Aims Early neoplasia arising from Barrett esophagus (BE) can be curatively treated by endoscopic resection. The choice of the resection technique – endoscopic mucosal resection (EMR) or submucosal dissection (ESD) – largely depends on the assumed infiltration depth of the lesion and hence on preprocedural judgment by the endoscopist. To clarify the accuracy of the endoscopic assessment and treatment allocation, we performed a study showing endoscopic photographs to BE experts simulating a second opinion procedure.
Aims Endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) of solid extraintestinal lesions is an established standard procedure in daily routine. The goal of this study was to investigate how representative the obtained material is and how often complications occur.
Aims ERCP in children older than 3 years and adolescents is a common procedure for detecting and treating congenital or acquired biliopancreatic disorders. For infants younger than 3 months ERCP represents a complex and challenging technique to diagnose or treat rare, mostly congenital diseases. Still there is poor data regarding diagnostic yield, therapeutic outcome and above all the savety of ERCP in infants younger than 3 months.
Ziele Bei der Durchführung einer real-time-CADx-Studie wurde bei einem Teil der Polypen, die von der KI und/oder einem Endoskopiker als Adenom erkannt wurden, eine negative histopathologische Diagnose gestellt. In der vorliegenden Studie stellten wir uns die Frage, ob kleine adenomatöse Läsionen in tieferen Schnitten innerhalb des verbleibenden eingebetteten Gewebes gefunden werden können.
Background Computer-aided detection (CADe) systems could assist endoscopists in detecting early neoplasia in Barrett's oesophagus, which could be difficult to detect in endoscopic images. The aim of this study was to develop, test, and benchmark a CADe system for early neoplasia in Barrett's oesophagus. Methods The CADe system was first pretrained with Image Net followed by domain-specific pretraining with GastroNet. We trained the CADe system on a dataset of 14 046 images (2506 patients) of confirmed Barrett's oesophagus neoplasia and non-dysplastic Barrett's oesophagus from 15 centres. Neoplasia was delineated by 14 Barrett's oesophagus experts for all datasets. We tested the performance of the CADe system on two independent test sets. The all-comers test set comprised 327 (73 patients) non-dysplastic Barrett's oesophagus images, 82 (46 patients) neoplastic images, 180 (66 of the same patients) non-dysplastic Barrett's oesophagus videos, and 71 (45 of the same patients) neoplastic videos. The benchmarking test set comprised 100 (50 patients) neoplastic images, 300 (125 patients) non-dysplastic images, 47 (47 of the same patients) neoplastic videos, and 141 (82 of the same patients) non-dysplastic videos, and was enriched with subtle neoplasia cases. The benchmarking test set was evaluated by 112 endoscopists from six countries (first without CADe and, after 6 weeks, with CADe) and by 28 external international Barrett's oesophagus experts. The primary outcome was the sensitivity of Barrett's neoplasia detection by general endoscopists without CADe assistance versus with CADe assistance on the benchmarking test set. We compared sensitivity using a mixed-effects logistic regression model with conditional odds ratios (ORs; likelihood profile 95% CIs). Findings Sensitivity for neoplasia detection among endoscopists increased from 74% to 88% with CADe assistance (OR 204; 95% CI 173-242; p<00001 for images and from 67% to 79% [235; 190-294; p<00001] for video) without compromising specificity (from 89% to 90% [107; 096-119; p=020] for images and from 96% to 94% [094; 079-111; ] for video; p=046). In the all-comers test set, CADe detected neoplastic lesions in 95% (88-98) of images and 97% (90-99) of videos. In the benchmarking test set, the CADe system was superior to endoscopists in detecting neoplasia (90% vs 74% [OR 375; 95% CI 193-805; p=00002] for images and 91% vs 67% [1168; 385-4753; p<00001] for video) and non-inferior to Barrett's oesophagus experts (90% vs 87% [OR 174; 95% CI 083-365] for images and 91% vs 86% [294; 099-1140] for video). Interpretation CADe outperformed endoscopists in detecting Barrett's oesophagus neoplasia and, when used as an assistive tool, it improved their detection rate. CADe detected virtually all neoplasia in a test set of consecutive cases.
Aims While performing a realtime CADx study a negative histopathological diagnosis was obtained in a portion of specimens from lesions detected as adenoma by CADx and/or endoscopists. In the present study, we asked, if small adenomatous lesion can be found in the deeper section within the remaining embedded tissue.
Einleitung Die Entwicklung künstlicher Intelligenzsysteme (KI) zur Weiterentwicklung der GI Endoskopie läuft mit Hochdruck. Seit kurzer Zeit sind Systeme zur Detektion und Charakterisierung von Kolonpolypen kommerziell erhältlich. Die Datenlage aus den ersten Bild- und Videostudien ist vielversprechend. Allerdings gibt es bisher kaum Studien aus der klinischen Versorgungsrealität. In dieser Studie soll die Genauigkeit eines kommerziell erhältlichen KI-Systems zur Polypencharakterisierung evaluiert werden.

