Spinal cord lesions have a real diagnostic and prognostic role in multiple sclerosis. Thus, optimizing their detection on MR imaging has become a central issue with direct therapeutic impact. In this study, we compared the 3D-MP2RAGE sequence with the conventional Magnetic Resonance Imaging in Multiple Sclerosis (MAGNIMS) set for cervical cord lesion detection in 28 patients with multiple sclerosis. 3D-MP2RAGE allowed better detection of cervical lesions (+62%) in this population, with better confidence, due to optimized contrast and high spatial resolution.
L’encéphalite/encéphalopathie avec lésion réversible du splénium du corps calleux (SCC) aussi appelé MERS est un syndrome clinico-radiologique rare. Les patients présentent habituellement des symptômes neurologiques centraux modérés. Il existe deux types de MERS selon la topographie lésionnelle. Le type I est une lésion unique atteignant la ligne médiane du SCC. Dans le type II, il y a une atteinte bilatérale et symétrique de la substance blanche des centres semi ovale ou une extension à la partie antérieure du corps calleux (CC) [1]. Les lésions se présentent sous la forme d’un œdème cytotoxique réversible [2]. La récupération clinico-radiologique complète survient en moins d’un mois, sans séquelle [3]. Dans une revue de la littérature récente de 2017, seulement 29 cas adultes ont été recensés [4]. L’objectif de notre étude était de rapporter notre expérience locorégionale sur les résultats et l’évolution clinique, biologique et radiologique des patients présentant un MERS. Tous les patients présentant un symptôme neurologique d’apparition récente associé à une lésion réversible du SCC de juillet 2011 à janvier 2018 au sein des CHU de Marseille et Nice étaient inclus. Les caractéristiques cliniques, biologiques incluant la ponction lombaire (PL), et radiologiques étaient recueillies. Les patients devaient avoir eu des IRM en séquences pondérées T1 sans et avec injection de produit de contraste, T2, T2 FLAIR, diffusion. Les lésions du SCC étaient classées en 3 sous type : (1) une petite lésion arrondie ou ovale au centre du SCC, (2) une lésion du centre du SCC s’étendant latéralement à la substance blanche (SB) adjacente via les fibres calleuses, (3) une lésion postérieure du SCC, s’étendant à la portion antérieure du CC. Le type de MERS étaient également recueillis. La lésion devait présenter une restriction de diffusion transitoire et avoir complètement régressé sur l’imagerie de suivi disponible. Douze patients d’un âge moyen de 28,4 ans ont été inclus, avec un seul cas pédiatrique. Les étiologies étaient infectieuses chez 11 patients et chez le dernier, l’étiologie retenue était celle des traitements anti épileptique. En dehors de la fièvre, le signe clinique prédominant était le trouble de la vigilance chez 8 patients. Les 8 PL réalisées ont montré 6 méningites lymphocytaires, 1 méningite purulente et une était normale. Les délais de régression complète des symptômes étaient variables avec un délai moyen de 14,6 jours. En imagerie, les 3 sous types de lésions du SCC ont été retrouvés avec 3 types 1, 7 types 2 et 1 type 3 (Fig. 1). Un patient a présenté initialement une lésion de type 1 qui a secondairement évolué en type 3 (Fig. 2). Il y avait 8 MERS de type I et 4 de type 2. Toutes les lésions étaient en hyper signal T2, FLAIR, diffusion avec chute de l’ADC, iso ou discret hypo signal T1 sans rehaussement après injection de produit de contraste. Elles étaient réversibles sur l’imagerie de suivi disponible dans un délai moyen de 105,6 jours avec une médiane 26 j. Le MERS est un syndrome relativement rare avec une présentation clinique et radiologique typique. De multiples pathologies peuvent en être à l’origine avec dans tous les cas une évolution neurologique excellente. L’IRM est la modalité de choix pour le diagnostic initial et le suivi. La sémiologie radiologique du MERS et le spectre des étiologies doivent être connues des radiologues afin d’en informer les cliniciens et éviter les traitements et investigations inappropriés.
BACKGROUND:Phlebologic diseases have become extremely common and have major socio-economic impact. However, the percentage of dermatologists working in phlebology appears to be decreasing according to the data of the German Society of Phlebology (DGP).METHODS:To investigate the reasons for this development, we--on behalf of the DGP--sent a questionnaire to 120 German Departments of Dermatology in autumn 2012.RESULTS:In 76 returned questionnaires, the number of physicians with additional fellowship training in phlebology averaged 1.5; the average number of those who fulfill the criteria for training fellows in phlebology was 0.9. In 71.1 % of the departments there was a phlebologist. A special phlebologic outpatient clinic existed in 73.7 % of the departments. Sonography with Doppler (89.5 %) and duplex (86.8 %) was used as the most frequent diagnostic tool. For therapy, compression (94.7 %), sclerotherapy (liquid 78.9 %, foam 63.2 %, catheter 18.4 %), endoluminal thermic procedures (radio wave 28.9 %, laser 17.1 %) and surgery (especially crossectomy and stripping 67.1 %, phlebectomy of tributaries 75 %) were used. The average number of treatments was very heterogenous in the different departments.CONCLUSIONS:Phlebology definitely plays an important role in dermatology. Most departments fulfill the formal criteria for the license to conduct advanced training in phlebology. A wide spectrum of phlebological diagnostic and therapeutic procedures is available.
