Exposure to fine particulate matter (PM2.5) is a major global health concern, particularly due to its role in cardiovascular diseases. To investigate the causal effect of PM2.5 on systemic inflammation, we performed a two-sample Mendelian randomization study using genetic data from the IEU OpenGWAS project. We focused on four inflammatory biomarkers: C-reactive protein (CRP), myeloperoxidase (MPO), tumor necrosis factor-alpha (TNF-alpha), and interleukin-6 (IL-6). The primary inverse-variance weighted (IVW) analysis identified significant positive causal associations of PM2.5 exposure with levels of CRP (OR=1.23, 95% CI: 1.02-1.48, p=0.029), MPO (OR=2.26, 95% CI: 1.16-4.42, p=0.017), and IL-6 (OR=1.44, 95% CI: 1.02-2.02, p=0.037). No significant association was found for TNF-alpha (p=0.582). These findings were robust across sensitivity analyses. Our study provides genetic evidence that PM2.5 exposure is causally linked to increased systemic inflammation, underscoring the imperative for stricter air quality regulations and highlighting specific inflammatory pathways for future research.
Myopia has been a major public health problem worldwide, with the highest incidence of myopia in East Asia. Investigating the incidence of myopia (non-cycloplegic) and its associated factors among children in primary and junior high school in underdeveloped regions of China. This observational cohort study was completed in Shihezi, China, which was drawn from nine primary and secondary schools with a follow-up period from 2018 to 2023, all of which were randomly selected in proportion to the number of schools in the city. This study aimed to explore and compare the role of cumulative exposure to four blood pressure (BP) markers [systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), and pulse pressure (PP)] in predicting cardiovascular disease (CVD) risk in the Uyghur population. Background and aimsMaternal malnutrition is a major global public health problem that can lead to serious maternal diseases. This study aimed to analyze and predict the spatio-temporal trends in the burden of maternal disorders attributable to malnutrition, and to provide a basis for scientific improvement of maternal malnutrition and targeted prevention of maternal disorders.MethodsData on maternal disorders attributable to malnutrition, including number of deaths, disability-adjusted life years (DALYs), population attributable fractions (PAFs), age-standardized mortality rates (ASMRs), and age-standardized DALY rates (ASDRs) were obtained from the Global Burden of Disease Study 2019 to describe their epidemiological characteristics by age, region, year, and type of disease. A log-linear regression model was used to calculate the annual percentage change (AAPC) of ASMR or ASDR to reflect their temporal trends. Bayesian age-period-cohort model was used to predict the number of deaths and mortality rates to 2035.ResultsGlobal number of deaths and DALYs for maternal disorders attributable to malnutrition declined by 42.35 and 41.61% from 1990 to 2019, with an AAPC of –3.09 (95% CI: −3.31, −2.88) and –2.98 (95% CI: −3.20, −2.77) for ASMR and ASDR, respectively. The burden was higher among younger pregnant women (20–29 years) in low and low-middle socio-demographic index (SDI) regions, whereas it was higher among older pregnant women (30–39 years) in high SDI region. Both ASMR and ASDR showed a significant decreasing trend with increasing SDI. Maternal hemorrhage had the highest burden of all diseases. Global deaths are predicted to decline from 42,350 in 2019 to 38,461 in 2035, with the ASMR declining from 1.08 (95% UI: 0.38, 1.79) to 0.89 (95% UI: 0.47, 1.31).ConclusionMaternal malnutrition is improving globally, but in the context of the global food crisis, attention needs to be paid to malnutrition in low SDI regions, especially among young pregnant women, and corresponding measures need to be taken to effectively reduce the burden of disease.
