Objective: This study aimed to determine whether sCD163, a soluble macrophage marker up-regulated in numerous inflammatory disorders, is predictive of accelerated atherosclerosis associated with systemic lupus erythematosus (SLE).Methods: Carotid ultrasound was prospectively performed, at baseline and during follow-up, in 63 consecutive SLE patients asymptomatic for cardiovascular disease (CVD) and 18 volunteer health workers. Serum sCD163 level was determined at baseline using enzyme-linked immunosorbent assay. The primary outcome was the presence of a carotid plaque. Factors associated with carotid plaques were identified through multivariate analysis.Results: Despite a low risk for cardiovascular events according to Framingham score in both groups (2.1 ± 3.8% in SLE vs 2.1 ± 2.9% in controls; p = 0.416), ultrasound at baseline showed a carotid plaque in 23 SLE patients (36.5%) and two controls (11.1%) (p = 0.039). Multivariate analysis showed that SLE status increased the risk for carotid plaque by a factor of 9 (p = 0.017). In SLE patients, sCD163 level was high (483.7 ± 260.8 ng/mL vs 282.1 ± 97.5 ng/mL in controls; p < 0.001) and independently associated with carotid plaques, as assessed by stratification based on sCD163 quartile values (p = 0.009), receiver operating characteristics (p = 0.001), and multivariate analysis (p = 0.015). sCD163 at baseline was associated with the onset of carotid plaque during follow-up (3 ± 1.4 years) in SLE patients who had no carotid plaque at the first evaluation (p = 0.041).Conclusion: sCD163 is associated with progressing carotid plaque in SLE and may be a useful biomarker for accelerated atherosclerosis in SLE patients at apparent low risk for CVD.
Les événements cardiovasculaires sont la première cause de mortalité des patients lupiques. Les outils d’évaluation comme le score de Framingham ne donnent pas de bonne estimation du risque cardiovasculaire dans cette population. L’objectif de notre étude est de déterminer si le CD163 soluble (sCD163), marqueur de surface des macrophages impliqués dans différents processus inflammatoires, est associé à la maladie athéromateuse infraclinique des patients atteints de lupus systémique (LS). La présence de plaque carotidienne (définie par une épaisseur de la paroi supérieure à 1,5 mm) a été évaluée par échographie doppler des carotides de manière prospective chez 63 patients suivis pour un LS et ne présentant aucun antécédent cardiovasculaire et 18 témoins. L’échographie doppler a été effectuée à l’inclusion et répétée durant le suivi par le même investigateur. Le taux de sCD163 a été mesuré à l’inclusion chez tous les participants par technique ELISA. Le score de Framingham, évaluant le risque cardiovasculaire à 10 ans, était bas chez les patients lupiques et les témoins (respectivement 2,1 % ± 3,8 et 2,1 % ± 2,9 ; p = 0,416). Cependant, une plaque carotidienne était mise en évidence chez 23 patients lupiques (36,5 %) et 2 témoins (11,1 % ; p = 0,039). Le lupus, en analyse mutivarié, augmentait par 9 le risque d’avoir une plaque carotidienne (OR : 9,16, intervalle de confiance à 95 % :1,82-77,97 ; p = 0,017). Les taux sériques de sCD163 étaient plus élevés chez les patients lupiques que chez les témoins (483,7 ng/ml ± 260,8 vs 282,1 ng/ml ± 97,5 ; p < 0,001). Chez les patients lupiques, le taux sérique de sCD163 était positivement associé à la présence de plaque carotidienne (stratification en quartiles (p = 0,009), courbe ROC (p = 0,001), analyse multivariée (p = 0,015)). Les taux sériques de sCD163 étaient également corrélés aux titres d’anticorps anti-dsADN (r = 0,27 ; p = 0,034). La recherche de plaque carotidienne a été renouvelée 2,6 ans (1–5,4) après la 1ere évaluation chez 20 des 40 patients qui ne présentaient pas de plaque carotidienne initialement. L’apparition d’une plaque carotidienne était notée chez 8 patients (40 %) correspondant à l’apparition d’une nouvelle plaque par an chez 2,7 patients. Le score de Framingham ne différait pas entre les patients ayant développé (1 % ; 0,5–9) ou non (0,5 % ; 0,5–3) une nouvelle plaque. Le taux sérique de sCD163 à l’inclusion était par contre plus élevé chez les patients qui développaient une nouvelle plaque carotidienne dans le temps (548,5 ng/ml ± 263,8 vs 324,8 ng/ml ± 131,7 ; p = 0,041). Ce travail préliminaire montre que sCD163 est associé de façon indépendante à la progression de l’athérome infraclinique chez les patients lupiques à faible risque cardiovasculaire apparent. sCD163 pourrait contribuer à une meilleure stratification du risque cardiovasculaire en contexte lupique.
