Objective Under the closed reduction of external fixator-assisted the BO of distal tibial comminuted fractures.Methods 31 distal tibial comminuted fracture cases were randomly divided into two groups:observation group(16) were treated under the external fixator with closed reduction under image intensifier and Wellenplate fixation;control group(15) with conventional open reduction and internal fixation.Results After being evaluated according to Johner-Wruhs scoring,in observation group,there were 12 cases as being excellent,3 as good,1 as fair;in the control group,there were correspondingly 8,3,3,1.The compared between two groups(P0.05).Conclusions Under the closed reduction of external fixator-assisted the treatment of distal tibial comminuted fractures provides biological fixation with better healing and lower complications rates,which is an effective and economical methods.
目的:探讨腓骨闭合复位内固定方法治疗踝关节骨折疗效分析。方法:将98例踝关节骨折患者随机分为观察组和对照组,观察组45例,对照组53例。对照组采用腓骨常规切开复位内固定术治疗,观察组采用腓骨闭合复位内固定术治疗。结果:根据Baird-Jackson踝关节评分标准评价,观察组优38例,良5例,及格1例,差1例,对照组分别为39、5、4、5例。两组相比有显著性差异。结论腓骨闭合复位内固定方法治疗踝关节骨折具有损伤小,固定可靠、安全,骨折愈合率高的优点。
目的观察抑制miR-125b的表达对人骨髓间充质干细胞(MSC)成骨潜能的影响。方法由健康青年男性髂骨取骨术中抽取骨髓分离培养MSC,将其诱导分化为成骨细胞,通过实时定量PCR检测miR-125b在MSC及其诱导分化细胞中的表达。然后进行miR-125b在MSC中的功能试验,使用miR-125b的抑制物GMR-mi RTM microRNAinhibitors通过RNAi-Mate转染试剂瞬时转染MSC,然后再加入成骨诱导剂进行培养,并同时设立转染microRNAinhibitor NC的阴性对照组。细胞培养7天后,进行细胞形态、碱性磷酸酶(改良钙钴法染色)、钙盐结节(茜素红染色)等项目的检测。结果 miR-125b的表达在MSC向成骨分化过程中呈明显下降。转染miR-125binhibitor后的MSC在培养7天后细胞形态逐渐由梭形变为多角形,与阴性对照组类似;ALP染色显示阴性对照组培养7天后为阳性,而试验组在培养7天就呈现强阳性;茜素红染色显示试验组培养7天后即出现了大小不等的钙盐沉积结节,而阴性对照组则暂未出现钙盐结节。结论 miR-125b inhibitor的转染可以促进MSC的成骨分化潜能,间接证实了miR-125b对人骨髓间充质干细胞成骨潜能的抑制作用。
目的探讨急性膝关节前交叉韧带(ACL)损伤关节镜下采用LARS人工韧带移植重建的近期疗效。方法 2007年6月~2008年8月以LARS人工韧带移植重建术治疗23例急性ACL损伤患者。其中男17例,女6例;年龄21~54岁。损伤原因:运动损伤5例,交通伤14例,其他原因4例。受伤至手术时间3d~3周。手术前后按照Lysholm膝关节评分标准进行评价。结果术后23例均随访10~14个月,平均11.2个月。Lysholm评分从术前(40±4)分增加至术后(90±4)分,差异有统计学意义(P<0.01)。结论关节镜下采用LARS人工韧带移植重建膝关节ACL,患者创伤小,合并症少,且可同时处理合并伤,对于早期稳定膝关节及防止膝关节继发损伤有重要意义。
