BACKGROUNDWest Nile virus (WNV) is a zoonotic mosquito-borne pathogen increasingly reported in Europe.AIMWe aimed to characterise heterogeneities in the average annual human risk of WNV infection (force of infection, FOI) and in WNV surveillance across Europe.METHODSWe conducted a systematic review following the PRISMA guidelines to identify serological studies on WNV in humans with IgG-based assays in Europe. We then used mathematical models fitted to both age-stratified serosurvey and case data to reconstruct spatially explicit FOI estimates, the sensitivity of syndromic surveillance and age-dependent trends in case reporting.RESULTSWe extracted 92 serosurvey datasets from 21 countries. Based on 10 age-stratified serosurvey datasets from Greece, Hungary, Italy, Romania and Spain and case data from seven countries (Austria, Cyprus, Greece, Hungary, Italy, Romania and Spain), we estimated the WNV FOI for 119 European nomenclature of territorial units for statistics level (NUTS) 0-3 regions. We found evidence of spatial heterogeneities in transmission intensity and estimated that on average less than 0.2% of human WNV infections were notified, with country variability and age-dependent trends in the propensity of reporting WNV disease.CONCLUSIONThis study shows that the intensity of WNV transmission, the average annual incidence of infection and the sensitivity of surveillance are heterogeneous across Europe. Due to differences in case reporting across countries, the incidence of reported WNV cases does not necessarily reflect the same proportion of WNV infections and hence the actual infection incidence, which highlights the importance of conducting WNV seroprevalence surveys.
Advanced age, comorbidities, and immunocompromised states remain major risk factors for severe or persistent COVID-19 despite vaccination and antivirals, underscoring the need for innovative treatments such as adoptive T-cell therapy (ATT). In this prospective single-center study, we evaluated the safety, feasibility, and efficacy of two ATT approaches in immunocompromised patients with high-risk or persistent SARS-CoV-2 infection: interferon-γ cytokine capture system virus-specific T cells (IFN-γ CCS VST, n = 12; median age 59) and CD45RA + T-cell depleted donor lymphocyte infusion (CD45RA+ TCD DLI, n = 11; median age 46). Background The place of extracorporeal membrane oxygenation (ECMO) in resuscitational guidelines has become unquestionable, but the exact time to start the treatment and appropriate patient selection remains unclear. The aim of this study is to find correlations between pre-ECMO parameters and subsequent survival indicators of patients requiring ECMO treatment, which could help intra-/prehospital providers to more easily identify potential ECMO candidates among their patients. Methods This research is a single-center retrospective observational study. The subjects of the research are patients who required ECMO support between January 2019 and November 2024. A total of 101 patients were included in the research (n=101). Results The median age was 49 years (IQR 37.5-62.5). 65.6% were male and 36.4% were female (65 vs. 36). The median duration of ECMO support was 6 days (IQR 6.5-8.5). Overall survival was 29.3% (101 vs. 29). Out of the 101 patients, 63 underwent cardiopulmonary resuscitation (CPR) before starting ECMO, 25 cases were out of hospital cardiac arrest (OHCA) and 38 were in hospital cardiac arrest (IHCA). Return of spontaneous circulation (ROSC) occurred in 18 cases. Cannulation was performed during continuous CPR in 44 cases, with a median low-flow time of 30 minutes (IQR 20-60). No CPR was performed in 39 cases. The group of patients who underwent ECMO with previous continuous CPR had a significantly better survival in the IHCA group (p <.001). An achieved ROSC during CPR was not associated with significantly better survival in comparison with the group that underwent full CPR (p = 0.066), but survival was significantly higher compared to the group of patients who were cannulated during continuous CPR (p = 0.01). Conclusion The key point of ECMO treatment is the selection of the right patients and the initiation of treatment at the right time. The inclusion/exclusion criterias of current guidelines can provide adequate assistance in this matter.
Bevezetés: A Magyar Máltai Szeretetszolgálat gyermekszemészeti szűrőprogramja 2020 óta biztosít gyermekszemészeti ellátást Magyarország felzárkózó településein. Kutatásunk a program első négy évének egészségügyi hatásait vizsgálja, különös tekintettel a fénytörési hibák eloszlására és a tompalátást okozó refraktív faktorok jelenlétére. Célkitűzés: Célunk a program működésének és hatékonyságának bemutatása a „megmentett látás” fogalmával, amely a tompalátás (amblyopia) megelőzésére tett beavatkozásokat jelenti, mint a szemüvegfelírás, a szemészeti műtétek és a tompalátás tapaszolásos kezelése. Tanulmányunk első részében a refraktív hibák előfordulásával és azok korrigálásával foglalkozunk. Módszer: A vizsgált dioptriaeloszlások és -kategóriák a szűrésen kézi videorefraktométer által mért adatok alapján kerültek meghatározásra. A látásszűrés hatékonysági mutatójaként vizsgáltuk az optometristához történő továbbutalást, azon belül is az indokolatlan továbbutalásokat. A szemüvegfelírásoknál külön kategóriát képeztek az amblyogen fénytörési hibákkal rendelkező gyermekek, mivel a korrekció időben történő felírásával megelőzhető a tompalátás kialakulása. Megbeszélés: A szűrési gyakorlat hatékonysága javult, a téves továbbküldési (fals pozitív) arány 2%-on stabilizálódott. A program több mint ötéves jelenléte révén javult a szülői együttműködés és a szemüvegviselési arány, amely 2024-re meghaladta a 70%-ot. Eredményeink alátámasztják a program jelentőségét. Következtetés: A Magyar Máltai Szeretetszolgálat gyermekszemészeti szűrőprogramja egyedülálló Magyarországon, mivel átfogóan vizsgálja a felzárkózó településeken élő gyermekek szemészeti állapotát, és a látásszűréstől a műtétek megszervezéséig széles körű ellátást biztosít. Hosszú távú célja a szemészeti problémák korai felismerésével és korrigálásával az érintett gyermekek életminőségének javítása. Orv Hetil. 2025; 166(30): 1181–1194.
