Cerrahi süreçler, hastaları fiziksel ve psikolojik yönden etkileyen dönemlerdir. Ameliyat öncesi yaşanan belirsizlik, anksiyete ve korku; ameliyat sonrasında ise ağrı, hareket kısıtlılığı, uyku bozuklukları ve yaşam kalitesinde azalma gibi sorunlar bireyin iyileşme sürecini olumsuz yönde etkileyebilmektedir. Bu nedenle cerrahi bakımda sadece fizyolojik değil, aynı zamanda psikososyal gereksinimlerin de ele alınması önem kazanmaktadır. Geleneksel ve tamamlayıcı tıp uygulamaları arasında yer alan meditasyon ve yoga, cerrahi bakım sürecinde etkili birer destekleyici yöntem olarak ön plana çıkmaktadır. Meditasyon; zihni sakinleştirme, dikkati odaklama ve içsel farkındalık geliştirme temeline dayanırken, yoga, fiziksel duruş, nefes kontrolü ve gevşeme teknikleriyle bedensel ve zihinsel dengeyi sağlamayı amaçlar. Meditasyon ve yoga hastaların anksiyete düzeylerinin azaltılmasında kullanılan etkin yöntemlerdir. Yoga özellikle çok az düzeyde hareket içeren pozisyonlarla ameliyat sonrası mobilizasyonu destekleyebilir, dolaşımı artırabilir ve komplikasyonların önlenmesine katkıda bulunabilir. Cerrahi bakım sürecinde bireyin bireysel ve bütüncül gereksinimlerini karşılamayı hedefleyen hemşirelik uygulamaları için meditasyon ve yoga, klinik karar verme süreçlerine entegre edilmesi gereken önemli geleneksel ve tamamlayıcı tıp uygulamalarındadır. Surgical processes are periods that affect patients physically and psychologically. Uncertainty, anxiety, and fear experienced before surgery, and problems such as pain, limited mobility, sleep disturbances, and a decrease in quality of life after surgery, can negatively affect the individual’s recovery process. Therefore, in surgical care, it becomes important to address not only physiological needs but also psychosocial needs. Meditation and yoga, which are among traditional and complementary medical practices, stand out as effective supportive methods during the surgical care process. While meditation is based on calming the mind, focusing attention, and developing inner awareness, yoga aims to achieve physical and mental balance through physical postures, breath control, and relaxation techniques. Meditation and yoga are effective methods used to reduce patients’ anxiety levels. Yoga, particularly through positions involving very minimal movement, can support postoperative mobilization, enhance circulation, and contribute to the prevention of complications. For nursing practices that aim to meet the individual and holistic needs of individuals during the surgical care process, meditation and yoga are important traditional and complementary medical practices that should be integrated into clinical decision-making processes.
Purpose This study aimed to investigate its effects on self-esteem and quality of life in patients with stoma. Methods This randomized controlled trial was conducted through the “Association of Colostomy and Ostomy Patients” between November 2021 and April 2023. The research population consisted of 192 patients who attended the association, and the sample consisted of 74 patients who met the inclusion criteria. After obtaining institutional and ethics committee approval, a total of four sessions of laughter therapy were delivered to participants in the intervention group via video calls over WhatsApp by the certified researcher. The data were collected using a Personal Information Form, the Rosenberg Self-Esteem Scale, and the City of Hope-Quality of Life-Ostomy Questionnaire. Data were analyzed using SPSS version 26. Results After laughter therapy, in the posttest and the 3rd-month follow-up measurements, the participants in the intervention group were determined to have significantly higher self-esteem and quality of life levels than those in the control group (p<0.001). In addition, it was determined that the quality of life of the individuals in the intervention group increased more in each measurement. Conclusion Laughter therapy is an effective complementary intervention for improving self-esteem and quality of life in patients with stoma. Its low cost, ease of application, and lack of side effects support its wider use in nursing practice. Additionally, future studies including larger samples and participants from different sociocultural backgrounds are recommended to enhance the generalizability of the findings.
