RAMSES (NCT04077528) est une cohorte nationale observationnelle, prospective, de patients adultes asthmatiques sévères. Cette étude compare les caractéristiques phénotypiques et le parcours de soins des patients en fonction du centre d’inclusion : centres universitaires (CU) ou centres non universitaires (CNU : CHG, clinique, cabinet privé). Au 30/06/2022, 1963 patients étaient inclus par 43 centres (19 CU, 24 CNU) : 62 % de femmes, d’âge moyen 53 ans, 65 % recevaient ou avaient reçu une biothérapie, 16 % une corticothérapie orale au long cours (CSO). À l’inclusion, 66 % avaient un ACT < 20 avec une moyenne de 4 exacerbation/an/patient. Un phénotype T2 était fréquent : éosinophiles sériques ≥ 300/mm3 (49 %), FeNO ≥ 20 ppm (70 %), sensibilisation à un aéroallergène perannuel (53 %). Les CU ont inclus 1407 patients (72 %), caractérisés par des comorbidités plus fréquentes (RGO 36 % vs 21 %, p < 0,0001 ; rhinosinusite chronique 49 % vs 32 %, p < 0,0001), un traitement plus fréquent par LAMA (55 % vs 46 %, p = 0,03) et macrolides (10 % vs 4 %, p = 0,003). Les patients inclus par les CNU (n = 556, 28 %), étaient plus fréquemment traités par montélukast (39 % vs 30 %, p = 0,04), hospitalisés pour asthme dans l’année précédente (23 % vs 16 %, p = 0,02), et présentaient une altération plus prononcée de la qualité de vie (AQLQ 4,5 vs 4,8, p < 0,0001). Il n’existait pas de différences en termes de démographie, de tabagisme, de contrôle de l’asthme, de fonction pulmonaire, de taux d’éosinophiles ou de traitement par CSO ou biothérapie. L’analyse du parcours de soin a montré une prise en charge multidisciplinaire plus fréquente en CU (éducation thérapeutique 27 % vs 18 %, p = 0,003 ; réentraînement à l’effort 25 % vs 11 %, p < 0,0001). Une bronchoscopie (44 % vs 29 %, p < 0,0001), un bilan allergologique (76 % vs 62 %, p < 0,0001), une mesure du FeNO (63 % vs 23 %, p < 0,0001) étaient plus fréquemment réalisés en CU. La cohorte RAMSES apparaît représentative de la population des asthmatiques sévères. S’il existe des différences discrètes de phénotype entre les patients suivis en CU et CNU (comorbidités, sévérité des exacerbations), des différences du parcours de soin (exploration, stratégie thérapeutique hors biothérapie) sont observées. Un recours plus systématique au bilan allergologique, à la mesure du FeNO et à l’éducation thérapeutique pourrait permettre une progression globale de la prise en charge des patients.
BACKGROUND: Since the previous French guidelines were published in 2017, substantial additional knowledge about idiopathic pulmonary fibrosis has accumulated. METHODS: Under the auspices of the French-speaking Learned Society of Pulmonology and at the initiative of the coordinating reference center, practical guidelines for treatment of rare pulmonary diseases have been established. They were elaborated by groups of writers, reviewers and coordinators with the help of the OrphaLung network, as well as pulmonologists with varying practice modalities, radiologists, pathologists, a general practitioner, a head nurse, and a patients' association. The method was developed according to rules entitled "Good clinical practice" in the overall framework of the "Guidelines for clinical practice" of the official French health authority (HAS), taking into account the results of an online vote using a Likert scale. RESULTS: After analysis of the literature, 54 recommendations were formulated, improved, and validated by the working groups. The recommendations covered a wide-ranging aspects of the disease and its treatment: epidemiology, diagnostic modalities, quality criteria and interpretation of chest CT, indication and modalities of lung biopsy, etiologic workup, approach to familial disease entailing indications and modalities of genetic testing, evaluation of possible functional impairments and prognosis, indications for and use of antifibrotic therapy, lung transplantation, symptom management, comorbidities and complications, treatment of chronic respiratory failure, diagnosis and management of acute exacerbations of fibrosis. CONCLUSION: These evidence-based guidelines are aimed at guiding the diagnosis and the management in clinical practice of idiopathic pulmonary fibrosis.