Les événements cardiovasculaires en lien avec l'athérosclérose sont la première cause de mortalité des patients lupiques. Les facteurs de risque cardiovasculaire classiques (hypertension artérielle, dyslipidémie, intoxication tabagique…) contribuent à ce surrisque cardiovasculaire mais n'expliquent pas la totalité du phénomène. De fait, le score de Framingham sous-estime le risque cardiovasculaire dans cette population. L'objectif de ce travail était de déterminer si la mesure sérique de la troponine C ultrasensible (cTnT-US) aide à l'identification des patients lupiques à risque cardiovasculaire. Étude monocentrique prospective menée de janvier 2012 à janvier 2013. La présence de plaque carotidienne (carotide interne) en échographie doppler a été analysée chez 63 patients lupiques consécutifs asymptomatiques sur le plan cardiovasculaire et 18 sujets contrôle. La cTnT-US était mesurée dans le sérum par électrochimiluminescence pour un seuil de détection à 3 ng/L. Les facteurs associés à la présence d'une plaque carotidienne ont été identifiés et analysés. Soixante-trois patients ayant un lupus (39,8 ± 10,2 ans, 82,5 % sexe féminin) ont été comparé à 18 volontaires sains (42,4 ± 11,2 ans, 77,8 % sexe féminin). Le risque cardiovasculaire calculé selon le score de Framingham était bas dans les 2 groupes (2,1 ± 3,8 % groupe lupus et 2,1 ± 2,9 % groupe contrôle ; p = 0,416). La présence de plaques carotidiennes a été détectée chez 23 (36,5 %) patients ayant un lupus contre 2 (11,1 %) dans le groupe contrôle (p = 0,039). En analyse multivariée, seuls l'âge (p = 0,006) et le lupus (p = 0,017) étaient indépendamment associés à la présence de plaques carotidiennes. La cTnT-US était détectable chez 37 (58,7 %) patients ayant un lupus et 6 (33,3 %) patients du groupe contrôle (p = 0,057). Quatre-vingt-sept pour cent des sujets lupiques ayant une plaque avaient un taux sérique de cTnT-US détectable (contre 42 % des sujets lupiques sans plaque SLE ; p < 0,001). Cinquante-cinq pour cent des sujets lupiques ayant une cTnT-US détectable avaient une plaque carotidienne (contre 11 % des sujets avec cTnT-US indétectable ; p < 0,001). Chez les sujets lupiques, les facteurs associés à la présence de plaques carotidiennes était l'âge (p < 0,001), le score de Framingham (p = 0,002), le surpoids (p < 0,001) et un taux serique cTnT-US détectable (p < 0,001). En analyse multivariée, seuls le surpoids (p = 0,006) et le taux sérique cTnT-US détectable (p = 0,033) étaient associés à la présence de plaques carotidiennes chez les patients ayant un lupus. Les taux sériques de cTnT-US sont élevés chez les sujets lupiques et fortement associés à la présence d'une athérosclérose infraclinique indépendamment des facteurs de risque cardiovasculaire classiques. La cTnT-US peut aider à l'identification des patients lupiques à risque cardiovasculaire malgré un score de Framingham bas.
