phase gazeuse à haute performance chez 349 cas et 550 témoins. L’association entre le risque de CHC et la concentration de différents AG découpée en tertiles a été évaluée par régression logistique multivariée. Les cas étaient plus âgés que les témoins [âge médian : 64,2 versus 58,9 ans, (p = 0,04)] et plus souvent des hommes. Une association positive était mise en évidence entre le risque de CHC et la concentration en acide linoléique (18 :2 n–6) [Odds Ratio (OR) = 2,22 [Intervalle de Confiance (IC) à 95 % 1,43–3,43] (p < 0,001)] et en AG saturés spécifiques des produits laitiers (15 :0 + 17 :0) [OR = 4.53 [1,97–9,63] (p = 0,002)]. À l’inverse, les concentrations en acide stéarique (18 :0) et en acide arachidonique (20 :4 n–6) étaient plus faibles chez les cas que chez les témoins (respectivement [OR = 0,36 [0,23–0,56] (p < 0,001)], et [OR = 0,27 [0,15–0,50] (p < 0,001)]. Les profils en AG des membranes érythrocytaires étaient différents selon la présence ou non de CHC chez les personnes atteintes de cirrhose. Les activités enzymatiques semblaient également perturbées. Ces résultats et les hypothèses mécanistiques devront être confirmés dans une étude prospective.
Purpose: To assess the usefulness of a second biopsy when the first one was inconclusive in patients with a liver nodule found during the follow-up for chronic liver disease. Materials and methods: Among 381 patients (544 nodules) included in a prospective study designed to evaluate the accuracy of imaging for the diagnosis of small hepatocellular carcinoma (HCC) in chronic liver disease, 254 nodules were biopsied. The following histological results were considered as conclusive: HCC, dysplastic or regenerative nodule, and other identified tumors (benign or malignant). For nodules with inconclusive results (e.g. fibrosis or no definite focal lesion), a second biopsy was suggested, but was not mandatory. Results: A total of 242 patients (194 men, 48 women; mean age, 61.9 +/- 9.5 [SD]; range: 40.2-89.0years) with 254 nodules underwent a first biopsy. Mean nodule diameter was 19.2 +/- 5.4mm (range: 10-33mm). The first biopsy was conclusive in 189/254 nodules (74.4%): 157 HCCs (83.1%), 11 regenerative nodules (5.8%), 10 dysplastic nodules (5.3%), 3 cholangiocarcinomas (1.6%), and 8 other tumors (4.2%). Among the 65 nodules for which the first biopsy was inconclusive, a second biopsy was performed for 17 nodules in 16 patients within 6 months of the first one. It was conclusive in 13/17 nodules (76.5%): 10 HCCs (76.9%), 2 dysplastic nodules (15.4%), and 1 other tumor (7.7%). In 4/17 nodules (23.5%), no definitive diagnosis could be provided. Conclusion: The diagnostic yield of a second biopsy of a suspicious lesion suggestive of HCC in chronic liver disease is not decreased compared to the first one. Repeated biopsy after a first negative one could be an alternative option to the follow-up of patients with chronic liver disease. (C) 2018 Societe francaise de radiologie. SummaryBackgroundThe outcome of cholangiopathy developing in intensive care unit (ICU) is not known in patients surviving their ICU stay.AimTo perform a survey in liver units, in order to clarify the course of cholangiopathy after surviving ICU stay.MethodsThe files of the liver units affiliated to the French network for vascular liver disease were screened for cases of ICU cholangiopathy developing in patients with normal liver function tests on ICU admission, and no prior history of liver disease.ResultsBetween 2005 and 2015, 16 cases were retrieved. Extensive burns were the cause for admission to ICU in 11 patients. Serum alkaline phosphatase levels increased from day 11 (2‐46) to a peak of 15 (4‐32) × ULN on day 81 (12‐511). Magnetic resonance cholangiography showed irregularities or frank stenosis of the intrahepatic ducts, and proximal extrahepatic ducts contrasting with a normal aspect of the distal common bile duct. Follow‐up duration was 20.6 (4.7‐71.8) months. Three patients were lost to follow‐up; 2 patients died from liver failure and no patient was transplanted. One patient had worsening strictures of the intrahepatic bile ducts with jaundice. Nine patients had persistent but minor strictures of the intrahepatic bile ducts on MR cholangiography, and persistent cholestasis without jaundice. One patient had normal liver function tests.ConclusionsIn patients surviving their ICU stay, ICU cholangiopathy is not uniformly fatal in the short term or clinically symptomatic in the medium term. Preservation of the distal common bile duct appears to be a finding differentiating ICU cholangiopathy from other diffuse cholangiopathies.

