Introduction: Prehabilitation is defined as preoperative conditioning of patients in order to improve post-operative outcomes. Some studies showed an increase in functional recovery following colorectal surgery, but its effect in hepato-pancreato-biliary (HPB) surgery is unclear. The aim of this study was to realize a systematic literature review and meta-analysis on the current available evidence on prehabilitation in HPB surgery.Materials and methods: A systematic review and a metanalysis were carried out on preha-bilitation (physical, nutritional and psychological interventions) in HPB surgery (2009-2019). Assessed outcomes were postoperative complications, length of stay (LOS), 30-day readmission, and mortality.Main results: Four studies among the 191 screened were included in this systematic review (3 randomized controlled trials, 1 case-control propensity score study), involving 419 patients (pre -habilitation group, n = 139; control group, n = 280). After pooling, no difference was observed on LOS ((-4.37 days [95% CI: -8.86; 0.13]) or postoperative complications (RR 0.83 [95%CI: 0.62; 1.10]), reported by all the included studies. Two trials reported on readmission rate, but given the high heterogeneity, a meta-analysis was not realized. No deaths were reported among the included studies.Conclusion: No effect of prehabilitation programs in HPB surgery was observed on LOS or postoperative complications rate. Future trials with standardized outcomes of measure, and adequately powered samples calculations are thus required. PROSPERO registration: CRD42020165218.(c) 2021 Elsevier Masson SAS. Introduction La préhabilitation peut être définie comme un conditionnement préopératoire des patients afin d’améliorer les résultats fonctionnels après chirurgie. Certaines études ont montré une meilleure récupération fonctionnelle après chirurgie colorectale, mais son impact en chirurgie hépato-bilio-pancréatique (HPB) n’est pas démontré. L’objectif de cette étude est de réaliser une revue systématique de la littérature et une méta-analyse des données actuellement disponibles sur la préhabilitation en chirurgie HPB. Matériaux et méthodes Une revue systématique et une méta-analyse ont été réalisées sur la préhabilitation (interventions physiques, nutritionnelles et psychologiques) en chirurgie HPB (2009–2019). Les critères de jugement évalués étaient les complications postopératoires, la durée du séjour, la réadmission à 30jours et la mortalité. Principaux résultats Quatre études parmi les 191 examinées ont été incluses dans cette revue systématique (trois essais contrôlés randomisés, une étude cas-témoin sur score de propension) portant sur un total de 419 patients (groupe de préhabilitation, n=139 ; groupe témoin, n=280). Après analyse, aucune différence entre les deux groupes n’a été observée sur la durée d’hospitalisation (−3,1jours [IC à 95 % : −6,3 ;0,1]) ni sur le taux de complications postopératoires (RR 0,83 [IC à 95 % : 0,62 ;1,10]). Deux essais ont évalué le taux de réadmission, mais compte tenu d’une hétérogénéité élevée, aucune méta-analyse n’a été réalisée. Aucun décès n’a été rapporté parmi les études incluses. Conclusion Sur la base des données disponibles il n’a pas été observé d’impact des programmes de préhabilitation en chirurgie HPB sur la durée d’hospitalisation ou le taux de complications postopératoires. Objective: To produce French guidelines on Management of Liver failure in general Intensive Care Unit (ICU). Design: A consensus committee of 23 experts from the French Society of Anesthesiology and Critical Care Medicine (Societe francaise d'anesthesie et de reanimation, SFAR) and the French Association for the Study of the Liver (Association francaise pour l'etude du foie, AFEF) was convened. A formal conflict-ofinterest (COI) policy was developed at the start of the process and enforced throughout. The entire guideline process was conducted independently of any industrial funding. The authors were advised to follow the principles of the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system to guide their assessment of the quality of evidence. The potential drawbacks of making strong recommendations in the presence of low-quality evidence were emphasised. Some recommendations were ungraded. Methods: Two fields were defined: acute liver failure (ALF) and cirrhotic patients in general ICU. The panel focused on three questions with respect to ALF: (1) Which etiological examinations should be performed to reduce morbidity and mortality? (2) Which specific treatments should be initiated rapidly to reduce morbidity and mortality? (3) Which symptomatic treatment should be initiated rapidly to reduce morbidity and mortality? Seven questions concerning cirrhotic patients were addressed: (1) Which criteria should be used to guide ICU admission of cirrhotic patients in order to improve their prognosis? (2) Which specific management of kidney injury should be implemented to reduce morbidity and mortality in cirrhotic ICU patients? (3) Which specific measures to manage sepsis in order to reduce morbidity and mortality in cirrhotic ICU patients? (4) In which circumstances, human serum albumin should be administered to reduce morbidity and mortality in cirrhotic ICU patients? (5) How should digestive haemorrhage be treated in order to reduce morbidity and mortality in cirrhotic ICU patients? (6) How should haemostasis be managed in order to reduce morbidity and mortality in cirrhotic ICU patients? And (7) When should advice be obtained from an expert centre in order to reduce morbidity and mortality in cirrhotic ICU patients? Population, intervention, comparison and outcome (PICO) issues were reviewed and updated as required, and evidence profiles were generated. An analysis of the literature and recommendations was then performed in accordance with the GRADE (R) methodology. Results: The SFAR/AFEF Guidelines panel produced 18 statements on liver failure in general ICU. After two rounds of debate and various amendments, a strong agreement was reached on 100% of the recommendations: six had a high level of evidence (Grade 1 +/-), seven had a low level of evidence (Grade 2 +/-) and six were expert judgments. Finally, no recommendation was provided with respect to one question. Conclusions: Substantial agreement exists among experts regarding numerous strong recommendations on the optimum care of patients with liver failure in general ICU. (C) 2019 The Authors. Editor The COVID-19 outbreak greatly impacted surgical activities. Restrictions were imposed globally based on precautionary principle. Little is known of the nosocomial risk for COVID-negative patients undergoing surgery, nor the risk of operating on COVID patients during the epidemic1. We reviewed patients who underwent surgery in an academic hospital in the particularly affected Paris area during the 4 initial weeks of surgical restrictions, encompassing the epidemic peak.
