Sowohl die Hyperlaktatämie als auch eine metabolische Azidose sind Gegenstand wissenschaftlicher Untersuchungen als prädiktive Parameter zur Abschätzung des neurologischen Behandlungsergebnisses nach kardiopulmonaler Reanimation. Laktat wird dabei als Surrogatparameter für eine globale Hypoperfusion nach zirkulatorischem Kollaps gewertet. Wir präsentieren den Fall eines reanimierten Patienten mit einem initialen pH von 6,45 und einer Laktatkonzentration von 21 mmol/l mit begleitender Ketonämie, dessen Laktatazidose nicht maßgeblich auf einen kardialen Arrest, sondern auf eine metforminassoziierte metabolische Entgleisung zurückzuführen ist. Der Patient überlebte eine 4‑minütige Reanimation und einen 45-tägigen Intensiv- und Krankenhausaufenthalt mit einer Fußparese als einzigem neurologischem Defizit. Bei nicht hypoxischen Laktatazidosen (Typ B) ist die prognostische Aussagekraft von pH-Wert und Laktat eingeschränkt zu werten.
Critically ill non-trauma patients are increasingly managed in the emergency department (ED) for prolonged periods because of hospital crowding, intensive care unit (ICU) capacity constraints, and diagnostic or procedural delays. During this interval, ED teams are required to provide emergency critical care (ECC) that extends beyond initial resuscitation and includes continued organ support, target-based reassessment, prevention of secondary complications, and structured handover. However, unlike trauma and cardiac arrest care, ongoing ECC for boarded non-trauma patients is less consistently supported by ED-specific practice frameworks. This Policy and Practice Review introduces OSCARinED, a pragmatic framework designed to organize ED-based ECC after initial ABCDE stabilization. OSCARinED integrates seven recurrent care domains: oxygenation and ventilation; sedation and analgesia; catecholamines, fluid therapy, and catheter management; anti-infective therapy; risk-adapted positioning and prophylaxis; electrolyte, blood gas, glucose, and nutrition management; and delirium prevention and treatment. The framework links trigger recognition and framework activation to explicit treatment targets, scheduled reassessment, documentation, and disposition planning. OSCARinED is intended as a safety bridge during ED boarding, not as a substitute for ICU admission. Its implementation may support standardization of care, reduce omission-prone tasks, improve handover reliability, and provide a structure for local quality improvement. Future studies should evaluate OSCARinED using implementation-science designs, focusing on process reliability, team performance, patient-centered outcomes, and ED operational endpoints.
Background and importance : In contrast to trauma care, evidence on the epidemiology and outcomes of non-traumatic critically ill (NTCI) patients admitted to emergency departments (EDs) remains limited. Objectives : This study examined the epidemiology, incidence and outcomes of NTCI patients in nine German and Danish EDs using a shared transnational research approach. Design : Data from adult NTCI patients across four German centres (2014–2021) were pooled and compared with data from patients triaged as the most urgent level (red in Danish Emergency Process Triage) in five EDs in Southern Denmark (2016–2018). Main results : Altogether, 408,673 Danish and 174,306 German ED admissions were analysed. In Denmark, 1.4% of ED admissions (range 0.7–2.0%) involved non-traumatic critical illness, similar to 1.6% (range 0.5–1.6%) in Germany. Mean patient age was 63–70 years, with 51–60% male across sites. Most patients presented with acute breathing (B) problems (21.7–45.8%), circulation (C) problems (31.2–47.4%), or disability (D) problems (11.3–41.2%). Admission rates to intensive or intermediate care were markedly higher in German centres (76.0–85.5%) compared with Danish sites (9.3–20.0%). Thirty-day mortality was also higher in Germany (31.6%) than in Denmark (26.5%). Conclusion : Approximately 1.4% of all ED patients required care for non-traumatic critical illness, most commonly presenting with acute breathing, circulation, or neurological impairment. These patients show high mortality and substantial need for advanced in-hospital care, underlining the need for targeted clinical pathways. Differences between German and Danish centres, particularly in admission strategies and mortality, highlight the necessity for standardized 24/7 management and future research on organizational factors and optimal treatment pathways for NTCI patients.