This increase may be due to factors such as high HCV viral load in blood and semen, sex with risk of mucosal damage, a higher number of sexual partners, presence of concomitant ulcerative sexually transmitted diseases and the use of recreational drugs. The aim of our study was to investigate the dynamics of HCV transmission in an outbreak of acute hepatitis C in HIV-infected MSM in Barcelona. Methods: Between 2008 and 2013, 113 cases of acute hepatitis C in HIV-infected MSM were diagnosed in the Infectious Diseases Unit, Hospital Clinic, Barcelona. Phylogenetic analysis of the HCV NS5B gene was performed in a total of 70 patients. Viral RNA was extracted from serum samples collected from each patient at the time of diagnosis. Massive sequencing was performed using the Roche 454 GS Junior platform. To define possible transmission networks, phylogenetic trees and multidimensional scaling maps were constructed from genetic distance matrices (Da). Results: At the time of diagnosis of acute hepatitis C, 53 of the 70 (76%) patients included in the study were receiving antiretroviral therapy. HIV viral load was undetectable in 48 patients (69%) and the mean CD4 cell count was 923 cells /ul. HCV viral load was 6.37 log IU/mL (range 3.73–6.99). Thirty-five of 53 (66%) patients treated with pegIFN and ribavirin achieved a sustained virological response. The prevalence of HCV genotypes was: 4d 51% (n =36), 1a 40% (n =28), 1b 7% (n =5) and 3a 1% (n =1). Phylogenetic analysis showed the existence of at least 13 monophyletic groups: 5 of genotype 1a, 2 of genotype 1b and 6 genotype 4d. Molecular analysis showed that the genetic distances between genotype 4d viruses (Da 5.42) were significantly lower than those of the subtypes 1a (Da 18.50, p < 2.2×10−16) and 1b (Da 15.25, p < 1.1×10−6). This result may suggest the existence of a single source of infection for genotype 4d and different sources for subtypes 1a and 1b. Conclusions: HCV infection spreads rapidly among HIV-infected MSM through a local network in Barcelona. The implementation of public health campaigns and preventive measures, as well as treatment interventions with the new direct-acting antivirals will allow the development of strategies to reduce the HCV transmission of HCV within these high-risk groups.
Summary Background A phase I dose‐escalation trial of transarterial chemoembolisation ( TACE ) with idarubicin‐loaded beads was performed in cirrhotic patients with hepatocellular carcinoma ( HCC ). Aim To estimate the maximum‐tolerated dose ( MTD ) and to assess safety, efficacy, pharmacokinetics and quality of life. Methods Patients received a single TACE session with injection of 2 mL drug‐eluting beads (DEBs; DC Bead 300–500 μm) loaded with idarubicin. The idarubicin dose was escalated according to a modified continuous reassessment method. MTD was defined as the dose level closest to that causing dose‐limiting toxicity (DLT) in 20% of patients. Results Twenty‐one patients were enrolled, including nine patients at 5 mg, six patients at 10 mg, and six patients at 15 mg. One patient at each dose level experienced DLT (acute myocardial infarction, hyperbilirubinaemia and elevated aspartate aminotransferase ( AST ) at 5‐, 10‐ and 15‐mg, respectively). The calculated MTD of idarubicin was 10 mg. The most frequent grade ≥3 adverse events were pain, elevated AST , elevated γ‐glutamyltranspeptidase and thrombocytopenia. At 2 months, the objective response rate was 52% (complete response, 28%, and partial response, 24%) by modified Response Evaluation Criteria in Solid Tumours. The median time to progression was 12.1 months (95% CI 7.4 months – not reached); the median overall survival was 24.5 months (95% CI 14.7 months – not reached). Pharmacokinetic analysis demonstrated the ability of DEB s to release idarubicin slowly. Conclusions Using drug‐eluting beads, the maximum‐tolerated dose of idarubicin was 10 mg per TACE session. Encouraging responses and median time to progression were observed. Further clinical investigations are warranted ( NCT 01040559).