Objectives: whereas it is widely admitted that valve interstitial cells (VICs) change their phenotype and become osteoblast-like when cultured in pro-osteogenic conditions, little is known about the nature of the endogenous stimuli. We hypothesize that the putative triggers are contained in plasma which, when in contact with VICs could trigger an inflammatory response and initiate the osteoblastic-like phenotype switch.
L’évolution rapide des méthodes de dosage des troponines cardiaques (cTn) vers une meilleure sensibilité analytique (cTn de haute sensibilité ou cTn HS) s’accompagne de nombreuses données de la littérature, mais encore incomplètes. En l’absence de standardisation des cTn et de données consensuelles sur l’utilisation et l’interprétation des résultats, les auteurs de cette revue proposent, à partir d’une revue de la littérature, et de façon multidisciplinaire, des éléments de réponses aux questions fréquemment posées. En conclusion, le bon usage des cTn HS repose sur la connaissance : 1) des caractéristiques propres de la méthode utilisée, en particulier de la précision obtenue au 99e percentile d’une population de référence ; 2) des facteurs de variation de la valeur du 99e percentile ; 3) de la forte individualité des dosages de cTn HS, pour lesquels la notion de cinétique individuelle est plus informative que la simple référence à des valeurs usuelles. La significativité des variations entre deux dosages n’est pas encore documentée pour toutes les méthodes HS. La collaboration entre cliniciens et biologistes est nécessaire à une meilleure utilisation des troponines au quotidien.
Travail réalisé sous la direction de M. Dehoux et C. Quesnel. Les fonctions des macrophages dépendent de leur polarisation pro-inflammatoire (M1) ou anti-inflammatoire (M2a et M2c), et ont été impliqués dans la réparation tissulaire et diverses pathologies. Au cours du syndrome de détresse respiratoire aiguë (SDRA), les monocytes sont recrutés au niveau alvéolaire et se différencient en macrophages. La participation des macrophages dans la réparation alvéolaire reste inconnue chez l’homme au cours du SDRA. Nos objectifs sont d’évaluer in vitro : – l’effet du liquide broncho-alvéolaire (LBA) de patients en SDRA sur la polarisation de macrophages ; – l’effet de cette polarisation sur la réparation épithéliale alvéolaire. Des monocytes humains issus de témoins sains ont été cultivés et différenciés en macrophages en présence de GM-CSF et de M-CSF, puis polarisés par des cytokines vers le phénotype M1 (rhIFNγ), M2a (rhIL-4) et M2c (rhIL-10) ou polarisés par des LBA issus de patients à la phase précoce de SDRA (n = 12), de témoins ventilés (TV, n = 10) et de témoins non ventilés (TNV, n = 4). La caractérisation de la polarisation a été réalisée par qPCR à l’aide des marqueurs M1(CD80), M2a (CD200R) et M2c (CD163). L’effet des surnageants de macrophages polarisés a été estimé dans un modèle de plaie épithéliale calibrée (insert) utilisant une monocouche de cellules pulmonaires A549. L’implication de l’IL-1β dans cet effet a été spécifiquement évaluée à l’aide d’un anticorps monoclonal anti-IL-1β. Le LBA de patients en SDRA favorise la polarisation M1. Le LBA de TV entraîne à l’inverse la polarisation M2c caractérisée par la surexpression du CD163. Les surnageants de macrophages polarisés M1 par rhIFNγ ou par les LBA de SDRA induisent la migration épithéliale, avec un ratio de migration respectivement de 1,39 (p = 0,01) et 1,36 (p < 0,001) fois le témoin, alors que ceux des macrophages polarisés M2c (rh IL-10 ou LBA de TV) ne la modulent pas. L’anti-IL-1β diminue la fermeture induite par les surnageants de macrophages M1 de 44 % (p = 0,004). L’environnement alvéolaire au cours du SDRA précoce induit une polarisation macrophagique M1. Nous montrons pour la première fois que la polarisation M1 influence la réparation alvéolaire en favorisant la migration épithéliale, à l’inverse de la polarisation M2c. L’IL-1β secrété par les M1 est l’un des médiateurs impliqués dans cet effet.