Since the latest 2017 French guidelines, knowledge about idiopathic pulmonary fibrosis has evolved considerably.Practical guidelines were drafted on the initiative of the Coordinating Reference Center for Rare Pulmonary Diseases, led by the French Language Pulmonology Society (SPLF), by a coordinating group, a writing group, and a review group, with the involvement of the entire OrphaLung network, pulmonologists practicing in various settings, radiologists, pathologists, a general practitioner, a health manager, and a patient association. The method followed the "Clinical Practice Guidelines" process of the French National Authority for Health (HAS), including an online vote using a Likert scale.After a literature review, 54 guidelines were formulated, improved, and then validated by the working groups. These guidelines addressed multiple aspects of the disease: epidemiology, diagnostic procedures, quality criteria and interpretation of chest CT scans, lung biopsy indication and procedures, etiological workup, methods and indications for family screening and genetic testing, assessment of the functional impairment and prognosis, indication and use of antifibrotic agents, lung transplantation, management of symptoms, comorbidities and complications, treatment of chronic respiratory failure, diagnosis and management of acute exacerbations of fibrosis.These evidence-based guidelines are intended to guide the diagnosis and practical management of idiopathic pulmonary fibrosis.

La polyarthrite rhumatoïde (PR) est une maladie chronique affectant 1 % des Européens. La morbidité est liée à la destruction articulaire et la mortalité est causée majoritairement par les manifestations extra-articulaires. Parmi celles-ci, les lésions interstitielles pulmonaires ont une prévalence de 19 à 56 %. Ce travail a pour objectif d’étudier les caractéristiques des pneumopathies interstitielles diffuses (PID) au cours de la PR, d’en analyser l’évolution et les facteurs pronostiques. Une étude rétrospective multicentrique a été menée dans les centres hospitalo-universitaires de Lille, Paris et Bruxelles, dans des centres hospitaliers et cabinets libéraux du Nord-Pas-de-Calais, sur 49 patients présentant une pneumopathie interstitielle diffuse (PID) en lien avec la PR. Le recueil des données a été réalisé à partir des dossiers médicaux. La dégradation fonctionnelle était définie comme une diminution d’au moins 10 % de la capacité vitale forcée et/ou d’au moins 15 % de la capacité de diffusion du monoxyde de carbone. Les lésions étaient de type pneumopathie interstitielle commune (PIC) dans 47 % des cas et pneumopathie interstitielle non spécifique (PINS) dans 27 % des cas. La médiane de survie était de 65 mois. Le pronostic était médiocre avec une survie à 10 ans de 44 %. Bien qu’il n’ait pas été noté de différence significative concernant la survie entre les groupes PIC et PINS (HR : 4,292 ; IC95 % : 0,75–24,64 ; p = 0,102), il semblait exister une tendance à la surmortalité dans le sous-groupe PIC par rapport au sous-groupe PINS. L’absence de dégradation fonctionnelle était associée à une survie de 100 % indépendamment de la sévérité initiale. Les autres facteurs associés à la mortalité étaient le sexe féminin et la dyspnée d’intensité élevée. Cette étude met en valeur la nécessité d’un suivi respiratoire rapproché des patients atteints de PID sur PR afin d’identifier les patients présentant une dégradation fonctionnelle, dont le pronostic est péjoratif.