Aims The adenoma detection rate (ADR) varies significantly between different endoscopists leading to an adenoma miss rate (AMR) of up to 26 %. To improve endoscopic quality and to reduce the rate of interval cancer artificial intelligence (AI) systems can be valuable. We evaluated the efficacy of an AI system in real time colonoscopy and its influence on the AMR and the ADR in a clinical setting.

Für den Gastrointestinaltrakt wurden bisher keine systematischen Untersuchungen zum Einfluss des Geschlechts auf den Verlauf von malignen Erkrankungen durchgeführt und das „Geschlecht“ war bisher meist kein Stratifikationskriterium in Studien. Es ist jedoch unumstritten, dass sich die gastrointestinalen Tumoren bezüglich Risikofaktoren, Inzidenz und Prognose geschlechtsspezifisch unterscheiden. Diese Übersichtsarbeit fasst die wichtigsten Kenntnisse zu epidemiologischen Faktoren, pathologischen Grundlagen, Prognose, Rezidivmustern und unterschiedlichem Ansprechen auf multimodale Therapiekonzepte aus der Sicht der Gendermedizin zusammen. Frauen haben häufiger nichtintestinale Tumoren mit niedriger Differenzierung und Siegelringzellen. Zum Ansprechen auf eine Chemotherapie liegen keine konklusiven Daten vor, allerdings zeigt sich ein Trend Richtung schlechteres Ansprechen des weiblichen Geschlechts sowie eines häufigeren Auftretens einer Peritonealkarzinose als Rezidivlokalisation. Trotzdem haben Frauen mit Magen- und Ösophaguskarzinom analog zu weiteren Tumorentitäten in epidemiologischen Daten eine bessere Prognose. Insgesamt bleiben viele Fragen offen. Die systematische Aufarbeitung des Geschlechteraspekts bei gastrointestinalen Tumoren bietet aber viele interessante Ansatzpunkte und sollte in zukünftige Studien eingebunden werden.
Der hepatische Hydrothorax ist nicht selten schwierig zu diagnostizieren, da meist kaum Ascites vorliegt und die „neben“ dem großen Pleuraerguss vorliegende Leberzirrhose fälschlicherweise kompensiert erscheint. Die Kontrastmittesonografie ist geeignet, die Diagnose rasch und einfach zu sichern.
To in vivo predict colorectal polyp histology, the BASIC classification incorporates both morphological and pit/vessel descriptors domains. To define the new BASIC classes for adenomatous and hyperplastic polyps, we analytically assessed the strength of predictions of these descriptors individually and in combination.
Zusammenfassung Rektale Blutungen sind ein häufiges Symptom in der Proktologie. Meist können durch strukturierte Anamnese und proktologische Basisdiagnostik häufige Ursachen wie die Analfissur oder das Hämorrhoidalleiden diagnostiziert und behandelt werden. Nicht selten findet sich jedoch eine Proktitis als Ursache rektaler Blutungen, die eine interdisziplinäre Diagnostik und Therapie erfordert. Neben den mit einer chronisch-entzündlichen Darmerkrankung assoziierten Proktitiden stellen mechanisch induzierte, radiogene, ischämische, infektiöse oder venerische Proktitiden wichtige Differenzialdiagnosen dar. Schließlich muss bei der rektalen Blutung ein Karzinom als Ursache ausgeschlossen werden. Letztlich sind die meisten Ursachen der rektalen Blutung bei entsprechender Sorgfalt sicher zu diagnostizieren und effektiv zu behandeln, jedoch können spezielle Umstände eine interdisziplinäre Diagnostik und Therapie – konservativ, topisch, interventionell oder chirurgisch – notwendig machen.
BACKGROUND & AIMSEndoscopic detection of early Barrett's esophagus-related neoplasia (BORN) is a challenge. We aimed to develop a web-based teaching tool for improving detection and delineation of BORN.METHODSWe made high-definition digital videos during endoscopies of patients with BORN and non-dysplastic Barrett's esophagus. Three experts superimposed their delineations of BORN lesions on the videos using special tools. In phase one, 68 general endoscopists from 4 countries assessed 4 batches of 20 videos. After each batch, mandatory feedback compared the assessors' interpretations with those from experts. These data informed the selection of 25 videos for the phase 2 module, which was completed by 121 new assessors from 5 countries. A 5-video test batch was completed before and after scoring of the four 5-video training batches. Mandatory feedback was as in phase 1. Outcome measures were scores for detection, delineation, agreement delineation, and relative delineation of BORN.RESULTSA linear mixed-effect model showed significant sequential improvement for all 4 outcomes over successive training batches in both phases. In phase 2, median detection rates of BORN in the test batch increased by 30% (P < .001) after training. From baseline to the end of the study, there were relative increases in scores of 46% for detection, 129% for delineation, 105% for agreement delineation, and 106% for relative delineation (all, P < .001). Scores improved independent of assessors' country of origin or level of endoscopic experience.CONCLUSIONSWe developed a web-based teaching tool for endoscopic recognition of BORN that is easily accessible, efficient, and increases detection and delineation of neoplastic lesions. Widespread use of this tool might improve management of Barrett's esophagus by general endoscopists.
