BACKGROUND:Bevacizumab, an anti-VEGF monoclonal antibody, has recently emerged as a new option for severe forms of hereditary hemorrhagic telangiectasia (HHT). Its utilization in this orphan disease has rapidly spread despite the lack of randomized trials and international guidelines. The objective of this study is to report the main clinical data (baseline characteristics, dose schedule, efficacy, adverse events and deaths) of HHT patients treated by intravenous bevacizumab in France.METHODS:Retrospective observational study of HHT patients treated with bevacizumab for a severe form of the disease in the 14 centers of the French HHT network.RESULTS:Forty-six patients (median age: 68 years) were treated between March 2009 and May 2015. Ten patients were treated for high output cardiac failure, 20 patients for severe hemorrhages and 16 for both indications. The standard protocol (6 infusions of 5mg/kg every 2 weeks) was initially used in 89% of the cases but diverse strategies were subsequently applied. A clinical improvement was noted by the referent physician for 74% of the patients with a median effect's duration of 6 months. Wound healing complications led to 2 amputations. Arthralgia/arthritis and arterial hypertension occurred in 5 patients each. One third of the patients were dead at the time of the final update, coherently with age and the poor prognosis of these highly symptomatic patients.CONCLUSION:Intravenous bevacizumab seems to provide a clinical benefice in severe HHT patients. Precautions concerning wound healing and vascular pathologies must be respected. Prospective double blinded versus placebo trials are needed.
Les syndromes hyperéosinophiliques constituent un groupe d’affections hétérogènes définies par une hyperéosinophilie (> 1,5 109/L) et une atteinte d’organe directement liée aux éosinophiles. Le mépolizumab, un anticorps monoclonal anti-interleukine (IL)-5, a permis une épargne cortisonique et montré une bonne tolérance sur 36 semaines dans un essai pivot incluant des syndromes hyperéosinophiliques non liés à FIP1L1-PDGFRA et corticodépendants [1]. Il existe peu de données sur l’efficacité et la tolérance du mépolizumab à long terme. Notre objectif était d’évaluer le maintien d’efficacité et la tolérance du mépolizumab à long terme au cours du syndrome hyperéosinophilique. Suivi prospectif unicentrique de 20 patients avec un syndrome hyperéosinophilique (10 lymphoïdes, 10 idiopathiques) ayant reçu du mépolizumab dans le cadre des essais sponsorisés par GlaxoSmithKline. Recueil systématique de la durée d’exposition au traitement, des doses de prednisone, des rechutes cliniques, des effets indésirables, effets indésirables graves. Recherche de facteurs prédictifs de réponse au mépolizumab. La durée d’exposition moyenne était de 330 semaines (25–572) et était supérieure à 8 ans pour 10 patients. Au total, le mépolizumab a été considéré comme efficace chez 18/20 patients. Nous avons observé 2 échecs ayant justifié l’arrêt de traitement. Dans ces 2 cas, il existait une corticorésistance initiale, alors que l’ensemble des 18 autres patients étaient corticosensibles, mais dépendants de doses variables de prednisone. Dix patients traités depuis plus de 8 ans restent en rémission clinique et hématologique complète. La dose de corticoïdes a été baissée en moyenne de 10,7 mg/j (de 6 mg/j à 17 mg/j). Le mépolizumab a été maintenu chez 16 patients 20. Il y a eu 2 arrêts pour effets indésirables graves : un hépatocarcinome survenu chez un patient ayant une hémochromatose HFE de type 1 sans cirrhose, et une poussée de la maladie liée à l’inefficacité du traitement. Sur une période de 126 patients/années, 315 effets indésirables dont 18 graves ont été notés. L’épargne cortisonique est importante, avec un excellent profil de tolérance. Les deux arrêts pour effets indésirables graves relèvent du principe de précaution et non d’une relation d’imputabilité forte. Le mepolizumab a donc un meilleur profil de tolérance que l’interféron et l’hydroxyurée. Son utilisation est pour l’instant limitée par sa disponibilité. L’exposition à long terme du mépolizumab apparaît sûre et son efficacité sur l’épargne cortisonique perdure, y compris à très long terme.
