原发性甲状腺功能减退症(甲减)是由于甲状腺激素(TH)合成和分泌减少导致的全身代谢减低综合征,其最常见的病因是桥本甲状腺炎,主要分为临床甲减(OH)和亚临床甲减(SCH)两种类型.一项甲状腺 功 能 筛 查 显 示,妊 娠 期 女 性 SCH 发 病 率 为9.6%[1].而维持正常的妊娠及哺乳与甲状腺功能息息相关,甲减可能增加不良妊娠结局风险[2],损害子代神经系统发育[3],还可能影响产后哺乳[4].甲减产后管理与妊娠期管理同样重要.本文从甲减产后管理的重要性、安全性、指南及研究进展三个方面进行文献综述,旨在提高临床医师对妊娠期甲减产后管理的认识.
目的 探讨自身免疫病相关性噬血细胞性淋巴组织细胞增多症(autoimmune associated hemophagocytic lymphohistiocytosis,AAHLH)患者的临床特点.方法 收集并整理16例AAHLH患者的基础疾病、 并发感染情况、 临床、 实验室检查、 治疗及预后情况.结果 基础疾病:系统性红斑狼疮(systemic lupus erythematosus,SLE)7例,类风湿关节炎(rheumatoid arthritis,RA)1例,SLE并发RA 1例,原发性干燥综合征(primary Sjogren's syndrome,pSS)1例,皮肌炎(dermatomyositis,DM)1例,未分化结缔组织病(undifferentiated connective tissue disease,UCTD)5例.部分患者同时并发感染,包括病毒感染4例,细菌感染3例,混合感染7例,感染部位以肺部为主.临床表现:高热(100%)、 肝肿大(18.8%)、 脾肿大(68.8%)、 淋巴结肿大(75%)、 皮疹(62.5%)、 关节炎(68.8%),并发弥漫性血管内凝血(disseminated intravascular coagulation,DIC)3例(18.8%).实验室检查:血细胞减少(100%)、 肝功能异常(87.5%)、 高甘油三脂血症(100%)、 低纤维蛋白原血症(43.8%)、 铁蛋白>500μg/L(100%)、 可溶性CD25(sCD25)≥2400 U/ml(75%)、NK细胞活性降低(100%).骨髓检查有成熟的单核巨噬细胞吞噬血细胞现象占56.3%.经过积极抗感染、 治疗原发病以及经人免疫球蛋白、 糖皮质激素、 免疫抑制剂治疗,存活12例,病情恶化自动出院3例后死亡,在院死亡1例.结论 AAHLH临床表现多样,容易并发感染,病死率高.sCD25水平高可能是AAHLH预后的不良因素.
目的 观察达格列净对早期2型糖尿病肾病(DN)患者肾功能和尿微量白蛋白的影响,探讨其与患者血清促炎症因子变化的关系.方法 纳入30例初诊2型糖尿病(T2DM)、50例早期DN患者与30例正常对照(NGT).T2DM组和DN组使用达格列净治疗12周,观察其疗效、肾功能和血清促炎症因子的变化.结果 治疗后,T2DM组和DN组除空腹血糖和糖化血红蛋白降低外,体重和血压也下降(均P<0.05);两组的肾小球滤过率均有所下降(均P<0.05);DN组的尿白蛋白/肌酐比值降低(P<0.05);糖尿病患者较正常对照组血清促炎症因子增加,而治疗后单核细胞趋化因子-1、白细胞介素-6和肿瘤坏死因子-α水平降低(均P<0.05).结论 达格列净能改善早期DN患者的尿蛋白漏出.除降糖、降压和减重外,该作用可能与促炎症因子水平的降低直接有关.
