Das Zertifizierungssystem „Zertifiziertes Gefäßzentrum“, das seit 2003 eine externe Überprüfung der Struktur‑, Prozess- und Ergebnisqualität anbietet, besitzt heute eine hohe Akzeptanz und Außenwirkung. Ziel der Arbeit ist eine Darstellung der Entwicklung des interdisziplinären Zertifizierungssystems in den Anfangsjahren und der Anpassungen im Verlauf und nun im Besonderen in Hinblick auf die Krankenhausreform in Deutschland. Derzeit sind 117 Zentren zertifiziert, und es wurden auch Zentren in Österreich, der Schweiz und Luxemburg erfolgreich begutachtet. Neben einer Verbesserung der Struktur- und Prozessqualität konnten wissenschaftliche Arbeiten aus der Versorgungsforschung nachweisen, dass die Ergebnisqualität der Versorgung von Patienten mit Bauchaortenaneurysmen, mit peripherer arterieller Verschlusserkrankung oder einer Carotisstenose durch die Zertifizierung signifikant verbessert werden konnten. Das Zertifizierungssystem wurde weiterentwickelt und durch die Digitalisierung des Antragswesens beschleunigt. Weiterhin wurde ein Gefäßzentrumssignet entwickelt, um die Außenwirkung des Zertifikates klarer darzustellen. Man bemühte sich, eine Zentrumsvergütung über die Gemeinsamer Bundesausschuss (G‑BA)-Zentrumsregelung zu erreichen. Eine Kooperation mit der Weissen Liste führte zu einer verbesserten Außendarstellung und als eines der wenigen Zertifizierungssysteme zu einer Erstaufnahme in den Bundes-Klinik-Atlas. Die Überprüfung des Zertifizierungssystems im Rahmen der Krankenhausreform durch das Institut für Qualitätssicherung und Transparenz im Gesundheitswesen (IQTIG) führte zu einer Weiterentwicklung der Zertifizierungskriterien mit Erstellung und Veröffentlichung eines Statuts und der Evidenzbasis des Zertifikates. Diese Optimierungen und der zu erwartende Strukturwandel im Rahmen der Krankenhausreform mit Fokus auf eine Zentralisierung werden zu einer weiteren positiven Entwicklung des Zertifizierungssystems und der Marke „Zertifiziertes Gefäßzentrum“ führen.
Die periphere arterielle Verschlusskrankheit (pAVK) ist eine häufige Manifestation der Atherosklerose mit hoher Morbidität und Mortalität. Der Beitrag fasst die aktuellen Empfehlungen der S3-Leitlinie „Diagnostik, Therapie und Nachsorge der pAVK“ (AWMF 2024) zusammen und beleuchtet die Auswirkungen der Krankenhausreform. Diagnostisch stehen Anamnese, klinische Untersuchung, Ankle-Brachial-Index (ABI) und Duplexsonographie im Vordergrund; bei Verdacht auf chronisch extremitätengefährdende Ischämie (CLTI) ermöglicht der WIfI-Score eine strukturierte Risikoeinschätzung des Amputationsrisikos. Therapeutisch werden strikte Sekundärprävention (LDL [Low-Density-Lipoprotein] < 3,1 mmol/l, Rauchstopp), strukturiertes Gehtraining und eine optimierte antithrombotische Therapie (Clopidogrel bevorzugt; „dual pathway inhibition“ bei Hochrisiko oder nach Revaskularisation) empfohlen. Revaskularisationen erfolgen endovaskulär oder chirurgisch, zunehmend in spezialisierten Zentren. Die Krankenhausreform stärkt Zentralisierung komplexer Eingriffe und Ambulantisierung, wodurch die Rolle nichtgefäßchirurgischer Abteilungen in Früherkennung und Überleitung betont wird.
Peripheral arterial occlusive disease (PAOD) is a frequent manifestation of atherosclerosis associated with high morbidity and mortality. This article summarizes the current recommendations of the German S3 guidelines "Diagnostics, treatment and follow-up of PAOD" (Association of the Scientific Medical Societies in Germany, AWMF 2024) and discusses the implications of the ongoing hospital reform. Key diagnostic modalities include patient history taking, clinical examination, ankle-brachial index measurement and duplex ultrasonography. In cases of suspected chronic limb-threatening ischemia (CLTI), the WIfI score enables a structured risk assessment for amputation. Treatment management emphasizes strict secondary prevention (low-density lipoprotein, LDL < 3,1 mmol/l, smoking cessation), supervised exercise training and optimized antithrombotic therapy (clopidogrel is preferred; dual pathway inhibition for high-risk patients or after revascularization). Revascularization can be performed endovascularly or surgically and increasingly in specialized centers. The German Hospital Reform supports the centralization of complex procedures and the expansion of outpatient care, thereby underscoring the pivotal role of nonvascular surgical departments in early detection and coordinated referral.