Les céphalées dites circonstancielles « à la toux, à l'effort et à l'activité sexuelle » sont incluses par l'International Headache Society dans le groupe 4 des « autres céphalées primaires ». Notre travail a pour but de réaliser une étude clinique et neuroradiologique chez les patients présentant ce type de céphalées circonstancielles afin d'essayer d'avancer dans la compréhension des mécanismes physiopathologiques de ces céphalées. Il s'agit d'une étude nationale multicentrique menée au cours de laquelle les patients ont été évalués cliniquement par un même questionnaire et ont été explorés avec une IRM cérébrale et une véno IRM. Cette dernière a été réalisée selon des séquences préétablies et interprétées de manière consensuelle par deux neuroradiologues séniors, aveugle vis-à-vis du diagnostic clinique des sujets. Une VRM a été également réalisée au sein d'un groupe de patients « témoins » non céphalalgiques venant pour un dépistage ou un bilan d'extension. Quarante et un patients présentant une céphalée circonstancielle ont été recrutés au sein de six centres français : 20 patients avec une céphalée liée à l'activité sexuelle, 11 avec une céphalée à l'effort et 10 patients avec une céphalée à la toux. Les patients présentant une céphalée à la toux ont une moyenne d'âge plus élevée, une comorbidité avec la migraine moins fréquente que dans les céphalées liées à l'effort ou à l'activité sexuelle. Les céphalées d'effort et liées à l'activité sexuelle ont une présentation clinique semblable caractérisée par une céphalée de durée plus longue que celle observée lors de la céphalée à la toux. L'association avec la maladie migraineuse est plus fréquente. Sur le plan neuroradiologique, la VRM révèle des modifications du système veineux situées soit sur les sinus transverses, soit sur les jugulaires. Dans la population céphalée à la toux, un seul patient a un système veineux normal ou avec des modifications de calibre < 50%. Dans le groupe des céphalées à l'effort les patients ne semblent pas avoir de lésions jugulaires significatives, mais ce segment n'a pas été exploré de façon constante en fonction des centres. Enfin, les patients présentant des céphalées liées à l'activité sexuelle ont des sténoses veineuses à la fois sur les sinus transverses et les jugulaires. Les patients présentant une céphalée circonstancielle semblent présenter un nombre plus important de modifications du système veineux cérébral. Cependant ce type de céphalée reste relativement rare et cette hypothèse nécessite d'être confirmée sur un échantillonnage plus important afin de voir la proportion précise de patients présentant des modifications du système veineux cérébral.
Purpose: To prove the feasibility of arterial spin labeling (ASL) to explore brain tumors by comparing dynamic susceptibility contrast-enhanced MRI to ASL at 3T MR. Materials and Methods: Twenty-seven patients were included presenting 9 gliomas, 10 metastases and 8 meningiomas. All were explored by a pseudo-continuous ASL and dynamic susceptibility contrast-enhanced T2* perfusion sequence. Two neuroradiologists analyzed the cerebral blood flow (CBF) maps to assess feasibility, examination quality and quantitative comparison. The Spearman nonparametric correlation test and the Bland-Altman graphic test were used to analyze our quantitative results. Results: 92% of ASL CBF maps were informative. ASL detected all lesions as well as dynamic susceptibility contrast-enhanced MRI. Both sequences provide relative quantitative CBF values closely correlated. Conclusion: On a 3T MR unit, ASL is a good alternative to dynamic susceptibility contrast-enhanced MRI when contrast medium is contraindicated or intravenous injection is not possible. Its results on relative CBF parameters are similar to contrast-injected perfusion.