Valid and reliable assessment tools are essential for evaluating surgical hand washing skills, which play a crucial role in preventing surgical site infections. This study aimed to develop and validate an objective assessment tool for evaluating surgical hand washing competency among nursing students. Methodological validation study. This study was conducted between November and December 2023 at Istanbul University-Cerrahpaşa with 105 second-year nursing students enrolled in surgical nursing courses. Data were collected using a 13-item Assessment Tool for Surgical Hand Washing Skills (ATSHWS) and demographic questionnaire. Content validity was established through expert review, and reliability was assessed through inter-observer agreement and test-retest analysis. Content validity was excellent (CVI: 0.976). Inter-observer reliability demonstrated strong agreement (ICC: 0.951, p<0.001), and test-retest reliability confirmed temporal stability. The ATSHWS provides a valid and reliable method for objectively evaluating surgical hand washing skills among nursing students and may be useful for educational assessment and quality improvement initiatives.
Effective bowel preparation is essential for successful colonoscopy, allowing for optimal mucosal visualization and polyp detection. While standard educational materials are commonly used, mobile health technologies offer potential for improving patient adherence and preparation quality. This study aimed to evaluate the impact of mobile application-based bowel preparation training on bowel preparation compliance, quality, and anxiety levels in patients scheduled for colonoscopy. This prospective, single-blind, randomized controlled trial included 160 adult outpatients scheduled for elective colonoscopy. Patients were randomly assigned to either a mobile application–based education group (n = 80) or a control group receiving standard verbal and written instructions (n = 80). All patients received the same purgative regimen. The primary outcome was bowel preparation quality assessed by the Boston Bowel Preparation Scale (BBPS). Secondary outcomes included compliance with dietary and preparation protocols and pre-procedural anxiety measured by the State-Trait Anxiety Inventory (STAI). Advances in web-based education and technology-mediated learning environments offer opportunities to enhance knowledge access and active learning, particularly in infection control, a critical public health issue. This study aims to assess the quality and reliability of e-learning materials used in infection control education for nursing students by evaluating materials related to hand hygiene, personal protective equipment, isolation measures, and standard precautions in healthcare colleges across Estonia, Finland, Portugal, Spain, and Turkey. The materials were assessed by ten experts from each country (only six experts from Finland) via the Learning Object Review Instrument (LORI). In our study, the Cronbachâs alpha coefficient for e-learning materials ranged from 0.736 to 0.932. In terms of the mean LORI total score, the highest mean score was for personal protective equipment (3.983 ± 0.619), whereas the lowest was for standard precautions (3.850 ± 0.550). When comparing the quality of materials related to infection control measures among the countries, materials related to hand hygiene, personal protective equipment, isolation measures, and standard precautions from Spain had the highest LORI total scores. In contrast, materials from Portugal had the lowest mean scores (with the exception of isolation measures). The interobserver agreement coefficients were calculated as 0.516â0.805 for Spain, 0.614â0.729 for Estonia, 0.511â0.711 for Turkey, 0.749â0.938 for Finland, and 0.566â0.882 for Portugal. As the first study to evaluate the quality and reliability of infection control materials via LORIs, the findings contribute valuable insights to the literature and provide a foundation for future research on improving e-learning in nursing education.