Le VQ11 est un auto-questionnaire de qualité de vie liée à la santé spécifique à la BPCO. Nous avons voulu évaluer l'intérêt du VQ11 pour le suivi thérapeutique. Notre étude a évalué l'impact sur le VQ11 et ses sous-dimensions d'un traitement bronchodilatateur de longue durée d'action initié chez les patients BPCO en état stable ainsi que les autres auto-questionnaires et échelles analogiques. Il s'agit d'une étude ouverte, prospective, multicentrique. Les patients ont ainsi été évalués à l'instauration (ou ajout) d'un traitement bronchodilatateur (V1) et à 3 mois (V2) par les questionnaires suivants : le VQ11, le questionnaire de Saint-Georges (SGRQ) et des échelles analogiques de dyspnée et de gêne dans la vie quotidienne liée à la respiration. Soixante-seize patients de 60 ± 9 ans ont été inclus, dont 54 (71 %) hommes. Il y avait 66 % de BPCO stade II et 31 % de stade III (GOLD 2010). À V1, le score VQ11, (24,3 ± 8,6) était corrélé au SGRQ (p = 0,001), à l'EVA dyspnée (p = 0,023) et à l'EVA gêne respiratoire dans la vie quotidienne (p = 0,0004). À V1, un VQ11 élevé était significativement associé à une amélioration de la qualité de vie. Le risque relatif d'amélioration de la qualité de vie était de 2,77 [1,32 ; 5,78] pour un score VQ11 à l'inclusion > 24 comparé à un score VQ11 ≤ 24 (p = 0,006). En analyse multivariée, le score VQ11 à l'inclusion était prédictif d'amélioration de la qualité de vie à 6 semaines (RR = 1,03 [1,00 ; 1,05] ; p = 0,049). Le VQ11 semble un outil pertinent pour le suivi sous traitement de la qualité de vie chez le patient BPCO.
La bronchopneumopathie chronique obstructive (BPCO), dont le tabagisme est le principal facteur de risque, est un problème majeur de santé publique pour lequel il n’existe que des traitements symptomatiques. La progression de la maladie fait intervenir des épisodes d’exacerbations favorisés par une susceptibilité de ces patients aux infections. Des travaux du laboratoire ont montré le rôle du défaut d’IL-17 et d’IL-22 dans la susceptibilité à l’infection par S. pneumoniae et H. influenzae. Notre objectif est d’évaluer la capacité d’immunostimulants agonistes de TLR (LPS, Flagelline et CpG) à moduler la réponse IL-17 et IL-22 mais aussi d’IFN-γ. Les taux d’IL-17, d’IL-22 et d’IFN-γ en réponse au LPS, à la Flagelline et au CpG ont été mesurés par Elisa dans les surnageants de cellules mononucléées (CMN) de : – sujets témoins non-fumeurs ; – sujets fumeurs ; – patients BPCO. Les cellules productrices de ces cytokines en réponse à ces immunostimulants ont été identifiées par cytométrie en flux. L’impact de la fumée de cigarette (CSE) sur la réponse aux immunostimulants a été évalué in vitro chez des sujets non-fumeurs. Le rôle des cellules présentatrices d’antigènes (APC) a également été analysé après exposition aux immunostimulants en présence ou non de CSE. Parmi les immunostimulants testés, seule la Flagelline induit une réponse IL-17 et IL-22 chez les patients BPCO. Cette réponse Th17 est observée principalement dans les cellules NK, NKT et Lineage−. Cette réponse est liée à une action directe sur les APC, puisque la Flagelline et le LPS augmentent la sécrétion des cytokines pro-Th17 par les monocytes et les cellules dendritiques de sujets témoins. L’exposition in vitro au CSE inhibe l’action de la Flagelline avec un effet direct sur les APC entraînant une diminution de production de cytokines pro-Th17. Ce travail montre l’efficacité de la Flagelline à restaurer une réponse de cytokines Th17 par les cellules immunitaires innées chez les patients BPCO. Cependant, l’effet de ce ligand du TLR5 est contrecarré par l’exposition au CSE. L’utilisation de cet immunostimulant pourrait représenter une nouvelle stratégie thérapeutique pour restaurer une immunité compétente chez les patients BPCO.
