Background Corrective osteotomy for severe Madelung deformity with a radioulnar inclination exceeding 50 degrees and additional diaphyseal bowing of the radius remains a surgical challenge. Virtual preoperative planning based on three-dimensional (3D) evaluation of the deformity may enhance understanding and improve the execution of multiplanar correction. Materials and Methods Four female patients (mean age: 17 +/- 1 years) underwent double osteotomy for correction of severe Madelung deformity. Quantitative assessment of the deformity included measurements of radioulnar inclination, lunate subsidence, and carpal alignment. Virtual planning was performed in a standardized manner, and surgery was executed using patient-specific, 3D-printed sawing and cutting guides as well as custom-designed implants. Results At a mean follow-up of 2.5 years, radioulnar inclination was reduced from 57 to 31 degrees, lunate subsidence was normalized (stage 0), and carpal alignment was restored in all patients. The desired radial joint level was achieved without the need for any additional ulnar procedures. Conclusion In severe Madelung deformity involving the diaphysis, double osteotomy can restore distal radial alignment and radial bow. Incorporating 3D preoperative planning with printed surgical guides and patient-specific plates increases accuracy and reliability in managing this complex pathology.
Fractures of the distal radius show a wide spectrum of different fracture patterns. Although standard X-ray images are sufficient for extra-articular fractures, the exact analysis of intra-articular fractures requires the use of computed tomography (CT) with coronal, sagittal and axial sectional images. The classification is based on the Working Group for Osteosynthesis Questions (AO) criteria. The treatment strategy can be more precisely defined by a CT-based classification. Special attention must be paid to the presence of the key corners, as they have a high risk for primary or secondary dislocation if they not adequately stabilized.
Introduction: Posttraumatic or congenital ulna-minus variance with altered shape of the sigmoid notch and increased tension of the distal oblique band of the interosseous membrane (DIOM) can lead to painful impingement in the distal radioulnar joint (DRUJ) during rotation and loading of the forearm. As an operative treatment concept, a new method was described in 2016. Its goal is to restore the osseous congruency, which is required for normal painless function. The hypothesis is based on remodelling of the joint surface and the decompression of the DRUJ by releasing the DIOM. The purpose of this study is to analyze the results of performed operations with detailed focus on posttraumatic cases. Materials and Methods: : The indication for the operation is the impingement and incongruency in the DRUJ with ulna-minus variance. The surgical procedure is based on shortening and closed-wedge osteotomy of the distal radius with an ulnar translation of the radial shaft. Fifty-nine operations were performed between 2011 - 2022 on 52 patients (13 men, 39 women). Twenty-four patients were operated on the right side, 21 on the left side and 7 bilaterally. In 45 cases the operation was indicated because of congenital, in 12 cases due to posttraumatic incongruency and in 2 cases because of iatrogenic impingement after previously performed excessive ulnar shortening osteotomy. Modified Mayo-Wrist-Score, patient questioning, VAS and ROM were used to evaluate the results. Results: Significant reduction of pain on VAS from 7.22 to 1.98 (p <.001) was achieved. The pre- and postoperative range of motion did not show any significant changes (mean total arc of motion 301,94. vs. 295,20., p =.300). Specific complications we observed included a too distally performed osteotomy, DRUJ instability, de Quervains tenosynovitis, persistent pain and conversion into an ulna-plus variance. Conclusion: Under consideration of the indication criteria and correct execution of the osteotomy, in about 90 % of the cases this operation leads to good-to-excellent results with pain reduction and improvement of weightbearing and power. The preoperative examination, verification of the DRUJ stability and the radiological diagnostics are crucial for a good outcome.
