Die akute, aber auch die chronische Herzinsuffizienz ist weiterhin mit einer hohen Morbidität und Mortalität verbunden; dekompensierte Herzinsuffizienz ist der häufigste Grund für eine Krankenhausaufnahme in Deutschland. Bereits 2018 hat die Deutsche Gesellschaft für Kardiologie und Kreislaufforschung (DGK) das Curriculum Herzinsuffizienz erarbeitet, welches jetzt eine Überarbeitung erfahren hat. Das Curriculum Herzinsuffizienz führt zur Vertiefung kardiologischen Wissens und der technischen Fertigkeiten im Bereich der Herzinsuffizienz sowie zur Erlangung einer diesbezüglichen Zusatzqualifikation innerhalb der Kardiologie. Die erhebliche Erweiterung der diagnostischen und therapeutischen Möglichkeiten für herzinsuffiziente Patienten erfordert detaillierte Kenntnisse und Fertigkeiten. Die Einrichtung spezialisierter Ambulanzen und Stationen (Heart Failure Units) in vielen Kliniken und Schwerpunktpraxen hat dem bereits Rechnung getragen. Das Curriculum „Herzinsuffizienz“ definiert den Rahmen, innerhalb dessen interessierte Kollegen vertiefte Kenntnisse und praktische Fähigkeiten auf dem Gebiet der Herzinsuffizienz erwerben können und diese auch von der DGK attestiert bekommen. Das Curriculum ist modular aufgebaut (1. Jahr: Grundkenntnisse, 2. Jahr: Spezialisierung) und erlaubt damit unterschiedliche Schwerpunktsetzungen. Im Rahmen der Spezialisierung sind mindestens 2 aus 4 Modulen für jeweils 6 Monate abzuleisten: fortgeschrittene Herzinsuffizienz inklusive ventrikuläre Unterstützungssysteme/Transplantation; Device-Therapie bei Herzinsuffizienz; interventionelle Herzinsuffizienztherapie; ambulante Versorgung/Rehabilitation/Psychokardiologie.
To determine whether cardiac sympathetic nervous dysfunction is present, in this single center prospective, non-randomized trial non-invasive SPECT/CT imaging using the radiotracer 123 I-metaiodobenzylguanidine was performed in 33 recovered COVID-19 patients without pre-existing cardiac conditions. Increased cardiac sympathetic activity, as indicated by late HMR, was observed in 67.7% of patients. At 6–8 months, 82% of these subjects (27/33) received follow-up, and cardiac sympathetic innervation abnormalities were still present in 70.4% (19/27). Additionally, at 12–15 months post-diagnosis, persistently abnormal HMRs were found in 9 individuals who initially had abnormal sympathetic innervation. Further follow-up is needed to investigate potential long-term cardiovascular consequences of COVID-19.
Abstract Aim Patients with severe mitral regurgitation (MR) and acute heart failure (HF) have refractory symptoms without adequate response to medical therapy. The objective of this analysis was to assess the impact of the MitraClip device in acutely ill HF patients, characterized by NYHA Class IV at baseline, in a real‐world, contemporary setting. Methods and results EXPAND was a prospective, multicenter, international study enrolling patients with MR who consented to receive the MitraClip System at 57 sites globally. The study outcomes included acute procedural success (APS), quality of life, heart failure hospitalizations (HFH), and all‐cause mortality. The study population comprised 1,041 patients, with 118 patients having baseline NYHA Class IV, and 922 having baseline NYHA Class I/II/III. NYHA Class IV patients had a significantly higher rate of baseline co‐morbidities and secondary MR aetiology compared with NYHA Class I/II/III patients. APS was achieved in 92.4% of NYHA Class IV patients and significant improvement in MR grade to ≤Mild (1+) in 90.7% of subjects at 30 days and 92.9% at 1 year was observed. 1‐year‐mortality was higher in the NYHA Class IV subjects compared with the NYHA Class I/II/III subjects (29.2% vs. 17.7%, P < 0.01). Significant improvement in functional capacity assessed by NYHA Functional Class and Quality of Life assessed through KCCQ score was observed. At 1 year, 72.6% of NYHA Class IV subjects improved to NYHA Class I/II and ΔKCCQ was 31.2 (24.1, 38.3) compared with baseline. Conclusion In the prospective, real‐world EXPAND study, MitraClip in patients with severe MR and NYHA Class IV was found to be safe and effective in treating MR, and significantly improving QoL and long‐term clinical outcomes.
