Nous rapportons le cas d'un homme lao présentant un paraffinome pénien dans un contexte d'infection par le VIH. Un patient lao infecté par le VIH était hospitalisé pour la prise en charge thérapeutique d'une tuberculose ganglionnaire. Les lymphocytes totaux étaient à 1200/mm3 (les lymphocytes CD4 étaient indisponibles). À l'examen clinique de la région génitale, on notait une infiltration circonférentielle asymptomatique du pénis. Il n'y avait pas d'ulcération génitale associée ni d'adénopathies inguinales. Le TPHA et le VDRL étaient négatifs. Le patient rapportait que, marin en Thaïlande, il avait effectué des injections intra-préputiales d'huile végétale. Ce procédé avait été entrepris avec les autres marins. Il s'agissait donc d'un paraffinome pénien chez un patient lao infecté par le VIH. La biopsie cutanée n'était pas réalisée (Fig. 1 et 2). La RDP Lao est un pays, en voie de développement, enclavé de l'Asie du Sud-Est. Le pays compte près de 7 millions d'habitants et a un Produit Intérieur Brut de 13,8 milliards de dollars. Il comptait, en 2003, 12 dermatologues (soit 1 dermatologue pour plus de 450 000 habitants, comparé à 1 dermatologue pour 20 000 habitants en France) travaillant principalement dans certaines capitales provinciales. La RDP Lao compte environ 80 groupes ethniques (répartis dans 3 groupes principaux : Lao Loum, Lao Teung et Lao Song). Les nodules péniens artificiels (APN) ont un précurseur (le Higo-zuiki) depuis le 18e siècle au Japon ; le paraffinome pénien est une technique plus récente par injection de huile végétale ou minérale. Ce sont 2 chirurgiens français qui décrivirent en 1947, les injections de paraffine chez 6 patients qui avaient un trouble de l'érection. Cette pratique se rencontre en Asie et en Europe de l'Est. Une analyse socioculturelle menée en Bulgarie sur 25 patients montre certaines caractéristiques : moyenne d'âge de 28 ans, tous sont prisonniers ou mendiants et 92 % sont des gitans. Les paraffinomes sont effectués par des individus, ni médecins ou infirmiers, pour 2 raisons principales : augmentation de la taille du pénis et du plaisir de la partenaire. Cela réalise également un rituel initiatique chez certaines communautés masculines : prisonniers, marins, militaires et yakusa. Le paraffinome peut induire certaines complications : complications allergiques et infectieuses, douleur à l'érection, nécrose et ulcération, paraphimosis. Cette pratique réalisée en groupe peut aussi favoriser l'infection par le VIH, le VHB et le VHC. Les paraffinomes péniens constituent un rituel initiatique dans certains groupes masculins. L'infection par le VIH ne semble pas être responsable de complications de ces derniers.
