AIM: To investigate the effect of blocking short wave visible light(400-500nm)on tear film stability in healthy people. METHODS: In this prospective intervention experiment, 26 college students were selected, including 12 males and 14 females, aged 22.1±1.4 years, from February to March 2020. At the same time of two adjacent days, the subjects watched the video for 1h under the same environmental conditions. For the first time, there was no short wave visible light blocking, and for the second time, they wore short wave visible light blocking myopia or flat glasses. The subjects were examined the radius of curvature of lacrimal River curved surface, tear osmolality, tear film rupture time, corneal fluorescein sodium staining score, the basic state before watching the video and after watching the video respectively. And the questionnaire of visual perception was completed at the end of the experiment. RESULTS: There were significant differences in the radius of curvature of lacrimal River curved surface, tear osmolality, tear film rupture time, corneal fluorescein sodium staining score between the basic state and the short wave visible unstopped state(t=4.50, 5.72, 4.437, 3.245, P<0.05). There was no significant difference between the short wave visible light blocking and the basic state(t=1.972, 1.993, 1.921, 1.641, P>0.05). There was also significant difference between the short wave visible light blocking and the short wave visible light unblocking(t=2.825, 3.771, 2.610, 3.028, P< 0.05). The results of the questionnaire showed that: eighteen subjects(69%)said that wearing the short wave visible light blocking glasses or without wearing short wave visible light blocking glasses had slight eye dryness, pain, foreign body sensation, visual fatigue and other discomfort after watching the video. Sixteen subjects(62%)said that they were more comfortable after blocking the short wave visible light. Twenty-four subjects(92%)said that they were willing to block the short wave visible light. CONCLUSION: The stability of tear film could be reduced by watching the display screen for a short time. Blocking the short wave visible light could protect the stability of tear film of the ocular surface.
Background: The present paper established a model of optic neuritis injury by intrathecal injection of lipopolysaccharide (LPS) into the optic nerve, then explored the protective effect of Lycium Barbarum Polysaccharide (LBP) on optic nerve tissue and RGCs after injury. Material and methods: LPS was microinjected into the optic nerve of SD rats to induce optic neuritis. The histopathological variations of HE staining were observed in each group. The expressions of ED1/CD68 and GFAP were determined by immunofluorescence. The optic nerve repairment was evaluated by lux solid blue staining. TUNEL detects retinal cell apoptosis. Western blot was used to detect differences in expressions of related proteins. Results: LBP can alleviate the degree of optic nerve damage in rats with optic neuritis injury, inhibit the increase of LPS-induced macrophages and astrocytes in the optic nerve and retina, mitigate LPS-induced retinal RGC damage, inhibit LPS-induced expressions of proinflammatory cytokines TNF-α and IL-6, COX2 and iNOS in the optic nerve injury model of optic neuritis in rats, as well as inhibit the activation of STAT1, STAT3 and NF-κB pathways. Conclusion: LBP can ameliorate optic nerve damage, and the mechanism may be realized through inhibiting NF- B and STAT signaling pathways.
