Je 10 Probanden erhielten eine Infusion von 500 ml Dextran 40 mit Zusätzen von 0%, 5%, 10% und 20% Maltose über einen Zeitraum von 60 min. Durch Kombination von 10%iger Maltoselösung mit Dextran 40 konnte eine signifikante Steigerung der Diurese von 132 ml/h auf 315 ml/h am Ende der Infusion erzeugt werden. Insgesamt wurden in 3 h 685 ml, 185 ml mehr als infundiert, ausgeschieden. Die bei Zusatz von 5%iger Maltoselösung aufgetretene Steigerung der Urinausscheidung von 53 ml/h auf 173,5 ml/h stellte eine Wasserdiure-se dar. Die Maltosekonzentration der 5%igen Kombinationslosung reichte zur Induktion einer merkbaren osmotischen Diurese nicht aus. Nach Applikation der 20%igen Maltose/Dextran-40-Lösung konnte eine ausgeprägte osmodiüretische Wirkung nachgewiesen werden. Eine gegenüber der Dextran-40-Lösung ohne Maltosezusatz signifikante Steigerung der Urinausscheidung konnte jedoch nicht nachgewiesen werden. Aufgrund des hohen kolloidosmotischen Druckes und der hohen Osmolalität der verabreichten Kombinationslosung wurden Mechanismen aktiviert, die der induzierten Osmodiurese entgegen-wirken. Bei der Kombination mit Maltoselösung wurden sowohl systemische, als auch intrarenale Mechanismen aktiviert, die eine osmotische Diurese induzierten. Hervorzuheben ist die durch die intrarenale Spaltung von Maltose in Glukose hervorgerufene lokale Steigerung der intratubulären Osmolalität um den Faktor 4 bei unver-änderter intravasaler Osmolalität. Es wurde vorgeschlagen, die Kombinationslosung von niedermoleku-larem Dextran mit lO&percent;iger Maltose als Onko-Osmo-Therapeutikum beim drohenden Nierenversagen anzuwenden.
40 healthy men received an infusion solution of low molecular dextran with either 0%, 5%, 10% or 20% Maltose over a 60 min period. The combination of LMWD with 10% maltose solution showed a significant increase of urine excretion from 132 ml/h to 315 ml/h at the end of the infusion. The overall excretion rate in 3 h was 683 ml, 185 ml more than infused. The increase in urine excretion from 53.0 ml/h to 173.5 ml/h after the application of 5% maltose/dextran 40 solution was only due to a water diuresis. The maltose concentration was not high enough to induce a visible osmotic diuresis. The application of 20% maltose/dextran 40 solution showed a clear osmotic effect. Nevertheless, there was no significant increase in the urinary excretion rate compared to the LMWD solution without any maltose. The high oncotic and osmotic pressure of this combination activated mechanisms, which were opposed to the induced osmotic diuresis.
Nine healthy women received a new type of beverage over a period of 14 days. The daily dose was 1400 ml containing 14 g lactate. Acceptance was good. Blood cholesterol dropped from 193 +/- 21 mg/dl to 165 +/- 16 mg/dl (p less than 0.01). Blood potassium increased from 3.7 +/- 0.8 mmol/l to 4.3 +/- 0.2 mmol/l (p less than 0.01). Magnesium and calcium, however, showed a significant augmentation. No metabolic side effects were observed.
Bei 9 gesunden Probandinnen wurde über einen Zeitraum von 14 Tagen ein lactathaltiges Getränk aus Getreide in einer Dosis von 1,4 l/Tag verabreicht. Das Getränk wurde gut vertragen. Auffallend war ein selektiver signifikanter Abfall der Cholesterinwerte von 193±21 mg/dl auf 165±16 mg/dl (p<0,01). Die Konzentrationen für die Elektrolyte Kalium, Calcium und Magnesium stiegen in den Normalbereich an. Die Kaliumkonzentration erhöhte sich von 3,7±0,8 mmol/l auf 4,3±0,2 mmol/l (p<0,01). Die Calciumkonzentration stieg zwar geringgradig, aber doch in allen Fällen: Die Ausgangskonzentration betrug 2,40±0,1 mmol/l, die Endkonzentration 2,50±0,1 mmol/l. Die Magnesiumausgangswerte betrugen 0,86±0,10 mmol/l, die Endwerte 0,90±0,04 mmol/l. In allen Fällen kam es zu einem milden Anstieg der Serum-Magnesiumwerte. Das periphere Blutbild, die Retentionswerte für Harnsäure und Kreatinin, der Blutzucker und die Enzyme des Leberstoffwechsels blieben unverändert.
Unter „funktionelle Störungen“ werden ganz unterschiedliche Erkrankungen des Gastrointestinaltraktes zusammengefaßt. Am längsten bekannt sind Obstipation, vermehrte intestinale Gasbildung (Meteorismus) und Diarrhoe. Später kam die selektive Beschreibung von funktionellen Störungen an der Speiseröhre, dem Magen, den Gallenwegen, dem Dünndarm und dem Colon hinzu.
