Einleitung: Neuroendokrine Tumore des Pankreas (PNET) sind seltene Tumore, welche sich bezüglich ihres biologischen Verhaltens, ihrer Diagnostik, Therapie und insbesondere ihrer Prognose erheblich von Pankreaskarzinomen unterscheiden.

Kontrovers diskutiert wird nach wie vor, ob diese Revisionseingriffe laparoskopisch oder offen durchgeführt werden sollen. Diese Untersuchung analysiert unser Krankenkollektiv im Bezug auf die Anzahl der notwendigen Reeingriffe und die Gesamtmorbidität.
Recently published data from our center have demonstrated the feasibility of a nephrotoxicity- and atherogenicity-free, mycophenolate mofetil (MMF)-based immunosuppressive protocol for elderly recipients of kidneys from elderly cadaveric donors. We investigated a therapeutic regimen of strictly monitored MMF (target mycophenolic acid [MPA] trough levels between 2–6 μg/mL) and steroids combined with a polyclonal-monoclonal induction regimen consisting of a low-dose, single shot of rabbit ATG (ATG-Fresenius) and the interleukin-2 receptor (IL-2R)-antibody basiliximab (d0 and d4). Between 1997 and 2007, we treated 175 elderly patients with an MMF-based, calcinearin inhibitor (CNI)-free immunosuppressive protocol. For the present cohort, 30 elderly recipients (67.8 ± 3.8 years) of renal transplants from deceased donors (69.4 ± 13.3 years) were recruited consecutively for this 5-year prospective, open, single center, pilot trial. One-year results of this clinical trial were patient and renal allograft survivals of 87% and 83%, respectively; death-censored 1-year graft survival was 97%. Mostly steroid-sensitive rejection episodes were observed in 46% of patients, with only 3 patients requiring serum antibody therapy. Renal allograft function was satisfactory, as reflected by a mean serum creatinine of 1.78 ± 0.45 mg/dL and a Nankivell glomerular filtration rate (GFR) of 48.8 ± 13.9 mg/dL at 6 months. Twenty-three percent of all patients demonstrated cytomegalovirus (CMV) infections; however, only 3.3% developed CMV disease. Application of a combined polyclonal-monoclonal induction regimen using a nephrotoxicity- and atherogenicity-free, MMF-based immunosuppressive maintenance protocol in elderly cadaveric kidney transplant recipients led to acceptable short-term outcomes, albeit at the expense of an increased rejection rate, comparable to that previously published for elderly (>50 years) recipients of allografts from elderly (>50 years) cadaveric donors.
Sepsis and septic multiorgan failure are still associated with a high mortality. Recent pathophysiological studies could show that a substantial depletion of the semi-essential amino acid arginine occurs during sepsis. However, the effects of a high-dosed supplementation of L-arginine on the microcirculation have not been well characterised. This study addresses the effect of an intravenous L-arginine application on the microcirculation in a well-established model of normotensive endotoxemia.

Anticoagulation, coagulation disorders and haemorrhage are causing considerable morbidity and mortality in surgical patients. Reversal of vitamin K anticoagulants and treatment of perioperative coagulopathy can be achieved by prothrombin complex concentrates (PCC). However, the effects on coagulation parameters, and any side effects on organ function, have yet to be determined.
Surgical Therapy of Neuroendocrine Nonpancreatic Gastrointestinal Tumors Engagement with rare gastrointestinal neuroendocrine tumors ( NET) is important, since these tumors are related to an overall better 5 year survival rate of about 70%, despite development of liver metastases and frequent relapses. Dignity and further clinical course are often difficult to estimate. As there is no effective chemotherapy thus far, only surgical treatment can cure the patient. Moreover, the surgeon can improve intraoperative staging by intraoperative diagnostics. Also general surgeons can be confronted with NET in emergency situations ( e. g. intestinal obstruction) where operation tactics are important. Even in case of liver metastasis, resection treatment and debulking operations can improve survival rates and may be indicated. Carcinoids of the appendix represent the most frequent NET manifestation and are therefore discussed in detail. Also NET in the stomach, small bowel, colon and rectum are discussed. Since NET are rare, treatment guidelines are mainly based on expert opinions. Over the last 10 years several new therapy options have been developed - from traditional aggressive surgery of metastasis over liver transplantation to sophisticated interventional techniques - procedures which can only be applied in interdisciplinary tumor centers with high expertise.

