Resections of primary pelvic bone tumors are frequently complicated by surgical site infections (SSIs), thereby impairing the functional prognosis of patients, especially in case of implant removal. Although prophylactic antibiotics play an essential role in preventing SSIs, there are presently no recommendations that support their appropriate use. This study aimed to assess the impact of a 24 h prophylactic protocol on the bacterial ecology, the resistance pattern, and the SSI healing rate. We hypothesized that this protocol not only limits the emergence of resistance but also results in a good cure rate with implant retention in case of SSI. A retrospective study was performed that included all patients with an SSI following a pelvic bone tumoral resection between 2005 and 2017 who received a 24 h antibiotic prophylaxis protocol. Twenty-nine patients with an SSI were included. We observed a 75.9% rate of polymicrobial infection, with a high prevalence of digestive flora microorganisms and a majority of wild-type phenotypes. We confirmed that there was no significant emergence of resistant flora. After first-line debridement, antibiotics (DA) if any implant was used, or debridement, antibiotics, and implant retention (DAIR) whenever possible, we obtained a 79.3% cure rate, with implant removal in 20% of cases. The absence of an implant was significantly associated with SSI healing. Early infection management and low resistance profiles may also have a positive effect, but this needs to be confirmed in a larger cohort. In light of this, the use of a 24 h prophylactic protocol in primary pelvic bone tumor resections is associated with a favorable infection cure rate and implant retention in case of SSI, and minimal selection of resistant microorganisms.
Los tumores óseos del miembro superior son menos frecuentes que los del miembro inferior. Esta localización no presenta ninguna especificidad epidemiológica, ya que todas las entidades patológicas pueden desarrollarse en el miembro superior. El condrosarcoma, el sarcoma de Ewing y el osteosarcoma son los tumores malignos más frecuentes. El objetivo de la resección es la exéresis del tumor en bloque, sin invadirlo, para las patologías malignas. Este procedimiento quirúrgico forma parte de un programa terapéutico multidisciplinario y sólo puede considerarse tras la práctica de una biopsia y la discusión en un comité multidisciplinario (CM) de los tumores del aparato locomotor. La resección se planifica y se lleva a cabo sobre la base de una reciente evaluación preoperatoria por pruebas de imagen (al menos por resonancia magnética), en un centro especializado en ortopedia oncológica, para conciliar los objetivos contradictorios de una resección oncológica completa y adaptada y la mejor preservación posible de la función. En más del 90% de los casos, el tratamiento puede ser conservador. La reconstrucción debe cumplir los requisitos de estabilidad y resistencia: la calidad del tejido blando musculotendinoso es fundamental para los resultados funcionales. En función de la localización anatómica, la extensión de la resección en los tejidos blandos, el estado del paciente y la enfermedad, la reconstrucción se basa en técnicas protésicas, biológicas de injertos óseos o compuestas. En todos los casos, existen numerosas complicaciones, tanto precoces como tardías, y justifican una estrecha coordinación con el equipo de oncología médica en el período perioperatorio para no dificultar el tratamiento de la enfermedad y el seguimiento a largo plazo en un centro ortopédico especializado.