Dipeptidyl peptidase-4 (DPP4) inhibitors are a novel therapy widespread used in type 2 diabetes mellitus. We describe 3 cases of polyarthritis which delay of appearance strongly suggests a link with DPP4 inhibitors. Three patients presented with bilateral, symmetrical, seronegative polyarthritis after introduction of DPP4 inhibitors (sitagliptine (n = 2) and vildagliptine (n = 1)). Two patients also developed xerostomia and xerostomia, and laboratory test results showed normal values of CRP and erythrocyte sedimentation rate. Joints X-rays were normal. One patient was diagnosed with primary Sjögren's syndrome and treated with hydroxychloroquine, methotrexate and prednisone, with a poor efficacy. When sitagliptine was stopped, all symptoms disappeared, leading to methotrexate and prednisone discontinuation within a month. There were no immunological abnormalities in the 2 other patients, but a chronic viral hepatitis B was found in one patient. Eventually, discontinuation of DPP4 inhibitors led to resolution of symptoms in 1 and 3 weeks for both patients. DPP4 inhibitors seemed to trigger bilateral, non-erosive, seronegative polyarthritis in our 3 patients. DPP4, also known as CD26, is expressed on many cells including lymphocytes and fibroblasts, and its inhibition may lead to immunomodulating effect as suggested by clinical and in vitro studies.
La maladie de Rendu-Osler (MRO) est une affection héréditaire transmise sur le mode autosomique dominant ; trois gènes (ENG, ACVRL1 et MADH4) sont impliqués dans la genèse de la maladie, qui codent pour des protéines interagissant avec la superfamille du transforming growth factor (TGF)-β. La MRO est une angiodysplasie multisystémique, pouvant se compliquer de malformations vasculaires viscérales, et notamment hépatiques.Nous rapportons l'observation d'une patiente atteinte de MRO, révélée par une cholestase anictérique. Le scanner abdominal a objectivé une dilatation de l'artère hépatique et des fistules artérioportales multiples. Lors du suivi à deux ans, la patiente a développé une insuffisance cardiaque à débit élevé compliquant l'hépatopathie liée à la MRO.Les atteintes hépatiques sont fréquentes (8–31 %) et potentiellement graves au cours de la MRO, pouvant se compliquer d'insuffisance cardiaque à débit élevé, d'hypertension portale et de cholangite ischémique. De fait, les malformations vasculaires hépatiques devraient être recherchées systématiquement chez tous les patients ; la plupart des équipes recommandent, aujourd'hui, la réalisation d'une échographie-doppler hépatique lors du bilan initial et du suivi des patients.Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant vascular dysplasia; mutations in at least three genes (ENG, ACVRL1 et MADH4), which are components of transforming growth factor (TGF)-β, may lead to the clinical picture of HHT. HHT is a multisystemic angiodysplasia, resulting in multiple vascular malformations, involving notably the liver.We report the case of a patient with anicteric cholestasis, revealing HHT. Abdominal CT-scan revealed hepatic artery dilation and multiple arteriovenous fistula. At 2-year follow-up, the patient exhibited liver involvement-associated high output cardiac insufficiency.Liver involvement is frequent in HHT, occurring in 8–31% of patients; it may lead to life-threatening complications, such as high output cardiac failure, portal hypertension or severe cholangitis. Abdominal Doppler ultrasonography is a non-invasive accurate method, suitable for first-line imaging of the liver in patients with HHT; it should be done in all patients, in order to detect HHT-related hepatic vascular malformations.
Nous rapportons une observation de périartérite noueuse (PAN) révélée par des hémorragies cérébrales par rupture d'anévrismes. Une patiente de 40 ans a présenté à 12 jours d'intervalle deux hématomes intracérébraux, le second par rupture d'anévrismes. Le diagnostic de PAN a été porté sur l'histologie de l'anévrisme montrant des lésions de vascularite, sur l'artériographie évocatrice de PAN par ses localisations rénales et spléniques, sur l'altération de l'état général, et sur la présence depuis plusieurs années de nouures cutanées caractéristiques. L'évolution a été globalement favorable sous corticothérapie et cyclophosphamide. Les accidents vasculaires hémorragiques cérébraux sont rares dans la PAN et exceptionnellement révélateurs de la maladie.We report a case of polyarteritis nodosa revealed by intracranial haemorrhage. A 40-year-old woman presented two episodes of cerebral haemorrhage twelve days apart, the second due to an aneurysm rupture. The diagnosis of polyarteritis nodosa (PAN) was based on the following criteria: histological aneurysm examination, angiography suggesting PAN with cerebral, renal and splenic localizations, loss of weight and cutaneous nodules. Cerebral haemorrhage in PAN is rare and exceptionally the presenting feature of the disease.