目的:研究双环己酮草酰二腙(cuprizone,CPZ)诱导C57BL/6小鼠建立的精神分裂症样模型小鼠海马内有髓神经纤维体积改变及其与行为学改变之间的关系.方法:19只雄性6周龄小鼠,随机分为实验组(9只)和对照组(10只),实验组用含0.2%(质量分数百分比,w/w)CPZ的混合鼠饲料饲喂6周,建立精神分裂症样小鼠模型;对照组用标准鼠饲料饲喂6周.运用旷场实验、高架十字迷宫实验、Morris水迷宫实验、转棒实验和探孔实验测试行为学改变;髓鞘碱性蛋白(myelin basic protein,MBP)免疫组化染色及透射电镜定性观察髓鞘结构变化;体视学方法测量并计算海马总体积及海马内有髓神经纤维体积;统计学方法分析行为学改变与海马内有髓神经纤维体积改变之间的关系.结果:与对照组相比,实验组小鼠体质量下降,差异具有统计学意义(P<0.05);行为学实验结果表明:实验组小鼠存在异常的焦虑行为(P=0.018)及空间认知能力障碍(P=0.037),但运动能力(P=0.443)、探索习性(P=-0.306)及学习记忆能力未见受损(P=0.462);MBP免疫组化染色发现海马区域免疫阳性染色变浅;透射电镜定性观察发现海马内有髓神经纤维存在脱髓鞘改变;体视学定量测定发现,实验组小鼠海马总体积未发生显著性改变(P=0.955),而海马内有髓神经纤维体积减小(P=0.009);统计学相关分析发现,闭臂路程百分比和海马内有髓神经纤维体积呈正相关(rs=0.83,P=0.003),开臂路程百分比和海马内有髓神经纤维体积呈负相关(rs=-0.65,P=0.043).结论:进一步证实CPZ模型小鼠可以出现类似精神分裂症样症状的行为学改变及海马内有髓神经纤维存在脱髓鞘改变,并且发现海马内有髓神经纤维体积和行为学表现间具有一定的相关性.
BACKGROUND:Simple vertebral canal decompression for degenerative lumbar scoliosis with stenosis cannot obtain long-period curative effects. Simple decompression is considered aiatrogenic destabilization of lumbar vertebra, and aggravates lumbar malformation. <br> OBJECTIVE:To analyze the effectiveness of posterior decompression, fixation and fusion in the treatment of degenerative lumbar scoliosis with stenosis and to evaluate corrective angle during half-year fol ow-up. <br> METHODS:A total of 23 cases of degenerative lumbar scoliosis with stenosis underwent posterior decompression, fixation and fusion. Visual Analogue Scale, Oswestry disability index, 36-Item Short-Form Health Survey scale and the Cobb angle on the lumbar coronal film were used to assess the changes in quality of life and corrective angle before treatment and during final fol ow-up. <br> RESULTS AND CONCLUSION:Al patients were fol owed up for at least 6 mouths. Mean Cobb angles were (23.94±11.4)° pretreatment and (10.28±6.93)° posttreatment. Mean Visual Analogue Scale scores were significantly lower posttreatment than pretreatment (P<0.05). Oswestry disability index scores were significantly lower posttreatment than pretreatment (P<0.05). The excellent and good rate of Oswestry disability index score was 83.33%. The eight scaled scores of 36-Item Short-Form Health Survey scale were significantly higher posttreatment than pretreatment (P<0.05). During half-year fol ow-up, there was no obvious loss in corrective angle. Shift of interbody cages was not displaced. No internal fixator breakage appeared. The rate of fusion for bone graft was 100%.
目的:探讨雌激素替代治疗(estrogen replacement treatment,ERT)对卵巢切除(ovariectomy,OVX)大鼠海马CA1区有髓神经纤维及其髓鞘的影响.方法:对10~11月龄的雌性SD大鼠进行双侧卵巢切除术,术后1月将成功切除卵巢的大鼠随机分为安慰剂对照组(OVX+O组)和雌激素替代治疗组(OVX+E组),前者给予连续4周安慰剂(芝麻油)处理,后者给予连续4周17β-雌二醇替代治疗,4周后采用Morris水迷宫对2组大鼠的空间学习能力进行测试,用化学发光法测定2组大鼠体内循环雌激素浓度,用透射电子显微镜和体视学方法对两组大鼠海马CA1区有髓神经纤维及其髓鞘进行定量研究.结果:OVX+E组大鼠体内循环雌激素浓度明显高于OVX+O组(t=5.688,P=0.000); OVX+E组大鼠空间学习能力明显提高(F=9.645,P=0.006).2组大鼠CA1区体积(t=1.451,P=0.181)、有髓神经纤维体积(t=-1.158,P=0.294)、有髓神经纤维髓鞘体积(u=-0.642,P=0.521)、有髓神经纤维平均直径(t=0.630,P=0.545)和有髓神经纤维髓鞘厚度(u=-1.746,P=0.081)均不存在统计学差异,但是OVX+E组直径在0.6~0.7 μm的有髓神经纤维髓鞘厚度比OVX+O组大鼠增加14.1%(t=2.428,P=0.038),OVX+E组直径在0.8~0.9μm的有髓神经纤维直径与轴突直径差比OVX+O组大鼠增加18.2%(t=2.432,P=0.038).结论:短期雌激素替代治疗可以改善大鼠空间学习能力并对海马CA1区有髓神经纤维髓鞘具有保护作用.