Understanding the burden associated with occupational asbestos exposure on a global and regional scale is necessary to implement coordinated prevention and control strategies. By the GBD Study 2019, we conducted a comprehensive assessment of the non-communicable diseases burden attributable to occupational asbestos exposure. In 2019, 239,330 deaths and 4,189,000 disability-adjusted life years (DALYs) worldwide due to occupational asbestos exposure occurred. 1990–2019, deaths and DALYs attributed to occupational asbestos exposure increased by 65.65% and 43.66%, respectively. Age-standardized mortality rate (ASMR) and age-standardized DALYs rate (ASDR) decreased, with the most rapid declines in high Socio-Demographic Index (SDI) regions, with average annual percent change (AAPC) of − 1.05(95%CI: −1.2, −0.89) and −1.53(95%CI: −1.71, −1.36), respectively. Lung cancer, mesothelioma and ovarian cancer were the top three contributors to the increase in deaths and DALYs, accounting for more than 96%. AAPCs of ASMR and ASDR were positively associated with SDI. Global deaths from occupational asbestos exposure were predicted to increase and ASMR to decrease by 2035, mostly in males. Due consideration should be given to the susceptibility of the elderly, the lag of asbestos onset, and the regional differences, and constantly improve the prevention and control measures of occupational asbestos exposure and related diseases.
目的:探讨兵团医学领域科研合作网络现状及演化趋势.方法:采用社会网络分析法,以1996-2021年石河子大学医学院在CNKI数据库中发文数据为研究对象,对发文量、网络密度、关联度、中心性、小世界特性等指标展开分析.结果:发文量整体上呈上升趋势,节点数与关系条数在2016年以前增长较快,网络密度、平均最短路径和关联度均呈上升趋势;近三年科研合作率为54.92%.中心性分析显示,石河子大学医学院、石河子大学第一附属医院等机构在科研合作网络中占据中心位置,预防医学、病理学等学科科研合作频率较高;科研合作网络满足小世界特性.结论:兵团医学领域科研合作网络规模和质量不断提升.
将石河子大学2018级临床医学专业1~4班学生作为实验组,5~8班学生作为对照组.对照组采用传统教学法,实验组在传统教学法的基础上融入课程思政,探索课程思政融入医学统计学课程教学的效果及意义.
ObjectivesUnderstanding the spatio-temporal patterns of the global burden of various diseases resulting from lead exposure is critical for controlling lead pollution and disease prevention.MethodsBased on the 2019 Global Burden of Disease (GBD) framework and methodology, the global, regional, and national burden of 13 level-three diseases attributable to lead exposure were analyzed by disease type, patient age and sex, and year of occurrence. Population attributable fraction (PAF), deaths and disability-adjusted life years (DALYs), age-standardized mortality rate (ASMR) and age-standardized DALYs rate (ASDR) obtained from the GBD 2019 database were used as descriptive indicators, and the average annual percentage change (AAPC) was estimated by a log-linear regression model to reflect the time trend.Results and conclusionsFrom 1990 to 2019, the number of deaths and DALYs resulting from lead exposure increased by 70.19% and 35.26%, respectively; however, the ASMR and ASDR decreased by 20.66% and 29.23%, respectively. Ischemic heart disease (IHD), stroke, and hypertensive heart disease (HHD) showed the highest increases in deaths; IHD, stroke, and diabetes and kidney disease (DKD) had the fastest-growing DALYs. The fastest decline in ASMR and ASDR was seen in stroke, with AAPCs of -1.25 (95% CI [95% confidence interval]: -1.36, -1.14) and -1.66 (95% CI: -1.76, -1.57), respectively. High PAFs occurred mainly in South Asia, East Asia, the Middle East, and North Africa. Age-specific PAFs of DKD resulting from lead exposure were positively correlated with age, whereas the opposite was true for mental disorders (MD), with the burden of lead-induced MD concentrated in children aged 0-6 years. The AAPCs of ASMR and ASDR showed a strong negative correlation with the socio-demographic index. Our findings showed that the global impact of lead exposure and its burden increased from 1990 to 2019 and varied significantly according to age, sex, region, and resulting disease. Effective public health measures and policies should be adopted to prevent and control lead exposure.