Zusammenfassung Hintergrund 2016 wurde die dekomprimierende Radiuskorrekturosteotomie zur Behandlung des schmerzhaften Impingement-Syndroms des distalen Radioulnaren Gelenks (DRUG) erstmals beschrieben. Die Vorstellung der betroffenen Patienten erfolgt wegen belastungsabhängigen Schmerzen im DRUG, wobei bei der klinischen Untersuchung ein positiver Kompressionstest im DRUG richtungsweisend ist. Die Diagnose wird durch den radiologischen Nachweis einer Ulna minus-Varianz mit Deformierung der Incisura ulnaris bestätigt. Genese dieser Inkongruenz im DRUG kann entweder kongenital, posttraumatisch oder iatrogen sein. Durch die Radiuskorrektur wird ein Remodelling im DRUG erreicht und gleichzeitig resultiert durch Entspannung der distalen Membrana interossea (DIOM) eine Druckreduktion. Diese Studie widmet sich der Auswertung der bisher durchgeführten Operationen in unserem Zentrum bei ausschließlich kongenitaler Ursache. Patienten und Methoden Die Verbesserung der Kongruenz der Gelenkflächen des DRUG wird durch Radiusverkürzung und Änderung der Neigung der Sigmoid notch mittels closed wedge Osteotomie erzielt. Die Entspannung der Membrana interossea erfolgt durch ulnare Translation des Radiusschaftes. Das Patientengut beinhaltet nur Patientinnen und Patienten mit angeborener Inkongruenz im DRUG. Zur Auswertung der Ergebnisse benutzten wir die visuelle Analogskala (VAS), prä- und postoperative Messung des Bewegungsumfanges (ROM) in der sagittalen und transversalen Ebene, Kraftmessung und den Wrist Score nach Krimmer. Ergebnisse Im Verlauf von 11 Jahren fanden 45 Operationen an 38 Patienten mit unserer Methode statt, davon bei 17 auf der rechten, bei 14 auf der linken Seite und 7 Patienten operierten wir bilateral. Bei radiologisch bilateral nachgewiesener Inkongruenz wurde nur die symptomatische Seite operiert. Die statistische Evaluation zeigte in der VAS postoperativ eine signifikante Schmerzlinderung von 7,2 auf 2 (p<0,001). Durch die Osteotomie wurde keine signifikante Änderung des Bewegungsausmaßes verursacht (p=0,812). Der Wrist Score nach Krimmer erbrachte in fast 90% der Fälle ein gutes bis sehr gutes Ergebnis. Schlussfolgerung Bei korrekter Indikationsstellung und Operationstechnik führt diese Methode zur Verbesserung der Funktionalität und Lebensqualität der Patienten. Der präventive Aspekt mit Vermeidung degenerativer Prozesse bleibt weiteren Studien vorbehalten. Diese Operation gilt als sichere Methode und lässt den Weg für alternative Maßnahmen offen.
Die distale Radiusfraktur (DRF) umfasst eine Vielzahl unterschiedlicher Frakturtypen. Während für extraartikuläre Frakturen Standardröntgenaufnahmen ausreichend sind, erfordert die exakte Analyse intraartikulärer Frakturen eine CT mit koronalen, sagittalen und axialen Schnittbildern. Die Klassifikation erfolgt nach den AO-Kriterien. Die Behandlungsstrategie kann durch eine CT-basierte Klassifikation präzisiert werden. Besonderes Augenmerk muss auf das Vorliegen von Schlüsselfragmenten gelegt werden, da sie ein hohes Risiko für eine primäre oder sekundäre Dislokation aufweisen, wenn sie nicht adäquat stabilisiert werden.