Amaç: Kadınların sağlık okuryazarlığı ile meme kanseri farkındalığı arasındaki ilişkiyi araştırmaktır.Yöntem: Bu çalışma, Haziran-Aralık 2023 tarihleri arasında kesitsel ve tanımlayıcı olarak yürütülmüştür. Örneklem, en az ilkokul mezunu, internet erişimi olan ve 18 yaş ve üzeri 354 sağlıklı kadından oluşmaktadır. Verilerin toplanmasında Microsoft Forms üzerinden çevrimiçi olarak Tanıtıcı Bilgi Formu, Sağlık Okuryazarlığı Ölçeği ve Meme Kanseri Farkındalık Ölçeği kullanıldı. This study explored operating room nurses' awareness, practices, and perceived barriers regarding environmental sustainability. The study was conducted between January and February 2025 using in-depth, semi-structured interviews with 15 operating room nurses. Data were collected using an interview form and a personal information form, employing maximum variation and snowball sampling techniques. The data were analyzed using content analysis, which yielded four main themes and 10 subthemes: (1) knowledge and awareness of environmental sustainability, (2) experiences with environmental sustainability in the operating room, (3) nurses' role in environmental sustainability, and (4) recommendations for environmental sustainability. Operating room nurses had limited awareness of environmental sustainability, revealing gaps in knowledge, support, and practices. The findings highlighted both positive and negative experiences related to sustainability practices. To improve sustainable practices, it is essential to increase awareness, expand educational opportunities, and develop institutional policies that are crucial. The limited awareness of environmental sustainability highlights the need for targeted educational programs aimed at increasing nurses' knowledge. Enhancing nurses' awareness and understanding is pivotal in implementing sustainable practices in the operating room, contributing to efficient resource utilization and benefiting both the environment and healthcare institutions.
PURPOSE:Postoperative nausea and vomiting (PONV) is a group of symptoms that develop in patients within 24 hours after surgical intervention and are undesirable outcomes complicating patients' postoperative recovery. The purpose of this study was to establish the impact of ginger and peppermint essential oil inhalation for PONV in patients who underwent total thyroidectomy. DESIGN:Our study was designed as a randomized controlled experimental study. METHODS:Participants consisted of patients who underwent thyroidectomy surgery at Bezmialem Vakif University General Surgery Clinic between February 2021 and August 2022. The subjects were first divided randomly into three groups: a ginger group, a peppermint group, and a routine care group. Patients' pain, nausea, and vomiting severity were measured at 2, 6, and 24 hours. Patients' outcome was assessed using the visual analog scale and the Rhodes Nausea and Vomiting Index. FINDINGS:Significantly lower pain and nausea severity was found in the ginger group compared with the control group. Similarly, the peppermint group also showed significantly lower pain and nausea severity compared with the control group. Rhodes 1 item score was 0.48 ± 0.51 in the ginger group, 1.11 ± 0.64 in the peppermint group, and 1.37 ± 0.49 in the control group (P < .001). These findings suggest that both ginger and peppermint essential oils are effective in alleviating PONV symptoms, with each group showing significant improvement compared with the control group. CONCLUSIONS:Results support that ginger and mint might have a positive effect if used to improve postoperative recovery quality. This nursing intervention is recommended to prevent and minimize the PONV occurrence and is made of ginger and mint.
Adherence to hand hygiene procedures is crucial for all populations, and the World Health Organization (WHO) has implemented specific guidelines for infection control. Frequent and correct hand hygiene can prevent infections, but non-compliance with hand hygiene is pervasive. Nursing students address this issue from the beginning of their training. In nursing training, self-efficacy is crucial in enhancing students’ competence, motivation, and clinical performance. We performed a cross-sectional multicenter study in five European countries, with a cross-sectional design with an online application of an instrument measuring hand hygiene knowledge based on WHO guidelines and general self-efficacy and specific self-efficacy for infection control. A total of 638 first-year nursing students participated in this study. The mean percentage of correct answers was 67.9%, with a considerable difference depending on the items. The worst results were obtained for questions related to sources of infection and types of hand hygiene methods in different situations. Finnish students displayed significantly (p < 0.001) higher scores in HH knowledge, whereas Estonian students had significantly (p < 0.001) higher levels of self-efficacy. There were significant correlations between the hand hygiene knowledge score and the self-efficacy score (p < 0.001). A multivariate analysis by linear regression analysis showed significant associations between the hand hygiene knowledge survey score and the students’ age (p < 0.001, OR = 0.18, 95% CI 0.04–0.10), as well as their country of origin (p = 0.01, OR = 0.09, 95% CI 0.03–0.34). HH knowledge is quite low among nursing students, and is correlated with self-efficacy, although the strongest predictors are age and country of origin. Different nursing curricula must favor HH knowledge, with varying degrees of emphasis depending on the country.
