目的 阐明哺乳期过量喂养对脂代谢基因、蛋白和成年肥胖的影响,明确成年肥胖的发育源机制及早期防治策略.方法 109只Wistar新生雄性大鼠,通过调整哺乳期母亲哺乳子代的数目形成正常喂养(对照组)和高能喂养(高能组),干预3周后断乳,断乳后各组均基础膳食喂养至成年,高脂膳食诱导肥胖.在不同生长发育阶段各组各处死8只鼠以动态观察,酶法测量血脂,计算体脂质量,FQ-PCR法和WB法检测ACC1、FAS、CPT1、UCP3基因和/或蛋白的含量.结果 与对照组相比,高能组大鼠在成年高脂诱导肥胖后的体脂质量(F= 11.840,P= 0.008,P = 0.009)、体重(F =11.913,P= 0.008,P= 0.008)增加;血清 TG(F= 9.871,P= 0.013,P = 0.011)、血清 TC 增加(F = 7.436,P= 0.024,P =0.022),HDL-C 减少(F= 9.407,P= 0.014,P= 0.013);ACC1(F= 121.782,P<0.001,P<0.001)、FAS基因(F=92.354,P<0.001,P<0.001)的含量增加,UCP3(F= 36.236,P<0.001,P<0.001)、CPT1基因(F= 21.427,P= 0.002,P= 0.001)和UCP3(F= 29.016,P= 0.001,P = 0.001)、CPT1 蛋白(F= 41.222,P<0.001,P<0.001)的含量减少.结论 大鼠哺乳期营养过剩可通过持续降低脂肪分解,增加脂肪合成而导致脂代谢紊乱、体脂质量增加和成年肥胖.
<正>腰椎间盘突出症是临床中引起腰腿疼痛的常见病,多发病,其主要表现为腰部疼痛,伴单侧或双侧下肢放射性疼痛、麻木,一般具有腰部活动功能障碍,部分患者有明显的脊柱侧弯畸形,本病好发于20~40岁的青壮年,男性多于女性。
Objective: Checking early syphilis infection in pregnant women,taking preventive measures to reduce the incidence of congenital syphilis.Methods: Syphilis serology inspection using gelatin particles(TPPA) and syphilis toluidine red test(TRUST) for women during early pregnancy.Syphilis during pregnancy is diagnosed in case of two positive results,and anti-luetic treatment or termination of pregnancy may be adopted.Results: In 3670 cases of women during early pregnancy,29 cases are diagnosed as Syphilis in Pregnancy,including 8 cases of TRUST titer greater than 1:8,21 cases of less than 1:8;12 patients receive anti-luetic treatment;17 patients choose to terminate the pregnancy.Conclusion: Syphilis serology inspection should be carried out for women during early pregnancy and measures should be taken in the early stage to reduce the incidence of congenital syphilis.
目的:探讨妊娠合并卵巢肿瘤的临床特点和诊治方法。方法:对62例妊娠合并卵巢肿瘤患者均行手术治疗,其中在剖宫产术中发现的患者同时行卵巢肿瘤剥除术,在孕中期发现的13例患者中,5例因发生肿瘤急性蒂扭转行一侧附件切除术,其余8例均于足月时行剖宫产术+卵巢肿瘤剥除术。在孕早期发现的患者于孕16~20周行卵巢肿瘤剥除术,分娩过程当中发现的均于产后行卵巢肿瘤剥除术。结果:6例于孕中期发生肿瘤蒂扭转及8例于孕早期发现者均给予保胎治疗成功后至足月妊娠;8例于足月后正常分娩。62例孕产妇中有44例行剖宫产术结束妊娠,新生儿全部存活,情况良好。结论:妊娠早期妇科检查是筛查妊娠合并卵巢肿瘤最方便有效的方法。在剖宫产术中认真探查双侧附件,对于发现卵巢肿瘤也很重要。只有早期诊断和正确辨别良、恶性肿瘤,根据个体情况及时采取手术等治疗方法,才能保证母儿平安。
Objective:To explore the effect of maternal nutritional manipulation on the expression of fetal uncoupling protein 3(UCP3),find out an optimal maternal diet.Methods:Wistar pregnant rats received a standard diet(SD) and a high and medium-chain fatty diet(HMCFAD),respectively.Their male offsprings were divided into control group and high and medium-chain fatty acid group(HMCFA).Offspring were fed with a standard diet from weaning to adult.Then rats in control group were allocated to two groups: one group was fed with SD,on group was HFCON(high fat diet).HFCON and HMCFA groups were fed with high-fat diet(HFD) for 6 weeks to induce obesity.After birth,weaning,adult and HFD-induced obesity,offspring were sacrificed for analyzing the expression of UCP3 mRNA.Results:Compared with the offspring of SD rats during pregnancy,body weight and obesity rate of offspring of HMCFAD rats during pregnancy were significantly decreased(P<0.05),and the expression of UCP3 mRNA increased persistently.Conclusion:HMCFAD during pregnancy can prevent offspring from HFD-induced obesity in adult,the mechanism is upregulation of UCP3 and increase of fat oxidation and heat dissipation.