In recent years, effective and safe patient handover concepts have been established in the resuscitation room. The utilisation of a structured scheme and the establishment of a 'handover culture', wherein all team members recognise the significance of this phase, serve as crucial milestones. In the interest of patient safety, it is recommended that a "5-second round" is conducted prior to the initiation of the handover process. The purpose of this procedure is to facilitate the evaluation of the patient's stability during the handover phase, thereby identifying the necessity for critical interventions to be performed beforehand. Originally developed for trauma patients, this tool is also relevant for critically ill non-trauma patients. The objective of this article is to provide a more detailed description of the original 5-second round, to adapt its application for use in critically ill non-trauma patients and to give some examples of concrete actions.
An increasing proportion of critically ill patients receive prolonged, high-acuity care in the emergency department (ED) before admission to the intensive care unit. Despite its clinical relevance, this early phase of care remains poorly characterized and difficult to capture using existing metrics. This Perspective argues that this early phase of critical illness represents a distinct but underrecognized component of the critical care continuum, commonly referred to as emergency critical care (ECC). Rather than being defined by location or specialty, ECC is characterized by time-critical decision-making, dynamic trajectory, and evolving care demands. A major challenge in studying this phase is its limited measurability within current frameworks. We therefore propose a pragmatic conceptualization of ECC based on three complementary dimensions: disease severity, clinical care intensity, and therapeutic organ support. Together, these dimensions describe key aspects of early critical illness and provide a pragmatic basis for translating ECC dimensions into observable clinical and administrative variables suitable for research, governance, and system evaluation without imposing rigid definitions or thresholds. Conceptualizing ECC along clinically meaningful and observable dimensions offers a practical way to improve its visibility in research and system evaluation. Recognizing ECC as a spectrum of critical care delivery rather than a binary state may provide a pragmatic basis for describing and studying early critical illness.
BACKGROUND:Poisoned patients represent a substantial proportion of cases in emergency medicine and critical care. OBJECTIVE:This study aimed to assess the perceived current practice of managing poisoned patients in German prehospital emergency services, emergency departments, and intensive care units from the perspective of frontline staff. MATERIALS AND METHODS:A prospective cross-sectional survey was conducted among personnel in German prehospital emergency services, emergency departments, and intensive care units from 3-19 October 2025. RESULTS:A total of 563 participants, predominantly from hospital-based and prehospital emergency care, estimated that poisoned patients accounted for a median of 5% (interquartile range [IQR] 3-10) of all cases in their clinical practice and required monitoring in 70% (IQR 40-90) of cases. Alcohol- and drug-related intoxications were most commonly encountered, whereas poisonings involving medications, chemicals, plants/mushrooms, and animals were less frequent. Most participants rated their formal training in the management of poisoned patients as insufficient. Many expressed a need for further education, specifically trained clinical staff, and a German-language digital information platform. Information was commonly obtained through independent internet searches or poison information centers. However, poison information centers were contacted in only about 30% of cases. CONCLUSION:The findings point to a perceived gap in toxicological training and knowledge transfer in emergency and critical care. Poison information centers were regarded as valuable but not sufficient as the only source of expertise. Strengthening toxicological competencies through structured training and digital information resources may support acute care practice.
Neurologische Probleme im Sinne von „Disability“, entsprechend den D‑Problemen des ABCDE-Schemas, stellen einen häufigen Anteil in der Versorgung kritisch kranker PatientInnen im Rettungsdienst dar. Die Vigilanzminderung und das akute fokal-neurologische Defizit sind hierbei die führenden Symptome. Da die initial unklare Vigilanzminderung infolge vieler zugrunde liegender Notfallbilder auftreten kann, sind eine strukturierte Anamneseerhebung und Differentialdiagnostik notwendig, um kritische Diagnosen zu identifizieren, eine mögliche Arbeitsdiagnose zu erarbeiten und die PatientInnen einer geeigneten Zielklinik zuzuweisen.