Objective To explore an appropriate dose of cuprizone(CPZ) for the CPZ-fed animal model as a novel schizophrenia animal model,and to investigate the possible relationship among CPZ dose,demyelination of nerve fibers in hippocampus and behavior alternations in CPZ-fed mice.Methods C57BL /6 mice were fed with 0.2%,0.3% and 0.4%(w / w) of CPZ for 6 weeks,respectively,and the mice without any treatment were used as control.The body weight,survival time and behavior changes were compared between the CPZ-fed mice and the control mice.The myelin basic protein(MBP)-positive nerve fibers in CA1 and DG regions of hippocampus were stained with immunohistochemical methods,and the mean optical density(OD) of MBP-positive nerve fibers in the hippocampal CA1 and DG regions of each group were calculated and compared.The changes of the myelinated nerve fibers in CA1 and DG regions of hippocampus between the control group and 0.2% CPZ group were investigated with a stereological method.Results The survive time of the 0.4% CPZ group was significantly less than that of control group(P < 0.01).The body weight of the CPZ-fed groups was significantly lower than that of the control group(P < 0.05),and the weight loss increased along with the increase of CPZ doses.In open field test,the mice fed with 0.2% of CPZ spent significantly more time in the central area of box compared with the control group(P < 0.05).The ODs of MBP-positive fibers in CA1 region of hippocampus in the 0.2% CPZ group and 0.3% CPZ group were significantly lower than that in the control group(P < 0.01).The OD of MBP-positive fibers in DG region of hippocampus in the 0.2% CPZ group was significantly lower than that in the control group and 0.3% CPZ group(P < 0.01).The volumes of CA1 and DG regions of the hippocampus in the 0.2% CPZ group were not significantly different from those in the control group(P > 0.05).The length density and total length of the myelinated nerve fibers in CA1 and DG regions of hippocampus in the 0.2% CPZ group were significantly less than those in the control group(P < 0.05).Conclusion 0.2% CPZ(w / w) was the appropriate dose for CPZ-fed animal model to mimic schizophrenia.The myelinated fibers in CA1 and DG regions of hippocampus undergo demyelination when mice are exposed to 0.2% CPZ for 6 weeks.

尽管弥漫大B细胞淋巴瘤(diffuse large B-cell lymphoma,DLBCL)患者的总体预后有所改善,但仍有大约三分之一患者属于难治性/复发性DLBCL,这是导致其发病率和病死率增加的主要原因。临床上治疗难治性/复发性DLBCL的方法仍以大剂量化疗以及对无并发症的患者进行大剂量化疗-自体干细胞移植(high-dose chemotherapy-autologous stem cell trans-plant,HD-ASCT)为主。但对于给予利妥昔单抗联合CHOP化疗(rituximab-CHOP,R-CHOP)治疗后无反应的难治性DLBCL患者进行HD-ASCT,预后极差。因此,如何提高难治性/复发性DLBCL患者总生存期成了新的研究热点。本文主要从治疗方面对难治性/复发性DLBCL进行综述。
Objective To investigate the changes of cerebral cortex volume and myelinated fibers in the cerebral cortices of cuprizone(CPZ)-induced schizophrenia-like mice.Methods Six-week old male C57BL/6 mice were divided into a CPZ group and a control group.The mice in the CPZ group were fed with mixed standard rodent chow containing 0.2% CPZ,while those in the control group were fed with standard lab chow.After six weeks,behavioral tests were performed to confirm the success of schizophrenia-like animal model.Then the cerebral cortex volume and myelinated fibers in the cerebral cortices were determined with transmission electron microscopy and stereological methods.Results Mice in the CPZ group showed schizophrenia-like performance.Stereological results showed that there was no significant change in the volume of the cerebral cortex between CPZ group and control group(P>0.05).Compared with the control group,the length density and total length of myelinated fibers in the cerebral cortices of the CPZ group significantly decreased by 64.3% and 68.9%,respectively(P<0.01).Compared with the control group,the mean diameter of myelinated fibers in the CPZ group significantly increased by 17.8%(P<0.01),and the total length of the myelinated fibers with diameters of 0.2 to less than 0.4 μm,0.4 to less than 0.6 μm and 0.6 to less than 0.8 μm significantly decreased by 4.317,3.313 and 0.940 km,respectively(P<0.01).There was no significant difference in the total length of other myelinated fibers between the two groups.Conclusion In CPZ group,the total length of myelinated fibers in cerebral cortex decreases but the mean diameter of the myelinated fibers increases,which is due to the loss of small-diameter myelinated fibers.