Purpose: The aim of this study was to develop a valid and reliable scale to evaluate and measure phantom limb pain. Material and Methods: This study, which was designed in a methodological type, was conducted with a total of 258 patients. A demographics form and a draft scale developed by the research authors were used to collect the study data. Kuder-Richardson Formula 20 was used to provide descriptive statistics and reliability analyses for the study data. Exploratory Factor Analysis was used in the development of the phantom limb pain rating scale, and Reliability and Confirmatory Factor Analysis were used for the study’s validity and reliability evaluations. Results: The Kuder-Richardson 20 value, which shows the internal consistency of the questions of the 16-item the rating scale, was found to be 0.921. The total score of the rating scale ranged from 1 to 16, with an average of 11.19±4.94. Postoperative nausea and vomiting (PONV) are common and distressing side effects after surgery and anesthesia. Pharmacological treatment is not sufficient to manage these two symptoms. Although the use of certain anesthetic agents is considered the main basis for this phenomenon, the exact factors are unknown. Aromatherapy is effective in reducing PONV in adults. The aim of this review is to scan the studies evaluating the effect of aromatherapy on the management of nausea and vomiting in the postoperative. A literature search was performed by scanning Cochrane Reviews, PubMed, OVID, Sciences Direct, and Google Scholar. Keywords “aromatherapy,” “use of aromatherapy,” “aromatherapy oils,” “nausea,” “vomiting,” “postoperative nausea and vomiting,” and “PONV” combined with Boolean operators, including AND, OR, and NOT. Data gathered from studies published from 2013 to 2023 were reviewed for the effect of aromatherapy on PONV. As a result of the screening, 12 studies out of a total of 706 studies were examined based on the inclusion criteria. In the studies, aromatherapy was used either by inhalation or orally. The plants that were used the most in these studies were ginger and mint. When the literature was reviewed, it was found that aromatherapy reduced the rate of nausea and vomiting and the need for antiemetics, especially in the first minutes after surgery. Using aromatherapy in surgical patients may relieve common postoperative symptoms. The limitations of drug therapy have led to an increase in the use of alternative drugs for the management of nausea and vomiting. Aromatherapy was favored by most patients and is an effective treatment option for PONV.
OBJECTIVE:Due to numerous stressors in intensive care, common psychosocial problems arise in patients. Among these, decreased anxiety and sleep quality are observed. This study aims to determine the effect of lavender and bergamot oil applied by inhalation on anxiety and sleep quality in surgical intensive care unit patients. METHODS:Fifty-four patients hospitalized in the intensive care unit of a hospital in Istanbul, Turkey, were included in this study. They were randomly divided into three groups (Lavender, Bergamot, Control). Intervention groups were exposed to 3 drops of lavender oil or bergamot oil on pillows for 20 min, which were then placed 10 cm away from the patient's head. This intervention was applied for two nights. The patients' sleep quality and anxiety level were evaluated using the "State-Trait Anxiety Inventory (STAI)" and "Richard-Campbell Sleep Scale (RCSS)." Data were analyzed using Chi-square, independent t-test, One-way ANOVA tests. RESULTS:According to our results, anxiety scores significantly decreased in the lavender and bergamot groups. Sleep quality scores significantly increased (p = <0.001). The control group showed lower sleep quality scores. CONCLUSION:Lavender and bergamot oil inhalation appeared effective in reducing anxiety and improving sleep quality in surgical intensive care unit patients.
