Ziele: Wertigkeit präoperativer CT und MRT bei distaler Radiusfraktur: Vergleich mit AO-Klassifikation, Diagnostik von Begleitverletzungen sowie Konsequenzen für das funktionelle Ergebnis Methode: Bei 92 Pat. mit einer röntgenologisch gesicherten distalen Radiusfraktur wurde prospektiv ein präop. CT u. MRT des Handgelenks u. der Handwurzel durchgeführt. Die Fraktureinteilung erfolgte nach der AO-Klassifikation (Typ A=extraartikulär, Typ B=partiell intraartikulär, Typ C=total intraartikulär). In der MSCT (4×1mm Kollimation) an einem Somatom Volume Zoom (Siemens) lag das Augenmerk auf dem Ausmaß der knöchernen Verletzungen (insbesondere Proc. styl. ulnae (PSU) sowie Beteiligung der radiokarpalen u. der DRUG-Gelenkfläche (distales Radio-Ulnar-Gelenk)). An einem 1.5 T Magnetom Sonata (Siemens) erfolgte insbesondere die Beurteilung des triangulären fibrokartilaginären Komplexes (TFCC), des scapholunären Bandes (SL-Band) u. des radioulnaren Bandapparates (RU-Band). Die klin. Nachuntersuchung (NU) erfolgte 1 Jahr postoperativ (CASTAING-Score [Scorebereich 0–27; Bereich 1–5=„gut'']). Ergebnis: Die 92 Pat. wurden anhand der Röntgenaufnahme klassifiziert als 45 A-, 9 B-, 38 C-Frakturen nach AO. Von den 45 röntgenologisch klassifizierten A-Frakturen konnten lediglich 5 in der CT bestätigt werden, in 40 Fällen waren die DRUG- bzw. die radiocarpale Gelenkfläche CT-morphologisch frakturiert. Bei C- und B-Frakturen fanden sich in >50% Rupturen der RU-Bänder. Bei CT-gesicherter Fraktur des PSU fand sich in 90% eine TFCC-Ruptur im MRT (54/60). 76/92 Pat. zeigten eine Fraktur der radiocarpalen Gelenkfläche, das DRUG war bei 80/92 frakturiert, in 60/92 Fällen ergab sich eine Fraktur des PSU, in 11/92 Fällen eine carpale Fraktur. In der NU ergab sich ein mittlerer Score nach CASTAING von 4,4 Pkt. (range 1–12). Es ergab sich keine statistisch signifikante Korrelation zwischen TFCC-Läsionen im MRT und NU-Score nach 1 Jahr, ebenso wurde keine signifikante Korrelation zwischen SL-Band-Läsionen im MRT und der NU nach 1 Jahr gefunden. Schlussfolgerung: Bei distaler Radiusfraktur findet sich eine hohe Inzidenz für karpale Begleitverletzungen in CT und MRT. CT/MRT zur präop. Diagnostik und OP-Planung bei distaler Radiusfraktur sind wertvoll, aber es lässt sich keine positive Korrelation zwischen karpalen Begleitverletzungen und funktionellem Ergebnis statistisch belegen. Bei Frakturen des PSU liegt in 90% aller Fälle eine TFCC-Läsion vor, für die in der NU keine statistisch signifikante Relevanz nachzuweisen ist.
OBJECTIVE:Injuries to the chest contribute significantly to the morbidity and mortality in multiple injured patients. This systematic review focuses on evidence based initial diagnostics and emergency room management of chest trauma. METHODS:Clinical trials was systematically collected (Medline, Cochrane and hand searches) and classified into evidence levels (1 to 5 according to the Oxford system). RESULTS:There are only a few studies that document the impact of injury mechanism and clinical examination of the patient. There is a positive correlation between crash severity or lateral impact with injury severity. Auscultation was found to be very sensitive in the detection of pneumothorax. Helical CT of the chest is most important in the initial work-up. Aortography is only indicated in selected cases. Whether tube thoracostomy is necessary in patients with occult pneumotharaces is still a matter of discussion. Indications for endotracheal intubation are poorly investigated and predominantly based on expert opinion. CONCLUSION:Numerous comparative studies (LE 2) dealing with emergency diagnostics and therapy of chest trauma are available, however only a few randomized studies do exist. Based on the available data a rational therapy of chest trauma is possible.
Thoraxverletzungen tragen wesentlich zur Morbidität und Mortalität von Schwerstverletzten bei. Diese Übersicht untersucht die im Rahmen der Schockraumversorgung notwendige klinische Diagnostik und erforderliche Sofortmaßnahmen.