Objective To investigate the changes of the myelinated fibers in the corpus callosum of the cuprizone(CPZ)-induced mouse model of schizophrenia,and the relationship between the demyelination changes and the behavioral abnormalities in this animal model.Methods Nineteen male C57BL/6 mice were randomly divided into 2 groups,control group(n=10) and CPZ-treated group(n=9).The mice in the later group were fed with a diet of 0.2%(w/w) CPZ mixed into standard rodent chow for 6 weeks.The mice in control group were fed standard lab chow and lab water.The behavioral changes of the animals were investigated with a series of behavioral tests.The changes of the corpus callosum and the myelinated fibers in the corpus callosum were studied with immunohistochemical assay,transmission electron microscopy and stereological methods.Results The CPZ-treated mice displayed more activities in the central area of open field(P<0.05)and in the open arms of elevated plus maze(P<0.05),which suggested that the anxiety behavior in the CPZ-treated mice was reduced when compared to the control.Morris water maze demonstrated that there was no working memory deficit in the CPZ-treated mice compared to the control(P>0.05).There was no abnormality in the motor function and basic exploration ability in the CPZ-treated mice when compared to the control.There were demyelination changes in the corpus callosum of the CPZ-treated mice.There was no significant difference in the total volume of the corpus callosum between the CPZ-treated mice and the control(12.66±1.07 vs 13.53±2.79 mm3).The length density and total length of the myelinated fibers in the corpus callosum were significantly decreased in CPZ-treated mice than control mice(0.70±0.17 vs 1.47±0.17 km/mm3,9.06±2.56 vs 19.75±3.70 km,P<0.05).Conclusion CPZ-treated mice display schizophrenia-like behaviors and demyelination change in the corpus callosum.
Objective To explore the role of white matter injuries in the schizophrenia induced by the NMDA re-ceptor antagonist. Methods Adult male C57BL/6J mice (8 week old) were equally divided into four groups. One group was sub-chronically treated with saline solution, and the other three groups were intraperitoneally treated with MK-801 at dose of 0.025 mg/mL (M1), 0.050 mg/mL (M2) and 0.100 mg/mL (M3) in a volume 10 ml per kilogram body weight. All ani-mals were tested using Morris water maze at the 9th-15th day and using the Hole Board exploration as well as Rota Rod performance tests on the 16th day. The myelin basic protein (MBP) and the ultrastructure of the myelin sheaths in the cor-pus callosum were then examined using immunohistochemical methods, transmission electron microscope technique and stereological methods. Results The repeated sub-chronic MK-801 treatment did not induce impairment of spatial learning and memory in Morris water maze. The MK-801 treatment at 0.25 mg/kg and 1.00 mg/kg but not at 0.50 mg/kg resulted in less exploration to a new environment. The myelin staining with anti-MBP antibody was less intense in all three schizo-phrenic groups when compared to saline control group (P<0.01). Furthermore, MK-801 treatment caused pathological al-terations of the myelin sheaths including segmental demyelination of myelinated fibers and splitting of myelin sheath lamel- lae in schizophrenic groups. The ratio of the injured myelinated nerve fibers in the corpus callosum of MK-801 treated mice [M3 group, (22.42 ± 4.24)%] was significantly higher when compared to the control mice [(3.84 ± 1.35)%,P<0.01)]. Conclusions The present study demonstrated the white matter damages, mainly low MBP expression and segmental demye-lization in the corpus callosum in the mice sub-chronic treated with MK-801, indicating that the white matter changes might be involved in the schizophrenia induced by NMDA antagonist.