Objective: This study was conducted with a comparative and descriptive research design to evaluate the self-care agency and self-efficacy of patients who experienced phantom pain after limb amputation. Methods: The population of the study consisted of 54 patients who underwent limb amputation operations in a university hospital in Istanbul between 1 January 2018 and 1 January 2020. Sample selection was not made, all patients included the population were reached by phone after their discharge. When the data were collected, one patient was excluded due to being younger than 18 years of age, 12 patients died, 9 patients had stump pain, and the study was completed with 32 patients. Data were collected using a "Personal Information Form", the Visual Analog Scale, the Exercise of Self-Care Agency Scale and the Self-Efficacy Scale. Results: The mean Exercise of Self-Care Agency Scale score of the patients was found to be 79.37±21.55. Their mean total Self-Efficacy score was 70.00±16.10. There was no statistically significant relationship between postoperative phantom pain and scale scores. Conclusion: It was determined that patients who experienced phantom pain after amputation had moderate self-care agency and perceived self-efficacy levels.
Amaç: Bu araştırmanın amacı; genel cerrahi kliniklerinde çalışan hemşirelerin Cerrahi Sonrası Hızlandırılmış İyileşme (Enhanced Recovery After Surgery-ERAS) protokolü uygulamalarına ilişkin değerlendirmelerini belirlemektir. Yöntem: Tanımlayıcı ve kesitsel tipteki bu araştırma, genel cerrahi kliniğinde çalışan 258 hemşire ile 20 Mayıs-20 Haziran 2021 tarihleri arasında gerçekleştirildi. Genel cerrahi kliniklerinde çalışan hemşirelerin sosyo-demografik, mesleki özellikleri ile ERAS protokolleri ve uygulamalarına ilişkin değerlendirmelerini içeren 37 sorudan oluşan veri toplama formu, çevrimiçi olarak hemşirelere iletildi. Veriler, SPSS 25.0 programıyla analiz edildi. Bulgular: Araştırmaya katılan hemşirelerin yaş ortalaması 28,81±7,63 yıl olup, %86’sı (n=222) kadın ve %38’i (n=98) lisans mezunudur. Hemşirelerin %38,8’nin (n=100) ERAS protokolleri hakkında bilgisi olmadığı, %58,9’unun (n=152) ise ERAS protokolüne yönelik herhangi bir eğitim almadığı ve %36,4’ünün (n=94) çalıştığı klinikte ERAS protokollerinin uygulanmadığı belirlendi. Lisansüstü eğitim düzeyine sahip olan (p<0,01) ve ERAS protokollerine ilişkin bilgi sahibi olan hemşirelerin (p<0,001) protokolün ameliyat öncesi, sonrası ve bileşenlerin toplamının uygulanmasına yönelik sorulara doğru yanıt verme oranlarının istatistiksel olarak anlamlı ve yüksek olduğu belirlendi. Çalıştığı hastanedeki hasta-hemşire oranı 1-5 arasında olan (p<0,05) ve ERAS protokollerine ilişkin eğitim alan (p<0,01) hemşirelerin ise ameliyat sonrası bileşenlerin uygulanmasına yönelik sorulara doğru yanıt verme oranlarının istatistiksel olarak anlamlı ve yüksek olduğu bulundu. Sonuç: Lisansüstü eğitim düzeyine sahip ve ERAS protokollerine ilişkin bilgisi olan hemşirelerin ameliyat öncesi ve sonrası uygulamalara ilişkin sorulara doğru yanıt verme oranının yüksek olduğu belirlendi. Çalıştığı hastanede en fazla 5 hastaya bakan ve ERAS protokollerine ilişkin eğitim almış olan hemşirelerin ise ameliyat sonrası uygulamalara yönelik sorulara doğru yanıt verme oranının yüksek olduğu bulundu. Cerrahi hemşirelerinin bilgi düzeyini artırmaya yönelik hemşirelik eğitiminde ve mezuniyet sonrası hizmet içi eğitim programlarında ERAS protokollerine yer verilmesi, cerrahi hemşirelerinin ERAS protokolüne yönelik güncel gelişmeler ile kanıta dayalı rehberleri takip etmeleri ve cerrahi hastasının bakımında disiplinler arası ERAS protokolü uygulamalarının temel alınarak hasta bakımına aktarılması önerilmektedir.