Zur Schockraumversorgung nichttraumatologisch kritisch kranker Kinder liegen bislang wenig Daten vor. Die Versorgung findet in Kindernotaufnahmen, in Akutversorgungsbereichen von Kinderintensivstationen und in den Schockräumen von Zentralen Notaufnahmen statt. Durch selbstständige Vorstellung von Patienten bzw. lokale Rettungsdienstdispositionen erfolgen teilweise aber auch Erstversorgungen in Notaufnahmen ohne angebundene Fachabteilung für Kinder- und Jugendmedizin. Für eine strukturierte Schockraumversorgung nichttraumatologisch kritisch kranker Kinder existiert bislang keine einheitliche Struktur, die die Versorgungsphasen von der Alarmierung und Vorbereitung bis zur Disposition der Patienten umfasst. Für erwachsene Patienten liegt mit dem (PR_E‑)AUD2IT-Algorithmus ein solcher Leitfaden bereits vor. Background: There are differing opinions among various professional associations and societies regarding the introduction of a specialist title for emergency medicine in Germany. The aim of this survey was to assess the attitudes of the emergency medicine junior professionals on this topic. Aim: This study examines the perspective of medical students and training assistants on a specialist title in emergency medicine. Methods: An online survey (www.empirio.de) was conducted with a total of nine questions targeted at medical students and training assistants of the Young Emergency Medicine of the Association of Emergency Physicians in North Rhine-Westphalia. Participation was voluntary and anonymous. Results: A total of 198 individuals (53% male) primarily from the age group of 21-30 years (70%) participated in the survey. The majority, with 85.4% of respondents, support the introduction of a specialist in emergency medicine. The respondents emphasize the importance of aspects such as training, working time models, remuneration, and career prospects. The emergency medicine junior professionals particularly express a desire for structured training, individualized working time models, and appropriate remuneration. A forward-looking approach to address existing shortages in primary care is to allow physicians who have completed a specialist qualification in emergency medicine to subsequently establish themselves as general practitioners. Conclusion: The present survey demonstrates the strong desire among emergency medicine-interested students and training assistants for the introduction of a specialist in emergency medicine.
Die Einführung einer Facharztweiterbildung für Notfallmedizin unterliegt in Deutschland einer umfassenden Debatte und es gibt unterschiedliche Ansichten seitens verschiedener Berufsverbände und Fachgesellschaften. Ziel der vorliegenden Umfrage war es, die Einstellung des notfallmedizinischen Nachwuchses zu diesem Thema einzuordnen. Untersuchung der Sichtweisen von Medizinstudierenden und Weiterbildungsassistent:innen in Bezug auf den Facharzt für Notfallmedizin. Es erfolgte eine Online-Befragung ( www.empirio.de ) mit insgesamt neun Fragen unter Medizinstudierenden und Weiterbildungsassistent:innen der Jungen Notfallmedizin der Arbeitsgemeinschaft Notärzte in Nordrhein-Westfalen (AGNNW). Für den internationalen Spielbetrieb im Fußball existieren Vorgaben der Union of European Football Associations (UEFA) für die medizinische Betreuung. Für die spezielle notfallmedizinische Versorgung zum Beispiel bei einem plötzlichen Herztod („sudden cardiac arrest“ [SCA]) existieren ebenfalls Empfehlungen zur Notfallversorgung. Besondere organisatorische Herausforderungen bestanden im Rahmen der Fußballeuropameisterschaft der Herren (UEFA EURO 2024) zur Notfallversorgung im Stadion und auf dem Spielfeld. In der Vorbereitung der UEFA EURO 2024 in Deutschland wurde für den Standort Düsseldorf ein umfangreiches Sicherheitskonzept entwickelt, welches auch eine spezielle notfallmedizinische Versorgung am Spielfeldrand vorsah. Spezielle Versorgungsalgorithmen wurden hierbei erstellt und trainiert. Während der fünf Spieltage in der Arena Düsseldorf wurde eine einzelne Hilfeleistung am Spielfeldrand dokumentiert. Die Erfahrung mit dem ausgearbeiteten Versorgungskonzept wurde jedoch äußerst positiv bewertet. Die entwickelten und trainierten Konzepte zur notfallmedizinischen Versorgung am Spielfeldrand können als Best-Practice-Beispiel für weitere Sportveranstaltungen dienen, sowohl für die Einsatzorganisation als auch medizinisch.