La contamination par Candida des liquides de conservation (LC) d’organes peut être responsable de graves complications mettant en jeu le pronostic vital chez le receveur. Ces complications consistent, principalement, en des anévrysmes mycotiques et/ou des artérites. Ces contaminations peuvent provenir du LC contaminé pendant le transport du greffon après une brèche digestive. Le but de cette étude était de déterminer l’incidence de la contamination des LC par Candida, sur une période de 10 ans, dans un grand centre français de transplantation ainsi que les complications cliniques. De janvier 2004 à décembre 2013, une analyse rétrospective d’une cohorte de patients transplantés de foie, de rein et de cœur, a été réalisée à l’hôpital Henri Mondor. Les résultats mycologiques des LC analysés ont été collectés à partir de notre base de données. Les données des patients receveurs d’organes avec un LC positif à Candida ont été collectées avec, en particulier, la recherche d’une infection à distance de la greffe, le traitement reçu et le devenir du patient. Durant cette période, 1584 transplantations d’organes (818 reins, 633 foies et 133 cœurs) ont été réalisées dans notre centre. L’examen mycologique du LC a été réalisé pour 1459 (92,1 %) transplantations. Depuis les recommandations de l’Agence Française de Biomédecine en 2008, le pourcentage de LC analysés a augmenté significativement (90,5 % entre 2004 to 2008 [1] (période A) vs. 94,3 % entre 2009 et 2013 (période B), p < 0,01). Candida a été isolé à partir de 33 (2,2 %) LC parmi les 1459 analysés avec respectivement des incidences de 2,5 %, 2,4 % et 0 % dans les LC de foie, de rein et de cœur. La contamination a diminué entre la période A et la période B (2,8 % vs. 1,7 %). Cette diminution est observée pour chaque type d’organe transplanté avec 3,3 % vs. 1,9 % et 3,1 % vs. 1,8 % pour le foie et le rein respectivement. Candida albicans est l’espèce la plus fréquemment observée (61 %), suivie de C. glabrata (17 %), C. krusei (9 %) et d’autres espèces (3 %). La CMI des antifongiques par Etest® a été réalisée sur tous les isolats. Tous les C. albicans étaient sensibles au fluconazole et aux échinocandines. Les isolats de C. glabrata étaient intermédiaires au fluconazole mais sensibles aux échinocandines. Soixante-dix-huit pourcent des patients ont été traités. Le traitement a été maintenu au minimum 2 semaines. Trente et un patients (94 %) étaient en vie à 1 an de suivi. Un patient transplanté de foie a développé un anévrysme mycotique et est décédé 2 mois après la transplantation. La contamination des LC d’organe peut mettre en jeu le pronostic vital. L’examen mycologique du LC devrait être systématiquement réalisé, comme cela est recommandé, afin d’instaurer rapidement un traitement antifongique en cas de contamination à Candida et de limiter ainsi les complications.