Objective: To study clinical and genetic characteristics of a Leber hereditary optic neuropathy (LHON) family with the heteroplasmic m.14484T>C mutation. Methods: A cross-sectional study. The objects of the study included a 31-year-old male LHON patient with the heteroplasmic m.14484T>C mutation (the proband) who visited Department of Ophthalmology in the Affiliated Central Hospital of Qingdao University in March 2015 and other 36 matrilineal relatives in a four-generation family (12 males and 24 females aged 2-81 years, median 27 years). The visual acuity, intraocular pressure, fundus, color vision, visual field, visual evoked potential and optical coherence tomography were evaluated in maternal members. The mitochondrial DNA (mtDNA) sequence of fragments including m.14484 loci was detected by Sanger sequencing in 33 members. The sequencing peaks were analyzed by QSVanalyzer software to get the heteroplasmy levels of m.14484T>C mutation. The mtDNA of the proband was amplified by PCR and sequenced. Assembled sequence of mtDNA was compared with the updated consensus Cambridge sequence. The differences in visual evoked potential, optical coherence tomography and heteroplasmy levels were compared between two groups by the t-test, and among multiple groups by the single factor variance analysis. Results: Among the 33 maternal members of the family, 4 patients, 28 carriers and 1 person without a mutation were confirmed. The penetrance was 12.5% (4/32) . In addition to 4 patients with obvious abnormality on the ophthalmic examination, 5 carriers also appeared anomaly on the electrophysiological and visual function examinations. Compared to carriers, the amplitude of P100 was obviously decreased in the LHON patients[ (5.6±2.6) μV vs. (15.6±9.6) μV, t=2.880, P=0.006]. Significantly reduced values were seen in the average retinal nerve fiber layer thickness[ (71±17) μm vs. (99±11) μm, t=5.969, P< 0.001], in each side of the sub-area macular thickness, and in the nasal side of the lateral sub-area macular thickness [ (260±16) μm vs. (291±12) μm, t=5.593, P<0.001] between the LHON patients and carriers. The heteroplasmic levels were 80%±3% in the LHON patients, and 27%±18% in the unaffected members;the difference was significant (t=-8.395, P<0.001). The average degree of heteroplasmy had no difference between male and female members (48%±34% vs. 35%±28%, t=-1.147, P=0.258). The average mutation load was 29%±14% in the second generation members, 36%±29% in the third generation members, and 51%±36% in the fourth generation members;the differences were not statistically significant (F=1.152, P=0.330). The difference in the heteroplasmic levels was not statistically significant between mothers and their offspring (31%±25% vs. 42%±32%, t=1.165, P=0.251). Compared to Cambridge consensus sequence, 41 mutations was found in mtDNA of the proband, of which, 10 were missense mutations, including mutations m.4216T>C and m.3394T>C. According to the phylogenetic tree, the haplotype of the proband was M9a (M9a1a1c1a). Conclusions: In the family with the heteroplasmic m.14484T>C mutation, clinical manifestations of LHON appear in the individuals whose heteroplasmic level is more than 75%, and all of patients show typical chronic optic atrophy on the ophthalmic examination. The carriers with the m.14484T>C mutation also appear anomaly on the electrophysiological and visual function examinations. The heteroplasmic level of m.14484T>C mutation has a tendency to increase during the transmission in the family. The primary mutation m.14484T>C coordinate mutations m.4216T>C and m.3394T>C to increase the penetrance and incidence of abnormal visual function in carriers. (Chin J Ophthalmol, 2018, 54: 526-534).
目的 探讨可吸收泪小管栓子VisiPlug植入后对中、重度干眼症病人相关指标的影响.方法 中、重度干眼症病人18例(36眼),给予双眼上下泪小管植入可吸收泪小管栓子VisiPlug,分别于治疗前和治疗后的第1月、1个月、3个月、6个月进行主观症状评分、基础泪液分泌实验、角膜荧光素染色评分、泪膜破裂时间观察和泪河高度检查.结果 无病人出现泪小管栓子脱落,1例病人出现不能耐受的泪溢症状,将下泪小管内的栓子冲洗入泪囊后缓解.治疗后第1周、1个月、3个月、6个月病人主观症状评分、角膜荧光素染色评分、泪膜破裂时间和泪河高度与治疗前比较,差异有显著性(F =26.79~259.18,P<0.01);治疗后第1、3、6个月与治疗后第1周相比,主观症状评分、角膜荧光素染色评分、泪膜破裂时间比较差异有显著性(P<0.05);治疗前与治疗后第1周、1个月、3个月、6个月基础泪液分泌测量结果比较差异无显著性(P>0.05);治疗后第1周、1个月、3个月、6个月泪河高度比较差异无显著性(P>0.05).结论 可吸收泪小管栓子VisiPlug植入是安全的,并且在未降解期间可以显著改善干眼症病人的相关指标.
