    1Non-incised Papillae Surgical Approach for Intrabony Defects Utilizing Advanced Platelet-Rich Fibrin/xenograft Composite.

    BACKGROUND:The present study aimed to assess and evaluate the clinical and radiographic outcomes of the non-incised papilla surgical approach (NIPSA) with advanced platelet-rich fibrin (A-PRF)/xenograft composite to enhance periodontal regeneration in intrabony defects. METHODS:A total of 10 systemically healthy patients aged 25-55 years with 3- or 2-wall intrabony defects involving the buccal wall and defect depth ≥3 mm were selected. Microsurgical technique NIPSA was employed, and grafting was done using A-PRF/xenograft composite, followed by A-PRF membrane placement. The radiographic cone beam computed tomography measurements of total defect depth and bone density in Hounsfield-Units (HUs) were taken at baseline and at the end of 6 months. Clinical parameters, including probing depth, clinical attachment level, and gingival thickness, were recorded at baseline and at 6 months postoperatively. RESULTS:From baseline to 6-month study showed a mean reduction of 2.69 mm in probing depth and a mean gain of 2.68 mm in clinical attachment and 2.81 mm gain in gingival thickness. Radiographically, a mean reduction of 2.80 mm in total defect depth and gain in bone density from 22.20 to 507.10 HUs was observed. All the parameters showed clinical and statistical improvement. CONCLUSIONS:NIPSA technique, along with the A-PRF/xenograft composite, can be an effective and minimally invasive regenerative procedure for the management of intrabony defects in the maxillary and mandibular anterior region. KEY POINTS:To the best of our knowledge, this is the first study to demonstrate the effectiveness of the non-incised papilla surgical approach technique in combination with advanced platelet-rich fibrin (A-PRF)/xenograft composite for the management of 2- and 3- wall intrabony defects. Minimally invasive non-incised papillae preservation approach effectively maintained the integrity of soft tissues and graft stability, both of which are critical factors for successful regenerative outcomes. A harmonious biomaterial A-PRF/xenograft composite graft utilizes the biological activity of PRFs with structural support of the graft material in escalating the regenerative treatment. PLAIN LANGUAGE SUMMARY:This study assessed a minimally invasive surgical procedure to help restore the bone and supporting tissues around teeth damaged by periodontal disease. Ten healthy adults with specific types of bone defects were treated using this procedure in the anterior region. A biologically active material prepared from the patient's own blood was combined with a bone graft to restore the defect. After 6 months, patients showed significant improvements such as increased gum attachment, gum thickness, and reduced pocket depth. In a three-dimensional scan, bone level and bone density were also increased. These changes indicate that the treated sites healed with stronger and more stable supporting structures. The outcomes suggest that this approach may offer an effective and less traumatic option for managing bone defects in the esthetic region, with encouraging clinical and radiographic outcomes.

    2Rubinstain -Taybi Syndrome: an Unusual Case Report

    RUBINSTAIN -TAYBI SYNDROME is an unusual genetic disorder which affects many systems of the body, characterized by growth and developmental delays, distinctive craniofacial features & intellectual disability. A 12- year- old girl reported in the department of oral medicine and radiology with a chief complaint of decayed teeth. Patient was medically challenged. Extraoral features revealed distinctive cranio-facial appearance with broad thumbs and halluces and intraoral features observed were talon cusp in the maxillary central incisors, narrow maxillary arch, carious teeth and plaque accumulation. Considering the history, clinical characteristics and radiographic investigations, patient diagnosed as chronic irreversible pulpitis w.r.t 11.21.36.42,46. Chronic generalized marginal gingivitis with RUBINSTAIN -TAYBI SYNDROME type I. For effective management and good health of the patient, preventive and curative dental procedure and treatment plan had been taken. As an oral physician, understanding RUBINSTAIN -TAYBI SYNDROME is crucial because it requires a multidisciplinary approach to manage the oral health issues associated with it. Early intervention and continuous support are essential to maintaining the well-being of affected individuals.

    3Energy Dispersive Spectroscopy of Open Dentinal Tubules on Application of Nanocrystalline Hydroxyapatite: an in Vitro Study