La Maladie de Wilson peut se présenter sous une forme hépatique ou neurologique. Nous rapportons le cas d'un patient présentant une insuffisance hépato-cellulaire aiguë et une anémie hémolytique conduisant à ce diagnostic. Un patient de 23 ans s'est présenté aux urgences pour ictère et vomissements. Dans ses antécédents, on retient deux paralysies faciales périphériques (1999 et 2011) idiopathiques, ainsi qu'un syndrome dépressif traité par Prozac® depuis début 2013. L'examen clinique note un ictère flamboyant et une asthénie majeure depuis 48 h, avec plusieurs épisodes [1] de vomissements alimentaires. Les urines sont foncées et les selles décolorées. Il n'y a pas de douleur abdominale spontanée ni provoquée, pas d'organomégalie, pas de fièvre. Le bilan biologique retrouve un ictère mixte, avec bilirubine totale à 244 μmol/L, dont respectivement 137 et 107 μmol/L de bilirubine conjuguée et libre, une insuffisance hépato cellulaire majeure (TP à 35 %, facteur V à 27 %), sans stigmate biologique de consommation. Les ALAT sont normales, les ASAT à 2 N, les GGT à 1,5 N, le PAL sont abaissées à 36 UI/L (N : 45–117). Il existe une anémie hémolytique à 9 g/dL, avec Coombs négatif et schizocytes < 1 %, ainsi qu'une thrombopénie à 98 000. La CRP est à 12 mg/L, la créatinine à 147 μmol/L et l'urée à 9,2 mmol/L. L'échographie abdominale est normale en dehors d'une splénomégalie à 16 cm. La sérologie de la leptospirose, des hépatites virales sont négatives, les ANA sont positifs au 1/640 avec fluorescence membranaire, sans auto-anticorps associés aux maladies hépato-biliaires auto-immunes. La céruléoplasmine est abaissée à 0,09 g/L, orientant le diagnostic vers une maladie de Wilson. Il n'est pas retrouvé d'anneau de Kayser-Fleisher. Une biopsie hépatique retrouve des lésions histologiques évocatrices de cirrhose (avec activité et fibrose marquées), et le dosage pondéral du cuivre hépatique est élevé à 8,55 μmol/g de tissu (N < 4 μmol/g). La fibroscopie œsogastrique objective des varices œsophagiennes de grade I. Enfin, les cuprémies libre et échangeable sont respectivement à 7,99 et 1,51 μmol/L, définissant un rapport cuivre échangeable/cuivre plasmatique à 18,9 %. La cupriurie des 24 h est élevée à 180 μg (N < 40 μg/24 h), et il est conclu à une maladie de Wilson. Des signes cliniques de cirrhose décompensée apparaissent rapidement. Le patient bénéficie d'une transplantation hépatique le 14/09/2014. La maladie de Wilson est maladie de surcharge rare dont la prévalence est estimée à 12 à 25 cas/million d'habitants, caractérisée par un déficit fonctionnel en ATP7B (mutation homozygote du gène codant pour cette protéine, sur le chromosome 13), entraînant un défaut d'excrétion biliaire du cuivre et ayant pour conséquence une accumulation tissulaire, avec un tropisme essentiellement hépatique et neurologique central, définissant 2 formes cliniques principales. Outre les tableaux d'hépatopathie chronique avec apparition progressive d'une cirrhose, sont également rapportés de rares cas d'hépatite fulminante (5 %), 1 comme dans le cas de notre patient. Il faut savoir évoquer le diagnostic de maladie de Wilson devant une insuffisance hépatocellulaire aiguë associée à une anémie hémolytique, d'où un ictère mixte, notamment chez des patients dans la 2e ou 3e décennie. L'atteinte neurologique est généralement absente, tout comme l'anneau de Kayser-Fleisher. Le diagnostic différentiel principal à évoquer est la leptospirose. Biologiquement, les PAL abaissées ainsi que l'absence de cytolyse sont de bons éléments d'orientation pour la maladie de Wilson. Le diagnostic est posé sur un faisceau d'arguments [1], et sera ensuite confirmé par la biologie moléculaire. Les outils utiles au diagnostic sont multiples : dosages de la céruléoplasmine, du cuivre plasmatique, cupriurie des 24 h, et enfin dosage pondéral du cuivre intrahépatique. Néanmoins, de nombreux faux-positifs existent, c'est pourquoi de nouvelles techniques de dosage basées sur le ratio cuivre échangeable/cuivre plasmatique ont été récemment étudiées et semblent prometteuses [2]. Il faut savoir évoquer le diagnostic de maladie de Wilson devant l'association d'une anémie hémolytique et d'une insuffisance hépatocellulaire aiguë.
