78 Kranke mit waschungsbedürftiger Bauchfellentzündung aus drei chirurgischen Kliniken werden nach Ursprungsort, Art und Ausmaß der Peritonitis analysiert. Die operative Therapie wird dargestellt. Bei diesen 78 Kranken wird als zusätzliche Behandlung das neue Chemotherapeuticum und Antiendotoxin Taurolin anstelle eines Antibioticum angewandt. Über den postoperativen Verlauf wird detailliert berichtet (Temperaturen über 38 °C, Sekundärheilung, Entlassungszeitpunkt und ernstere Komplikationen). Eigenschaften des postoperativen Verlaufs lassen sich mit prä- und intraoperativen Befunden korrelieren: 1. postoperatives Fieber über 38 °C tritt bei der Hälfte der Patienten auf, überdurchschnittlich häufig nach Perforation von Magen und Zwölffingerdarm; 2. jeder zweite Patient heilt sekundär, vermehrt häufig nach gallenwegsbedingter und appendicitischer Peritonitis; 3. ernstere Komplikationen ereignen sich bei 29,5 % der Patienten, besonders nach Peritonitis ausgehend von Magen, Dünn- oder Dickdarm; 4. die Gesamtletalität von 11,5 % wird bei Dünn- oder Dickdarmbedingter Peritonitis deutlich überschritten. Der Peritonitisindex gebildet aus prä- und intraoperativen Faktoren kann signifikant zwischen den später Verstorbenen und Überlebenden unterscheiden. Bei 80 % dieser 78 Kranken mit schwerer Peritonitis war Taurolin ohne zusätzliche Gabe eines Antibiotikum erfolgreich.
One of the most frequent causes for peritonitis is acute appendicitis. Extent and degree of peritonitis determine the further course of the disease. A retrospective analysis of acute appendicitis with concomitant peritonitis relates pre- and intraoperative findings to the postoperative development. Resulting aspects may simplify therapy in special cases and permit a more reliable prognosis.
From three different surgical departments 78 patients with purulent peritonitis are analyzed according to cause, origin, and extent of peritonitis. Operative therapy is presented. As additional antibacterial therapy the new chemotherapeutic agent, Taurolin, with antiendotoxin-effect is used. The postoperative course (temperature exceeding 38 degrees C, secondary wound healing, day of discharge, and serious complications) is correlated with pre- and intraoperative parameters; for instance, 1. postoperative fever occurs in one half of the patients, more frequently following perforation of stomach and duodenum; 2. every second patient shows secondary wound healing, this happens even more often in peritonitis arising from biliary disease or appendicitis; 3. in 29.5% of the patients serious complications arise, especially in patients with peritonitis originating in stomach, small intestine or large bowel; 4. overall mortality of 11.5% is surpassed in patients with peritonitis originating from small intestine or large bowel. The so-called peritonitis-index (calculated on the basis of pre- and intraoperative factors) shows a significant difference between survivors and patients dying during the postoperative course. In 80% of the 78 patients with purulent peritonitis taurolin was effective as a substitute for the usual antibiotics.
A prospective randomized study on the value of additional antibacterial therapy in surgically treated, purulent peritonitis (69 patients) is presented. A new chemotherapeutic agent Taurolin with antiendotoxin activity is tested against conventional antibiotic therapy. There was an increased rate of secondary wound healing and overall complications in the Taurolin group (especially bronchopneumonia). Local complications and lethality due to peritonitis occur similarly frequently in both groups. The bactericidal and antiendotoxic effects of Taurolin, its lacking toxicity (possibility of higher dosage), and the choice of combination with antibiotics make it a most interesting substance for additional antibacterial therapy for purulent peritonitis following surgical treatment.
