Les particules diesel, les hydrocarbures polycycliques et les particules fines sont classés cancérigènes pour le poumon par le CIRC. Ce mécanisme d'oncogenèse reste mal connu. Des données récentes suggèrent que les particules fines jouent le rôle de promoteur des tumeurs avec mutation EGFR [1]. L'objectif de notre étude est d'étudier le lien entre l'exposition aux polluants, aux radiations ionisantes et les altérations moléculaires et le statut PD-L1 des cancers bronchiques non à petites cellules (CBNPC). La cohorte KBP-2020 est une étude observationnelle multicentrique ayant inclus tous les patients avec un diagnostic de cancer du poumon en 2020. Seuls les patients atteints d'un CBNPC non épidermoïde dont le lieu de résidence était correctement rempli et situé en France métropolitaine ont été inclus dans cette analyse. Les niveaux d'exposition moyenne au cours des 20 dernières années aux polluants suivants ont été segmentés en quartiles : PM2,5, PM10, NO2, O3, rayonnements telluriques et cosmiques. L'exposition au radon a été segmentée selon les trois classes de l'IRSN. Les résultats étaient ajustés sur le tabac, l'âge et le sexe. Ils sont exprimés en OR IC95 %. Au total 7006 cas de CBNPC non épidermoïdes ont été identifiés. Pour 2900 cas où la mutation EGFR avait été recherchée, il existait un lien significatif et linéaire entre la moyenne des concentrations en PM2,5 et la présence de la mutation EGFR (OR du 4e quartile vs 1er quartile : 2,34 p = 0,002). Il existait également une association significative entre mutation EGFR et moyenne des concentrations en NO2 (OR du 4e quartile vs 1er quartile : 2,21 p = 0,003). Pour l'O3, on observait une association inverse (OR : 0,53 p = 0,003). Les autres altérations moléculaires n'étaient pas associées aux facteurs environnementaux, en dehors de la présence de STK11 avec les moyennes de PM10 (OR : 1,63, p = 0,016). Aucun lien significatif n'a été retrouvé entre le statut PDL1 et les expositions environnementales. Un lieu d'habitation à haut potentiel d'exposition au radon n'était pas associé à la présence d'altérations moléculaires. Ces résultats démontrent l'association entre mutations EGFR et les PM2,5 et le NO2, indépendamment du tabagisme, alors qu'aucun lien n'est retrouvé pour les autres altérations touchant les drivers oncogéniques. Ils suggèrent que la lutte contre la pollution pourrait réduire la survenue de certains cancers du poumon.
Au cours de la pandémie COVID-19, l’utilisation du pembrolizumab à 400 mg toutes les 6 semaines et du Nivolumab 480 mg toutes les 4 semaines a été proposée pour limiter les venues de patients porteurs d’un CBNPC avancé. Cette proposition reposait uniquement sur des données de pharmacocinétique et pharmacodynamie compatible. L’étude IDEE a pour objectif principal de déterminer la sécurité et l’efficacité de ce schéma thérapeutique en condition de vie réelle. Nous avons réalisé une étude observationnelle, rétrospective, multicentrique dans 4 centres hospitaliers bretons. Tous les patients atteints d’un CBNPC avancé et ayant reçu au moins une dose de schéma adaptée entre le 1er mars 2020 et 1er mars 2021 ont été inclus dans l’étude. Le suivi a été effectué jusqu’en mars 2022. Les données cliniques ont été recueillies à partir des dossiers médicaux. Quatre-vingt-onze patients ont été inclus dans l’étude (38 ont reçu le nivolumab toutes les 4 semaines, 53 le pembrolizumab toutes les 6 semaines). L’âge médian à l’inclusion était de 68 ans. La plupart des patients avaient un CBNPC de