Objectives: The aim was to determine the characteristics of patients who developed Cutibacterium acnes spinal implant-associated infection (SIAI) and the associated risk factors. Methods: We conducted two parallel caseecontrol studies comparing 59 patients with SIAI caused by C. acnes (cases 1) and 93 patients with SIAI caused by other microorganisms (cases 2) diagnosed during 2010-2015 with 302 controls who underwent spinal instrumentation without subsequent infection. Results: Late-onset infections (median time to diagnosis, 843 days versus 23 days; p < 0.001) were more common in cases 1 than in cases 2. However, 20/59 (34%) of cases 1 occurred within the first 3 months after the index surgery. In addition, cases 1 were less likely to have fever (27%, 16/59 versus 58%, 54/93; p 0.001) or wound inflammation (39%, 23/59 versus 72%, 67/93; p < 0.001). Moreover, 24/59 (40%) of cases 1 presented with polymicrobial infections, and staphylococcal pathogens accounted for 22/24 (92%) of the co-infections. By comparing and contrasting the two multivariate risk models (cases 1 versus controls and cases 2 versus controls), the following factors associated with C. acnes SIAI development were identified: age <54 years (adjusted odds ratio (aOR) 2.43, 95% confidence interval (CI) 1.09-5.58, p 0.03), a body mass index <22 kg/m(2) (aOR 2.47, 95% CI 1.17-5.29, p 0.02), and thoracic instrumentation (aOR 16.1, 95% CI 7.57-37.0, p < 0.001). Conclusions: Future therapeutic and prophylactic studies on C. acnes SIAI should focus on young, thin patients who undergo spinal instrumentation procedures involving the thoracic spine. AbstractObjectives:We longitudinally observed and assessed the impact of the operating room (OR) staff movements and door openings on surrogates of the exogenous infectious risk using a new technology system.Design and setting:This multicenter observational study included 13 ORs from 10 hospitals, performing planned cardiac and orthopedic surgery (total hip or knee replacement). Door openings during the surgical procedure were obtained from data collected by inertial sensors fixed on the doors. Intraoperative staff movements were captured by a network of 8 infrared cameras. For each surgical procedure, 3 microbiological air counts, longitudinal particles counts, and 1 bacteriological sample of the wound before skin closure were performed. Statistics were performed using a linear mixed model for longitudinal data.Results:We included 34 orthopedic and 25 cardiac procedures. The median frequency of door openings from incision to closure was independently associated with an increased log10 0.3 µm particle (ß, 0.03; standard deviation [SD], 0.01; P = .01) and air microbial count (ß, 0.07; SD, 0.03; P = .03) but was not significantly correlated with the wound contamination before closure (r = 0.13; P = .32). The number of persons (ß, −0.08; SD, 0.03; P < .01), and the cumulated movements by the surgical team (ß, 0.0004; SD, 0.0005; P < .01) were associated with log10 0.3 µm particle counts.Conclusions:This study has demonstrated a previously missing association between intraoperative staff movements and surrogates of the exogenous risk of surgical site infection. Restriction of staff movements and door openings should be considered for the control of the intraoperative exogenous infectious risk.
Background. - In France, the most severe bone and joint infections (BJI), called "complex'' (CBJI), are assessed in a multidisciplinary team meeting (MTM) in a reference center. However, the definition of CBJI, drawn up by the Health Ministry, is not consensual between physicians. The objective was to estimate the agreement for CBJI classification. Methods. - Initially, five experts from one MTM classified twice, one-month apart, 24 cases as non-BJI, simple BJI or CBJI, using the complete medical record. Secondly, six MTMs classified the same cases using standardized information. Agreements were estimated using Fleiss and Cohen kappa (k) coefficients. Results. - Inter-expert agreement during one MTM was moderate (k = 0.49), and fair (k = 0.23) when the four non-BJIs were excluded. Intra-expert agreement was moderate (k = 0.50, range 0.27-0.90), not improved with experience. The overall inter-MTM agreement was moderate (k = 0.58), it was better between MTMs with professor (k = 0.65) than without (k = 0.51) and with longer median time per case (k = 0.60) than shorter (k = 0.47). When the four non-BJIs were excluded, the overall agreement decreased (k = 0.40). Conclusion. - The first step confirmed the heterogeneity of CBJI classification between experts. The seemingly better inter-MTM than inter-expert agreement could be an argument in favour of MTMs, which are moreover a