Imatinib is the treatment of choice for FIP1L1/PDGFRA (F/P)-associated chronic eosinophilic leukemia (F/P+ CEL), but its optimal dosing, duration, and possibility of discontinuation are still a matter of debate. A retrospective multicenter study was conducted with 44 F/P+ CEL patients identified in the French Eosinophil Network and treated with imatinib. The most frequently involved systems were skin (57%), spleen (52%), and lung (45%), and eosinophilic heart disease was observed in 15 patients (34%). Complete hematologic response (CHR) was obtained in all patients, and complete molecular response (CMR) in 95% of patients (average initial imatinib dose, 165 mg/d). For 29 patients the imatinib dose was tapered with a maintenance dose of 58 mg/d (+/- 34 mg/d), allowing sustained CHR and CMR. None of the patients developed resistance during a median follow-up of 52.3 months (range, 1.4-97.4 mo). Imatinib was stopped in 11 patients; 6 of the patients subsequently relapsed, but 5 remained in persistent CHR or CMR (range, 9-88 mo). These results confirm that an initial low-dose regimen of imatinib (100 mg/d) followed by a lower maintenance dose can be efficient for obtaining long-term CHR and CMR. Our data also suggest that imatinib can be stopped in some patients without molecular relapse.

La maladie de Still de l'adulte, atteignant des sujets d'âge moyen dans les deux sexes, se définit cliniquement par l'association d'un état fébrile prolongé, d'une éruption cutanée fugace et d'arthralgies, dans un contexte biologique très inflammatoire. L'hyperleucocytose notable, sans disparition de l'éosinophilie, est la seule donnée paraclinique qui puisse orienter l'enquête, mais elle manque beaucoup de spécificité. Aucun examen paraclinique ne permet d'affirmer le diagnostic. Sa place vis-à-vis de la maladie de Still de l'enfant reste discutée, et sa pathogénie totalement mystérieuse. Le traitement est dominé par la corticothérapie régulièrement efficace, mais de sevrage difficile. Les traitements de fond méritent d'être proposés, mais leur bénéfice n'est pas encore démontré avec certitude.The adult form of Still's disease affecting middle aged patients of both sexes is defined clinically by the association of a chronic pyrexia, transient cutaneous eruption and arthralgia with very inflammatory biological findings. A high leukocyte count without disappearance of eosinophilia is the only paraclinical finding which suggests the diagnosis but it lacks specificity. There is no diagnostic paraclinical investigation. Its relation to the childhood form of Still's disease remains controversial and its pathogenesis is completely unknown. The treatment centres around steroid therapy which is regularly effective but difficult to withdraw. Maintenance therapy should be proposed but its benefits have not be proved.