CONTEXT:Glucocorticoid therapy is being used in a wide variety of systemic disorders. Reference papers, published more than 20 years ago, showed no correlation between adrenal insufficiency risk and dose or duration of glucocorticoid therapy.OBJECTIVE:Our objective was to evaluate the extent to which long-term glucocorticoid therapy damages the pituitary-adrenal axis in patients with systemic inflammatory disorders.DESIGN:We conducted a retrospective observational study from January 2011 to August 2012.SETTING:This was a monocentric study at the Department of Internal Medicine, Bichat Hospital, Paris-Diderot University, Paris, France.PARTICIPANTS:Sixty consecutive patients who were receiving long-term prednisone therapy for systemic inflammatory disorders and in whom discontinuation of glucocorticoid treatment was planned.INTERVENTION:A short Synacthen test was performed. A bolus of 0.25 mg 1-24-ACTH was injected in the morning, 24 hours after the most recent dose of prednisone. Cortisol was measured at baseline and 60 minutes after Synacthen injection.MAIN OUTCOME MEASURES:We assessed frequency and risk estimate of pituitary-adrenal dysfunction.RESULTS:Twenty-nine patients (48.3%) had adrenal insufficiency defined by a plasmatic cortisol <100 nmol/L (n = 13) at baseline (time 0) or <550 nmol/L (n = 16) 60 minutes after Synacthen injection. Cumulative dose (area under the receiver operating characteristic curve = 0.77 [95% confidence interval = 0.62-0.91], P = .007) and exposure (area under the receiver operating characteristic curve 0.80 [95% confidence interval = 0.67-0.93], P = .002) to prednisone were predictive for adrenal insufficiency based on a T0 <100 nmol/L. Prednisone was stopped in 29 of 31 patients (93.5%) showing a normal response to short Synacthen test; none of these patients required hydrocortisone replacement with a mean follow-up of 10 (± 6) months.CONCLUSION:Adrenal insufficiency is frequent in patients treated with long-term glucocorticoids for systemic inflammatory disorders and is related to duration and cumulative dose of steroids.
Background In showing no correlation between adrenal insufficiency risk and dose or duration of steroid treatment, previous reference papers have blurred our comprehension and practice (1). Objectives To determine the extent to which long-term corticosteroid therapy damages the pituitary-adrenal axis in patients treated with prednisone medication for systemic disorders. Methods We retrospectively studied all consecutive patients followed in our department from January 2011 to August 2012 in whom short synacthen test (SST) was assessed when withdrawal of prednisone was planned. Age, sex, disease status, duration and cumulative dose of glucocorticoid therapy were systematically recorded. SST was considered blunted when the 60 minutes plasma cortisol concentration was below 550 nmol per liter. Results Sixty consecutive patients suffering various systemic diseases were studied (Table 1). The mean daily dose of prednisone at study time was 7 (±2) (range: 5-10) mg. Median duration and cumulative dose of prednisone were 3.8 years (range: 0.3-32) and 16.6 grams (1-132), respectively. The SST response was blunted in 29 patients (48.3%) and normal in 31. Blunted responses were associated with higher cumulative prednisone dose (p=0.04) and treatment duration (p=0.04) (Table1 and 2). Basal cortisol concentrations negatively correlated with both duration (r=-0.33, p=0.01) and cumulative dose (r=-0.28, p=0.03) of prednisone. Steroids were stopped in 29/31 (93.5 %) patients showing a normal response to SST. In this case, no patient required hydrocortisone replacement for a mean follow-up of 10 (± 6) months. Conclusions Adrenal insufficiency is frequent in patients treated with long-term glucocorticoid for systemic disorders. Background: Chronic stress during pregnancy has been associated with worsened maternal and fetal outcomes. Acute stress immediately before spinal anaesthesia for caesarean section may contribute to hypotension. Therefore objective measures of acute stress may help identify women at risk of adverse outcomes. Salivary alpha-amylase is a stress biomarker that has so far been poorly investigated during pregnancy. The reference change value is the difference between two sequential results that must be exceeded for a change to be considered clinically relevant. Our first aim was to determine if salivary