privileged place to enhance expertise. La prise en charge des infections ostéo-articulaires sur prothèse (IOAp) peut nécessiter une approche médico-chirurgicale palliative dont les données sont limitées. L’objectif était ici de décrire l’utilisation de la fistulisation comme alternative de traitement palliatif chez les patients avec une IOAp complexe. Étude rétrospective descriptive multicentrique incluant les patients avec une IOAp traitée par fistule spontanée ou dirigée de janvier 2013 à décembre 2018. Les critères de jugement étaient la survenue d’un événement (hospitalisation ou décès lié à l’IOAp) et la gravité des séquelles. Sur 3 centres, 48 patients ont été inclus d’âge médian 78 ans (48–95) avec un sex-ratio 0,92. Le score ASA médian était de 3 (1–4) et 34 patients (71 %) avaient au moins une comorbidité sévère. Une fistule spontanée depuis au moins 1 an était déjà présente chez 29 patients (60 %) lorsque la décision de traitement a été prise en RCP. Ces IOAp touchaient la hanche (n = 26 ; 54 %), le genou (n = 17 ; 35 %), l’épaule (n = 4 ; 9 %) et le coude (n = 1 ; 2 %). Elles étaient précoces pour 9 patients (18 %), tardives pour 17 (36 %) et retardées pour 21 (46 %). Une documentation bactériologique était obtenue pour 37 patients, plurimicrobiens dans 7 cas. Les germes isolés étaient : Staphylococcus aureus (13 ; 25 %) dont 3 résistants à la méthicilline, autres staphylocoques (4 ; 8 %), entérobactéries (9 ; 17,5 %), enterocoques (9 ; 17,5 %), streptocoques (7 ; 14 %), pseudomonas (4 ; 8 %), corynebacterium (2 ; 4 %), cutibacterium (1 ; 2 %), autres anaérobies (2 ; 4 %). Huit patients (18 %) ont reçu une antibiothérapie suppressive (ATS). Dix-huit patients (40 %) ont été hospitalisés pour des complications de l’IOAp, 2 (4 %) sont décédés en lien avec l’IOAp, 3 patients ont été perdus de vue et 25 (56 %) n’ont eu aucun événement. Dans 19 cas (42 %), aucune séquelle fonctionnelle n’a été notée, 14 patients (31 %) avaient des séquelles non graves (gêne à la fonction articulaire) et 12 (27 %) des séquelles graves (perte de la fonction articulaire). Il n’y avait pas de différence significative entre le groupe avec ou sans ATS concernant le risque d’hospitalisation, de décès (p = 0,62), ou sur de séquelle (p = 0,38). Le traitement palliatif des IOAp par fistulisation semble donner un bon taux d’efficacité et un maintien de la fonction articulaire et pourrait constituer une option raisonnable pour les patients avec des comorbidités et un score ASA 3. L’intérêt de l’association fistule et ATS reste à démontrer.
An obese woman was treated with oral tedizolid 200 mg once daily for pseudoarthrosis 10 years after Roux-en-Y bypass surgery. Total plasma peak concentration was 2.12 mg/liter 3 h after intake, and area under the concentration-time curve from 0 to 24 h (AUC0-24) was 28.3 mg/liter · h. The AUC0-24/MIC ratio for unbound concentrations and for sensitive Staphylococcus and Streptococcus strains was ≥10.8, higher than the target ratio of 3. These results support the use of tedizolid without adjustment after bariatric surgery.
BackgroundIn 2015, our center replaced vancomycin with linezolid for the postoperative empirical treatment of osteoarticular infections (OAI).ObjectivesTo assess the bacteriological relevance of linezolid for orthopedic postoperative probabilistic antibiotic therapy.MethodsAnalysis of an observational cohort of patients empirically treated with a combination of linezolid and piperacillin/tazobactam during the immediate postoperative stage for an OAI between July 1st 2015 and July 1st 2016, in a French reference center.ResultsSeventy-seven of 126 patients who received a probabilistic postoperative combination of linezolid with piperacillin/tazobactam had microbiological proof of infection. Sixty-six of 77 OAI involved material, including an osteosynthesis in 45 cases (68%) and prosthesis in 21 cases (32%). Infection was due to Gram-positive bacteria in 62 cases (80.5%), mostly S. aureus (n=32, 41.6%), and S. epidermidis (n=14, 18.2%) accounting for 74.2% of Gram-positive bacteria. Among 14 OAI due to S. epidermidis, 11 (78.6%) were due to methicillin-resistant strains. All the S. aureus and S. epidermidis strains were susceptible to linezolid (MICs ≤ 4 mg/L), except in one patient previously treated with linezolid who was infected with a linezolid-resistant S. epidermidis strain (MIC > 256 mg/L).ConclusionLinezolid can be used empirically in postoperative antibiotic therapy of OAI before obtaining definitive microbial results. Although linezolid resistance is rare in this population, previous oxazolidinone treatment should be documented before initiation of probabilistic postoperative treatment to highlight potential linezolid resistance.