目的 探讨雷帕霉素(RAPA)脂质体滴眼液的制备方法及其对大鼠角膜新生血管(CNV)的治疗作用.方法 应用薄膜分散法制备RAPA脂质体滴眼液,以正交设计法研究制备工艺中脂质体制备质量的主要影响因素,筛选出较理想处方.用透射电镜观察其形态,以激光散射法测定其粒径大小.将102只健康Wistar大鼠随机分为正常对照组(A组,6只)、碱烧伤空白对照组(B组,24只)、碱烧伤空白脂质体对照组(C组,24只)、碱烧伤RAPA脂质体治疗组(D组,24只)、碱烧伤RAPA豆油治疗组(E组,24只).A组不处理;B、C、D、E均制作右眼碱烧伤动物模型,然后D组应用RAPA脂质体滴眼液滴眼,E组用RAPA豆油滴眼液滴眼,C组用空白脂质体滴眼液滴眼,B组不予处理.术后第1天开始每日裂隙灯显微镜观察眼局部情况,记录碱烧伤后4、7、14 d大鼠角膜反应与CNV生长情况.结果 所制RAPA脂质体为完整的类球形,平均粒径145.2 nm,包封率90.02%,可作为滴眼液用于动物实验.4组大鼠CNV面积均随时间增加而增加,差异均有显著性(F=906.43~1 682.16,P<0.01).碱烧伤4、7、14d时,B、C组CNV面积比较差异无显著性(P>0.05);其余各组间比较,D、E组CNV面积较B、C组减小,D组面积较E组减小,差异均有显著性(F=309.37~574.92,q=11.98~47.90,P<0.01).结论 脂质体是RAPA良好的药物载体.RAPA能显著抑制CNV生长,有望成为防治CNV的一种有效方法.
病人,男,43岁,因右眼渐进性视物不清1年入院.病人无眼红、眼痛,无异常分泌物,否认有高血压、糖尿病等全身疾病,无手术史、药物过敏史及跟部外伤史.眼科检查:视力OD为0.25,验光视力不提高,眼压2.67 kPa,右眼角膜透明,前房中深,房水清,瞳孔圆,直径约2.5 mm,对光反应存在,晶状体前囊和皮质不均匀混浊,晶状体颞下方悬韧带部分断裂,眼底视盘界清,色正,C/D约0.3,黄斑中心凹光反射存在,A∶V为2∶3.入院诊断为年龄相关性白内障(右)、晶体半脱位(右).
AIM:To investigate the expression of Fas/FasL in fetal cornea after deep cryopreservation.METHODS: Immunohistochemical SABC method was used to study the expression of Fas/FasL in paraffin-embeded sections of 33 fetal corneae, including both nomal fetal corneae and cryopreserved corneae.RESULTS: All fetal corneal cells Fas/FasL was stained in cytoplasm and cell membrane. The positive expression of Fas protein was detected in all the 33 fetal corneae, so was FasL protein. Fas protein was expressed in corneal epithelial, stromal and endothelial cells. FasL ligand protein, however, was detected in corneal epithelial and endothelial cells, but not in stromal cells. There were no differences in expression of Fas/FasL in either normal fetal corneae or cryopreserved corneae.CONCLUSION: Cryopreservation of fetal cornea does not affect the expression of Fas protein or Fas ligand protein.

12例眼睑恶性肿瘤患者术中采用保存异体睑板作为睑板替代物修复全层眼睑缺损,均无肿瘤复发、上睑下垂、眼睑内外翻,仅2例局部发生倒睫,成功率100%.结果提示异体睑板是较理想的睑板替代物,是眼睑缺损修复的良好方法.