The management of critically ill patients, arriving at the emergency department (ED), requires structured care in critical care facilities, particularly in the resuscitation room. This study examines the significance of initial vital signs and blood gas analysis (BGA)-derived values as clinically useful early indicators of mortality risk in critically ill patients, both during in the resuscitation room care and within the following 30 days, with a focus on evaluating the individual predictive performance of accessible clinical parameters. We pooled data from two consecutive retrospective observational studies in a German university ED to analyze an unselected patient population of non-traumatic critically ill patients. Vital signs, such as heart rate, systolic blood pressure, and BGA values (including pH, bicarbonate, carbon dioxide, glucose, lactate, electrolyte levels) on admission to the ED, were used to estimate the impact on both resuscitation room and 30-day mortality. In 1,536 critically ill patients, pH, lactate and bicarbonate were found to be potential predictors of resuscitation room mortality. In contrast, vital signs showed limited reliability in predicting outcomes. Während die Versorgung kritisch kranker Erwachsener in den letzten Jahren verstärkt in den Fokus gerückt ist, gibt es vergleichsweise wenig Forschung zur Organisation und Struktur für nichttraumatologisch kritisch kranke Kinder und Jugendliche. Die Umfrage untersucht den Status quo in deutschen Notaufnahmen. Diese deutschlandweite Querschnittsstudie wurde als Online-Umfrage von den Arbeitsgruppen „Kindernotaufnahme“ und „Schockraum“ der Deutschen Gesellschaft für Notfallmedizin e. V. (DGINA) entwickelt. Acute bleeding often occurs in a wide variety of forms in clinical emergency medicine. In traumatic bleeding, soft tissue injuries to the head or extremities are common, while severe bleeding is less common in chest injuries or pelvic trauma. In nontraumatic emergencies, gastrointestinal bleeding is the leading cause, but many other bleeding entities are possible. Structured management helps to detect the critically ill patient, identify the possible source of bleeding and provide appropriate radiological diagnostics and therapy. Clinical examination, blood gas analyses and emergency ultrasound are crucial in the initial phase of diagnosis. Initially, symptomatic therapy is usually indicated to stabilize the patient, and then interventional or surgical care is indicated in the course of the disease, depending on the location of the bleeding. Volume and catecholamine therapy, coagulation management and emergency transfusion are important components of interdisciplinary emergency care, in addition to primary bleeding control, especially in the case of open injuries.
Mit der Einführung von Advanced Critical Illness Life Support (ACiLS) wurde in Deutschland im Jahr 2022 ein eigenes Kurskonzept für die nichttraumatologische Schockraumversorgung eingeführt. Inhaltliche Schwerpunkte sind die Teamkommunikation, die Durchführung ausgewählter kritischer Interventionen, ein strukturierter Ansatz für Differenzialdiagnostik und Sonographie sowie ein standardisiertes Vorgehen nach dem (PR_E-)AUD2IT-Algorithmus. Ziel des ACiLS-Kurskonzepts ist es, die Versorgung kritisch kranker nichttraumatologischer Patienten im Schockraum zu jeder Tages- und Nachtzeit in Krankenhäusern aller Notfallstufen auf einem hohen Standard zu vereinheitlichen.
Penetrierende Verletzungen sind eine seltene, aber immer wieder vorkommende Einsatzsituation in der Notfallmedizin. Das Ziel der Untersuchung war es, die Häufigkeit und Verletzungscharakteristika penetrierender, gewaltassoziierter Verletzungen einer Metropolregion über einen 5‑jährigen Zeitraum zu ermitteln. In der retrospektiven Untersuchung wurden anhand einer Datenbankabfrage der Leitstelle des Rettungsdienstbereichs Düsseldorf sämtliche rettungsdienstliche Einsätze mit penetrierenden, gewaltassoziierten Verletzungen in den Jahrgängen 2015, 2017 und 2019 identifiziert und deskriptiv analysiert. Für diejenigen Patienten, die dem überregionalen Traumazentrum (ÜTZ) zuverlegt wurden, erfolgte neben der präklinischen eine weitergehende transsektorale Analyse des innerklinischen Verlaufes.