Les mycobactéries atypiques (MA) sont des pathogènes opportunistes rarement impliqués dans des infections ostéo-articulaires (IOA). Décrire les caractéristiques cliniques, paracliniques, les facteurs de risque et le devenir des patients atteints d'IOA à MA. Étude rétrospective monocentrique recensant les cas d'IOA à MA entre 2012 et 2017. Critère d'inclusion : au moins un prélèvement ostéo-articulaire positif en culture à MA. Il s'agit de 5 patients (3 H et 2 F) dont la moyenne d'âge est de 62 ans (51–78). Deux patients avaient une spondylodiscite (SD) associée à une atteinte du genou dans un cas, et à une atteinte multiple (sacrum, sternum, clavicule, côte) dans l'autre cas. Deux patients avaient uniquement des localisations extravertébrales impliquant le poignet et le genou pour l'un, et la cheville et le tibia pour l'autre. Un patient présentait une arthrite du poignet isolée. Il n'y avait aucune localisation extra-osseuse. Des facteurs de risque étaient identifiés chez 4 patients : maladie auto-immune (n = 4), traitement immunosuppresseur (n = 4), aquariophile (n = 1), antécédent d'infection à MA (n = 1). Tous les patients étaient négatifs pour le VIH. L'examen clinique retrouvait une douleur (n = 3), de la fièvre (n = 2), une compression médullaire (n = 1) et des signes locaux (n = 2). L'imagerie objectivait des anomalies dans 100 % des cas (n = 5). Le délai diagnostique moyen était de 7,5 mois (0,5–21). Les prélèvements étaient tous négatifs à l'examen direct et positifs en culture à Mycobacterium avium (n = 2), M. intracellulare (n = 1), M. parascrofulaceum (n = 1) et M. marinum (n = 1). L'examen anatomopathologique était contributif dans 75 % des cas (3/4), révélant des granulomes ou des infiltrats lymphoplasmocytaires nécrotiques. Pour un patient, les prélèvements ostéo-articulaires étaient également positifs en culture à S. aureus. Les souches du complexe aviaire étaient sensibles à la clarithromycine. Une antibiothérapie était débutée pour 4 patients, complétée par une chirurgie dans 2 cas. L'évolution était favorable pour le patient atteint d'une arthrite à M. marinum après 1,5 mois de traitement par clarithromycine + doxycycline et pour le patient ayant une SD avec localisations extravertébrales multiples après 12 mois de traitement par clarithromycine + rifabutine + éthambutol. Deux patients sont décédés. Un patient était perdu de vue. Le diagnostic des IOA à MA est difficile en raison du caractère non spécifique de l'imagerie et de la clinique. L'examen anatomopathologique reste d'une aide précieuse. Il est important d'évoquer ce diagnostic rare, en particulier chez les patients immunodéprimés hors VIH, pour réduire le délai diagnostique préjudiciable chez des patients fragiles.
After rifampin and levofloxacin treatment for a Staphylococcus aureus bone infection, a pyogenic granuloma due to a newly described Cutibacterium species, C. namnetense developed on the tibia former external fixator. This rifampin resistant bacterium, selected during treatment, harbored a mutation in the rpoB gene. This case illustrates the possible in vivo selection of resistant mutant most likely due to the bacterial burden and therefore the importance of adequate bone infection treatment.