stade IV (n = 72 ; 79 %), de type adénocarcinome (n = 68 ; 74 %). La majorité des patients étaient en 2e ligne ou plus (n = 53 ; 58 %). La durée moyenne sous schéma standard avant adaptation posologique était de 10,7 mois. La durée médiane de suivi sous double immunothérapie était de 7 mois. Au total, 36/91(39,5 %) patients ont présenté ou ont maintenu une réponse partielle avec le schéma double dose ; 8/91 (8,7 %) ont présenté ou ont maintenu une réponse complète ; 29/91 (31,8 %) ont arrêté le schéma double dose pour progression. Au total, 16/91 (17,5 %) étaient toujours sous ce schéma au 1er mars 2022. La plupart des effets indésirables (EI) rapportés sous double dose étaient attendus : digestifs (10), cytolyse (3), cutanéomuqueux (30), arthralgies (10), dysthyroïdie (12), neuropathie (1) insuffisance surrénalienne (1), de grade 1–2. En termes de toxicité sévère, un grade 3 et un grade 4 pulmonaire ont été observés. 33/91 (36,3 %) ont présenté une aggravation ou un nouvel effet indésirable lors du passage en double dose. Il n’y a pas eu de décès imputable au traitement double dose. Quinze patients ont arrêté le schéma modifié pour toxicité, comprenant 4 (4,4 %) arrêts définitifs. Ce travail basé sur des données de vie réelles est rassurant concernant l’efficacité et la sécurité d’utilisation du schéma modifié en double dose et à intervalle prolongé de l’immunothérapie dans le CBNPC avancé. Each decade since 2000, the French College of General Hospital Respiratory Physicians (CPHG) conducts a real-life nationwide prospective epidemiological, observational, multicentre study on primary lung cancer (PLC). First data on patient and tumour characteristics from 2020 cohort are here reported and compared with 2000 and 2010 results. Collection of all new PLC histologically or cytologically confirmed between 01/01 and 12/31/2020 in non-academic public hospital pulmonology or oncology units. A Scientific Committee controlled inclusion exhaustivity and quality. These data have been compared to those from KBP-2000 and KBP-2010, conducted according to the same methodology. 82 centres included 8,999 patients (vs 137 and 104 centres; 5,667 and 7,051 patients in 2000 and 2010 respectively). Compared with both previous decades, patients were significantly older (mean age 67.8±10.3 vs65.5 ± 11.3 in 2010 and vs 64.3 ± 11.5 in 2000, P< 0.0001), the proportion of women has increased markedly (34.6% vs 24.3% in 2010 and 16.0 % in 2000, P< 0.0001) and the proportion of non-smokers was also higher (12.6% vs 10.9% in 2010 and 7.2% in 2000, P< 0.0001). Adenocarcinoma was still the main histology (51.9% vs 45.4% in 2010 and 29.0% in 2000, P< 0.0001) and most patients were still in stage IV at diagnosis (60.4%). In 2020, 87.7% of patients benefited from molecular biology testing (vs 30.5% in 2010) and the most frequently observed mutations were KRAS (37.9%), EGFR (14.9%) and STK11 (14.8%). Curative surgery represented 20.3% out of the first therapeutic strategies, radiotherapy 28.6%, chemotherapy 58.4%, targeted therapy 5.8% and immunotherapy 30.2%. Lung cancer characteristics have evolved during the last 20 years. The patients at diagnosis are older, women's proportion has significantly increased, non-smokers are more numerous and adenocarcinomas, already predominant in the previous cohort, increase further, molecular biology testing is carried out routinely in real-life setting. KBP-CPHG studies represent a unique data source on lung cancer evolution in France over the last 20 years.