Urinary tract involvement is rarely described in APS. We report the case of a 40 year-old man with primitive APS who was diagnosed an arterial and venous unilateral ureteral ischemia, revealed by a hydronephrosis. He had been on oral anticoagulation for six years because of two deep venous thromboses. He developed then a massive splenic infarct and an acute myocardial infarct, which required intensification of his anticoagulation, add of aspirin and high doses of corticosteroids. Three months later, he was explored for a latero-thoracic pain. Biology found an acute renal failure and microscopic haematuria. CT scan showed hypoperfusion of the left kidney, an ostial defect on renal artery and left hydronephrosis. Retrograde pyelography found a proximal ureteral stenosis. A dilatation with stenting failed, leading to a partial left ureteral resection. Histology confirmed ureteral segmental organized arterial thrombosis and venous thrombosis. Les troubles du sommeil sont fréquents chez les patients avec un trouble neurocognitif. Leur diagnostic et prise en charge chez ces patients peut s'avérer complexe en pratique clinique.Cette revue narrative offre une approche systématique basée sur les données probantes afin de diagnostiquer et traiter les troubles du sommeil concomitants à un trouble neurocognitif.La maladie d'Alzheimer est fréquemment associée aux troubles des rythmes circadiens, à l'insomnie chronique ainsi qu'au syndrome d'apnées-hypopnées du sommeil. Les alpha-synucléinopathies sont quant à elles souvent associées au trouble du comportement en sommeil paradoxal, au syndrome des jambes sans repos, à l'insomnie chronique et au syndrome d'apnées-hypopnées du sommeil. Une anamnèse ciblée permettra de diagnostiquer la majorité des troubles du sommeil. Une évaluation par polysomnographie est recommandée si un syndrome d'apnées-hypopnées du sommeil ou un trouble du comportement en sommeil paradoxal sont suspectés. Les traitements de première ligne sont souvent non-pharmacologiques, notamment la thérapie cognitivo-comportementale pour l'insomnie, les modifications de l'exposition lumineuse pour les troubles circadiens et la ventilation par pression positive continue pour le syndrome d'apnées-hypopnées obstructives du sommeil. Des recommandations pharmacologiques sont discutées, en particulier pour le trouble du comportement en sommeil paradoxal et le syndrome des jambes sans repos.Les interventions non pharmacologiques multiples et soutenues sont recommandées dans le traitement des troubles du sommeil associés aux troubles neurocognitifs. Les indications pharmacologiques demeurent actuellement restreintes, et davantage d'essais cliniques randomisés intégrant une approche multimodale sont nécessaires afin d'évaluer les traitements des troubles du sommeil spécifiques aux différents troubles neurocognitifs.Sleep disorders are prevalent in patients with a neurocognitive disorder, and diagnosis and treatment in these patients remain challenging in clinical practice.This narrative review offers a systematic approach to diagnose and treat sleep disorders in neurocognitive disorders.Alzheimer's disease is often associated with circadian rhythm disorders, chronic insomnia, and sleep apnea-hypopnea syndrome. Alpha-synucleinopathies (e.g., Parkinson's disease and Lewy body dementia) are often associated with a rapid eye movement sleep behavior disorder, restless legs syndrome, chronic insomnia, and sleep apnea-hypopnea syndrome. A focused history allows to diagnose most sleep disorders. Clinicians should ensure to gather the following information in all patients with a neurocognitive disorder: (1) the presence of difficulties falling asleep or staying asleep, (2) the impact of sleep disturbances on daily functioning (fatigue, sleepiness and other daytime consequences), and (3) abnormal movements in sleep. Sleep diaries and questionnaires can assist clinicians in screening for specific sleep disorders. Polysomnography is recommended if a rapid eye movement sleep behavior disorder or a sleep apnea-hypopnea syndrome are suspected. Sleep complaints should prompt clinicians to ensure that comorbidities interfering with sleep are properly managed. The main treatment for moderate to severe obstructive sleep apnea-hypopnea syndrome remains continuous positive airway pressure, as its efficacy has been demonstrated in patients with neurocognitive disorders. Medications should also be reviewed, and time of administration should be optimized (diuretics and stimulating medications in the morning, sedating medications in the evening). Importantly, cholinesterase inhibitors (especially donepezil) may trigger insomnia. Switching to morning dosing or to an alternative drug may help. Cognitive-behavioral therapy for insomnia is indicated to treat chronic insomnia in neurocognitive disorders. False beliefs regarding sleep should be addressed with the patient and their caregiver. The sleep environment should be optimized (decrease light exposure at night, minimize noise, avoid taking vital signs, etc.). Sleep restriction can be considered as patients with a neurocognitive disorder often spend too much time in bed. The need for naps should be assessed case by case as naps may contribute to insomnia in some patients but allow others to complete their diurnal activities. Trazodone (50 mg) may also be used under certain circumstances in chronic insomnia. Recent evidence does not support a role for exogenous melatonin in patients with a neucognitive disorder and insomnia. Patients in long-term care facilities are often deprived of an adequate diurnal exposure to light. Increasing daytime exposure to light may improve sleep and mood. Patients with circadian rhythm disorders can also benefit from light therapy (morning bright light therapy in case of phase delay and evening bright light therapy in case of phase advance). Rapid eye movement sleep behavior disorder can lead to violent behaviors, and the sleeping environment should be secured (e.g., mattress on the floor, remove surrounding objects). Medication exacerbating this disorder should be stopped if possible. High dose melatonin (6 to 18 mg) or low dose clonazepam (0.125–0.25 mg) at bedtime may be used to reduce symptoms. Melatonin is preferred in first-line as it is generally well tolerated with few side effects. Patients with restless legs syndrome should be investigated for iron deficiency. Medication decreasing dopaminergic activity should be reduced or stopped if possible. Behavioral strategies such as exercise and leg massages may be beneficial. Low-dose dopamine agonists (such as pramipexole 0.125 mg two hours before bedtime) can be used to treat the condition, but a prolonged treatment may paradoxically worsen the symptoms. Alpha-2-delta calcium channel ligands can also be used while monitoring for the risk of falls.Multiple and sustained nonpharmacological approaches are recommended for the treatment of sleep disturbances in patients with neurocognitive disorder. Pharmacological indications remain limited, and further randomized clinical trials integrating a multimodal approach are warranted to evaluate the treatment of sleep disorders in specific neurocognitive disorders.