Background Visual disturbances (VDs) are reported with an average rate of 1.4% after foam sclerotherapy (FS). Some clinical clues indicate that they could correspond to migraine with aura (MA). Aims To validate the hypothesis that VDs occurring after FS correspond to MA and are not transient ischaemic cerebro-vascular events. Method A prospective multicentre study was carried out by the French Society of Phlebology in collaboration with the Neurology Department of the Marseille University Hospital (France). We included prospectively and consecutively all patients who experienced VDs after FS using air to make the foam. The patients were assessed (1) clinically with a specific form describing procedures of FS and recording neurological symptoms, later analysed by a neurologist specialized in migraine; and (2) by a brain diffusion-weighted magnetic resonance imaging (MRI) (T1, T2, T2*, diffusion) carried out within two weeks and analysed by a neuroradiologist. Results Twenty patients, 16 women and four men, were included in 11 phlebology clinics. All kinds of veins were treated. VDs occurred in average seven minutes after FS. Clinical assessment showed that VDs presented characteristics of MA in all patients, with headache in 10 and without in 10. Paresthesia was observed in five patients and dysphasic speech disturbance in one. Fifteen patients (75%) had a personal history of migraine. Fifteen MRIs were performed within two weeks (mean: 8 days) and three were late (26 days). All of them were normal. MRI was not performed in two patients. Conclusion These results show that VDs occurring after FS correspond to MA and are not transient ischaemic cerebro-vascular events. We suggest a pathophysiological hypothesis resting on the release of endothelin that would reach the cerebral cortex through a paten foramen ovale.

This paper proposes a computationally efficient procedure for skew detection and text line position determination in digitized documents, which is based on the cross-correlation between the pixels of vertical lines in a document. The determination of the skew angle in documents is essential in optical character recognition systems. Due to the text skew, each horizontal text line intersects a predefined set of vertical lines at non-horizontal positions. Using only the pixels on these vertical lines we construct a correlation matrix and evaluate the skew angle of the document with high accuracy. In addition, using the same matrix, we compute the positions of text lines in the document. The proposed method is tested on a variety of mixed-type documents and it provides good and accurate results while it requires only a short computational time. We illustrate the effectiveness of the algorithm by presenting four characteristic examples.
BACKGROUND AND PURPOSE:Our aim was to describe the immediate or early complications and technical failures occurring during balloon kyphoplasty (BKP) procedures and attributable to balloon inflation.MATERIALS AND METHODS:We performed a retrospective review of all consecutive kyphoplasty procedures performed in our institution from May 2005 to October 2006. Fifty-one patients were treated by BKP at 75 spinal levels, and 137 vertebroplasties were performed as well.RESULTS:Several recurrent complications or procedural failures were observed during BKP: cortical or endplate fracture by balloon expansion (4 vertebrae), partial vertebral re-collapse after deflation (4 vertebrae), balloon rupture during inflation (5 vertebrae), and transient hyperalgia after the procedure (11 patients, 27.5%).CONCLUSIONS:Several symptomatic or asymptomatic complications and technical failures can occur during BKP. Some modifications of the usual kyphoplasty technique may decrease the frequency of these complications.
Background and Purpose. - Glioma and meningioma are the two most common types of primary brain tumor. The aim of the present study was to analyze, using dynamic susceptibility contrast MR perfusion imaging, the effect of angiogenesis on peritumoral tissue.Methods.-In this prospective study, conducted from December 2003 to March 2005, out of 18 patients recruited, 12 were included (six with meningioma, six with glioblastoma). Using rates of maximum signal drop (MSD), we drew regions of interest (ROI) starting near the lesion, and gradually moving outwards to areas of distant edema in axial and sagittal planes at 10, 20 and 30 mm from the tumor. We also drew ROI on the contralateral brain white matter to obtain a normal baseline for comparison (relative MSD; rMSD).Results. - In regions of peritumoral T2 hypersignals, we observed a decrease in rMSD with distance from glioblastoma due to reduced angiogenesis, and an increase in rMSD with distance from meningioma, probably due to a reduced mass effect.Conclusion. - In our study, dynamic susceptibility contrast MR perfusion imaging, using MSD as a parameter, revealed differences between meningioma and glioblastoma peritumoral tissue due to changes in angiogenesis. Although crucial in regulating intracranial hydrodynamics, the cerebral venous system has been rarely studied because of its structural complexity and individual variations. The purpose of our study was to evaluate the organization of cerebral venous system in healthy adults. Phase-contrast magnetic resonance imaging (PC-MRI) was performed in 18 healthy volunteers, in the supine position. Venous, arterial, and cerebrospinal fluid (CSF) flows were calculated. We found heterogeneous individual venous flows and variable side dominance in paired veins and sinuses. In some participants, the accessory epidural drainage preponderated over the habitually dominant jugular outflow. The PC-MRI enabled measurements of venous flows in superior sagittal (SSS), SRS (straight), and TS (transverse) sinuses with excellent detection rates. Pulsatility index for both intracranial (SSS) and cervical (mainly jugular) levels showed a significant increase in pulsatile blood flow in jugular veins as compared with that in SSS. Mean cervical and cerebral arterial blood flows were 714+/-124 and 649+/-178 mL/min, respectively. Cerebrospinal fluid aqueductal and cervical stroke volumes were 41+/-22 and 460+/-149 microL, respectively. Our results emphasize the variability of venous drainage for side dominance and jugular/epidural organization. The pulsatility of venous outflow and the role it plays in the regulation of intracranial pressure require further investigation.