L’immunothérapie s’est récemment imposée comme standard en 2e ligne thérapeutique des cancers bronchiques non à petites cellules (CBNPC) de stade avancé. Cependant, les données concernant son efficacité sont basées sur des populations sélectionnées ne reflétant pas toujours la pratique clinique quotidienne. Cette étude avait pour but d’évaluer l’efficacité du nivolumab dans la vraie vie et de déterminer un sous-type de patients plus à même d’en bénéficier. Étaient éligibles à l’inclusion les patients porteurs d’un CBNPC de stade avancé (IIIB-IV) traités par nivolumab entre le 01/09/2015 et le 30/09/2016 après échec d’au moins une ligne de chimiothérapie. Le nivolumab était administré à une dose de 3 mg/kg tous les 15 jours jusqu’à progression ou toxicité majeure. Le critère de jugement principal était le taux de réponse objective (TRO) selon RECIST 1.1. Les critères secondaires étaient la survie sans progression (SSP), la survie globale (SG) et la recherche de facteurs prédictifs d’efficacité. Au total, 259 patients ont été inclus dans neuf centres bretons. Les caractéristiques au diagnostic étaient : âge médian de 62 ans [29–85], 72,2 % d’hommes, 73 % avec un performance status ≤ 1, histoire tabagique dans 85,8 % des cas, avec une majorité de stades IV (85,3 %) et d’adénocarcinomes (63,7 %). La mutation KRAS était la plus fréquente et retrouvée chez 55 patients. Dans 61,4 % des cas, le nivolumab était administré en 2e ligne. Le TRO était de 22,4 % (58 patients). La SSP médiane et la SG médiane étaient de 2,3 et 11 mois, respectivement. L’histologie épidermoïde était associée à une meilleure réponse (HR = 1,98, IC95 % [1,08-3,61], p = 0,03). La survenue d’effets secondaires d’ordre immunitaire était associée à une meilleure réponse et à une amélioration de la SG (HR = 0,42, IC95 % [0,26–0,70], p < 0,01). Avec un TRO de 22,4 %, similaire aux essais cliniques, et une SG de 11 mois, le nivolumab semble tenir ses promesses dans la vraie vie. Dans cette étude, l’histologie épidermoïde et la survenue d’effets secondaires d’ordre immunitaire sont des facteurs prédictifs d’efficacité.

AP is classified into three groups: autoimmune AP defined by the presence of plasma autoantibodies anti-GM-CSF, the most frequent form (90% of all AP); secondary AP, mainly occurring as a consequence of haematological diseases, or following on from toxic inhalation or infections, and genetic AP, which affects almost exclusively children. AP diagnosis is suspected where chest CT-scan demonstrates interstitial lung disease with a crazy paving aspect; and confirmed by bronchoalveolar lavage, which has a milky appearance and contains periodic acid Schiff positive proteinaceous alveolar deposits. The use of surgical lung biopsy to confirm AP is less frequent nowadays. In this context, positive antibodies against GM-CSF indicates an auto-immune etiology of the AP, Concerning management, whole lung lavage is the gold standard therapy. In refractory AP, new treatments are available such as subcutaneous or inhaled GM-CSF supplementation, or rituximab infusions. The clinical course is unpredictable. Introduction: Malignant pleural effusions (MPE) are a major problem in advance malignancy resulting in severe dyspnea, poor quality of life and reduced survival. Management must be centered on palliative care and symptom control. In recent years TPC has emerged as a new therapeutic opportunity for these patients. Objective: To describe the use of TPC, results and its potential complications in a retrospective (2007-2012) multi-centric study. Studied population : Two hundred and thirty patients (mean age 63 years [19-96] , 56% of women) with proven MPE and 241 TPC insertions. Underlying malignancy included lung cancers (35%), gynecological cancers (12%), breast cancers (31%), mesothelioma (5%). Most patients were unsuitable for talc pleurodesis (trapped lung 39.4%, lymphangitis carcinoma 13.5% or lung mass 58.3%). All had failed with previous repeated thoracocentesis and thoracoscopic talcage. Results : Dyspnea improvement was noted in 87.1% of the cases. 73% of the patients were discharged from hospital with a home drainage by a trained nurse. Mean length of hospital stay was 4,9 days. Mean length of pleural drainage was 82 days [0-489]. Spontaneous pleurodesis occurred in 13% of the cases. Early complications were mainly pneumothorax (8.2%). Late complications included locoregional infections (7.64%), poor TPC tolerance (4.2%), TPC obstruction or dislodgement (5.45%) and 1 case of tract metastasis. Conclusion: Our results show that the management of recurrent MPE by the TPC is easy and safe. This technique avoids hospitalizations for iterative thoracocentesis and improves patients symptoms with a low rate of complication.
