Mobile C-arms with the option of 3D imaging like the Iso-C-3D allow for intraoperative 3D visualization of anatomical areas with complex three-dimensional structures like articular surfaces. In an 8-year period we performed 1,841 intraoperative control scans following osteosynthesis. Among these patients we registered the number of intraoperative adjustments of fracture reduction and implant position in correlation to the area of surgery. The majority of intraoperative examinations in 1,841 patients was performed in fractures of the calcaneus (20.5%) and the upper ankle joint (13.2%). Altogether we improved the reduction or the implant position intra-operatively in 21.5%. The majority of intraoperative revisions was seen in osteosynthesis of the calcaneus (40.3%), the upper ankle joint (30.9%) and fractures of the distal tibia (29%). The rate of revisions over the time was very stable. Intraoperative need for revision of reduction or implant position is not a rare phenomenon in our experience. Intraoperative 3D imaging is a valid tool to recognize and adjust suboptimal reduction or implant positioning. Intraoperative 3D imaging can improve the quality of osteosynthesis especially in fractures of joints and complex anatomical areas.
Delayed fracture healing and non-unions of the femoral neck after lag screw osteosynthesis occur particularly in multiply injured young patients, and then surgical revision is often required. Currently no evidence-based treatment guidelines exist and therapeutic options include both hip arthroplasties and femoral head-maintaining operations. Here we report on young patients with delayed fracture healing of the femoral neck. Patients underwent revision surgery by autologous bone cylinder transplantation with mechanical re-stabilisation by cannulated lag screws.
Emergency response management and rescue operations concerning the railway network in Germany need special attention and implementation in several ways. The emergency response concerning the German national railway network managed by Deutsche Bahn AG is subject to various rules and regulations which have to be followed precisely. Only by following these rules and procedures is the safety of all emergency staff at the scene ensured. The German national railway network (Deutsche Bahn AG) provides its own emergency response control center, which specializes in managing its response to emergencies and dispatches an emergency response manager to the scene. This person serves as the primary Deutsche Bahn AG representative at the scene and is the only person who is allowed to earth the railway electrical power lines. This article will discuss different emergency situations concerning railway accidents and the emergency medical response to them based on a near collision with a high speed train during a rescue mission close to the railway track. Injury to personnel could only be avoided by chance and luck. The dangers and risks for rescue staff are specified. Furthermore, the article details practical guidelines for rescue operations around the German national railway track system.
Dargestellt wird eine Beinahekollision mit einem Hochgeschwindigkeitszug im Rahmen eines Rettungseinsatzes im Gleisbereich. Nur durch Zufall und Glück konnten Personenschäden verhindert werden. Bei Rettungseinsätzen im Eisenbahnbereich müssen die Meldewege der Deutschen Bahn AG unbedingt eingehalten werden. Nur so wird die Sicherheit der Rettungskräfte am Ereignisort gewährleistet. Die Deutsche Bahn AG verfügt über eigene Notfallleitstellen, die mit entsprechender Kompetenz ausgestattet sind. Die exekutive Kraft mit technischer Qualifikation vor Ort ist der Notfallmanager der Deutschen Bahn AG, der als einziger Bahnerdungen vornehmen darf und vor Ort Mitglied der Einsatzleitung ist. Dieser Beitrag informiert über Gefahren bei Einsätzen im Gleisbereich und gibt Handlungsempfehlungen für die Praxis.
Obwohl Schuss- und Stichverletzungen in Deutschland selten sind, muss an Krankenhäusern jeglicher Versorgungsstufen die akute Erstversorgung entsprechend Verletzter – auch in größerer Anzahl, wie beispielsweise nach einem Amoklauf – gewährleistet sein. Wichtige Voraussetzungen für ein gutes Outcome sind ein prioritätenorientiertes standardisiertes präklinisches Management, ein standardisiertes Schockraummanagement sowie, falls erforderlich, die rasche, pragmatische erste operative Phase im Sinne von „damage control surgery“. Im vorliegenden Beitrag werden Maßnahmen für Präklinik, Schockraum und erste operative Phase nach penetrierenden Höhlenverletzungen an Thorax und Abdomen vorgestellt.

Die meisten intraoperativen Revisionen wurden mit 40,3% bei Osteosynthesen bei Kalkaneusfrakturen, mit 30,9% bei Verletzungen des oberen Sprunggelenkes und mit 29% bei Pilonfrakturen gesehen. Im zeitlichen Verlauf war die Gesamtrevisionsrate sehr stabil. Ein intraoperativer Revisionsbedarf ist kein seltenes Phänomen. Die intraoperative 3D-Darstellung ist ein geeignetes Instrument, um suboptimale Implantatpositionierungen und Repositionsergebnisse zu erkennen und in gleicher Sitzung zu korrigieren.