The high urgency (HU) criteria for heart transplantation published in 2004 have been more and more criticized due to the lack of evidence and too much room for interpretation. As a first step the criteria for patients with cardiac hypoperfusion syndrome and proven inotrope medication dependency were published on 19 December 2019 in harmonization with the organ committee of Eurotransplant. The deadlines for re-evaluation were extended to 4 and 8 weeks; however, the centers must perform an internal re-evaluation of the prerequisites for the HU status every 14 days with the obligation to downgrade the waiting list status if the HU criteria are no longer fulfilled. A further change is the accumulation of HU waiting times. In a second step the HU criteria for patients with complications on durable mechanical circulatory support (dMCS) were more strictly defined. These guideline changes came into force on 7 September 2021. Further HU criteria for special cases, e.g. congenital heart disease or uncontrollable life threatening ventricular arrhythmia etc. were published simultaneously.
Die 2004 eingeführten Kriterien für die Aufnahme eines Patienten auf der Hochdringlichkeits(„High-urgency“-, HU-)Warteliste für eine Herztransplantation waren aufgrund fehlender zugrunde liegender Evidenzen sowie zu großem Interpretationsspielraum zunehmend kritisiert worden. Als erster Schritt wurden am 19.12.2019 in Abstimmung mit Eurotransplant die HU-Kriterien für Patienten mit kardialem Hypoperfusionssyndrom und nachgewiesener Abhängigkeit von inotropen Medikamenten publiziert. Zeitgleich wurden die Fristen für die Reevaluierung auf 4 bzw. 8 Wochen verlängert. Zentrumsintern müssen die Voraussetzungen für den HU-Status alle 14 Tage überprüft und der Wartelistenstatus ggf. zurückgestuft werden. Als weitere Änderung werden HU-Wartezeiten nun akkumuliert. Im zweiten Schritt wurden die Kriterien für Patienten mit Komplikationen am dauerhaften Kreislaufunterstützungssystem („durable mechanical circulatory support“, dMCS) angepasst. Hier wurden die einzelnen Kriterien wesentlich deutlicher definiert. Diese Richtlinienänderung trat zum 07.09.2021 in Kraft. Es wurden gleichzeitig HU-Kriterien für Spezialfälle veröffentlicht, wie z. B. angeborene Herzfehler oder unkontrollierbare ventrikuläre Arrhythmien etc.
Mitral and tricuspid regurgitation are highly prevalent types of valvular heart disease, important drivers of a poor prognosis and subject to catheter-based treatment. The transcatheter reconstruction of the valves has meanwhile evolved as an important alternative to pure medicinal treatment and surgical interventions. Whereas transcatheter treatment of tricuspid valve insufficiency has recently shown effectiveness and safety in the first registries, transcatheter treatment of mitral valve insufficiency has meanwhile been tested in several randomized trials. This paper comments on the results of these trials and presents recommendations regarding the indications for the interventional treatment modalities available for mitral and tricuspid valve regurgitation. As part of this manuscript and in an effort to further improve the quality of care, the German Cardiac Society has updated the criteria for the certification of centers for transcatheter treatment of mitral valve insufficiency.

The Cardioband system enables percutaneous surgical-like direct mitral valve annuloplasty and, thereby, repair of severe functional mitral valve regurgitation (MR) in patients with advanced systolic heart failure (HF) and dilation of the left ventricular (LV) annulus. Since the device is anchored by screws in the LV annulus, limited myocardial injury is likely to occur.
Es wird über den Fall einer Kardiomyopathie bei einem 22-jährigen Patienten mit langjährigem Methamphetaminkonsum berichtet. Echokardiographisch zeigte sich eine dilatative Kardiomyopathie mit hochgradig eingeschränkter Pumpfunktion und hochgradiger Mitralklappeninsuffizienz. Durch Inotropika und Implantation zweier MitraClips® (Abbott Vascular, Santa Clara, CA, USA) konnte eine hämodynamische Stabilisierung erreicht werden. Die zunehmende Verbreitung des Methamphetaminkonsums sollte Anlass geben, in Fällen mit unklarer Kardiomyopathie bei jungen Patienten eine gezielte Anamnese und gegebenenfalls Urinanalyse durchzuführen.
Die eosinophile Myokarditis ist eine seltene, schwierig zu diagnostizierende, aber gut therapierbare Erkrankung. Das klinische Bild reicht von asymptomatischen bis hin zu lebensbedrohlichen Verläufen. Wir berichten über eine 44-jährige Patientin mit Schwindel, Brustschmerzen und Dyspnoe. Auffällig waren ein intermittierender atrioventrikulärer Block II° Typ Mobitz II° und eine gering reduzierte systolische linksventrikuläre Funktion. In Zusammenschau von Hypereosinophilie, kardialer Magnetresonanztomographie und Endomyokardbiopsie diagnostizierten wir eine eosinophile Myokarditis bei allergischer Reaktion auf Aspergillus fumigatus. Zwar fehlen prospektive Studien und standardisierte Therapieempfehlungen, doch gilt eine frühzeitige, hoch dosierte und mehrmonatige Steroidgabe als effektive Therapie der eosinophilen Myokarditis. Mit der vorliegenden Kasuistik möchten wir auf diese seltene Myokarditisform aufmerksam machen und zum raschen Erkennen und Behandeln dieser unterdiagnostizierten, tückischen Erkrankung beitragen.
