Background and purpose:Migraine is a highly prevalent neurological disorder affecting predominantly women and significantly impairing quality of life. The introduction of monoclonal antibodies targeting the calcitonin generelated peptide (CGRP) pathway represents a major advancement in preventive therapy. This study aimed to evaluate the realworld effectiveness of erenumab and fremanezumab in Hungarian migraine patients. Methods:In this retrospective multicenter study, data from 12 Hungarian headache centers were collected between May and Az alábbi dokumentumot magáncélra töltötték le az eLitMed.hu webportálról. A dokumentum felhasználása a szerzői jog szabályozása alá esik. December 2024. A total of 200 patients receiving erenumab or fremanezumab were included. Migraine subtype, sex, and treatment characteristics were recorded. The number of monthly migraine days and triptan use were compared between the 3 months prior to treatment initiation and the third month of therapy. Statistical analyses included paired t-tests, Wilcoxon signedrank tests, and permutation tests where appropriate. Results:Both treatments resulted in a significant reduction in monthly migraine days and triptan use in patients with episodic and chronic migraine (all p < 0.001). In fremanezumab-treated patients, median monthly migraine days decreased from 10.0 to 2.0 in episodic migraine and from 17.5 to 6.0 in chronic migraine. In the erenumab group, mean monthly migraine days decreased from 10.8 to 4.3 (episodic) and from 18.6 to 8.9 (chronic). Triptan use also significantly declined in both groups. Nonresponse was observed in a small proportion of patients. Treatment effectiveness was confirmed in both sexes, although statistical power was limited in male subgroups. Conclusion:Real-world data from Hungarian centers demonstrate that both erenumab and fremanezumab are highly effective in reducing migraine burden and acute medication use within the first three months of therapy. These findings support the use of CGRP-targeting monoclonal antibodies as effective preventive treatments, while highlighting the need for further studies on sex-specific treatment responses.
Background: Approximately 60% of orthopedic surgeons worldwide use tourniquets during primary Total Knee Arthroplasty (TKA) surgeries with varying protocols and without established guidelines or recommendations; we therefore aimed to conduct a systematic review and network meta-analysis comparing these protocols. Methods: Three major databases (PubMed, Cochrane, Embase) were systematically searched in October 2025. Randomized Controlled Trials operating primary Total Knee Arthroplasty comparing any of the four major Tourniquet (TQ) protocols were included. The risk of bias was assessed using the RoB2 tool, and the level of evidence was evaluated using the Confidence in Network Meta-Analysis (CINEMA) method. For continuous outcomes, the mean difference was used as the effect size measure, along with a 95% confidence interval. For dichotomous outcomes, the risk ratio was reported with a 95% confidence interval. Results: From our systematic search, we identified 7023 studies, from which 70 studies were deemed eligible. We also included 15 studies found through reference and citation chasing. The Full Tourniquet protocol (FT) resulted in significantly shorter surgery times (ST); however, the difference was only around 3 minutes, and it also led to a significantly higher risk of Deep Vein Thrombosis (DVT) compared to No Tourniquet (NT). We found no significant difference in Calculated Total Blood Loss (CBL) or in Transfusion requirements between FT and NT; however, using the Cementation Tourniquet (CT) protocol resulted in significantly higher CBL and increased transfusion requirements. Regarding the Length of Hospital Stay (LOS), Straight Leg Raise (SLR), and pain levels, NT performed the best, and the difference was statistically significant compared to the FT protocol. Conclusion: Registration: Overall, NT ranked best across most outcomes, including Deep Vein Thrombosis, LOS, SLR, and postoperative pain levels, while FT ranked best at Intraoperative Blood Loss, Surgery Time, and transfusion requirements.
This recommendation based on recent international guidelines, literature data and our experience deals with the follow-up, complex rehabilitation, palliative and nutritional therapy of breast cancer patients. Practising intensive follow-up is not justified but early detection and curative management of local/regional tumor relapse is still a priority, while special issues (breast cancer risk, pregnancy) must be handled with expertise. Social rehabilitation should be advised by competent social workers. Various diagnostic and therapeutic methods used for physical rehabilitation all along the management of the disease are presented. Detailed recommendations for the diagnosis, pharmacological therapy or alternative management of various psychooncological disorders are provided. A complex nutritional therapy guideline deals with the nutritional status of breast cancer patients, the recommended diet during and following their treatment or the nutrition therapy as a part of palliative care and ethical considerations. The principles of palliative care and its organisation in Hungary are described.
A sleeve gastrectomia évek óta a legnagyobb számban elvégzett metabolikus sebészeti beavatkozás. A műtétet követően jelentkező panaszok miatt hasi CT-vizsgálat történt, amely először a csőgyomor mellett elhelyezkedő haematomát véleményezett, melyet intervenciós radiológus drenált. Bevezetés: A kalcitoningénnel kapcsolatos peptid (CGRP-) útvonal gátlása új terápiás lehetőséget kínál a migrénes rohamok megelőzésében.