La prophylaxie antifongique avec une échinocandine [1] est recommandée chez les transplantés hépatiques. Celle-ci peut être responsable de l’émergence de résistance. Le but de ce travail était d’évaluer la résistance des Candida spp. aux échinocandines chez des patients transplantés hépatiques, traités en prophylactique ou en curatif par des échinocandines. Deux cohortes de patients transplantés hépatiques de façon consécutive au cours des 6 premiers mois de 2013 (étude rétrospective [2]) et de 2015 (étude prospective) ont été incluses. Des cartes fongiques hebdomadaires ont été réalisées ainsi que des prélèvements profonds en cas de suspicion de candidose invasive. Les isolats de Candida ont été identifiés par MALDI-TOF. La sensibilité des isolats à l’anidulafungine et la micafungine a été réalisée par Etest®. La résistance a été confirmée par EUCAST et par le séquençage des régions hot spots des gènes FKS. Quatre-vingt-quatorze transplantés hépatiques (52 durant la 1ère période et 42 durant la 2ème) ont été inclus. Trente-neuf patients étaient colonisés ou infectés et traités par une échinocandine. La résistance aux échinocandines est apparue chez 3 (7,6 %) patients avec un C. glabrata résistant (patient P1) et un C. dubliniensis (patient P2) durant la 1ere période et un C. albicans résistant (patient P3) durant la 2ème période. Pour chaque patient, les isolats résistants ont été retrouvés sur plusieurs sites et à différents temps. Pour P1, 22 isolats ont été retrouvés : urines (n = 11), anus (n = 6) et pli inguinal (n = 5). Pour P2, 5 isolats ont été retrouvés : collection abdominale (n = 4) et bouche (n = 1). Chez P3, 1 isolat a été retrouvé au niveau du pli inguinal. Les CMI de l’anidulafungine et de la micafungine ont été respectivement >0,125 mg/L et >0,06 mg/L (EUCAST). La résistance est apparue chez C. albicans, C. glabrata et C. dubliniensis 24, 14 et 27 jours respectivement après l’initiation du traitement par caspofungine et a disparu après l’arrêt du traitement. L’analyse moléculaire a retrouvé 1 mutation dans la région HS1 fks1 (S645P) de C. albicans et C. dubliniensis et 2 mutations successives dans la région HS1 fks2 (F659S and S663A) de C. glabrata. Ce travail confirme le risque d’émergence de résistance de Candida lors de traitements par échinocandine. La plupart des souches résistantes ont été isolées à partir de sites digestifs. La résistance peut apparaître rapidement, ce qui nécessite une surveillance rapprochée de la sensibilité des isolats aux antifongiques.
Extracorporeal membrane oxygenation (ECMO) support has become an integral part of contemporary advanced cardiovascular support. Traditional indications include: cardiogenic shock due to acute myocardial infarction (AMI), refractory heart failure (including myocarditis and intractable arrhythmias) and post-cardiotomy (PC). Recently, addition of extracorporeal cardiopulmonary resuscitation (E-CPR) as an indication has gained popularity. In order to offer this technology, the Montréal Heart Institute (MHI) created an ECMO interdisciplinary team and derived a protocol for clinical decision guidance. We herein report the outcomes of all veno-arterial ECMO supported patients according to local guidelines implantation.
Survivors of prolonged Intensive Care Unit (ICU) stay frequently have physical limitations and altered quality of life. However data on patients following prolonged ICU stay after cardiac surgery are scarce. Consequently, more information is needed about their outcome after discharge.
Infections remain a major cause of morbidity and mortality after liver transplantation. One possible cause of infection is preservation fluid contamination. Donor-derived pathogens, such as Candida albicans, have occasionally produced life-threatening complications in organ recipients, already described in renal transplantation. In the present case, we report the loss of a liver graft secondary to vascular complications because of C. albicans found in the preservation fluid. Our case report raises the question of implementing procedures, similar to those in renal transplantation, including early antifungal treatment and repeated radiological monitoring for the prevention and detection of vascular complications.
Background and Goal of Study: Previous studies have established that pneumonia is the most frequent postoperative complication af ter liver transplantation (LT). Pulmonary spirometric testing is part of the standard pretransplant evaluation in several centers but the correlation between the preoperative lung function and the complications developed af ter the LT has been insuf ficiently evaluated. Our goal was to assess the impact of pulmonary dysfunctions (particularly measured spirometric pat terns) on the postoperative outcome. Materials and methods: This study included 342 patients transplanted between july 2008 and may 2010 in two centers specialised in LT. We integrated in this retrospective analysis all patients who had spirometric tests in their preoperative evaluation. The mean age was 51 ± 12 years and 68% of these patients were male. The main indications for LT were: cirrhosis (42%), hepatocellulary carcinoma (30%) and familial amyloid neuropathy (11%). A total of 30 patients (8,7%) were transplanted in emergency because of acute liver failure. Results and discussion: A restrictive lung pattern (defined by a total lung capacity of less than 80%) was found in 21% of the patients evaluated for LT (71/342). Among these 71 patients, 67% presented ascities and 62% a pleural ef fusion that may have been implicated in the etiology of the restrictive pattern. An obstructive respiratory disease was found among 23% of patients (79/342). Patients presenting a restrictive lung pattern had a longer hospital and ICU stay (40 ± 32 vs. 31 ± 18 days, p=0,04 and 15 ± 29 vs. 9 ± 10, p=0,03) as well as a higher lung infection risk (29% vs. 18%, p=0,04). However, neither the restrictive pat tern, nor the obstructive one were associated with a higher mortality. Conclusion(s): Pretransplant restrictive lung pattern is associated with a higher risk of postoperative pneumonia in patients undergoing a LT. Specific management of these patients may diminish the postoperative pulmonary dysfunction rate.