The patient population treated by vascular medicine is becoming older, which goes along with an increasing number of patients presenting with frailty syndrome. The level of frailty determines the outcomes after vascular surgery or interventions. This position paper written by the German Society for Vascular Surgery and Vascular Medicine, the German Society for Geriatrics, and the German Society for Angiology - Society for Vascular Medicine summarizes the current evidence on the impact of frailty on the outcome after treatment for vascular diseases, and calls for a closer collaboration between geriatrics and vascular medicine and an integration of geriatric assessment in the preoperative or preinterventional evaluation of patients. Concrete and practical demands and tasks are formulated aiming at increasing awareness of geriatrization of the patient population, promoting research on the impact of frailty on the treatment of vascular patients and improving patients' safety, reducing complications and enhancing quality of care of older vascular patients.

Die schnelle Erkennung, Ersttherapie und Diagnostik der traumatische Gefäßverletzung und Auswahl der Zielklinik mit offen chirurgischen und endovaskulären Therapiemöglichkeiten sind entscheidend für das Outcome der – mit einer hohen Morbidität und Letalität behafteten – traumatischen Gefäßverletzung.
Background and Aims: Managing infrainguinal vascular graft infections (VGI) in bypass reconstruction is complex. Although an autologous vein is the preferred graft material, alternative conduits are required when autologous veins are unavailable. This study aimed to compare the efficacy of cryopreserved arterial homografts and biosynthetic glutaraldehydemodifiedovine collagen grafts (Omniflow II) for the treatment of infrainguinal VGI. Methods: The study was retrospective across two centers and included patients with infrainguinal VGI treated with cryopreserved arterial homografts or Omniflow II grafts between 2009 and 2019. Freedom from reinfection, primary patency rates, amputation-free survival, and overall survival were analyzed using 5-year Kaplan-Meier estimates. Results: Overall, 63 patients with infrainguinal VGI were treated with either Omniflow II grafts (n = 34) or cryopreserved arterial homografts (n = 29). At 5 years, freedom from reinfection was 97.1% for Omniflow II grafts and 93.1% for cryopreserved arterial homografts (p = .4). Primary patency was 50% for Omniflow II grafts and 55.2% for the cryopreserved arterial homografts (p = .5). Amputation-free survival was 52.9% for Omniflow II grafts and 55.2% for cryopreserved arterial homografts (p = .7). No graft degeneration or graft ruptures were observed during the follow-up. Conclusions: This study suggests that biosynthetic grafts are viable and accessible alternatives for traditional graft materials, offering similar efficacy and ease of use. Although autologous vein grafts remain the gold standard for VGI management, biosynthetic grafts may serve as a viable alternatives to cryopreserved arterial homografts in the treatment of infrainguinal VGIs.
OBJECTIVE:While the impact of annual caseload on short term outcomes after abdominal aortic aneurysm (AAA) repair remains under debate, current data from the most decentralised healthcare system in Europe may offer valuable insights into an area that has changed considerably due to the widespread use of endovascular aortic aneurysm repair (EVAR). METHODS:This was a retrospective observational study of multicentre quality registry data from Germany on EVAR for intact and ruptured AAA between January 2017 and December 2023. The impact of annual caseload and risk factors on in hospital death, complication rates, and failure to rescue after intact AAA repair was determined using multivariable logistic regression models. RESULTS:A total of 19 641 individuals (n = 2 576 females [13.1%]; median age 74 years, interquartile range [IQR] 68, 80) who underwent EVAR for intact (n = 18 763) or ruptured (n = 878) AAA were included from 194 centres. Among these, 44.3% of males were selected for treatment with a maximum diameter < 55 mm (31.8% of females < 50 mm). After a median length of hospital stay of six days (IQR 5, 8) and 10 days (IQR 6, 17) for intact and ruptured cases, 1.0% and 19.7% died, respectively. The rate of any post-interventional complication was 8.2% and 31.4%, respectively. An endoleak was apparent on the completion angiogram in 17.6% of intact cases (14.5% type II) and 18.2% of ruptured cases (11.0% type II). The median annual EVAR caseload per centre was 34.7 (IQR 22.1, 58.4), and 25.1% of patients were treated at centres above the 75th percentile. An American Society of Anesthesiologists (ASA) score ≥ 4, juxtarenal aneurysm extent, severe heart failure, female sex, larger aneurysm diameter, older age, and history of cancer, chronic obstructive pulmonary disease, and stroke were associated with in hospital death after intact AAA repair, but annual caseload and severe chronic kidney failure were not. Sensitivity analyses including different outcomes were confirmative, except for a statistically significant impact of annual caseload on the composite endpoint of any post-interventional complication (p = .008). CONCLUSION:This large registry analysis was unable to confirm an impact of caseload on short term results after EVAR for intact AAA. Future studies should address long term outcomes after EVAR as well as the underlying reasons for the considerably large proportion of small diameter AAA treatment.