The purpose of this study was to define the risk of progression and survival of patients with smoldering Waldenström macroglobulinemia (SWM). SWM is defined clinically as having a serum monoclonal IgM protein ≥ 3 g/dL and/or ≥ 10% bone marrow lymphoplasmacytic infiltration but no evidence of end-organ damage (anemia, constitutional symptoms, hyperviscosity, lymphadenopathy, or hepatosplenomegaly). We searched a computerized database and reviewed the medical records of all patients at Mayo Clinic who fulfilled the criteria of SWM between 1974 and 1995. During 285 cumulative person-years of follow-up of the 48 patients with SWM (median, 15.4 years), 34 (71%) progressed to symptomatic Waldenström macroglobulinemia (WM) requiring treatment, one to primary amyloidosis, and one to lymphoma (total, 75%). The cumulative probability of progression to symptomatic WM, amyloidosis, or lymphoma was 6% at 1 year, 39% at 3 years, 59% at 5 years, and 68% at 10 years. The major risk factors for progression were percentage of lymphoplasmacytic cells in the bone marrow, size of the serum M-spike, and the hemoglobin value. Patients with SWM should be followed and not treated until symptomatic WM develops. Treatment on a clinical trial for those at greatest risk of progression should be considered.
La syphilis est une infection bactérienne en recrudescence en France. La neurosyphilis en est une forme rare, se traduisant principalement par une atteinte méningée et vasculaire. Elle peut être une cause d'accident vasculaire ischémique. Les thromboses veineuses d'origine syphilitiques sont rarement décrites.Nous rapportons le cas d'un patient de cinquante ans hospitalisé pour baisse d'acuité visuelle bilatérale accompagnée de céphalées. Le scanner cérébral montrait une thrombose veineuse cérébrale du sinus latéral droit jusqu'au golf jugulaire. Il présentait un œdème papillaire bilatéral au fond d'œil. La ponction lombaire montrait une méningite lymphocytaire avec une sérologie sanguine et du liquide cérébro-spinal (LCS) évocatrices d'une neurosyphilis. Le patient a été traité avec une antibiothérapie par pénicilline G pendant quatorze jours et une anticoagulation curative durant trois mois. L'évolution était favorable.La thrombose veineuse cérébrale lors d'une neurosyphilis est une entité exceptionnellement décrite. Le cas rapporté ici confirme le statut de grande simulatrice de la syphilis. Dans le contexte de sa recrudescence mondiale, la syphilis doit être évoquée devant un trouble neurologique inexpliqué.Syphilis is a bacterial infection which is increasing in France. Neurosyphilis is a rare manifestation of syphilis, mainly involving the meninges and the blood vessels. It is a rare cause of cerebral vascular stroke. Venous thrombosis of syphilitic origin is rarely described.We reported a case of a fifty-year-old patient hospitalized for bilateral non-painful decreased visual acuity with headache. The CT scan showed cerebral venous thrombosis from the right lateral sinus to the jugular gulf. Patient presented a bilateral papillar oedema on the ocular fundus. Lumbar puncture showed lymphocytic meningitis with blood and CSF serology suggestive of neurosyphilis. The patient received antibiotic therapy with penicillin G for 14 days with curative anticoagulation for six months. The evolution was favorable.Cerebral venous thrombosis in neurosyphilis is a poorly described entity. This case report confirms the status of great simulator of syphilis. In the context of its worldwide recrudescence, syphilis must be evoked in front of an unexplained neurological disorder.