目的了解本地区孕产妇贫血发生情况,分析其发生的相关因素,采取有效干预措施,降低因孕妇贫血引起的围产期并发症。方法对在我院进行系统保健管理的2500例贫血孕妇,进行回顾性调查分析。结果2500例妊娠期妇女,贫血664例,患病率26.56%,贫血患病率随孕周增大而增加,贫血与体重增长过快有关,孕妇贫血城乡有差别。结论贫血对孕妇及胎儿均有危害,应引起重视,加强健康教育知识的宣传,做好围产保健,提高出生人口素质。
目的:探讨乳腺疾病发病的相关因素,制定有效的防护措施,降低乳腺疾病发生率。方法:应用近红外线乳腺诊断仪、B超,病理检查并结合触诊对1300例妇女进行乳腺体检。结果:乳腺患病732例,患病率为56.31%,乳腺增生580例,患病率为44.62%,乳腺癌2例,患病率为0.15%。结论:乳腺疾病与年龄、职业有相关性,加强宣教,及时体检,指导妇女健康饮食、生活习惯,降低乳腺疾病,及早发现乳腺癌。
目的:了解出生缺陷儿发病率,探讨其发生的相关因素,为更好落实干预措施提供依据。方法:根据我院2003-01/2007-12出生缺陷儿登记卡,进行回顾性调查分析。结果:监测围产儿10 298例,出生缺陷儿128例,出生缺陷儿发生率为12.43‰。城市与农村缺陷儿发生率无明显差别,母亲>35岁,出生缺陷儿发生率明显高于20~30岁组。结论:要广泛开展健康教育,普及优生优育知识,加强孕前,孕期保健和产前诊断工作,继续抓好出生缺陷监测工作,降低出生缺陷儿发生率。
OBJECTIVE:To investigate the effect of pregnant nutritional manipulation on offspring mRNA expression of fatty acid synthetase (FAS) and phosphoenolpyruvate carboxykinase (PEPCK).METHODS:Wistar pregnant rats fed on standard diet and high protein diet (HPD), respectively. The male offspring were divided into control and high protein group (HP) in accordance with the diets of their dams. Offspring were cross-fostered and fed on standard diet after weaning. At adult, some offspring randomly selected from control group, namely as high fat control (HFC), HFC and HP group rats fed on high-fat diet (HFD) to induce obesity. At different stages, tissue was collected for analyzing expression of FAS and PEPCK mRNA by fluorescent quantitation PCR.RESULTS:In HP, body weight and obesity rates were decreased after weaning and HFD (P < 0.05). The abundance of PEPCK mRNA was persistently decreased in HP, and the abundance of FAS mRNA in HP was down-regulated before and after HFD vs. control or HFC.CONCLUSION:The increase of protein intake during pregnancy could decrease the expression of FAS and PEPCK mRNA, and prevent offspring from HFD-induced obesity in adult.