Brüche der körperfernen Speiche sind die häufigsten Knochenbrüche Erwachsener. Die Beurteilung komplexer distaler Radiusgelenkfrakturen ist anhand von 2D-Röntgenprojektionen eingeschränkt und wird in der Diagnostik häufig durch eine computertomographische Schnittbilddiagnostik ergänzt. Intraoperativ liegen vergleichbare Umstände vor, zusätzlich bereitet die Beurteilung der Implantatlage Schwierigkeiten. Mit dem Siremobil® Iso-C3D und dem Arcadis® Orbic 3D der Fa. Siemens stehen Bildverstärker mit 3D-Option zur Verfügung. Aus 100 Einzelprojektionen in digitaler Radiographietechnik wird ein hoch auflösender isotroper 3D-Datenkubus von etwa 12 cm Kantenlänge generiert. Die Auflösung innerhalb des Kubus entspricht der einer Computertomographie. Im Rahmen einer prospektiven Studie wurden 122 Frakturen bei 114 Patienten untersucht. Durch die 3D-Darstellung wurden pathologische Befunde bei 41% der Untersuchungen aufgedeckt. Bei 36 Fällen (29%) wurde aufgrund dieses Ergebnisses die Indikation zur sofortigen Revision gestellt.
In most bone fractures sustained by adults the distal radius is the bone affected. Two-dimensional X-ray examination allows only limited assessment of complex distal radius fractures afffecting the joint and is often supplemented in the course of the diagnostic procedures by computer tomography. The situation during surgery is comparable, and in addition assessment of the implant's position also causes difficulty. Image intensifiers with a 3D option are available in the Siremobil (R) Iso-C 3D and the Arcadis (R) Orbic 3D from Siemens. A high-resolution isotropic 3D data cube with side length approximately 12 cm is generated from 100 individual projections obtained with a digital roentgenographic technique. The resolution within the data cube is as high as that of a computer tomograph. In a prospective study 122 fractures were investigated in 114 patients. The 3D imaging revealed pathologic findings in 41% of the examinations. In 36 cases (29%) this result was taken to indicate immediate revision surgery.
BACKGROUND:Nonunions of long bone fractures are considerable therapeutic and economic problems with increasing tendency. Basic surgical options are autogenous cancellous bone grafting, rod dynamization, reamed nailing, plate fixation with compression, and bone transport techniques. If these methods fail to work, alternative treatment options are needed.MATERIAL AND METHODS:Since May 2001 treatment with recombinant human (rh) bone morphogenic protein 7 [BMP-7 or osteogenic protein 1 (OP-1)] in combination with a type one collagen carrier has gained interest. BMP 7 induces the formation of new bone by differentiation of stem cells, thereby initiating the reaction cascade of osteogenesis. Nonunions over 9 months and unsuccessful bone grafting delineate the indication.RESULTS:We report our experience with 21 patients and nonunion of long bone fractures. Between July 2002 and June 2004, 23 units of BMP 7 were implanted. The implantation sites were 7 femora, 12 tibias, 2 humeri, and 2 forearms. In ten cases BMP 7 was combined with a new osteosynthesis and bone grafting and in five patients with bone grafting alone. In contrast in eight patients BMP 7 was applied as a single procedure without any bone grafting or any change of the osteosynthesis.CONCLUSION:There were no peri- or postoperative complications. Follow-up was obtained for a minimum of 6 months. Of 23 implantations, 22 were successful with bony healing revealed by clinical and radiological evaluation. In one patient no bony consolidation of the nonunion has been seen so far. In summary, based on our results we propose BMP 7 as an additional innovative therapy of long bone nonunions.
Rupturen der Achillessehne sind typische Sportverletzungen; die Inzidenz ist steigend. Der Altersgipfel liegt zwischen dem 30. und 45. Lebensjahr; Männer sind 5–10mal häufiger betroffen. Sie sind meist indirekte Folge einer plötzlichen Überdehnung der Sehne; eine direkte Gewalteinwirkung ist selten. Die Wahl der Therapie wird kontrovers diskutiert. Im Gegensatz zu den angloamerikanischen Ländern hat sich in Deutschland die operative Therapie durchgesetzt. Die offen-chirurgische Naht hat sich bewährt und gilt seit Jahrzehnten als Standardtherapie. Wesentlicher Vorteil der offenen Nahttechnik ist die geringe Rerupturrate; Nachteil der relativ hohe Anteil an Wundheilungsstörungen und Infekten. Durch Entwicklung perkutaner Nahttechniken konnte das Weichteiltrauma reduziert und die Wundheilungsstörungs- und Infektrate deutlich gesenkt werden; nachteilig ist jedoch eine deutlich erhöhte Rate an Läsionen des N. suralis. Die Entwicklung der Sonographie und der Magnetresonanztomographie bildet die Grundlage für die heute postulierte, differenzierte Therapie. Bei Adaptation der Sehnenstümpfe unter 20° Plantarflexion wird von vielen Autoren eine primär konservativ-funktionelle Behandlung empfohlen. Hohes Alter, Immunsuppression, pathologische Rupturen oder mangelnde Compliance des Patienten stellen Kontraindikationen zur Operation dar.