Introduction: The medical development in the previous 15 years and the changes in treatment reality of the comprehensive elective treatment of abdominal aortic aneurysms necessitate a re-evaluation of the quality assurance guidelines of the Federal Joint Committee in Germany (QBAA-RL). In the current version this requires a specialist further training quota for nursing personnel in intensive care wards of 50%. The quota was determined in 2008 based on expert opinions, although a direct empirical evidence base for this does not exist. Methods: Representatives from the fields of patient representation, physicians, nursing personnel and other relevant interface areas were invited to participate in a modified Delphi procedure. Following a comprehensive narrative literature search, a survey and focus group discussions with national and international experts, a total of three anonymized online-based voting rounds were carried out for which previously determined key statements were assessed with a 4-point Likert scale (totally disagree up to totally agree). In addition, the expert panel had also defined a recommendation for a minimum quota for the specialist training of nursing personnel on intensive care wards in the treatment of abdominal aortic aneurysms, whereby an a priori agreement of 80% of the participants was defined as the consensus limit. Results: Overall, 37 experts participated in the discussions and three successive voting rounds (participation rate 89%). The panel confirmed the necessity of a re-evaluation of the guideline recommendations and recommended the introduction of a shift-related minimum quota of 30% of the full-time equivalent of nursing personnel on intensive care wards and the introduction of structured promotional programs for long-term elevation of the quota. Conclusion: In this national Delphi procedure with medical and nursing experts as well as representatives of patients, the fundamental benefits and needs of professional specialist qualifications in the field of intensive care medicine were confirmed. The corresponding minimum quota for specialist further training of intensive care nursing personnel should generally apply without limitations to specific groups. The expert panel stipulates a shift-related minimum quota for intensive care nursing personnel with specialist training of 30% of the nursing personnel on intensive care wards and the obligatory introduction of structured and transparent promotion programs for the long-term enhancement.
Die elektive und notfallmäßige Behandlung von Menschen mit Bauchaortenaneurysma erfolgt in Deutschland aktuell an bis zu 500 Krankenhäusern. Um durch konsequentes Benchmarking der Behandlungsergebnisse die Qualität weiterzuentwickeln, hat sich ein Teil dieser Krankenhäuser unter dem Schirm der Deutschen Gesellschaft für Gefäßchirurgie und Gefäßmedizin (DGG) zu einem Qualitätsnetzwerk zusammengeschlossen, das vom Deutschen Institut für Gefäßmedizinische Gesundheitsforschung gGmbH (DIGG) organisiert wird. Zusammenfassung Einleitung Die medizinischen Weiterentwicklungen in den vergangenen 15 Jahren und die veränderte Versorgungsrealität bei der flächendeckenden elektiven Behandlung des Bauchaortenaneurysmas machen eine Reevaluation der Qualitätssicherungsrichtlinie des Gemeinsamen Bundesausschuss in Deutschland (QBAA-RL) erforderlich. Diese fordert derzeit in der aktuellen Fassung eine Fachweiterbildungsquote für Pflegefachpersonen der Intensivstation in Höhe von 50 %. Die Quote wurde 2008 auf dem Boden von Expertenmeinungen festgelegt, ohne dass bisher eine direkte empirische Evidenzbasis dafür existiert. Methoden Vertreter:innen aus den Bereichen Patient:innenvertreter, Ärzt:innen, Pflegefachpersonen sowie weiteren relevanten Schnittstellenbereichen wurden zur Teilnahme an einem modifizierten Delphi-Verfahren eingeladen. Nach einer umfassenden narrativen Literaturrecherche, einer Umfrage sowie Fokusgruppendiskussionen mit nationalen und internationalen Expert:innen erfolgte die Durchführung von insgesamt drei anonymisierten onlinebasierten Abstimmungsrunden, bei denen zuvor festgelegte Kernaussagen mit einer 4‑Punkt-Likert-Skala („stimme ganz und gar nicht zu“ bis „stimme voll und ganz zu“) bewertet wurden. Das Expert:innenpanel hat außerdem eine Empfehlung für eine Mindestquote für die Fachweiterbildung von Pflegefachpersonen auf der Intensivstation bei der Behandlung des Bauchaortenaneurysmas festgelegt, wobei a priori eine Zustimmung in Höhe von 80 % der Teilnehmenden als Konsensusgrenze festgelegt wurde. Ergebnisse Insgesamt haben 37 Expert:innen an den Diskussionen und drei sukzessiven Abstimmungsrunden teilgenommen (Teilnahmerate 89 %). Das Panel hat die