OBJECTIVETo investigate the effects of maternal nutritional manipulation on fetal mRNA abundance of uncoupling protein UCP2, UCP3 and carnitine palmityl transferase 1 (CPT1), and find out an optimal maternal diet and targets for pharmacological prevention and treatment of obesity.METHODSWistar pregnant rats were assigned to two groups which received a standard diet (SD) and a high protein diet (HPD) during pregnancy, respectively. After delivery, the male offspring were assigned to control group (CON) and high protein group (HP) according to their maternal diet, which were suckled by dams that received SD during pregnancy. Offspring were fed with SD from weaning (week 3) to week 8. Then CON were allocated to two groups: CON (SD during the whole experiment); HFCON (high fat control). HFCON and HP group rats were fed with high-fat diet (HFD) for 6 wk to induce obesity. At 0, 3, 8 and 14 wk of age, blood and tissue were collected for analyzing blood fat and abundance of UCP2, 3 and CPT1 mRNA.RESULTSIn HP body weight and TG were decreased after weaning (F = 4.589, P = 0.039; F = 27.001, P = 0.000) and HFD (F = 16.076, P = 0.00; F = 71.518, P = 0.000). Obesity rates were significantly decreased in HP after HFD (chi2 = 8.076, P = 0.004). The abundance of UCP3 and CPT1 mRNA was persistently higher in HP than in CON or HFCON, and the abundance of UCP2 mRNA was also persistently higher than in CON or HFCON after weaning. Moreover the abundance of CPT1 mRNA was significantly increased after weaning and HFD compared with that after SD, the abundance of UCP2, UCP3 mRNA was also increased after HFD compared with that after SD.CONCLUSIONSIncreasing protein intake during pregnancy might prevent offspring from HFD-induced obesity in adult, moreover might increase offspring the expression of UCP2, UCP3 and CPT1 mRNA. UCP2, UCP3 and CPT1 might participate in prevention and treatment of obesity by mediating fatty acid oxidation.
目的:进一步探讨子宫肌瘤的发生机理。方法:应用sp免疫组化方法测定30例子宫平滑肌瘤和相应子宫肌层组织的雌激素受体(ER)、孕激素受体(PR)和增殖细胞核抗原(PCNA)。结果:子宫肌瘤雌激素受体阳性率为70%,孕激素受体阳性率为63.33%,相应子宫肌层组织阳性率分别为43.33%、33.33%。两组间比较有显著性差异(P<0.05)。子宫肌瘤增殖细胞核抗原明显高于子宫肌层组织,两组间比较有显著性差异(P<0.01)。结论:子宫肌瘤的发生发展可能与雌激素,孕激素受体及PCNA有关。
哈尔滨医科大学附属第五医院妇保科(崔莉莉) 我院对2002年-2006年收治的57例患原发性肾病综合征及高凝状态小儿中的28例采用维生素E辅佐治疗,余29例作对照比较,取得了较满意的效果,现报告如下:
应用654-Ⅱ治疗重症肺炎30例,均于病情加重24小时内使用654-Ⅱ,全部治愈。病儿除吸氧,抗生素,抗病毒及补液等综合治疗外,使用654-Ⅱ,每次0.5mg~1mg/kg,总用量5mg~30mg。使用中如能注意早期,快速,足量,反复,可取得较好效果。

滴虫性阴道炎是最常见的阴道炎,儿童患病多不易被发现.最近我院收治2例,现报告如下:
乳腺组织增生属乳腺结构不良症的早期病变,此病多见于育龄期性机能旺盛的中年女性,30~40岁最多见,少数在2030岁时发病,尤以未婚女性或已婚未育或已育未哺乳者多见.主要以乳房的周期性疼痛为特征.起初为弥漫性隐痛,钝痛或胀堕感,后又为局限性刺痛或刀割样疼痛,累及一侧或双侧乳腺,50%在外上象限,且伴有压痛,有时疼痛可放射到同侧上臂内侧,腋窝部或上颈部,上肢带动及生气时加重.