Chronic obstructive pulmonary disease (COPD) is a major clinical challenge mostly due to cigarette smoke (CS) exposure. Invariant natural killer T (iNKT) cells are potent immunoregulatory cells that have a crucial role in inflammation. In the current study, we investigate the role of iNKT cells in COPD pathogenesis. The frequency of activated NKT cells was found to be increased in peripheral blood of COPD patients relative to controls. In mice chronically exposed to CS, activated iNKT cells accumulated in the lungs and strongly contributed to the pathogenesis. The detrimental role of iNKT cells was confirmed in an acute model of oxidative stress, an effect that depended on interleukin (IL)-17. CS extracts directly activated mouse and human dendritic cells (DC) and airway epithelial cells (AECs) to trigger interferonγ and/or IL-17 production by iNKT cells, an effect ablated by the anti-oxidant N-acetylcystein. In mice, this treatment abrogates iNKT-cell accumulation in the lung and abolished the development of COPD. Together, activation of iNKT cells by oxidative stress in DC and AECs participates in the development of experimental COPD, a finding that might be exploited at a therapeutic level.
En imagerie TEP, la quantification des données acquises peut être assurée en utilisant une série de techniques de correction pour minimiser les effets inhérents au processus d’imagerie par émission de positons. Des images quantitatives peuvent ainsi être reconstruites après avoir corrigé les données d’émission détectées de la présence de coïncidences aléatoires et diffusées, des effets d’atténuation, des effets associés aux différentes sensibilités des détecteurs, des effets liés à la géométrie de l’appareil et des effets du temps-mort. Tous ces effets sont bien pris en compte dans les imageurs TEP commercialisés. Néanmoins, d’autres effets viennent entacher d’erreurs les images TEP, comme la résolution spatiale finie engendrant des effets de volume partiel ou encore les mouvements du patient et, notamment, les mouvements respiratoires et cardiaques. Au niveau thoraco-abdominal, les effets du mouvement respiratoire sur les images d’émission tomographiques se caractérisent par une perte de sensibilité dans la détection en raison du flou qui lui est associé. De nombreuses méthodes ont été développées pour corriger ces effets, mais sans réellement encore trouver une légitimité en routine clinique. Ces domaines de recherche sont ainsi toujours très actifs.In PET imaging, quantification of the acquired data can be achieved by using a series of correction techniques to minimize the effects inherent in the imaging process of emission tomography. Quantitative images can be reconstructed after correcting the detected emission data for the presence of random and scattered coincidences, attenuation effects, effects associated with the variable detectors sensitivity, effects related to the geometry of the device and the effects of dead-time. All these effects are well accounted for in the PET imaging market. However, other effects can degrade the quality of the PET images, such as the finite spatial resolution generating partial volume effects, or the patient movements including respiratory and cardiac physiological movements. In the thoracic field, the effects of respiratory motion on PET images are characterized by a loss of detection sensitivity due to the associated blur. Several methods have been developed to correct for these effects, but still without a wide acceptance in clinical routine. These research areas are therefore still very active.

Idiopathic pulmonary fibrosis (IPF) is the most frequent chronic idiopathic interstitial pneumonia in adults. The management of rare diseases in France has been organised by a national plan for rare diseases, which endorsed a network of expert centres for rare diseases throughout France. This article is an overview of the executive summary of the French guidelines for the management of IPF, an initiative that emanated from the French National Reference Centre and the Network of Regional Competence Centres for Rare Lung Diseases. This review aims at providing pulmonologists with a document that: 1) combines the current available evidence; 2) reviews practical modalities of diagnosis and management of IPF; and 3) is adapted to everyday medical practice. The French practical guidelines result from the combined efforts of a coordination committee, a writing committee and a multidisciplinary review panel, following recommendations from the Haute Autorité de Santé. All recommendations included in this article received at least 90% agreement by the reviewing panel. Herein, we summarise the main conclusions and practical recommendations of the French guidelines.