alpha-amylase increased in pregnant patients when subjected to the stress of transfer to the operating room. Our second aim was to determine if changes in salivary alpha-amylase were likely to be clinically significant by measuring reference change value in healthy volunteers.Methods: In 15 pregnant patients undergoing planned caesarean section under spinal anaesthesia, salivary alpha-amylase, systolic blood pressure, heart rate, and immediate anxiety were measured on the morning of surgery on the ward and again in the operating room. The reference change value was calculated from 18 healthy volunteers.Results: A median 220% increase in salivary alpha-amylase activity (P = 0.0015) and a 17% increase in systolic blood pressure (P = 0.0006) were observed between the ward and operating room. No changes of immediate anxiety or heart rate were observed. Reference change value was +/- 76% in volunteers and 13 of the 15 pregnant patients had a salivary alpha-amylase increase greater than the reference change value.Conclusion: When pregnant women are taken to the operating room, a clinically and statistically significant increase in salivary alpha-amylase was observed. Identification of new therapeutic targets in idiopathic pulmonary fibrosis (IPF) is required. We hypothesized that, in the adult lung, SHH pathway plays a critical role during the alveolar repair process and lung fibrogenesis.Methods: We determined the expression pattern of several members of the SHH pathway in lung biopsies and primary cultured fibroblasts from controls and IPF patients. We also examined the expression profile of SHH pathway after TGF-β stimulation in fibroblasts. We characterized the action of recombinant SHH and cyclopamine, an inhibitor of SHH pathway, on the effect of TGF-β on fibroblast differentiation markers.Results: In lung biopsies, the ligand SHH, the receptor Patched (PTC), the transmembrane protein Smoothened (SMO) and the transcription factors GLI 1-2-3 were detected in both control and IPF tissues. Compared to controls, PTC was decreased in IPF, both in epithelial cells and fibroblasts. SMO was maintained and GLI2 was localized in the nuclei in IPF. In vitro, there was no difference between control and IPF fibroblasts in the expression of HH signalling components. After TGF-β stimulation, PTC, SMO and GLI3 mRNA were strongly inhibited while GLI2 was increased. Cyclopamine inhibited TGF-β induced expression of myofibroblast differentiation markers in control fibroblasts. IPF fibroblasts were less sensitive to cyclopamine treatment. Though, addition of recombinant SHH had no effect.Conclusions: Our results show that the signaling pathway downstream of SMO is activated in IPF. SMO activity is also necessary to TGF-β induced myofibroblastic differentiation. These data support a pro-fibrotic action of the SHH pathway in IPF.
4 5 Fibrocytes are mesenchymal progenitors from hematopoietic origin involved in normal and pathologic organ repair. During Background: acute lung injury (ALI) and acute respiratory distress syndrome (ARDS), an ineffective repair is associated with over-mortality. We investigated whether fibrocyte detection in bronchoalveolar lavage fluid (BAL) could be a predictor of outcome during acute lung injury. We prospectively collected BAL from 122 ventilated patients (62-ARDS, 30-ALI, 30-ventilated patients w/o ALI/ARDS). Fibrocytes, Methods: defined as cells expressing CD45 and collagen 1, were quantified by flow cytometry. Chemokines, growth factors and collagen 1 concentrations were measured in BAL supernatants. Ventilated patients were followed up for 28 days after BAL procedure and clinical data recorded. Fibrocytes were detected in 89% of BAL from ventilated patients. The median percentage of BAL fibrocytes was significantly Findings: increased in patients with ALI and ARDS (5.0 %) in comparison with ventilated controls (0.9%, p<0.0001). Fibrocyte percentage correlated with percentage of monocytes/macrophages in BAL (s=0.35, p=0.007). After adjustment for age, comorbidities, administered treatments and severity of illness in a multivariable COX proportional-hazard model, a percentage of BAL fibrocytes over 6% was independently associated with a lower proportion of patients weaned from mechanical ventilation at 28 days (HR (95% CI)= 0,34 (0,15-0,78), p=0,01) and a higher 28-day mortality in ALI/ARDS patients (HR (95% CI)= 6.15 (2.78-13.64), p<0.0001).