In der vorliegenden Arbeit konnte unter Nutzung zweier unterschiedlicher Versuchsmodelle eindrucksvoll gezeigt werden, dass bei septischen Bedingungen der hochdosierte Einsatz einer alleinigen AT-Gabe (250 U/kg) der klassischerweise mit Heparin durchgeführten Thromboseprophylaxe überlegen ist. Allerdings kam es unter diesem Regime zu einer Verlängerung der Blutungszeit. Erstmalig konnte somit in zwei gut etablierten experimentellen Sepsismodellen die antithrombotische Potenz von Antithrombin zur Verhinderung der mikrovaskulären Thrombosierung charakterisiert werden. Damit könnte in der adjuvanten Therapie septischer Patienten auf die Heparingabe verzichtet werden zugunsten von AT und dessen anti-inflammatorischen Potenz.
Trotz kontinuierlicher medizinischer Fortschritte in den letzten Jahrzehnten bleibt die Mortalität der Sepsis weiterhin unakzeptabel hoch. In multiplen Phase-III-Studien wurden in der letzten Dekade spezifische immunmodulatorische Ansätze bei der Sepsis getestet (Anti-Endotoxin-Antikörper, Anti-Zytokin-Antikörper, Zytokinrezeptorantagonisten, Cyclooxygenaseinhibitoren usw.) mit dem Ziel, die proinflammatorische Immunantwort zu modifizieren. Bei hervorragenden Ergebnissen im Tierexperiment mit hoher modellimmanenter Mortalität und meist prophylaktischem Einsatz waren diese Strategien bei humaner Sepsis erfolglos. Dies mag daran liegen, dass wesentliche Aspekte der Sepsispathophysiologie (Redundanz der Systeme, Phasen der Immunantwort bei der Sepsis, Veränderung der Immunantwort im Alter, Gerinnungsbeteiligung) nicht beachtet wurden und dass nicht nach Fokussanierung stratifiziert wurde. Mit natürlichen Gerinnungsinhibitoren (aktiviertes Protein C, Antithrombin und Gewebsfaktorinhibitor (TFPI)) stehen heutzutage Substanzen zur Verfügung, die neben ihren antikoagulatorischen Eigenschaften auch direkt immunmodulatorisch wirken. Erstmalig konnte in einer Phase-III-Studie durch die exogene Applikation von aktiviertem Protein C die 28-Tage-Mortalität von Patienten mit schwerer Sepsis signifikant gesenkt werden. Die Antithrombin-Gabe hingegen senkte in einer Phase-III-Studie nur in der Untergruppe der nicht gleichzeitig mit Heparin behandelten Patienten die 90-Tage-Mortalität signifikant, was auf einen klinisch relevanten Antagonismus zwischen Antithrombin und Heparin hinweist. Die Mechanismen der Immunmodulation durch Gerinnungsinhibitoren werden in dieser Arbeit diskutiert: Gerinnungsinhibitoren können durch Zellkernfaktor Beta (NFkB) Hemmung (aktiviertes Protein C, Antithrombin) und Prostazyklinfreisetzung (Antithrombin) nicht nur die Leukozyten-Thrombozyten-Endothelzell-Interaktion beeinflussen, sondern hierdurch auch die kapillare Perfusion verbessern. Die Perfusionsverbesserung stellt eine Voraussetzung zum Erhalt und zur Verbesserung der Organfunktion dar und sollte als Zielkriterium zur Effektivitätsbeurteilung einer adjuvanten Sepsistherapie etabliert werden. Dies impliziert, dass zur Beurteilung der mikrovaskulären Perfusion geeignete, am Menschen anwendbare Untersuchungsmethoden, für die Mikrozirkulation etabliert werden müssen.