Le traitement par l'lmatinib (Glivec®), inhibiteur de la tyrosine kinase BCR-ABL a transformé le pronostic de la leucémie myéldïde chronique (LMC) au cours des dernières années, permettant d'obtenir des taux de rémission complète cytogénétique de plus de 80 %. L'lmatinib, à la dose de 400 mg/j, est maintenant le traitement en première ligne de la LMC pour tousles patients. La réponse au traitement est évaluée dans un premier temps sur les données hématologiques et cytogénétiques, puis très rapidement la surveillance est principalement moléculaire avec quantification du transcrit BCR-ABL par RQ-PCR tous les trois mois. Un panel d'experts de l'European leukemia net a récemment défini les critères de la réponse optimale, suboptimale et de I'echec. Malgré les pourcentages très élevés de réponse cytogénétique compléte, la maladie résiduelle reste détectable chez la plupart des patients laissant supposer qu'il existe un réservoir de cellules souches leucémiques. La réponse à l'lmatinib est stable chez la plupart des patients, mais quelques uns développent une intolérance ou une résistance à l'lmatinib. La principale cause de ces résistances est représentée par des mutations dans le domaine kinase de BCR-ABL, mutations qui perturbent la fixation de l'lmatinib. Le Dasatinib et le Nilotinib permettent de restaurer une réponse hématologique et cytogénétique chez la plupart des patients présentant une résistance acquise ou un échec primaire. De nouvelles molécules sont actuellement testées chez les patients présentant une résistance au Dasatinib ou au Nilotinib. La thérapeutique ciblée de la leucémie myéloïde chronique a totalement transformé le pronostic, mais a rendu plus complexe la prise en charge thérapeutique et la surveillance biologique. Un suivi moléculaire trés rigoureux est indispensable, ainsi que le recours à des hématologistes expérimentes pour la prise en charge du traitement des résistances.The introduction of Imatinib (Gleevec), tyrosine kinase inhibitor, has revolutionized the drug therapy of chronic myeloid leukemia over the last few years with rates of complete cytogenetic remission of more than 80 % in newly diagnosed patients. First line treatment is now Imatinib 400 mg daily. Response is monitored, hematologically, cytogenetically and molecularly (with RQ-PCR every 3 months). Failure, optimal and suboptimal responses have been defined by the expert panel of the European Leukemia Net. Despite of high rates of complete cytogenetic remission residual disease remains detectable in the majority of patients, suggesting that they have reservoir of leukemic stem cells from which relapse may occur. Most of the responses to Imatinib are stable but some patients become intolerant or resistant to Imatinib. Mutations of the kinase domain of BCR-ABL, that impair Imatinib binding have been described as the leading cause of resistance. Dasatinib and Nilotinib restore hematological and cytogenetical remission in most of the patients with acquired resistance or primary failure. New drugs are tested in patients resistant to Dasatinib and Nilotinib. Treatment of chronic myeloid leukemia in an example of targeted therapy which requires specialized professionals. Follow up and decisions have to be taken with the assistance of an experimented centre.
Le diagnostic étiologique d'une anémie chronique est habituellement simple et standardisé, mettant en évidence le plus souvent un déficit en fer, en vitamines B9 ou B12, une insuffisance rénale chronique sévère, une maladie endocrinienne, un syndrome inflammatoire ou une atteinte médullaire. Parfois aucune cause n'est retrouvée. Les résultats du bilan biologique initial peuvent être mis en défaut en raison d'une méconnaissance des nuances analytiques, ceci impliquant parfois des erreurs dans le diagnostic d'orientation, source d'explorations inutiles. Le but de cette mise au point est de montrer un regard critique sur les résultats des différentes analyses initiales et de rappeler certaines étiologies d'anémies « non hémolytiques » plus rares qu'il faut savoir parfois rechercher.The most potential causes of "non hemolytic" anemias are iron, folate or vitamin B12 deficiencies, severe renal impairment, endocrine diseases, inflammation and medullary disorders. In a non-exceptionnal way no cause is found, sometimes because of a wrong interpretation of analysis results and sometimes because of a little known etiology. The goal of this review is to point out analytical difficulties and to remember some rarer etiologies.
