Les microangiopathies thrombotiques (MAT) médicamenteuses sont définies par une anémie hémolytique, une thrombopénie et une défaillance d’organe, dans les suites de l’administration d’un traitement, après avoir éliminé les autres causes de MAT. Le carfilzomib, nouvel inhibiteur irréversible sélectif du protéasome, ciblant sa protéine 20S, entraînant l’accumulation des protéines ubiquitinées pro-apoptotiques, est utilisé depuis peu dans le myélome en rechute, avec une injection bihebdomadaire sur 28 jours. Le développement de MAT aux inhibiteurs du protéasome est en augmentation progressive notamment au carfilzomib et au bortezomib avec 12 cas publiés en 2015 et 2016. Pour illustrer cette complication, nous rapportons le cas d’un patient de 73 ans atteint d’un myélome de stade III osseux IgG kappa à 62 g/L, associé à une hypercalcémie et à une anémie à 11 g/dL, sans atteinte rénale initiale. Il est traité en première ligne par neuf cures de bortezomib-melphalan-prednisone avec très bonne réponse partielle, complété d’un entretien par lenalidomide. Il présente une rechute à un an, traitée par bortézomib-thalidomide-dexaméthasone avec progression après sept cures. Il est placé sous carfilzomib-dexaméthasone. Alors que l’évaluation, montre une réponse partielle avec un pic passant de 12,8 g/L à 5,3 g/L, le patient présente au j8 du deuxième cycle un syndrome confusionnel sans fièvre. Le bilan montre une insuffisance rénale aiguë anurique, avec une créatininémie à 532 μmol/L, associée à une thrombopénie à 46 G/L, une anémie à 7,9 g/dL, une schizocytose à 5 % et une CRP à 63 mg/L. Il n’y a pas d’atteinte cardiaque et aucune lésion ischémique au scanner cérébral. L’activité ADAMTS13 est normale, il n’y a pas d’argument pour un syndrome hémolytique et urémique typique ou atypique. On conclut à une MAT au carfilzomib. Celui-ci est arrêté, des échanges plasmatiques (EP) et une hémofiltration quotidienne sont mis en place. La thrombopénie et l’anémie se corrigent après quatre séances d’EP et la schizocytose diminue à moins de 3 %. L’hémodialyse est sevrée après trois semaines et le patient reprend une diurèse, avec persistance d’une insuffisance rénale chronique de stade 3b. Il n’a pas reçu d’autre chimiothérapie depuis. L’étiopathogénie des MAT iatrogènes peut être immunologique ou toxique. Le mécanisme immunologique met en jeu des anticorps activant les cellules endothéliales et la cascade de coagulation avec des manifestations systémiques de survenue rapide telles qu’une insuffisance rénale aiguë avec anurie, dans les heures suivant l’administration du médicament. L’arrêt du médicament est obligatoire. Les échanges plasmatiques, souvent débutés devant l’hypothèse d’un purpura thrombotique thrombocytopénique, sont d’efficacité incertaine. L’évolution peut être marquée par une hypertension artérielle, une insuffisance rénale chronique voire terminale, favorisée par d’autres facteurs de risque déjà présents. Le mécanisme toxique, dose-dépendant, induit une perte graduelle de fonction rénale avec hypertension artérielle et parfois une présentation clinique sévère et rapide. Alors que l’anémie hémolytique et thrombopénie se résolvent souvent, l’atteinte rénale peut persister. En conclusion, on assiste à un nombre croissant de MAT de survenue précoce liées au carfilzomib sur une période de temps courte, ce qui n’était pas le cas avec l’utilisation du bortézomib. C’est une complication rare mais grave qui doit inciter à une grande vigilance dans notre pratique quotidienne.
Objective: We carried out a prospective study to evaluate the endemicity of Pseudomonas aeruginosa in intensive care units (ICUs). Pulsed-field gel electrophoresis (PFGE) was used to determine the genotypes of P. aeruginosa isolates. This allowed us to determine the importance of cross-colonisation and the colonisation routes of P. aeruginosa. Design: We screened epidemiological specimens (rectal swab, nose swab and tracheal aspiration) and routine clinical cultures from patients admitted to ICUs during a 2-year period, from 1st January, 1998, to 31st December, 1999. Setting: The study was carried out in four separate adult ICUs located in the Franche-Comté region of France. These four units admitted a total of 1,500 patients per year. Results: A total of 1686 specimens were collected from 473 patients; 122 of these patients were positive on admission, 351 became positive during hospitalisation. The overall incidence of P. aeruginosa was 15.7 cases per 100 patients and 15.1 cases per 1000 days of hospitalisation. Of 184 patients with at least one ICU-acquired positive clinical culture, 104 had been previously identified as carriers by a similar genotype. Typing of 208 non-replicate isolates revealed 101 major DNA patterns. Approximately 50% of P. aeruginosa carriage or colonisation/infection was acquired via cross-transmission; the other cases probably originated from endogenous sources. Conclusion: Cross-colonisation seems to play an important role in the general spread of P. aeruginosa in ICUs.
Patients with systemic inflammatory diseases can present with protracted fever or symptoms suggestive of severe infection. Investigations for an infection remain negative and empirical antimicrobial therapy fails to provide relief. This scenario is encountered in adult Still's disease, a condition for which there is no diagnostic laboratory test. Thus, the diagnosis of adult Still's disease is one of exclusion, Marked serum ferritin elevation strongly suggests adult Still's disease, although it is not among current criteria for this condition. The case of an 18-year-old who came to the emergency room for an "infection" is reported. The serum ferritin assay suggested adult Still's disease, and glucocorticoid therapy was dramatically effective.

Divers mecanismes toxiniques et immunologiques encore mal elucides sous-tendent ces differentes atteintes polyviscerales.

The clinical spectrum extends from a benign flu-like syndrome to a severe bacteremic form. Multiple organ involvement is the rule, with the most common targets being the kidneys, liver, nervous system, lungs, heart, and blood cells. A variety of poorly understood toxin-mediated and immunological mechanisms underlie the organ lesions.
Hemorrhagic fever with renal syndrome (HFRS) is a viral infection caused by viruses which belong to a new group (Hantavirus) of the Bunyaviidae Family. Human infection occurs indirectly through airborne viruses from feces and urines of various infected rodents. HFRS is due to Hantaan, Seoul, Puumala, Belgrade and Sin nombre viruses. In general, HFRS due to Hantaan and Belgrade viruses Is more severe and has higher mortality than those due to Puumala (nephropathia epidemica) or Seoul viruses. Hantaan is predominant in the Far East (Korea, Japan, China), Belgrade in the Balkans and Puumala In Western Europe and Scandinavia. Hantavirus Pulmonary Syndrome (HPS) is a recently described infection with high mortality (60 %) due to adult respiratory distress syndrome. The virus is a new Hantavirus called Sin nombre virus. The antiviral agent, ribavirin, if administred early in the illness, may be of some benefit.
There have been reports of interactions between miconazole and vitamin K antagonists, oral hypoglycemic agents, and anticonvulsant agents, with adverse experiences due to potentiation of either the therapeutic or the toxic effects of the drug given concomitantly with miconazole, A case of rhabdomyolysis induced by concomitant administration of miconazole and ciprofibrate is reported herein, The mechanism was probably displacement of ciprofibrate from plasma proteins by miconazole, which also has high affinity for plasma proteins.