L’ecthyma gangreneux est une infection sévère de la peau le plus souvent due à Pseudomonas aeruginosa. Il est caractérisé par des ulcérations nécrotiques limitées par un halo inflammatoire. Les lésions siègent habituellement dans les régions péri-anale, péri-génitale ou axillaire. L’ecthyma gangreneux survient en général chez des sujets immunodéprimés : chimiothérapie aplasiante, infection par le VIH, neutropénie ou déficit fonctionnel des polynucléaires neutrophiles, agammaglobulinémie ; il a plus rarement été décrit comme compliquant une varicelle.Nous rapportons un cas d’ecthyma gangreneux de la paupière survenu au décours d’une varicelle chez une fillette de deux ans et demi immunocompétente ; la lésion a déterminé une cicatrice rétractile invalidante.Le rôle favorisant des anti-inflammatoires non stéroïdiens (AINS) dans l’apparition de surinfections cutanées nécrosantes au cours de la varicelle a déjà été signalé. La localisation de telles complications sur une paupière peut non seulement avoir des conséquences néfastes sur la fonction de l’œil, mais aussi être source d’un préjudice esthétique important.Ecthyma gangrenosum is an acute ulcer necrotic skin infection frequently caused by Pseudomonas aeruginosa. It is characterised by necrotic ulcerations circumscribed by an inflammatory halo. Lesions are normally found in the anal, genital and axillary regions. Ecthyma gangrenosum is most commonly seen in immunodepressed patients (cytotoxic chemotherapy, HIV infection, neutropenia or functional neutrophil deficiency, agammaglobulinemia). It is a rarely described complication in chicken pox.Herein we report the case of a girl aged 2 ½ years presenting in our dermatology department with ecthyma gangrenosum on the right upper eyelid secondary to varicella. The disease course was marked by fibrous scarring of the inner canthus with ptosis of the right upper eyelid. The retractile scarring caused disability.There have been previous reports of the contribution of non-steroidal anti- inflammatory drugs (NSAIDs) to the appearance of necrotic cutaneous super infections during the course of chickenpox. The occurrence of such complications on an eyelid may be harmful not only for the function of the eye but it can also cause extensive aesthetic impairment. Subsequent aesthetic and functional impairment may be improved by corrective surgery.
Les investigations en dermato-allergologie nécessitent une bonne préparation. À cette fin, le groupe dermato-allergologie de la Société française de dermatologie avait proposé une fiche d’information sur le déroulement des patch-tests. Le but de notre étude était d’évaluer l’utilité et la bonne compréhension de cette fiche d’information.Une fiche d’information était envoyée au patient avant les tests. Un questionnaire lui était remis le jour des tests afin d’évaluer la compréhension de la fiche. Un autre questionnaire était remis en parallèle au médecin dermato-allergologue afin de vérifier le respect par le patient des recommandations figurant sur la fiche. Les deux questionnaires étaient évalués indépendamment et appariés pour apprécier la concordance entre patient et médecin.Huit services hospitaliers de dermato-allergologie ont participé à l’étude, permettant de recueillir, sur une durée de 18 mois, 921 questionnaires patient et 921 questionnaires médecin appariés. La majorité des patients avaient lu la fiche (96,2 %), l’avaient trouvée utile (98,8 %), facile à lire (97,4 %) et adaptée (91,5 %). Cependant, 10 % des patients avaient eu des difficultés de compréhension.Cette étude a montré que la fiche d’information était claire et explicite pour la grande majorité des patients. Néanmoins, grâce aux remarques de certains d’entre eux, la fiche a bénéficié de quelques modifications pour en améliorer encore la compréhension.Skin patch-tests in dermatology-allergology practice require good preparation. To this end, the dermatology-allergology group of the French Society of Dermatology introduced an information notice informing patients about patch testing procedures. The aim of this study was to evaluate the utility and understanding of the notice.The information notice was sent out to patients before testing. On the day of the test, a questionnaire was submitted to patients to evaluate their comprehension of the notice. Another questionnaire was submitted simultaneously to the dermatology-allergology practitioner to evaluate whether the patient had complied with the guidelines given in the information notice. Paired questionnaires were analyzed for this study.Eight dermatology-allergology hospital departments participated in the study and collected 921 paired questionnaires over a period of 18 months. Among the vast majority (96.2%) of patients who had read the information notice, most found it useful (98.8%), easy to read (97.4%), and appropriate (91.5%). Ten percent of patients had difficulty understanding.This study shows that the information notice was clear and explicit for the immense majority of patients. Thanks to the feedback of a number of patients, the information notice was further improved to enhance patient understanding.