The aim of this compilation is to review the role of the nurse in the patient who describes phantom sensation and phantom pain in the light of the literature. The problems that occur in individuals following operations such as amputation and mastectomy, which require removal of an organ / limb from the body, lead to disability in daily life activities in different dimensions, causing individuals to become fully or semi-dependent physically, economically and socially. The pain in the extremity, which is removed from the body after amputation, is called phantom pain and its mechanism is not fully understood yet. It is known that both peripheral and central nervous system play a role in phantom pain. Phantom Sensation (PS), which is defined as feeling that the limb or organ is still in place, and Phantom Pain (PP), seen as pain in the non-organ / limb, are common chronic problems in the postoperative period. These symptoms, which prolong the healing process in patients undergoing surgery, can only be identified by a comprehensive / complex assessment, and therefore the role of the nurse in management is large. In this compilation, we aimed to review the role of nurses in the treatment and care of patients with phantom sensation and phantom pain.
Aim This study aimed to investigate the preoperative fasting time and preoperative and postoperative well-being of patients. Method This is a descriptive study. In Istanbul, between January and June 2016, 130 patients from the Training and Research Hospital Urology Clinic were included in the study. In collecting the data of the research, the information form, which included the descriptive features and surgical intervention information of the patients, and together with the scale of “Quality of Recovery-40 Questions (QoR-40)” was used. Mean, Standard Deviation, Median, Frequency, Ratio, Minimum, Maximum, Students’ t-Test, Mann Whitney U-test, Paired Samples T-Test, Pearson and Spearman’s Correlation Analysis were used for data analysis. Results The mean age was 57.48 ± 11.12, 79.2% of the patients were male and 63.8% of them underwent robotic prostatectomy. It was observed that the mean of times for preoperative fasting, solid foods, liquid foods were 11.26 ± 2.17, 12.56 ± 2.47, 11.26 ± 2.17 hours. The percent score of patients who felt hungry, thirsty, and dryness of the mouth was 30.8, 48.5, and 40, respectively, and patients had complaints such as nausea, fatigue, and feeling anxious before surgery. Conclusion It was determined that patients were still hungry after midnight until surgery and remained hungry for a long time. The preoperative and postoperative well-being was adversely affected; however, there was no statistically significant relationship between the preoperative fasting time and postoperative well-being of patients, duration of hospitalization (p > .05).
Giriş: Hemşirelik, bireye ve topluma hizmet sunan toplumun sağlığını koruma, geliştirme ve hastalık durumunda iyileştirmeyi amaçlayan, bilim ve sanata dayalı profesyonel kendini sürekli yenileyen bir meslek grubudur. Amaç: Bu çalışma cerrahi hastalıkları hemşireliği dersinin klinik uygulamasını yapan öğrencilerin belirledikleri hemşirelik tanılarının önem sırasına göre değerlendirilmesi amacıyla yapıldı. Yöntem: Tanımlayıcı ve kesitsel yöntem kullanılarak yapılan çalışmada, bir Vakıf Üniversitesi Sağlık Bilimleri Fakültesi Hemşirelik Anabilim Dalı'nda 2018-2019 ve 2019-2020 eğitim öğretim yılında Cerrahi Hastalıkları Hemşireliği Klinik Uygulamasını gerçekleştiren 156 öğrencinin hemşirelik bakım planları incelenmiş ve öncelikli tanılar belirlenmiştir. Bulgular: Öğrencilerin hazırladıkları hemşirelik bakım planları öncelik sırasına göre incelendiğinde; birinci tanı %26,9 (n:42) ile kanama riski, ikinci tanı %30,1 (n:47) ile enfeksiyon riski, üçüncü tanı ise %21,2 ile (n:33) akut ağrı olarak belirlendi. Sonuç: Cerrahi hastalıkları dersini almış hemşirelik öğrencilerinin hemşirelik bakım planlarında, kanama riski, enfeksiyon riski, akut ağrı gibi tanıları öncelikli olarak belirledikleri görüldü. Diğer tanıları belirlemede ve hastaları bütüncül olarak değerlendirmede sorun yaşadıkları söylenebilir.