    Background Dentinal hypersensitivity is an enigmatic problem that has long bewildered the dental profession and is one domain where man has not yet achieved the elusive “gold standards” of management. Recently, lasers have been used owning to their various modes of action to reduce hypersensitivity. Thus, the aim of the present in vitro study is to evaluate and compare the effect diode laser along with and without Nanocrystalline Hydroxyapatite powder on occlusion of dentinal tubules under scanning electron microscope (SEM). Methodology Twenty specimens were obtained from dentin discs of 2 mm thickness prepared from the cemento-enamel junction (CEJ) portion of 20 extracted human third molar teeth. The specimens were divided in to four groups of 5 specimens each. The specimens were etched in 37% phosphoric acid for 60 seconds to simulate hypersensitivity condition. Specimens in Group 1 (control) received no further treatment. Group 2 (DL) specimens received irradiation with 810 nm diode laser at an output power of 1 W for 60 seconds in continuous wave, non-contact mode. Specimens in Group 3 received treatment with a single application of nanocrystalline hydroxyapatite powder alone, whereas, Group 4 received a combination treatment of Diode laser irradiation followed by application of nanocrystalline hydroxyapatite powder. Results The average tubular diameter of dentinal tubules observed in the specimens belonging to Group 1 (control) was 3.14 microns, and in the Group 2 (diode laser) the average dentinal tubular diameter to be 2.28 microns. Mean diameter of dentinal tubules in Group 3 (nanoHA alone) was 2.41 microns. The specimens belonging to Group 4 (DL + nanoHA) showed average dentinal tubular diameter were 1.74 microns. Conclusion The single application of nanocrystalline hydroxyapatite powder and diode laser irradiation, showed significantly greater tubular occlusion and increased reduction in dentinal tubular diameters showing greater potential in occluding open dentinal tubules when compared to diode laser alone.

    4Concurrent Validity of Cephalometric Analysis Using Smartphone Application (oneceph), Dolphin Imaging Software, and Conventional Method

    Objectives: The objective of the study is to compare the accuracy and reliability of cephalometric measurements using Dolphin Imaging software and Android application (OneCeph) to that of manual tracing. Material and Methods: 36 lateral cephalograms of patients between the ages of 18–25 years with Class I skeletal base were selected for this study. 15 parameters were selected for the study. All the cephalograms were traced manually as standard and then using Dolphin software and OneCeph application after calibration. The values obtained on selected parameters by manual, Dolphin Imaging software, and OneCeph were tabulated, and then agreement of selected cephalometric parameters by Dolphin Imaging software and OneCeph application was analyzed with that of manual tracing. Results: There was no significant difference found for any of the parameters selected done by different methods. Dolphin reported better correlation with regard to sella-nasion-point A (SNA) and sella-nasion-point B (SNB). Regarding A-nasion-point B (ANB), OneCeph reported better correlation. Regarding lower incisor-nasion point B plane [LI-NB (°)] and LI-NB (°) both OneCeph and Dolphin reported similar correlation. Dolphin reported better correlation with regard to N perpendicular to Point A, N perpendicular to Pogonion, Nasolabial angle. Regarding Jarabak’s ratio, OneCeph reported better correlation. Dolphin reported better correlation with regard to frankfort mandibular plane angle (FMA), frankfort mandibular incisor plane angle (FMIA), incisor mandibular plane angle (IMPA), and mandibular plane angle. Intraclass correlation coefficient values of Dolphin and OneCeph applications in relation to the manual method depicted that Dolphin software reported better correlation compared to OneCeph in assessing various parameters, considering the manual conventional method as gold standard reference. Conclusion: Android application (OneCeph) is a better alternative for cephalometric analysis but Dolphin Imaging software is more reliable.

    5Colour Stability of Siloczest LSR 120 Against Cosmesil M511 Maxillofacial Silicones under Simulated Environmental Ageing Conditions

    Background: Maxillofacial prostheses play a vital role in rehabilitating facial form and function, with long-term colour stability being a key determinant of patient satisfaction. These prostheses are routinely exposed to ultraviolet radiation, sweat, and cleaning agents, which may compromise their appearance over time. Aim: To compare the colour stability of two maxillofacial silicone elastomers, Siloczest LSR 120 and Cosmesil M511 under simulated ageing conditions replicating clinical exposures. Materials and methods: A total of 80 samples (40 per material) were fabricated and divided into four subgroups based on ageing conditions: sunlight, artificial sweat, immersion in neutral soap, and a combination of sunlight and sweat. Colour measurements were recorded before and after 6 months of ageing, except for neutral soap (30 hours) using the Commission Internationale de l’Eclairage Lab* ( CIE L*a*b*) system via a spectrophotometer, and ΔE values were calculated. Data were analysed using SPSS v28. Normality was assessed with Kolmogorov-Smirnov and Shapiro-Wilk tests. Independent t-test and one-way analysis of variance with Bonferroni correction were used for inter- and intra-group comparisons. Results: Both materials exhibited colour changes under all conditions. No statistically significant difference was observed between Siloczest LSR 120 and Cosmesil M511 under isolated sunlight or sweat exposure. However, Siloczest LSR 120 showed significantly higher discolouration under neutral soap (ΔE-1.48) and combined sunlight and sweat conditions (ΔE-6.68) against Cosmesil M511 (ΔE-0.68 & 3.82 respectively), (p < 0.05). The combination of sweat and sunlight caused the greatest colour degradation, particularly in Siloczest LSR 120. Conclusion: The results suggest that while Cosmesil M511 remains the more reliable option for long-term aesthetics, Silicozest LSR 120 demonstrates promising performance. Although its colour stability was comparatively poorer under neutral soap and combined exposure, the observed changes remained within clinically acceptable limits for darker skin tones (ΔE < 10.07), indicating that it can still be considered a viable material for clinical use in appropriate contexts.