The objective of this study was to examine the characteristics of cutaneous lupus erythematosus, excluding systemic lupus erythematosus (SLE), in patients of African descent. Indeed, since the description of subacute cutaneous lupus erythematosus (SCLE), which had been included in chronic cutaneous lupus erythematosus (CCLE), there has been no description of the disease in black patients. In 2000, we performed a retrospective epidemiological study by querying multiple sources to identify all patients with lupus in French Guiana – a part of France in South America having western living conditions, free healthcare and 157,000 inhabitants, most of whom are of African origin. We found 45 patients with pure cutaneous lupus, which included CCLE (mostly discoid), SCLE and bullous lupus. The disease characteristics of these patients exhibited few differences compared with those of the Caucasian patients cited in the literature. However, the age of onset for our patients of African descent was younger than that of Caucasian patients. In contrast to the race-related differences reported for SLE, we found no major differences in terms of demographic, clinical and biological presentation between this cohort of pure cutaneous lupus erythematosus patients of African origin and Caucasian patients with similar forms of lupus.
La Guyane est un département français situé au nord de l’Amérique du Sud, sa forêt équatoriale recouvre 96 % du territoire et abrite des vecteurs et des réservoirs de la leishmaniose. Les leishmanioses diagnostiquées dans cette région sont majoritairement cutanées, dues à L. guyanensis. Mais il semble que des leishmanioses cutanéo-muqueuses à L. braziliensis soient aussi retrouvées. Leur présence est importante à connaître : leur présentation initiale cutanée bénigne répond moins bien au traitement usuel, et est enclin à rechuter sous forme cutanéo-muqueuse invalidante par destruction du massif facial. Nous voulions faire le point sur les leishmanioses à L. braziliensis diagnostiquées en Guyane. Une analyse descriptive des dossiers de patients s’étant présentés dans le service de Dermato-Vénéréologie du Centre Hospitalier (CH) de Cayenne, et/ou qui ont bénéficié d’une identification d’espèce de leur leishmaniose au Centre National de Référence ou au Laboratoire Hospitalo-Universitaire de Parasitologie Mycologie du CH de Cayenne, entre Janvier 1994 et Décembre 2007 a été réalisée. Dix-sept patients avaient présenté une leishmaniose à L. braziliensis durant cette période. Deux lésions muqueuses au niveau de la cloison nasale étaient retrouvées. La première ligne de traitement aboutissait à un échec ou une récidive. La fréquence des leishmanioses à L. braziliensis diagnostiquées au CH de Cayenne augmente. Il faut savoir y penser lors du traitement initial, car elle met en échec le traitement classique des leishmanioses cutanées en Guyane et sa récidive, pouvant se produire des années après, peut être cutanéomuqueuse et délabrante. L’identification d’espèce devient capitale, afin de permettre une prise en charge adéquate.
Conclusion La fréquence des leishmanioses à L. braziliensis diagnostiquées au CH de Cayenne augmente. This review of literature was aimed to assess in vivo experiments which have evaluated the efficacy of dental pulp stem cells (DPSCs) for bone regeneration.An electronic search of English-language papers was conducted on PubMed database. Studies that assessed the use of DPSCs in bone regeneration in vivo were included and experiments evaluating regeneration of hard tissues other than bone were excluded. The retrieved articles were thoroughly reviewed according to the source of stem cell, cell carrier, the in vivo experimental model, defect type, method of evaluating bone regeneration, and the obtained results. Further assessment of the results was conducted by classifying the studies based on the defect type.Seventeen papers formed the basis of this systematic review. Sixteen out of 17 experiments were performed on animal models with mouse and rat being the most frequently used animal models. Seven out of 17 animal studies, contained subcutaneous pockets on back of the animal for stem cell implantation. In only one study hard tissue formation was not observed. Other types of defects used in the retrieved studies, included cranial defects and mandibular bone defects, in all of which bone formation was reported.When applied in actual bone defects, DPSCs were capable of regenerating bone. Nevertheless, a precise conclusion regarding the efficiency of DPSCs for bone regeneration is yet to be made, considering the limited number of the in vivo experiments and the heterogeneity within their methods.