目的 建立卷积神经网络(CNN)模型对流式细胞术(FCM)数据进行自动分析,实现急性髓系白血病(AML)的初步诊断,探究将CNN模型应用于FCM数据分析中的可行性.方法 以FlowRepository数据库和新疆维吾尔自治区人民医院临床检测中心获得的骨髓FCM数据进行CNN应用的探索性研究,数据均已被临床确诊是否患有AML.其中,公开数据按照6 ∶ 2 ∶ 2 划分训练集、验证集和测试集,本地数据作为外部测试集;为了使 FCM 数据能够适应 CNN 模型,提出一种基于图像矩阵原理的FCM数据结构,对原始数据进行预处理后,提取与AML初步诊断相关的变量,包括侧向散射光和CD45、CD13、CD33、HLA-DR、CD117、CD34 的各抗原表达水平,将各变量写入矩阵;对训练集使用细胞抽样和数据增强方法增大样本量,在Python中使用keras软件包构建LeNet-5 CNN模型,将训练集和验证集分别用于模型的训练和调参,评价模型在测试集上的性能.结果 CNN在两测试集上识别AML的准确率分别为0.931、0.851,灵敏度为0.667、0.636,特异度为 0.968、0.940,受试者工作特征曲线下面积(AUC)为 0.940 和 0.917.结论 基于提出的FCM数据结构,CNN 模型能够实现对 AML 的初步诊断,表明CNN在FCM数据分析中具有一定的应用价值.
目的 探讨以翻转课堂为核心的混合式教学模式在医学统计学教学中的应用效果和大面积推广的可行性,提高医学统计学的整体教学成效.方法 抽取某医学院2015级490名学生作为研究对象,对医学统计学课程采用翻转课堂为核心的混合式教学模式,课后采用问卷星收集学生对新教学模式的评价,并使用SPSS 20.0软件对学生的实习成绩和期末考试成绩进行统计分析.结果 专业知识提升方面:新教学模式实施后学生的实习成绩平均分为78.77±17.79分,期末考试成绩平均分为78.04±14.68分;经过混合教学上课后,学生对主要内容熟悉的比例达到了94.6%,较上课前有显著提升(P<0.001).综合能力提升方面:在新教学模式下学生的自学能力提升了7.6%(P=0.002).教学评价方面:大部分学生对该课程教学的总体评价较好(占94.2%),认为新教学模式较好(占85.7%),认可网络教学平台在学习过程中的作用(占87.2%),对教师的教授清晰度表示肯定(97.4%).结论 以翻转课堂为核心的混合式教学模式表现出良好的教学成效,可在大样本人群应用,在与医学统计学有相同特点的课程中推广.
目的:定量分析新疆各地州(市)级政府整合城乡居民基本医疗保险政策文件,分析政策工具使用情况,为提高城乡医保整合政策实施的有效性提供建议.方法:检索新疆各地州(市)卫生健康委、政府信息公开处关于医保整合的政策文件,以政策文本为研究对象.从政策工具视角出发,基于政策工具和实施阶段两个维度对政策文件进行编码、统计分类和分析,分析该地区整合政策的政策工具使用情况.结果:收集了21份政策文件,编码589条.从政策工具维度来看,环境型政策工具占比最高为71.14%,其次为供给型和需求型政策工具,分别为17.32%、11.54%;从实施阶段维度来看,第1阶段文件条目数占8.83%,第2阶段条目数占31.58%,第3阶段条目数占59.59%.结论:新疆城乡居民医保整合政策文本中环境型政策工具运用最多,供给型、需求型政策工具不足.
目的 探讨预防医学专业毕业生学业成绩与综合能力的关系.方法 以某高校预防医学专业2011—2015届毕业生为研究对象进行问卷调查,采用改进层析分析法计算毕业生的综合能力,采用Pearson相关分析法探讨学业成绩与综合能力的相关性.结果 毕业生综合能力与公共卫生实践技能、公共卫生理论水平、团队协作能力、组织沟通能力的相关系数分别为0.560、0.552、0.607、0.578,学业成绩与公共卫生实践技能、公共卫生理论水平、团队协作能力、组织沟通能力的相关系数分别为0.325、0.448、0.283、0.207,学业成绩与综合能力的相关系数为0.531.结论 预防医学专业毕业生综合能力与团队协作能力、组织沟通能力、公共卫生实践技能相关性较强;学业成绩与公共卫生理论水平相关性较强,而与公共卫生实践技能、团队协作能力及组织沟通能力相关性较弱.