The prognosis of symptomatic cardiac amyloidosis - either of light chain (AL) or transthyretin (ATTR) type - is poor. Heart transplantation (HTX) might improve prognosis, but, previous reports suggested unfavorable survival after HTX in this population compared to HTX recipients with other indications as ischemic or dilative heart disease. Here, we analyzed our single center experience regarding HTX for cardiac amyloidosis.
Left ventricular assist devices (LVAD) are an established pathway to treat severe heart failure. However, right heart failure (RHF) remains the major cause of early death post LVAD surgery. Temporary right ventricular assist support (RVAD) might prevent early RHF. Nevertheless, delayed implantation of temporary RVAD results in worse outcome. Conventional temporary RVAD strategies require additional surgery. We aimed to use the TandemHeart (TH) RVAD with the completely percutaneous implantation of the ProtekDuo dual lumen cannula via the jugular vein simultaneous to LVAD implantation. This approach allows for prevention of RHF without the need of re-thoracotomy for RVAD explantation.

Trotz jüngster Erfolge in der Therapie nimmt die Prävalenz weiter zu, und eine Herzinsuffizienz ist mittlerweile der häufigste Grund für eine Krankenhausaufnahme in Deutschland. Gleichzeitig hat sich das Spektrum der diagnostischen und therapeutischen Möglichkeiten in den letzten 2 Dekaden erheblich erweitert. Entsprechend sind die erforderlichen Kenntnisse und Fertigkeiten für eine State-of-the-art-Therapie gestiegen. Dieser Entwicklung wurde in vielen Kliniken und Schwerpunktpraxen bereits Rechnung getragen, u. a. durch die Einrichtung spezialisierter Ambulanzen und Stationen (Heart Failure Units). Das Curriculum „Herzinsuffizienz“, welches auf einem Vorschlag der Heart Failure Association (HFA) der Europäischen Gesellschaft für Kardiologie aufbaut, soll es Kardiologen ermöglichen, ihre Kenntnisse und praktischen Fähigkeiten auf dem Gebiet der Herzinsuffizienz zu vertiefen. Das Curriculum ist modular aufgebaut (1. Jahr: Grundkenntnisse, 2. Jahr: Spezialisierung) und erlaubt damit unterschiedliche Schwerpunksetzungen. Ziel dieses Curriculums ist neben der individuellen Qualifizierung eine höhere Qualität der Versorgung der Patienten mit Herzinsuffizienz.
AIMS:Impaired left ventricular diastolic function leading to elevated left atrial pressures, particularly during exertion, is a key driver of symptoms and outcomes in heart failure with preserved ejection fraction (HFpEF). Insertion of an interatrial shunt device (IASD) to reduce left atrial pressure in HFpEF has been shown to be associated with short-term haemodynamic and symptomatic benefit. We aimed to investigate the potential effects of IASD placement on HFpEF survival and heart failure hospitalization (HFH).METHODS AND RESULTS:Heart failure with preserved ejection fraction patients participating in the Reduce Elevated Left Atrial Pressure in Patients with Heart Failure study (Corvia Medical) of an IASD were followed for a median duration of 739 days. The theoretical impact of IASD implantation on HFpEF mortality was investigated by comparing the observed survival of the study cohort with the survival predicted from baseline data using the Meta-analysis Global Group in Chronic Heart Failure heart failure risk survival score. Baseline and post-IASD implant parameters associated with HFH were also investigated. Based upon the individual baseline demographic and cardiovascular profile of the study cohort, the Meta-analysis Global Group in Chronic Heart Failure score-predicted mortality was 10.2/100 pt years. The observed mortality rate of the IASD-treated cohort was 3.4/100 pt years, representing a 33% lower rate (P = 0.02). By Kaplan-Meier analysis, the observed survival in IASD patients was greater than predicted (P = 0.014). Baseline parameters were not predictive of future HFH events; however, poorer exercise tolerance and a higher workload-corrected exercise pulmonary capillary wedge pressure at the 6 months post-IASD study were associated with HFH.CONCLUSIONS:The current study suggests IASD implantation may be associated with a reduction in mortality in HFpEF. Large-scale ongoing randomized studies are required to confirm the potential benefit of this therapy.

Eine Herztransplantation (HTX) kann eine nachfolgende kausale Therapie der Amyloidose ermöglichen und damit möglicherweise die Prognose verbessern, die Datenlage hierzu ist aufgrund bisher nur kleiner Kohortenstudien und Fallberichten unzulänglich. In der vorliegenden Arbeit wurden die Daten aus dem Amyloidosezentrum des Universitätsklinikums Heidelberg zwischen 2001 und 2017 bezüglich der herztransplantierten Patienten analysiert.