Equinovarus foot is the most frequent complication in patients suffering from spastic paresis. Surgical treatment of equinovarus spastic foot includes different kinds of surgery, such as: selective tibial neurotomy, lengthening of the gastrocnemii, soleus, or Achilles tendon, tendon transfer, or rear- and midfoot arthrodesis. So far, there is a lack of precise guidelines about the surgery to consider, for these patients according to their clinical situation. To describe the outcome and complications of equinovarus foot surgery. We performed an analytic, retrospective, single centre study. We included all patients with spastic paresis acquired after 18 years who were operated for an equinovarus foot, on a 12-year period. Four time points were studied: the preoperative situation, postoperative at 3 and 12 months, and the last assessment during follow-up. Considering three main clinical situations (dynamic equinus, fixed equinus and fixed rear and midfoot deformities), we described the surgical procedures and their outcomes. From 200 patients who underwent surgery, 126 feet were included. Forty-eight percent of patients presented with a dynamic equinus foot and had a neurotomy; in such cases 10% of surgery failed, whereas only 6% failed in fixed equinus. The rate of subjective success (assessed by the patients) was 86%. Thirty percent of patients experienced a revision surgery, and 15% presented with complications. Fifteen percent of patients had claw toes after surgery in each group. Five patients had bone surgery because of a fixed varus. The occurrence of postoperative neuropathic pain, was associated with neurotomy of the plantar nerve (P < 0.01). Equinus/equinovarus foot is a complex situation in patients with spastic paresis. Several clinical pictures can be identified. An important variety of surgical procedures is performed in these patients. The predictive factors of failure and complications of such surgeries can be suggested from this study.
Objectives: To evaluate the difference in terms of overall complications between surgical and non-surgical management of lower limb fractures in patients with chronic spinal cord injury (SCI). Design: A 13-year retrospective study including patients with chronic spinal cord lesion admitted for sublesional lower limb fractures. Setting: University hospital SCI reference departments (Rehabilitation department and orthopedic department). Participants: Forty patients with SCI were included, 24 men and 16 women. Fifty-six distinct fracture occurrences were responsible for a total of 59 lower limb fractures. We compared the number of overall complications between surgical and non-surgical management of fractures. Results: Non-surgical management was realized for 19 fractures and surgery for 40. Characteristics of operated and non-operated patients at the time of each fracture occurrence did not differ concerning age (P = 0.430), sex (P = 0.890), lesion levels (P = 0.410) and AIS classification (P = 0.790). Data analysis highlighted 20 complications directly due to the fracture site for 16 distinct fractures. Seven medical complications were found in 5 distinct fracture events. Only 10 (25.0%) of 40 surgical managements had at least one medical or post-surgical complication, whereas 12 (63.2%) of 19 non-operative managements had at least one complication. Therefore, the overall rate of complications was significantly higher after non-surgical treatment (P = 0.044). Conclusion: Lower extremity fractures due to osteoporosis in patients with SCI are responsible for local and general complications. When possible, surgery may be the best management to propose because of fewer overall complications.
Introduction: Surgical site infections (SSI) represent the most common postoperative complication after limb sparing surgery for primary malignant bone tumors, with incidence ranging from 10 to 47%. There is no consensus concerning about the optimal surgical strategy, or the adequate antibiotic prophylaxis in pelvic resections. A greater knowledge of these infections and their surgical trajectories seem essential to obtain.Materials and methods: We retrospectively studied 45 cases of pelvic resection, including at least the periacetabular zone 2 of Enneking, performed between 1989 and 2013 in the same center. Infection rate, risk factors and surgical trajectories were analyzed. The impact of a postoperative infection on the quality of life and functional recovery was evaluated by the Musculoskeletal Tumor Society scoring system (MSTS).Results: Sixteen patients presented a SSI in the first post-operative year (35.6%). We found as risk factors the pre-operative ASA score, the age at surgery and the number of packed red cells transfused during surgery. In case of failure of an initial washout, an iterative procedure is responsible for a high failure rate of 88.9%. Irrespective of the type of reconstruction, our functional results show that this surgery is often a source of handicap with a MSTS score of 13.77 in infected patients versus 17.70 in non-infected patients, at two-year follow-up.Discussion: In case of failure of an initial wash, prosthetic material must be removed and a hip transposition procedure should be preferred to a second-look surgery. Concerning prophylactic antibiotherapy, a dual therapy for at least 48 h after surgery should probably be preferred. (C) 2017 Elsevier Ltd. All rights reserved.