La protéinose alvéolaire pulmonaire (PAP) est une maladie rare caractérisée par l’accumulation alvéolaire de composants du surfactant qui gêne les échanges gazeux. Les PAP sont séparées en trois groupes : les PAP auto-immunes caractérisées par la présence d’anticorps sériques anti-GM-CSF qui sont les plus fréquentes ; les PAP secondaires à une maladie sous-jacente, le plus souvent hématologique, mais aussi à l’inhalation de toxiques et à des infections ; enfin, les PAP d’origine génétique. Le diagnostic de PAP est évoqué sur le scanner thoracique avec une pneumopathie infiltrante diffuse ayant un aspect classique de crazy paving, puis confirmé par l’aspect laiteux du lavage bronchoalvéolaire, et la présence de matériel lipoprotéinacé PAS positif. La confirmation du diagnostic de PAP par biopsie pulmonaire chirurgicale est désormais rarement nécessaire. La positivité des anticorps sériques anti-GM-CSF signe le caractère auto-immun de la maladie. Le traitement de référence des PAP auto-immunes est le « grand lavage pulmonaire thérapeutique ». En cas de maladie réfractaire, d’autres traitements sont disponibles, tels que le GM-CSF en injection sous-cutanée ou en aérosol et le rituximab. L’évolution de la maladie est imprévisible. Une évolution spontanément favorable ou même une guérison spontanée peuvent survenir et la survie actuarielle à 5ans est de 95 %. Les principales complications sont infectieuses.

This paper deals with the modeling of different plasma flows interacting with a liquid jet of pure water. Using only fluid mechanic equations, the goal was to simulate and to understand the first moments of these interactions during suspension plasma spraying of nanomaterials. The obtained simulations were then validated by comparing the results with breakup mechanisms described for the same Weber number in a plasma cross-flow with constant temperature. The global model was applied to various suspension plasma spraying conditions: two plasma gas mixtures (Ar/H2 and Ar/He) and two types of liquid injections (train of droplets or continuous jet). These simulations demonstrated different trajectories of the liquid and, consequently, different modes of breakup. Droplet size distributions were also calculated in the global modeling field.
Pulmonary, alveolar proteinosis (PAP) is a rare, diffuse infiltrative lung disease of unknown etiology characterized by accumulation of abnormal lung surfactant in the alveoli and distal airways. Although it was,first described in 1958, few large scale studies have been published. The diagnosis is conventionally based on lung biopsy and the value of less invasive procedures such as bronchoalveolar lavage (BAL) remains controversial. A multi-center retrospective study was conducted in an attempt to update epidemiological, clinical (aid long term outcome and to assess the usefulness of chest CT scan and BAL in the management of a large homogeneous series of 41 patients. Inclusion criteria required a clinical picture compatible with PAP and either histological confirmation, or a BAL with a milky Macroscopic appearance coupled with positive staining to PAS. Mean age was 48.8 years with a 4: 1 male predominance. The Prevalence of smokers (80%) and occupational exposure to various inhaled dusts (39%) was high. The clinical, roentgenographic and functional features were in keeping with previous series. Worsening dyspnea was the most common symptom. Other common manifestations were dry cough, recurrent lung infections, weight loss, finger clubbing, crackles and cyanosis. LDH serum levels were increased in 51% of cases. Lung function tests usually showed a mild restrictive ventilatory defect with hypoxemia and reduced DLCO. Chest X ray revealed bilateral ground glass opacities with consolidation or air bronchogram involving peri-hilar or lower lobes. Chest CT scan, performed in 38 patients, revealed a similar pattern in most of them with diffuse intra-alveolar ground glass opacities (33118) (aid interlobular thickening (35/38), resulting in a crazy paving appearance in the typical geographic pattern (32138). This latter aspect was highly suggestive of the diagnosis. Segmental BAL led to a definitive diagnosis in 62% and lung biopsy was only required in 38% of cases. These data suggest that careful BAL analysis can be sufficient to provide a diagnosis without the need for a lung biopsy. During follow-up, spontaneous remission or improvement was reported in 26%. Segmental (n = 3) or whole lung lavage (it = 22) was required in 62.5% with a favorable course (complete remission 37%, improvement 34%, stable course 21%) in 92% of the cases. In this series the overall prognosis for PAP remained good, although unpredictable.
Tracheobronchomegaly is defined as a dilatation of the trachea and the large bronchi. It may occur as a familial condition or in association with a connective tissue disease, e.g. Ehlers-Danlos syndrome. Tracheobronchomegaly occurs late in adults. The predominant symptoms are bronchial irritation and recurrent bronchopulmonary infections (because of ineffective cough). Diagnosis is provided by thoracic imaging, particularly computed tomography that enables measuring the precise diameter of the trachea. We report the case of one patient with tracheobronchomegaly who was greatly improved after implantation of Ultraflex tracheobronchial prostheses.