Background In 2016, a new method was described to treat the painful impingement syndrome of the DRUJ: decompression corrective osteotomy of the distal radius. Clinical symptoms are based on a positive compression test; pain occurs with weight-bearing on the forearm. This phenomenon is seen in conjunction with a deformed sigmoid notch together with ulna minus-variance, which leads to increased tension in the distal oblique bundle of the interosseous membrane. The etiology of the condition can be either congenital, post-traumatic, or iatrogenic. Through the proposed osteotomy, decompression in the DRUJ is achieved. This study summarises the results of these surgical procedures performed in our hand centre exclusively in cases of congenital origin. Patients und Methods Remodelling of the DRUJ is achieved through the shortening of the distal radius together with closed wedge osteotomy. Relief of the interosseous membrane is accomplished by ulnar translation of the radial shaft. This study only included patients with congenital incongruency in the DRUJ. The results were evaluated using a visual analogue scale (VAS) and the Krimmer Wrist Score and by measuring the preoperative and postoperative range of motion as well as grip strength. Results Within 11 years, 45 procedures were performed with our method on 38 patients, of which 17 were treated on the right side, 14 on the left side, and 7 bilaterally. In cases of bilateral incongruency, only the symptomatic side was treated. The statistical evaluation showed a significant reduction of pain on the VAS from 7.2 to 2 (p<0.001). No significant changes were seen in the range of motion (p=0.812). The Krimmer Wrist Score showed good to excellent results in almost 90% of cases. Conclusion If the indication criteria are met, contraindications are avoided and the osteotomy is correctly performed, this technique leads to an improvement of patients' functionality and quality of life. From a preventive viewpoint, the influence on the progression of the degenerative changes is yet to be demonstrated in further studies. At any rate, this is a safe procedure, which leaves the path open for other possible options.
The complex kinematics of the saddle joint has been a challenge in the development of trapeziometacarpal prostheses. The introduction of the dual-mobility design increases the range of motion and limits strains on the cup fixation. Therefore, rates of loosening and dislocation have been markedly reduced. Furthermore, several studies showed a similar relief of pain, range of movement and grip strength compared to the results of resection arthroplasty. The preservation of the length of the first ray as well as the more stable and functional joint can be favourable in younger and active patients. Further investigation is needed to determine whether or not those higher demands affect long-term survivorship. The high grade of patient satisfaction, the shorter recovery time and the safe surgical technique motivates considering the implantation of a total endoprosthesis with a dual-mobility cup as an attractive alternative to resection arthroplasty in the operative treatment for trapeziometacarpal osteoarthritis.
Die komplexe Kinematik des Daumensattelgelenkes stellte die Entwickler von Endoprothesen vor große Herausforderungen. Mit der Einführung des Dual-Mobility-Prinzips konnten der Bewegungsspielraum erweitert und die Belastung auf die Pfannenverankerung entscheidend reduziert werden. Dadurch wurden Lockerungs- und Luxationsraten deutlich gesenkt. Zudem zeigte sich im Vergleich zur Resektionsarthroplastik eine gleich gute Schmerzlinderung, Beweglichkeit und Kraftentwicklung. Der Erhalt der Länge des 1. Strahles, sowie die höhere Stabilität und Führung bieten gerade für jüngere, aktive Patienten Vorteile. Wie sich eine stärkere Beanspruchung langfristig auf Standzeiten oder Revisionsraten auswirkt, gilt es abzuwarten. Aufgrund der hohen Patientenzufriedenheit, der kürzeren Rekonvaleszenzzeit und der sicheren operativen Technik halten wir die Daumensattelgelenksendoprothese nach dem Dual-Mobility-Prinzip für eine attraktive Alternative zur Resektionsarthroplastik bei Rhizarthrose.
Posttraumatic malunion or secondary dislocation can cause wrist joint incongruency. Uncorrected malalignment increases the risk of secondary degenerative changes and chronic pain. Therefore, early correction using the available fixed-angle devices, cancellous bone grafting only becomes necessary in larger bony defects. Premounting the plate through a palmar approach with regard to the desired correction angles leads to predictable results by precise correction. In case of posttraumatic growth arrest with larger discrepancy of the radius and the ulna, a two-staged procedure is advisable.