Objective: To investigate the effect of matrine on the expressions of SALL4 and downstream target genes of Wnt/β-catenin signaling pathway in erythroleukemia (EL) cell line TF-1. Methods: TF-1 cells were treated with matrine at various concentrations (0.5, 1.0 and 2.0 g/L) for 48 h, using those untreated as control. The expression levels of SALL4 mRNA and downstream target genes (β-catenin, C-myc and Cyclin DI) of Wnt/β-catenin signaling pathway in TF-1 cells of various groups were determined by real-time fluorescent quantitative PCR, between which the relationship was analyzed. The expression level of SALL4 protein in TF-1 cells was determined by Western blot. Results: The expression levels of SALL4, β-catenin, C-myc, Cyclin DI mRNAs as well as SALL4 protein in TF-1 cells 48 h after treatment with matrine at various concentrations decreased significantly in a dose-dependent pattern as compared with those in control group (P < 0.05). The expression of SALL4 gene was significantly related to those of β-catenin, C-myc and Cyclin DI genes, with rs values of 0.912, 0.818 and 0.832 respectively (each P < 0.01). Conclusion: The inhibitory effect of matrine on expressions of SALL4 gene and protein as well as downstream target genes β-catenin, C-myc and Cyclin DI of Wnt/β-catenin signaling pathway may play an important role in inhibiting cell proliferation and inducing cell apoptosis.
Objective: To investigate high sensitivity-C reactive protein(Hs-CRP) as a prognostic factor in non-Hodgkin lymphoma(NHL). Methods: Data for 85 consecutive non-Hodgkin lymphoma patients were followed up and reviewed to determine the value of Hs-CRP relative to known prognostic parameters. Results: The progression-free survival(PFS) and overall survival(OS) times of patients with pre-therapeutic baseline serum Hs-CRP levels of ≥4 mg/L were shorter than those of patients with basal serum Hs-CRP levels of <4 mg/L(P>0.05). However, the post-therapeutic, early-to-mid serum Hs-CRP level was not significantly correlated with the OS and PFS times(P>0.05). Multivariate Cox regression analysis showed that the pre-therapeutic baseline serum Hs-CRP level may be an important prognostic parameter for the relapse and survival of NHL patients(P<0.05). Conclusion: The baseline Hs-CRP level can be a major indicator of prognosis in NHL patients.
Objective :To investigate the stereological morphological changes in cerebral cortical capillaries of mice by UT - B deletion , and discuss pathological mechanism of depress -like behavior in UT -B null mice .Methods We observed depression -like behavior of wild -type and UT -B null mice with the forced swim test , performed immunohistochemistry and stereological techniques to estimate the total length , total volume , total surface area and average diameter of the capillaries in cortex of mice .Results Compare to wild -type ,UT -B null mice exhibited longer passive immobility time (123 ± 19 vs 86 ± 13 s , P=0 .0012<0 .05) .Volume shrinkage of wild -type and UT -B null mice were 0 .53 ± 0 .04、0 .51 ± 0 .07 (P>0 .05) ,respectively .The total length ,total volume ,total surface area and average diameter of cerebral cortical capillaries were (29 .20 ± 1 .20 vs 24 .00 ± 2 .45 m ,P=0 .093>0 .05) ,(1 .159 ± 0 .095 vs 0 .855 ± 0 .060 mm3 ,P=0 .035<0 .05) ,(6 .143 ± 0 .377 vs 4 .616 ± 0 .455 cm2 ,P=0 .042<0 .05) ,(6 .506 ± 0 .161 vs 5 .564 ± 0 .118 ?m ,P=0 .016<0 .05) respectively .Conclusion There were significant morphologic changes of cerebral cortical capillaries induce by UT -B deletion ,which possibly attribute to decreased cerebral blood perfusion and exhibited depression -like behavior .