Background: Disseminated intravascular coagulation (DIC) is a serious complication of sepsis that is associated with a high mortality. Objectives: Using the adapted International Society on Thrombosis and Haemostasis (ISTH) diagnostic scoring algorithm for DIC, we evaluated the treatment effects of high-dose antithrombin (AT) in patients with severe sepsis with or without DIC. Patients and Methods: From the phase III clinical trial in severe sepsis (KyberSept), 563 patients were identified (placebo, 277; AT, 286) who did not receive concomitant heparin and had sufficient data for DIC determination. Results: At baseline, 40.7% of patients (229 of 563) had DIC. DIC in the placebo-treated patients was associated with an excess risk of mortality (28-day mortality: 40.0% vs. 22.2%, P < 0.01). AT-treated patients with DIC had an absolute reduction in 28-day mortality of 14.6% compared with placebo (P = 0.02) whereas in patients without DIC no effect on 28-day mortality was seen (0.1% reduction in mortality; P = 1.0). Bleeding complications in AT-treated patients with and without DIC were higher compared with placebo (major bleeding rates: 7.0% vs. 5.2% for patients with DIC, P = 0.6; 9.8% vs. 3.1% for patients without DIC, P = 0.02). Conclusions: High-dose AT without concomitant heparin in septic patients with DIC may result in a significant mortality reduction. The adapted ISTH DIC score may identify patients with severe sepsis who potentially benefit from high-dose AT treatment.
Wir konnten in diesem »second-hit« Modell erstmals zeigen, dass die LPS-induzierte Sepsis zur Aggravierung der Mikrozirkulationsstörung im traumatisierten Skelettmuskel führt. Die Applikation von APC hingegen verbessert die nutritive Perfusion und verringert die inflammatorische Antwort insbesondere bei traumatisierten Tieren mit Endotoxinämie. Diese experimentellen Ergebnisse könnten den Mechanismus der APC Wirkung bei chirurgischen Patienten mit schwerer Sepsis repräsentieren.
Objective: To explore if patients with severe sepsis and with a predicted high risk of death (according to the Simplified Acute Physiology Score II) might have a treatment benefit from high-dose antithrombin III. Design: Subgroup analysis of a randomized, placebo-controlled, double-blind, prospective phase III study. Setting: Unifactorial and multifactorial reanalysis of prospectively defined populations from the KyberSept trial. Patients: We studied 1,008 patients (43.6% of the overall intention-to-treat population, n = 2,314) with a predicted mortality rate of 30–60% at study entry as defined by the Simplified Acute Physiology Score II. Interventions: Patients were randomized in a 1:1 fashion to receive either high-dose antithrombin III (30,000 IU intravenously over the period of 4 days) or placebo. Measurements and Main Results: In a Kaplan-Meier analysis of patients with a predicted mortality of 30–60%, the survival time when followed up for 90 days after admission was increased in the high-dose antithrombin III group compared with placebo (p = .04). If heparin was avoided during the 4-day treatment phase with high-dose antithrombin III (n = 140) or placebo (n = 162), the treatment effect appeared to be even more pronounced: 28-day mortality rate, 35.7% vs. 44.4% (risk ratio, 0.804; 95% confidence interval, 0.607–1.064); 56-day mortality rate, 39.9% vs. 52.2% (risk ratio, 0.764; 95% confidence interval, 0.593–0.984); 90-day mortality rate, 42.8% vs. 55.1% (risk ratio, 0.776; 95% confidence interval, 0.614–0.986). Like in the overall population, the percentage with any bleeding was increased in patients receiving high-dose antithrombin III compared with placebo. Survival rates were in favor of high-dose antithrombin III in patients both with and without bleeding complications. Conclusions: Treatment with high-dose antithrombin III may increase survival time up to 90 days in patients with severe sepsis and high risk of death. This benefit may even be stronger when concomitant heparin is avoided.

Mit natürlichen Gerinnungsinhibitoren (aktiviertes Protein C, Antithrombin, Gewebsfaktorinhibitor (TFPI), C1-Esteraseinhibitor) stehen Substanzen zur Verfügung, die neben ihren antikoagulatorischen Eigenschaften direkte immunmodulatorische Wirkung auf verschiedenen Ebenen aufweisen. Neben grundsätzlichen Mechanismen der Immunmodulation durch Gerinnungshemmung werden spezifische antiinflammatorische Effekte von natürlichen Gerinnungsinhibitoren diskutiert. Diese können über Zellkernfaktor Beta Hemmung (aktiviertes Protein C, Antithrombin) und Prostazyklinfreisetzung (Antithrombin) nicht nur die Leukozyten-Thrombozyten-Endothelzellinteraktion reduzieren, sondern hierdurch auch zu einer verbesserten kapillaren Perfusion führen. Die Perfusionsverbesserung stellt eine Voraussetzung zum Erhalt der Organfunktion dar und sollte als Zielkriterium zur Effektivitätsbeurteilung einer adjuvanten Sepsistherapie beachtet werden.
