Die Technik der Doppelradionuklidszintigraphie zur Placentalokalisation wird anhand von vier Anwendungsbeispielen demonstriert.
Bei insgesamt 24 Patienten einer internistischen Intensivstation wurde im Rahmen der totalen parenteralen Ernährung eine 5 %ige Fett-emulsion eingesetzt, bei der 25 % des Fettanteiles durch mittelkettige Triglyzeride (MCT) ersetzt sind. Der Beobachtungszeitraum erstreckte sich zwischen 3 und längstens 22 Tagen. Dabei kam es zu keiner iatrogenen Hyperlipidämie und zu keiner Beeinflussung von Leber- und Kohlenhydratstoffwechsel. Infusionsbedingte Nebenreaktionen wurden niemals beobachtet. Trotz relativ hoher Kalorienzufuhr (etwa 2600 kcal/Tag = 10 885 kJ/Tag) und hoher Aminosäurezufuhr (100 g/Tag) kam es zu einem Abfall des Albumins, der als Ausdruck einer Katabolie gewertet werden muβ. Der von uns beobachtete geringe Amylaseanstieg bedarf der weiteren Klärung. Die MCT-haltige Fettinfusion wurde von den Patienten gut toleriert. Somit stehen in der parenteralen Ernährung neben den Kohlenhydratinfusionen auch mittelkettige Triglyzeride als rasche Energiedonatoren zur Verfügung.
Nach intravenöser Dauerinfusion einer Kohlenhydratkombinationslösung, bestehend aus Fruktose, Glukose und Xylit (2/3n1/3n1) mit einer Zufuhrrate von 0,2 g/kg/h und gleichzeitiger Zufuhr von Aminosäuren mit einer Dosierung von 0,03 g/kg/h, fanden sich nur geringe Verluste von Fruktose, Glukose und Xylit. Nur bei 3 Patienten lag der Endverlust über 11%.
A twenty-four percent solution of carbohydrates 0.2 g/kg/h consisting of fructose, xylit and glucose in a 2:1:1 ratio and an aminoacid solution 0.03 g/kg/h were given to 13 patients of the internal medicine intensive care unit. There was an unimportant loss of carbohydrates, only, with but few exceptions, i.e. a loss of 11% in 3 patients. The results demonstrate that the carbohydrates of the combined solution become utilized independent of each other. The balance of water, sodium, potassium and magnesium were not only negative but also positive. The elimination of calcium was between 1-19 mval/24 h, of phosphate between 0.6-8.7 mval/24 h.
Sonography is suitable for the examination of thyroid glands respectively goiters. This method particularly gains in the differentiation of cold nodules or cold regions. In more than 50% of patients with cold nodules sonography showed cystic lesions. In addition, ultrasonically guided percutaneous fine needle biopsy is employed for emptying cysts or gaining material for cytological examination.
The technique of using double radionuclide-scintigraphy for placental localization is demonstrated by four examples of its use.