Objective: To investigate the influence of Bortezomib (Bor) on proliferation and apoptosis of acute promyelocyte leukemia NB4 cell strain and acute erythroleukemia TF1 cell strain as well as on expressions of SALL4 gene and C-mye and CCND1 genes downstream in Wnt/β-catenin signaling pathway. Methods: NB4 and TF1 cells were treated with 10, 30 and 50 nmol/L Bor for 12, 24 and 48 h separately, using those untreated as control, then determined for proliferation activity by MTT method, for apoptosis rate by flow cytometry, and for expressions of SALL4, C-mye and CCND1 genes by RT-PCR and fluorescent quantitative PCR. Results: Bor showed time- and dose-dependent inhibitory effect on proliferation of NB4 and TF1 cells, with IC50 values of 23.97 and 25.36 nmol/L respectively 48 h after treatment. Bor induced the apoptosis of two kinds of cells in dose-dependent mode. The apoptosis rates of NB4 cells 24 after treatment with 30 and 50 nmol/L Bor and TF1 cells 24 h after treatment with 50 nmol/L Bor showed significant difference with those in control group (both P < 0.05). Bor inhibited the expressions of SALL4, C-mye and CCND1 genes in the two kinds of cells significantly. Twenty-four hours after treatment with 50 nmol/L Bor, the expression levels of the three genes in both NB4 and TF1 cells were significantly higher than those in control group (each P < 0.05). The expression of SALL4 gene was closed related to those of C-mye and CCND1 genes (r. = 0.857, 0.929, both P < 0.01). Conclusion: The inhibition of SALL4 gene as well as target genes C-mye and CCND1 downstream in Wnt/β-catenin signaling pathway may play an important role in induction of apoptosis of NB4 and TF1 cells.

Objective To evaluate the clinical value of allogeneic hematopoietic stem cell transplantation(allo-HSCT) with reduced intensity Bu + Cy conditioning regimen in the treatment of patients with leukemia.Methods The reduced intensity Bu + Cy conditioning regimen including busulfan(Bu) 4 mg/(kg·d),for 3 d;cyclophosphamide(Cy) 50 mg/(kg·d),for 2 d,antithymocyte globulin(ATG) 2.5 mg/(kg·d),for 2/4 d,were used in 15 leukemia patients who underwent allo-HSCT in our department from April 2010 to March 2012.There were 2 cases of acute lymphocytic leukemia,3 cases of acute non-lymphoid leukemia,9 cases of chronic myeloid leukemia,and 1 case of Crisis phase of CML.Results All patients achieved successful hemopoietic reconstruction.The median time of the neutrophil recovery >0.5×109/L was 12(ranging from 10 to 17) d and the platelet recovery >20×109 /L was 15(12 to 27) d after transplantation.In 15 patients,short tandem repeat polymerase chain reaction(STR-PCR) confirmed that the donor cells were fully implanted on day 30.Transplant-related complications were acute graft-versus-host disease(aGVHD) in 6 patients(40%)(2 patients with aGVHD=grade Ⅰ,4 patients with aGVHD=grade Ⅱ,and none with aGVHD=grade Ⅲ-Ⅳ),chronic graft-versus-host disease(cGVHD) in 8 patients(53.3%)(2 cases of extensive-type,and 6 cases of limited-type),hemorrhagic cystitis in 3 patients(20%),and infected cytomegalovirus in 4 patients(26.7%).There was no other serious complication.All patients were followed up for a median time of 15.4 months(ranging from 6 to 30).One patient(6.7%) died of side effect related to the conditional regimen.Two patients(13.3%) got recurrence,and the rest(80.0%) survived event-free for 4 to 30 months.Conclusion After treated with reduced intensity Bu + Cy conditioning regimen-HSCT,leukemia patients have a better hematopoietic reconstitution and less transplant-related complications.It is also suggested that the pre-conditioning regimen has a low recurrence rate and a lower mortality.Therefore,this method maybe one of the safe and effective treatments for leukemia.