Notwendigkeit einer Reevaluation der Richtlinienempfehlungen bestätigt und empfahl die Einführung einer schichtbezogenen Mindestquote in Höhe von 30 % der Vollzeitäquivalente der Pflegefachpersonen der Intensivstation sowie die Einführung strukturierter Förderprogramme zur langfristigen Erhöhung der Quote. Background: Approximately 500 hospitals in Germany carry out the elective and emergency treatment of people with abdominal aortic aneurysms (AAA). A proportion of these hospitals are connected to a quality network under the umbrella of the German Society for Vascular Surgery and Vascular Medicine (DGG) to improve the quality of outcomes through rigorous benchmarking. Methods: A retrospective descriptive analysis of registry data on inpatient treatment of AAA between 1 January 2023 and 31 December 2023 was carried out. Results: In 2023 a total of 163 hospitals provided data on 5456 cases (15.4% women, median age 73 years). In median 28 procedures were conducted per center (7 open surgical aortic procedures). In 443 patients (8.1%) a rupture was documented, of which 83.3% were covered ruptures. In total 667 (14.3%) were detected during ultrasound screening and the median diameter of the AAA was 55 mm at the time of treatment. The length of hospital stay was 7 days in median and the elective endovascular aortic repair (EVAR) rate was 78%. Among patients with intact AAAs 461 (9.2%) were symptomatic, 12.4% had a juxtarenal AAA and in 26.8% the iliac arteries were involved. Conclusion: The current data of the quality improvement registry in Germany emphasize that the indications, treatment reality and inpatient results are comparable to international registry data.
Background: To report technical success as well as perioperative outcomes of patients who underwent endovascular aortic repair (EVAR) of penetrating abdominal aortic ulcers ≤35 mm in diameter (PAU). Patients and methods: The abdominal aortic aneurysm (AAA) quality registry of the German institute for vascular research (DIGG) was used to identify patients with standard EVAR for infrarenal PAU ≤35 mm between 1/1/2019 and 12/31/2021. Infectious, traumatic, inflammatory PAUs, PAUs associated with connective tissue disease, PAUs following aortic dissection as well as true aneurysms were excluded. Demographics, cardiovascular comorbidity, technical success as well as perioperative morbidity and mortality were determined. Results: Amongst 11 537 patients who underwent EVAR during the study period, 405 with PAU ≤35 mm were eligible from 95 participating hospitals across Germany (22% women, 20.5% octogenarians). The median aortic diameter was 30 mm (Interquartile range 27-33). Cardiovascular comorbidities were frequent with coronary artery disease (34.8%), chronic heart failure (30.9%), history of myocardial infarction (19.8%), hypertension (76.8%), diabetes (21.7%), smoking (20.8%), history of stroke (9.4%), symptomatic lower extremity peripheral arterial disease (20%), chronic kidney disease (10.4%) and chronic obstructive pulmonary disease (9.6%). Most patients were asymptomatic (89.9%). Among the symptomatic patients, 13 presented with distal embolization (3.2%) and 3 with contained ruptures (0.7%). Technical success of endovascular repair was 98.3%. Both, percutaneous (37.1%) or femoral cut-down access approaches (58.5%) were registered. Endoleaks of any type were present with type 1 (0.5%), type 2 (6.4%) and type 3 (0.3%) endoleaks. Overall mortality was 0.5%. Perioperative complications occurred in 12 patients (3.0%). Conclusions: According to this registry data, endovascular repair of PAU is technically feasible with acceptable perioperative outcomes, but further studies investigating mid- and long-term data are needed before invasive treatment of PAU disease in an elderly and comorbid patient population should be recommended.
Zusammenfassung Die Deutsche Gesellschaft für Gefäßchirurgie und Gefäßmedizin (DGG e. V.) hat 38 Jahre nach ihrer Gründung ein Leitbild entwickelt, um vor allem ihren Mitgliedern, aber auch Patientinnen und Patienten, dem studierenden Nachwuchs und der Öffentlichkeit eine werteorientierte Sichtbarkeit dessen zu geben, was ihr wichtig ist. Unter dem Motto „ Wir sind Gefäße “ positionieren sich die DGG und ihre Mitglieder. Die DGG steht für Kompetenz und Exzellenz in der Gefäßchirurgie und Gefäßmedizin. Das Leitbild wurde in der Mitgliederversammlung der DGG e. V. an der Dreiländertagung 2022 in Wien vorgestellt und verabschiedet. Das nun vorliegende Leitbild bildet die Grundlage für eine künftige Markenbildung (Branding) der DGG und des Faches Gefäßchirurgie. Die Anpassung und Weiterentwicklung dieses Leitbildes sind zukünftig möglich und ggf. erforderlich.