Die bakterielle eitrige Peritonitis hat auch heute eine hohe Letalität. Sie erfordert zunächst die chirurgische Sanierung der ursächlichen Erkrankung. Meist wird dann die Bauchhöhle ausgetupft, gewaschen oder gespült, drainiert, und prophylaktisch ohne Ergebnis der Resistenztestung ein Breitbandantibioticum gegeben. Die Wertigkeit jeder einzelnen dieser•Zusatzmaßnähmen ist nicht gesichert. Taurolin ist ein neues Chemotherapeuticum, das in vitro und in vivo baktericid und antiendotoxisch wirkt (1, 4). Erste Erfahrungen aus drei chirurgischen Kliniken zeigen die Wirkung einer kombinierten intraperitonealen und parenteralen Applikation (2). In dieser Arbeit soll die Wirksamkeit der konventionellen Zusatztherapie der Peritonitis mit der Applikation von Taurolin verglichen werden.
Unter 2153 Operationen an den extrahepatischen Gallenwegen (1973 —1977) 41 Kranke mit gallenwegsbedingter Bauchfellentzündung; Ursache in 90%: Gallensteine; operative Behandlung in 95%: Cholecystektomie; Letalität: 9,8 %. Bauchfellentzündung zu 2/3 lokal und zu 1/3 diffus, in 63 % auf dem Boden einer Perforation. Kranke mit eitrigem Exsudat hatten zur Hälfte postoperativ Fieber, zu 1/5 eine Sekundärheilung; letztere nur nach diffuser Peritonitis. Ein aus Alter, Risikofaktoren, prä- und intraoperativen Befunden gebildeter Peritonitis-Index wird vorgestellt.
Peritonitis originating in the biliary tract is analyzed in 41 patients (1973--1977). Cholelithiasis was the underlying disease in 90% of the cases, and one patient suffered from carcinoma of the biliary tree. Early cholecystectomy was performed in 95% of the patients. Peritonitis was localized in two-thirds and diffuse in one-third of the patients. A perforation was present in 63%. The grade and extent of peritonitis could be correlated with the postoperative course. One-half of the patients with turbid or purulent exudate developed postoperative fever; one fifth showed secondary wound healing. The diffuse form of peritonitis was followed more often by postoperative fever than the localized form. Although the presence or absence of perforation did not influence this parameter in the diffuse form, it did influence cases of localized peritonitis. Only the diffuse forms of peritonitis were followed by secondary wound healing. A peritonitis index was established from age, risk factors, and several preoperative and intraoperative findings. It could also be correlated with the postoperative course. The peritonitis index may classify the disease state of patients with peritonitis and may be found useful in evaluating prognosis at the time of the operation.
Unter 2153 Operationen an den extrahepatischen Gallenwegen der Jahre 1973–1977 fanden sich 41 Kranke mit gallenwegsbedingter Bauchfellentzündung. Ursächlich beteiligt waren zu 90% Gallensteine, nur einmal lag ein Carcinom zugrunde. In 95% wurde eine Cholecystektomie durchgeführt. Durch die heute praktizierte Frühoperation bei der ≫>akuten Galle≪< wurden intraoperativ verschiedene Stadien der Peritonitis angetroffen: 2/3 waren lokalisierte, 1/3 diffuse Bauchfellentzündungen, zu 63% auf dem Boden einer Perforation. Das Exsudat war zu 3/4 der Fälle trüb oder eitrig. 3/4 der untersuchten Patienten wiesen Enterobacteriaceae als ursächliche Keime auf. Grad und Ausmaß der Peritonitis ließen sich mit dem postoperativen Verlauf korrelieren. Die Hälfte der Kranken mit galligtrübem oder eitrigem Exsudat zeigten postoperativ Fieber, fast 1/5 heilten sekundär. Fibrinöse Peritonitis entwickelte postoperativ kein Fieber und keine Sekundärheilung. Fieber trat häufiger auf nach diffuser als nach lokaler Peritonitis. Die Tatsache der Perforation spielte bei der diffusen Peritonitis keine Rolle. Bei lokaler Peritonitis war das Auftreten von Fieber postoperativ nach Perforation häufiger als ohne Perforation. Sekundäre Wundheilung ergab sich nur nach diffuser Ausbreitung der Bauchfellentzündung. Ein aus Alter, Risikofaktoren und prä- und intraoperativen Befunden gebildeter Peritonitis-Index kann in der heutigen Form zur Klassifizierung der Peritonitis und erst nach weiterer mathematischer Verfeinerung eventuell auch zur Prognosestellung verwandt werden.