Depuis novembre 2015, une équipe mobile de pharmacie clinique (EPC) est déployée au sein d’un service de chirurgie orthopédique. Sa mission est de sécuriser la prise en charge médicamenteuse. Via des outils comme la conciliation médicamenteuse et l’analyse pharmaceutique, elle permet d’éviter oublis, erreurs, interactions médicamenteuses et propose des avis pour optimiser les prescriptions. L’objectif est d’analyser l’impact de l’EPC sur la prise en charge de ces patients. L’étude s’étend de début novembre 2015 à fin décembre 2016 dans un service de 84 lits. Pour tous les patients programmés et non programmés, une conciliation médicamenteuse est réalisée. L’EPC rédige une liste exhaustive des traitements du patient en recoupant au moins 3 sources d’information, telles que le médecin traitant, la pharmacie de ville, les ordonnances apportées et un entretien patient. La comparaison de cette liste avec la consultation d’anesthésie ou avec la première prescription hospitalière permet d’identifier des divergences non intentionnelles (DNI). En cas de DNI, le pharmacien intervient pour proposer des modifications de prescription. Elles sont appelées interventions pharmaceutiques (IP). En outre, au cours de l’hospitalisation, le pharmacien répond aux interrogations des équipes médicales et paramédicales et intervient pour optimiser les prescriptions et les transferts de patients. À 14 mois du déploiement de l’EPC, 4200 patients ont été conciliés avec 47 % de patients non programmés. Au total, 3240 IP ont été réalisées. Lorsque le pharmacien intervient, la prescription est modifiée dans 96 % des cas. Les problématiques principalement identifiées sont des oublis de prescriptions (48 %) et des erreurs de posologie (35 %). Deux grandes catégories de médicaments sont retrouvées : 29 % sont des médicaments qui agissent sur le système nerveux (antiépileptiques, neuroleptiques, antalgiques, antiparkinsoniens) et 20 % sur l’appareil cardiovasculaire (anticoagulants, antihypertenseurs, bêtabloquants). D’après la classification Remed de la Haute Autorité de santé, 25 % des IP analysées sont cotées comme de gravité significative. L’impact de l’EPC a été clairement établi par le nombre important de patients conciliés et par la quantité d’IP ayant entraîné une modification de prescription. Une étude de satisfaction à 6 mois auprès des chirurgiens, des anesthésistes, des internes, des cadres et des infirmiers vient conforter cette mise en place avec des résultats unanimes. Ces données soulignent l’intégration et l’importance de la présence pharmaceutique dans une unité de chirurgie orthopédique.