Es wurde das Ernährungsverhalten von 51 Patienten mit Diabetes mellitus untersucht. Die Männer verzehrten 223±55 g Kohlenhydrate, 111 ±22 g Fett und 112±27 g Protein. Die Energiezufuhr betrug 2560±433 Kalorien/ Tag. Die Frauen verzehrten durchschnittlich 1862±354 Kalorien. Davon waren 166 ±36 g Kohlenhydrate, 78±23 g Fett und 97±25 g Protein. Die Nährstoffrelation betrug bei Frauen 37% Kohlenhydrate, 39% Fett und 21% Eiweiß, bei Männern fand sich eine Nährstoffrelation von 36% Kohlenhydrate, 40% Fett und 18% Protein. Die Ballaststoffzufuhr betrug bei Männern 32,5±8,0 g/Tag und bei Frauen 28,3±6,4 g/Tag. Eine marginale Versorgung mit Linolsäure fand sich bei Männern in 15% und bei Frauen in 29%. Die durchschnittliche Linolsäurezufuhr betrug bei Männern 12,9±5,0 g/Tag und bei Frauen 9,2±4,1 g/Tag. Der P/S-Quotient lag in der Diabetesdiät bei 0,35.
Es sollte untersucht werden, ob Alkohol in einer Dosis von 1,22 g/kg und Tag bei gesunden Erwachsenen eine Störung des Harnsäurestoffwechsels auslösen kann.
8 gesunde Personen erhielten über einen Zeitraum von 3 Wochen eine neue bilanzierte, eiweißreiche Trink- und Sondennahrung für Diabetiker (Nutricomp D). Ab der zweiten Woche erhielten die Probanden zusätzlich 15 Beutel eines neuen Ballaststoffkonzentrates aus Sojakleie. Die Formeldiät wurde gut vertragen. Die Konzentration für das Hämoglobin blieb konstant. Die Serumtransaminasen, die Serumlipoproteine sowie die Retentionswerte Harnsäure und Kreatinin zeigten keine wesentliche Änderung. Die Konzentration der Spurenelemente Eisen und Kupfer blieb ebenfalls konstant.
Six healthy men received a purine-free formula diet. Uric acid was measured from day to day in blood and urine. After an adaptation period of five days, six apparently healthy volunteers received ethanol in a dosage of 1.22 g/kg bw for the next five days. The subjects drank the ethanol between 7 p.m. and 9 p.m. In the beginning the uric acid concentration in blood was 5.3 +/- 1.1 mg/dl. After five days of testing the uric acid concentration was 5.6 +/- 1.2 mg/dl (n.s.). No alterations of uric acid elimination during day (7 a.m. and 7 p.m.) and night (7 p.m. and 7 a.m.) was observed. Uric acid clearance remained constant. The total uric acid elimination rate was 568 +/- 133 mg/d during the adaptation period. After ethanol consumption the uric acid elimination was 682 +/- 66 mg/d (n.s.). The tested ethanol consumption of 100 g/d thus resulted in no significant alterations.
A procedure for the quantitative determination of O-hydroxyethylated rutosides by high-performance liquid chromatography is described, which can be used for the detection of these modified flavonoids in human serum. Serum samples are processed by the addition of acetone, which removes most of the proteins. After passing the supernatant through a microcolumn of Amberlite XAD-2 and washing with water, the hydroxyethylated rutosides are eluted with methanol. The eluate is concentrated in vacuo. The methanolic solution of the residue is chromatographed on RP-8 columns using UV and fluorescence detectors. The mono- to tetrahydroxyethylated constituents and their corresponding aglycones could be separated with a step gradient, starting with a solvent system of water-methanol-acetic acid (70:30:6) followed by a mixture of water-ethanol-acetic acid (70:30:6). Alternatively, the rutosides can be separated by a linear gradient of water-acetonitrile. An almost linear calibration curve and about 80% recovery are obtained. A detection limit of 1 mg/l is achieved. Pharmacokinetic studies in human volunteers are described.
In 8 subjects, the oral protein supply is modified in periods of 3 days each. With increasing protein intake, a decrease of the serum uric acid levels results as well as a significant increase in uric acid excretion and uric acid clearance. On the basis of the data obtained, it may be assumed that protein has a uricosuric action.
Elemental diets are in use for therapy of intestinal fistulas, diarrhoe of unknown etiology, short-bowel-syndrom and chronic inflammatory bowel diseases.
In a collective of 3899 out-patients, the effect of uric acid reduction of benzbromarone was tested. Ten days after beginning of treatment, a uric acid concentration of 5.1 to 5.7 mg/100 ml had been obtained. Three months after beginning of therapy, uric acid values were at 4.9 mg/100 ml, independently of age and sex. As side-effects were observed:diarrhoea in 1% (n = 41) of the treated patients, furthermore, 0,8% (n = 31) complained of gastro-intestinal trouble such as pressure over the stomach, nausea, sensation of fullness and epigastric pain. Only in 2 of the 3899 patients (0.05%) the appearance of a renal colic was observed.
Unter Verwendung einer Kohlenhydratkombinationslösung, welche 12,5 mval/Liter Kalzium und 12 mmol/Liter Phosphat enthielt, gelang es, die Konzentrationen für Kalzium und Phosphat während der parenteralen Infusionstherapie mit Kohlenhydraten mit einer Zufuhrrate von 0,5 g/kg/h und Aminosäuren mit einer Zufuhrrate von 0,1 g/kg/h konstant zu halten. Die Bilanzen für Kalzium und Phosphat waren positiv. Die Kalziumausscheidung lag geringfügig über der Norm, wie sie bei oraler Ernährung ermittelt wurde. Eine geringfügige Verminderung der Kalziumzufuhr durch eine Konzentrationsänderung der Lösung auf 10 mval/Liter wird vorgeschlagen.