Objective To observe the effect of trabeculectomy with adjustable sutures in patients with glaucoma. Methods Adjustable sutures combined with traditional trabeculectomy were applied to the operation for glaucoma. Results The intraocular pressure was controlled postoperatively and there were no significant differences as compared with traditional trabeculectomy(t=1.946,P0.05). Shallow anterior chamber could be observed to have lower incidence than that of ordinary and recovered rapidly by adjusting the sutures. The difference was significant(χ 2=11.148,P0.05). Conclusion Trabeculectomy with accommodative suture can lower the complications of the operation by adjusting the filtration of aqueous humor.
目的讨论白内障囊外摘除术后囊破裂的术中处理方式及效果.方法对37例(37只眼)白内障囊外摘除术后囊破裂的病例进行回顾性分析.结果后囊中央裂口<5mm,周边缺损<120°者18只眼;后囊缺损而前囊膜周边完整者8只眼,常规法植入人工晶体,术后反应轻微.后囊破裂超过上述范围11只眼,玻璃体处理后行双侧晶体襻悬吊术,术后反应重.术后裸眼视力0.1~0.3者5只眼,0.3~0.5者4只眼,>0.5者28只眼.结论根据后囊破裂的程度和部位,采取正确的处理方式,可以取得较好的临床效果.
目的:比较分析影像检查方法对眼眶皮样囊肿的定位及定性诊断的价值.方法:对21例眼眶皮样囊肿的B超、CT扫描及磁共振成像(MRI)检查结果进行观察分析.结果:B超、CT和MRI均可确定病变位置并提示病变的性质,但B超能较好显示病变内部结构和动态,CT发现病变及确定位置较精确,MRI特征为TiWI、T2WI均为高信号.结论:3种影像检查中,CT检查对眼眶皮样囊肿最有价值,3种手段相结合能得出最佳定性、定位诊断.肿瘤防治杂志,2001:8(4):386-387
Objective To study the induction of anterior chamber associated immune deviation (ACAID) of rabbits.Methods Soluble corneal classified antigens were prepared by glycine method, ACAID was induced by injecting glycine into anterior chamber(AC) of rabbits in different groups. The ear swelling and pathological results of the spleen were investigated. Results Corneal epithelium, endothelium, stroma and the whole corneal soluble antigens could induce ACAID .The induced concentration was 2.0g/L, and the success rate was 90%, 100%, 80% and 100%, respectively. Conclusion Corneal soluble antigen can induce ACAID of rabbits.
目的探讨角膜移植术后病人外周血免疫功能的动态变化.方法检测26例行带环形巩膜全角膜移植术(CST)和76例部分穿透性角膜移植术(PKP)病人的术前和术后不同时期体液与细胞免疫指标,并进行对比分析.结果角膜移植病人的细胞和体液免疫术前与术后比较有显著差异(P<0.05),带环形巩膜全角膜移植术病人体液免疫持续时间较部分穿透性角膜移植术组延长,细胞免疫持续时间差异无显著性.结论带环形巩膜全角膜移植术后体液免疫较部分穿透性角膜移植反应程度高,可能是排斥率高的原因之一.
本文对26例(32眼)白内障患者施行晶状体超声乳化联合人工晶状体植入术,所有患者晶状体核硬度均在Ⅱ~Ⅳ级范围,采用环形撕囊或邮票式截囊,对整体核采用高能量低负压的原则,将其分成数个核碎块,然后采用高负压低能量的原则进行乳化吸出;对软核先采用挖碗法吸出核较硬部分,后直接吸出核板.常见的并发症为后囊破裂、虹膜损伤和内皮水肿等.术后随访1~6mo,术后1wk裸眼视力≥0.5者22眼(68.8%),≥1.0者8眼(25.0%),术后3mo裸眼视力≥0.5者30眼(93.8%),≥1.0者11眼(34.4%).本文对超声乳化联合人工晶状体植入术的手术适应症、手术技巧和术中常见并发症的预防及处理进行了探讨.