L’usage du TMP/SMX est une alternative possible dans le traitement des IOA sur matériel ; mais est débattue. Notre objectif était d’évaluer le rapport bénéfice/risque (RBR) du TMP/SMX dans cette indication. Cette étude observationnelle rétrospective a été développée dans 4 centres du CRIOGO. Les adultes traités TMP/SMX (n = 50) pour une IOA sur matériel entre 2011 et 2012 ont été inclus (groupe+), puis comparé à un groupe control dont l’antibiothérapie ne comprenait pas de TMP/SMX (n = 139 ; groupe−). Les critères d’évaluation au terme du suivit étaient : 1/évolution favorable : infection traitée sans séquelles fonctionnels orthopédiques ou avec séquelles mineurs, 2/évolution défavorable : infection traitée avec séquelles majeurs (amputation, arthrodèse), décès, échec de traitement. Le RBR était évalué selon les critères suivants : clairement favorable (infection traitée sans effet indésirable (EI) induit par l’antibiothérapie), plutôt favorable (infection traitée et EI induit par l’antibiothérapie sans changement de molécule), plutôt défavorable (infection traitée et EI induisant un changement de l’antibiothérapie) et clairement défavorable (échec de traitement, décès). Les caractéristiques et la prise en charge thérapeutique entre les 2 groupes de patients étaient comparables. L’évolution était équivalente entre les 2 groupes, respectivement pour les groupes+ vs − évolution favorable (82,5 % vs 85,4 %), évolution défavorable (17,5 % vs 14,6 %) (p = 0,85). L’analyse multivariée confirme ce résultat (OR = 1,17 [0,37 ; 3,7] ; p = 0,78). De plus, un âge plus élevé (> 77 ans), une IOA complexe et une durée d’antibiothérapie plus élevée (> 6 semaines) étaient associés à un risque plus élevé d’évolution défavorable. Le nombre d’EI secondaire à l’antibiothérapie étaient comparables : groupe+ n = 10 (20 %), groupe− n = 22 (15,8 %) (p = 0,65). Le RBR était équivalent entre les 2 groupes avec respectivement pour le groupe+ vs − clairement favorable (69,6 % vs 70 %), plutôt favorable (6,5 % vs 5,4 %), plutôt défavorable (6,5 % vs 5,4 %), clairement défavorable (17,4 % vs 19,2 %) (p = 1). L’analyse multivariée confirme ce résultat (OR = 0,8 [0,32 ; 1,97], p = 0,62) et montre qu’une durée prolongée d’antibiothérapie plus élevée étaient associée à une RBR défavorable. Notre étude permet de conclure à une bonne tolérance et efficacité du TMP/SMX dans le traitement des IOA sur matériel.
Objectives Interleukin (IL)-38 is a newly characterised cytokine that belongs to the IL-1 family. This cytokine is expressed in the rheumatoid arthritis (RA) synovial tissue and IL-38 deficient mice have exacerbated arthritis. Here, we analysed the effect of IL-38 overexpression in the joints of arthritic mice, in human macrophages and synovial fibroblasts in vitro. Methods Articular injections of an adeno-associated virus (AAV) 2/8 encoding IL-38 were performed in collagen-induced arthritis (CIA), K/BxN serum transfer-induced arthritis (STIA) and antigen-induced arthritis (AIA) in mice. The effect of IL-38 overexpression was evaluated through clinical scores, immunohistochemistry, microCT, Luminex and RT-qPCR analysis. THP-1 macrophages were transduced with a lentiviral vector to overexpress IL-38. Results Clinical inflammatory scores were significantly decreased after AAV IL-38 injection in joints of mice with CIA and STIA, but not AIA. This decrease was accompanied by reduced macrophage infiltration and a decreased expression of Th17 cytokines (IL-17, IL-23, IL-22) and TNFα. However, IL-38 overexpression had no effect on cartilage or bone destruction. In vitro, the THP-1 monocytic cell line expressed less IL-6, TNFα and IL-23 after IL-38 overexpression. Conditioned media from these cells, containing released IL-38, also exert an anti-inflammatory effect on human primary macrophages and synovial fibroblasts from patients with RA. Conclusions This study shows for the first time that IL-38 overexpression attenuates the severity of experimental arthritis. IL-38 may exert its anti-inflammatory effects by decreasing the production of proinflammatory cytokines by macrophages and synovial fibroblasts. This effect can lead to the development of novel treatment strategies in arthritis.
The purpose of this study was to find clinical or ultrasound characteristics that might predict the failure of conservative treatment in de Quervain's syndrome. A total of 42 ultrasound-guided injections have been performed in 41 patients after clinical and ultrasound examination. Patients were immobilized for 3 weeks with a spica splint cast, and clinically evaluated at 3 and 6 weeks and by phone call at the end of the study. Ultrasound showed a septum between the tendons of the first comportment in 34% of the wrists. At last follow-up (mean 15.6 months after the injection) ten patients (24%) had undergone surgery. When comparing ultrasound and clinical characteristics of the operated and non-operated wrists, we found that patients with a high baseline visual analogue scale, with all positive clinical tests and with a persistent intracompartmental septum, had a significantly higher risk of failure following conservative treatment.