Pseudarthrosen stellen unverändert eine schwerwiegende Komplikation nach Schaftfrakturen dar und erfordern in vielen Fällen mehrfache operative Revisionen. Insbesondere bei therapierefraktären Pseudarthrosen werden zunehmend Wachstumsfaktoren zur Stimulation der Frakturheilung als Therapieoption diskutiert. OP-1 (bone morphogenetic protein 7, BMP 7) ist zur Zeit der einzige als Medikament zugelassene Wachstumsfaktor zur lokalen Applikation. Als Indikation gelten therapieresistente Pseudarthrosen nach erfolgloser autologer Spongiosaplastik. Im Rahmen einer prospektiven Studie wird zur Zeit in unserer Klinik die Wirkung bei alleiniger lokaler Applikation von OP-1 evaluiert. Es sollen nun die bisherigen vorläufigen klinischen Erfahrungen anhand erster klinischer Fallbeispiele dargestellt werden.
Computerassistierte Operationsverfahren in der Unfallchirurgie finden eine zunehmende Verbreitung. Das Prinzip liegt in einer Verknüpfung von Bildinformationen mit intraoperativen Instrumenten und Implantatpositionen. Ohne Navigationshilfe erfolgt diese Verknüpfung der teilweise sehr komplexen Bilddaten mit der Position des Instruments im Kopf des Operateurs. Die Bildinformationen können aus präoperativ angefertigten Bildern, wie z. B. der Computertomographie oder der Magnetresonanztomographie aber auch aus intraoperativ angefertigten Bildern mithilfe eines konventionellen C‐Bogens bestehen. Zur Bildgebung steht im Operationssaal in der Regel ein Röntgenbildverstärker, der eine Instrumentenposition in Echtzeit nur in einer Ebene verfolgen kann, zur Verfügung. Nur wenigen Zentren ist es vorbehalten, intraoperativ Schnittbilddiagnostik mit Computertomographie oder MRT, mit einem entsprechend erheblichen logistischen und personellen Aufwand, durchzuführen. In der unfallchirurgischen Routine kann ein für die Navigation ausgestatteter C-Bogen zu einer Verbesserung der Präzision und zu einer drastischen Reduktion der Strahlenbelastung für OP-Personal und den Patienten führen. Im folgenden Beitrag sollen das Prinzip, das Spektrum und der logistische und personelle Aufwand der C-Bogen-basierten Navigation dargestellt werden.
Zusammenfassung Anhand einer retrospektiven Follow-up-Studie unter der Verwendung von Matched-Pair-Variablen wurde der Heilverlauf von 64 individuell zugeordneten Patientenpaaren (128 Patienten) mit Frakturen des Unterarmschafts untersucht.Eine Gruppe wurde mit dem AO-Point-Contact-Fixator (PC-Fix), die andere mit 2,7 bzw. 3,5 mm DCP/LC-DCP behandelt. Die Heilverlaufsdaten stützen sich auf einen Zeitraum von 18 Monaten nach der Implantation. Es konnte bei allen Patienten ein Follow-up durchgeführt werden. Die Patienten wurden einander auf Grund der Kriterien: Frakturklassifikation, Weichteilschaden,Begleitverletzungen und Alter zugeordnet.Mögliche Komplikationen waren: implantationsbedingte Nervenschäden, Infektionen, Implantatversagen, Heilungsverzögerung,Pseudarthrosen,Bewegungseinschränkungen und Synostosen.Bei 13 Patienten traten Komplikationen auf (PC-Fix Gruppe: 8 vs. DCP Gruppe: 5). Die Signifikanztestung ergab bei einem p-Wert von 0,5811 keinen statistisch signifikanten Vorteil für eines der beiden Implantate.
After experimental and preclinical evaluation (HAP Paul Award 2001) of a CT-free image-guided surgical navigation system for acetabular cup placement, the system was introduced into clinical routine. The computation of the angular orientation of the cup is based on reference coordinates from the anterior pelvic plane (APP) concept. A hybrid strategy for pelvic landmark acquisition has been introduced involving percutaneous pointer-based digitization with the noninvasive biplanar landmark reconstruction using multiple registered fluoroscopy images. From January 2001 to May 2002, 118 consecutive patients (mean age 68 years, 82 male, 36 female, and 62 left and 56 right hip joints) were operated on with the hybrid CT-free navigation system. During each operation, the angular orientation of the inserted implant was recorded. To determine the placement accuracy of the acetabular components, the first 50 consecutive patients underwent a CT scan 7-10 days postoperatively to analyze the cup position relative to the APP. This was done blinded with commercial planning software. There was no significant learning curve observed for the use of the system. Mean values for postoperative inclination read 43 degrees (SD 3.0, range 37-49) and anteversion 19 degrees (SD 3.9, range 10-28). The resulting system accuracy, i.e., the difference between intraoperatively calculated cup orientation and postoperatively measured implant position, shows a maximum error of 5 degrees for the inclination (mean 1.5 degrees, SD 1.1) and 6 degrees for the anteversion (mean 2.4 degrees, SD 1.3). An accuracy of better than 5 degrees inclination and 6 degrees anteversion was achieved under clinical conditions, which implies that there is no significant difference in performance from the established CT-based navigation methods. Image-guided CT-free cup navigation provides a reliable solution for future total hip arthroplasty (THA).