To the Editors: The combination of atovaquone and proguanil is effective and safe in the prevention and treatment of malaria. We present the first report of proguanil-induced acute eosinophilic pneumonia (AEP) associated with skin eruption. On December 18, 2006, a 40-yr-old female was admitted to our hospital (Victor Provo Medical Centre, Roubaix, France) for shortness of breath and widespread skin lesions. She had a 10-pack-yr smoking history but no respiratory illness or relevant comorbidity. On the morning of December 8, 2006, she had initiated atovaquone–proguanil treatment for malaria prophylaxis prior to a trip to Senegal. She had taken the same treatment for the first time 1 yr previously without any complications. That evening she developed progressive dyspnoea with nonproductive cough and subacute fever (38°C). She only stayed in Senegal for 4 days and did not leave her hotel room during this time. After returning to France she continued to take atovaquone–proguanil for 1 week. Increased shortness of breath prompted her to consult her general practitioner, who prescribed amoxicillin plus clavulanic acid for bronchitis. The aim of the study was to determine whether the Borg dyspnoea scale could be a useful and simple marker to predict respiratory muscle weakness in amyotrophic lateral sclerosis (ALS). From April 1997 to 2001, respiratory function was perfomed in 72 patients together with the Borg score in both the upright (uBorg) and supine (sBorg) positions. Mean upright vital capacity (VC) was 81+/-24% predicted, sniff nasal inspiratory pressure (SNIP) was 55+/-26% pred, maximal inspiratory pressure (P(I,max)) was 57+/-26% pred and arterial carbon dioxide tension (P(a,CO(2))) was 41+/-6 mmHg. The mean Borg scores in the upright and supine positions were 1.7+/-1.5 and 2.2+/-2, respectively. A significant relationship between SNIP and uBorg (r = 0.4; p = 0.0007) and SNIP and sBorg (r = 0.58; p<0.0001) was observed. Upright VC, DeltaVC (measured as the supine fall in VC as a percentage of seated VC), P(I,max) and P(a,CO(2)) were significantly correlated with SNIP. A cut-off value of 3 on the sBorg scale provided the best sensitivity (80%) and specificity (78%) (area under the curve 0.8) to predict a SNIP < or =40 cmH(2)O, indicating severe inspiratory muscle weakness. Patients with a sBorg score > or =3 also exhibited significantly lower VC, P(I,max) and twitch mouth pressure during cervical magnetic stimulation, and slightly higher P(a,CO(2)) (43.7+/-7 versus 39.2+/-5 mmHg; p = 0.05). The Borg dyspnoea scale is a valuable noninvasive test for the prediction of inspiratory muscle weakness in ALS patients.
Cannabis is widely smoked in Europe and its increasing use is becoming a major public health problem. Adulterating cannabis with glass beads or sand is a new trick used by dealers to increase the weight and boost profits. These recent practices are not without danger.We report two cases of respiratory symptoms related to the use of this kind of adulterated cannabis, The first case is a 33 year-old patient admitted for an acute inhalation pneumonitis secondary to smoking cannabis adulterated with grit sand. The CT scan showed patchy ground-glass opacities, mainly in the upper lobes. A broncho-alveolar lavage, examined under polarized light, revealed birefringent intracellular particles, identified as silica in alveolar macrophages. Spontaneously clinical and radiological improvements were observed after stopping the use of contaminated cannabis. The second patient, who smoked cannabis mixed with glass beads, described epistaxis, mouth ulcers, sore throats and cough. CT scan and BAL were normal.Adulteration of cannabis with microscopic glass beads, alone or mixed with sand, is a recent and widespread practice in Europe These anecdotal reports prompted the French Department of Health to advise cannabis smokers of the harmfulness of these contaminants.