目的:探索广东省某医科大学基于教学学术的教师发展项目的实施效果及影响因素,为我国医学院校开展基于教学学术的教师发展项目提供参考.方法:选取广东省某医科大学美国国际医学教育与研究促进基金会区域中心2016级和2017级项目成员共38人为研究对象,采用问卷调查法和访谈法评价该项目的实施效果.结果:问卷调查显示学习者增长了医学教育和领导力的理论知识(P<0.001),提高了教学能力、教育研究能力和项目管理能力(P<0.01),从而促进了其职业发展(P<0.001).访谈结果表明基于教学学术的教师发展项目通过改变教师的知识、技能和态度来促成其行为的变化.结论:良好的教学学术环境是教师提高教学质量的必要条件之一,同时医学院校加强基于教学学术的教师发展项目的开发有助于促进教与学.
“新医科”背景下,加强具备临床经验、基本技能、决策能力及人文关怀的复合型医学人才的培养是临床医学教育 的根本目的,也是推动医学事业不断前进的坚实基础。循证医学的提出与发展为新型临床医学教育开辟了新的思路 与方向,也为临床医学实践提供了科学合理、真实客观的决策模式。BOPPPS 是一种注重学生能动性、参与度、活跃 度及提升课堂高阶性、创新性及挑战度的教学模式。本研究将BOPPPS 教学模式融入循证医学课程并做相应教学设 计。
目的:探讨新疆专业公共卫生机构卫生资源供给水平的区域差异及空间分布特征,为优化公共卫生体系资源配置提供参考.方法:利用熵值法测度卫生资源综合供给水平,利用泰尔指数、全局空间自相关及热点分析对其地区差异及空间分布进行分析.结果:新疆各地州专业公共卫生机构卫生资源供给水平空间分布不均衡;卫生资源全局空间相关性不显著;"热点区"主要为昌吉回族自治州,"冷点区"主要为喀什地区.结论:新疆专业公共卫生机构卫生资源供给水平存在不公平性;应针对空间分布特征,统筹规划,合理配置卫生资源.
目的 了解石河子大学医学院非预防医学专业医学统计学课程教学现状,为今后开展医学统计学混合式教学改革提供参考依据.方法 采用自制调查问卷,对石河子大学医学院2014—2016级临床医学、口腔、检验及护理专业已完成医学统计学课程学习的学生进行问卷调查,共1475人.调查内容包括个人一般情况,学生对教师教学能力的评价、网络教学综合平台使用情况及评价、教学模式的选择及评价、课程难易程度的评价5个方面.结果 在网络教学综合平台使用情况及评价方面,仅有18.7%的学生每周利用网络教学平台进行线上学习的时间在3小时以上;在教学模式的选择及评价方面,有62.4%的学生愿意接受多种方式相结合的教学模式;在课程难易程度的评价方面,学生认为最难的章节为卡方检验和参数估计,其次为方差分析、t检验及相关与回归.结论 教师在教学过程中应注重激发学生学习兴趣,增加理论课与实践课课时,改进教学模式,优化教学内容,提高教学效果.
目的:本次对石河子大学预防医学专业2011-2018届本科毕业生就业现状及满意度进行调查,分析影响大学生就业和择业需求的主要原因,指导学生树立正确的就业观,同时为我校预防医学专业本科教育教学发展工作提供借鉴与参考.方法:于2018年9月-10月对预防医学专业2011-2018届本科毕业生进行线上问卷调查.结果:回收有效问卷309份.我校毕业生就业率为93.85%;就业去向以疾控中心和医院为主;对工作薪资水平和工作不可替代性满意度低;就业毕业生自评岗位胜任率达87.50%;毕业生对母校教学总体满意度达98.71%.结论:专业对口难和薪资水平满意度低导致专业对口程度不高、工作满意度低;我校应加强就业指导,提升毕业生就业竞争力.
分析卫生统计学教学效果的影响因素,为开展和完善混合式教学以及提高教学效果提供参考依据.