Trotz aller Verbesserungen bezüglich der operativen Versorgung distaler Radiusfrakturen durch optimierte Osteosynthesematerialien und einem differenzierten Verständnis der Frakturmorphologie ist die in Fehlstellung verheilte distale Radiusfraktur immer noch eine herausfordernde Komplikation. Dieser Beitrag gibt einen Überblick über die verschiedenen Fehlstellungsformen und deren operative Korrekturmöglichkeiten.
Ligamentäre Stabilität und knöcherne Kongruenz sind Voraussetzung für eine ungestörte Funktion des distalen Radioulnargelenks (DRUG). Eine ausgeprägte Ulnaminusvarianz anlagebedingt oder verursacht durch übermäßige Ulnaverkürzung kann zu einem Impingement mit Schmerzen bei der Unterarmdrehung führen. Als Therapiekonzept führen wir eine distale Radiusosteotomie mit Verkürzung, Ulnartranslation des Radiusschafts und Korrektur der Inklination der Incisura ulnaris durch. Die Arbeitshypothese basiert einerseits auf der sekundären Adaptation des DRUG und andererseits auf einer Spannungsreduzierung des TFCC und der distalen Membrana interossea (distales schräges Band) durch die Radiusverkürzung mit gleichzeitiger Ulnartranslation des Radiusschafts. Mit dieser Form der Radiuskorrektur können diese seltenen Störungen am DRUG erfolgreich behandelt und Rettungsoperationen vermieden werden. Langzeitergebnisse werden zeigen, ob hiermit auch die drohenden arthrotischen Veränderungen vermieden werden können.
Trotz der Fortschritte in Diagnostik und Primärbehandlung von Handgelenkverletzungen spielen (Teil‑)Arthrodesen zur sekundären Rekonstruktion weiterhin eine wichtige Rolle. Teilarthrodesen stabilisieren den Karpus und ermöglichen über die verbleibenden Handgelenkanteile eine zufriedenstellende Restbeweglichkeit. Durch die Ausschaltung der schmerzhaften Gelenkbereiche kann eine Schmerzreduktion erreicht werden. Teilweise wird der Arthroseprozess am Handgelenk durch Teilarthrodesen verzögert oder gestoppt. Die vorliegende Übersichtsarbeit gibt einen Eindruck über die häufigsten angewandten (Teil‑)Arthrodesen, führt Indikationen sowie Kontraindikationen auf und stellt typische Komplikationen dar.
Carpal bone fusions for secondary reconstruction are still indispensable despite state of the art diagnostic tools and modern treatment techniques for wrist lesions. The former fusions stabilize the wrist and enable sufficient residual carpal mobility. Pain can be reduced significantly by arthrodesis of destroyed joints and the progress of osteoarthritis may be stopped or delayed. This review presents commonly used fusions with their inherent indications, contraindications and complications.
Fractures of the distal radius are very common. The majority of patients are elderly females. High impact trauma are often responsible for fractures in young men. Clinical and radiological diagnostics, including computer-assisted tomography (CAT) scan, are generally sufficient. The indication for conservative treatment is still recommended for specific fracture patterns. Application of palmar locking plates after open reduction proved to be efficacious for the majority of fracture patterns. Furthermore, precise detection and treatment of concomitant lesions are mandatory in order to prevent complications.
Die distale Radiusfraktur ist weiterhin die häufigste Fraktur des Menschen. Neben den überwiegend älteren, weiblichen Patienten tritt diese auch seltener durch Mittel- oder Hochrasanztraumata bei männlichen, jungen Erwachsenen auf. Meist sind zur Diagnostik die klinische Untersuchung und Röntgenaufnahme, ggf. Computertomographie ausreichend. Die Indikation zur konservativen Therapie ist weiterhin gegeben, operativ haben sich jedoch mittlerweile in vielen Fällen die offene Reposition und Osteosynthese mit einer palmaren winkelstabilen Platte durchgesetzt. Neben der eigentlichen Versorgung der Fraktur sind Begleitverletzungen und Komplikationen frühzeitig zu diagnostizieren und ggf. zu therapieren.