Furuncular myiasis is an infestation of the skin caused by Dermatobia hominis larvae known as "ver macaque" in French Guyana, "berne" in Brazil, "torsalo" in Colombia, or "human botfly" in English-language literature. It has identical features in man and domestic mammals. The primary lesion consists of a boil-like inflammatory papule with a central punctum exuding a serosanguinous discharge. The respiratory sinus of the D. hominis larvae may be visible through the punctum. Myiasis secondary to D. hominis accounts for 10% of imported tropical dermatosis observed in Paris. Diagnosis of furuncular myiasis should be considered in any patient with a history of travel or residence in an endemic area. Treatment depends mainly on mechanical removal that may be facilitated by injection of lidocaine into the lesion or prior application of a 1% solution of ivermectin.
We report the onset of an urban epidemic of human myiasis caused by Dermatobia hominis. To our knowledge, this is the first urban epidemic described for D. hominis. The epidemic was most likely related to exceptional weather conditions and notably high rainfall in January 2000, which may have facilitated the maturation of the pupae.
In French Guiana cutaneous leishmaniasis occurs mainly in the localized form with L. guyanensis accounting for more than 90% of cases. Mucocutaneous leishmaniasis is uncommon (less than 2% of cases) with L. braziliensis accounting for all previously reported cases. The purpose of this report is to describe a case of mucocutaneous leishmaniasis due to L. guyanensis that led to diagnosis of HIV infection in a patient living in French Guiana.
French Guyana borders Brazil with the second highest number of cases of leprosy in the world. The purpose of this retrospective study of leprosy cases diagnosed in Guyana between January 1997 and December 2006 was to calculate the incidence of the disease and to identify any special clinical and epidemiological features. A total of 90 new cases were recorded during the study period for a mean incidence of 0.53 cases/10,000 inhabitants/year. Since this incidence is below the 1/10,000 threshold defined by the World Health Organization, leprosy is no longer considered as a major public health issue in French Guyana. However it must be noted that while the number of "native leprosy" cases has declined, the number of Brazilian cases has increased (p<0.01). Brazilian leprosy has different epidemiological features, i.e., dominance of multibacillary forms and high incidence in gold panning and western regions of the country where the incidence is over 1/10,000 inhabitants/year.
The composition of 2977 Late Prehistoric glass objects was investigated to derive information on the nature of the colourants used. 2673 Late Iron Age Celtic (La Tène) bracelet fragments from the Netherlands and Austria and 51 Early Iron Age beads from the Netherlands were analysed. Hand-held XRF analyses demonstrated that all glass objects were of the soda-silica-lime type, which has a presumed origin in the Eastern Mediterranean. Copper was used as colourant, in the form of copper filings, in most of the Early Iron Age glass beads to give recycled glass a blue-green colour.The vast majority (98%) of the translucent Iron Age glass, was coloured using cobalt (blue), manganese (purple; colourless), antimony (colourless) and iron (green). Manganese, however, was added to all glass, contributing additional amounts of elements like copper, cobalt and iron. Opaque decorations were produced using antimony, or a combination of tin and lead.REE analyses on a selection of representative objects indicate that the manganese ores in translucent glass are of hydrogenetic-diagenetic (Early Iron Age) or hydrothermally influenced diagenetic (Late Iron Age) types. Strontium isotope ratios show mixing between a calcite-related seawater source (0.709) and manganese ores with isotope ratios of ∼0.70766. Lead isotope ratios are dominated by colourant-derived Pb. The isotope ratios of the manganese used to produce translucent glass and lead used for opaque glass decorations fall in the same range. The most likely general provenance of both lies on Lavrion or the Western Cycladic Islands, although an origin in the central Taurus or the Sinai mountains cannot be excluded. The conclusion is that manganese ore used for colourants contributes significantly to the REE concentration in the glass, including Nd, as well as to Sr and Pb. This needs to be taken into account when using concentrations or isotope ratios of these elements for provenancing other raw materials like sand and calcium carbonate. This appears to be the case for all antique soda-lime-silica glass.
