为了更加准确的计算车辆动载系数和路面不平整度,以及准确分析路面动响应和动荷载的关系.文中根据沥青路面不同平整度条件,利用MATLAB数学计算软件,通过建立七自由度四轴重载车辆的1/2模型,来计算沥青路面不同平整度的标准差和波形参数之间的关系,同时利用该模型还可以准确地分析出动载系数是如何受到波长、车速、振幅以及轴载影响的.研究结果表明,随着波长的增加,动载系数最大值为1.45;动载系数与振幅和载重、动载之间的关系分别是正相关、负相关和正相关.振幅越大,动载系数随车速的提高增长速度也越快.
Background: There is evidence supporting electroacupuncture (EA) for the treatment of major depressive disorder (MDD), but its characteristics have not been well investigated. Objective: To investigate the effectiveness and characteristics of EA in MDD. Methods: 60 subjects were enrolled—35 in the EA group and 25 in the selective serotonin reuptake inhibitor (SSRI) group based on their preferences—in an 8-week non-randomised controlled clinical trial. The 24-item Hamilton depression rating scale (HAMD-24) and clinical global impression (CGI) were adopted for clinical assessment. The Columbia suicide severity rating scale and adverse event form were used to measure safety and tolerability. The characteristics of EA and SSRIs were compared by analysing seven factors of the HAMD-24. Results: There was no significant difference between the two groups in terms of HAMD-24 response rate after intervention (P>0.05). Patients treated with EA demonstrated a significant reduction in CGI scores (P<0.05) with fewer adverse events compared with SSRIs (P<0.01). Although HAMD-24 factor analysis showed both EA and SSRIs could improve factor scores in cognitive impairment, diurnal variation, retardation, sleep disturbance, anxiety/somatisation and feelings of despair, EA showed greater improvement in anxiety/somatisation and feelings of despair than SSRIs (P<0.05). Conclusions: There was no significant difference between EA and SSRIs in the treatment of MDD with respect to our primary outcome. However, as a potential therapy for MDD, EA appeared to result in greater symptom improvement than SSRI treatment with respect to anxiety/somatisation and feelings of despair. The results of this secondary analysis should be interpreted cautiously given the inherent issues of multiple testing.
目的:观察电针与选择性五羟色胺再摄取抑制剂类抗抑郁药(SSRIs)治疗抑郁症的临床效果及两种干预措施的疗效特点.方法:本研究基于真实世界研究(RWS)原则,纳入60例抑郁症患者,按患者意愿分为电针组(35例)和西药组(25例),电针组选取百会、印堂作为主穴,随证配穴,西药组服用选择性5-羟色胺再摄取抑制剂类药物.治疗8周,治疗结束4周后随访.选用汉密尔顿抑郁量表(HAMD-24)和健康调查简表(SF-36)在治疗前后及随访时进行临床评价.结果:治疗8周后,电针组痊愈率为57.14%,西药组为12%,2组痊愈率比较差异有统计学意义,治疗后HAMD-24总评分差异有统计学意义.2种干预手段治疗前后抑郁症患者生存质量总评分、生理职能、总体健康、精力、社会功能、情感职能、精神健康6个领域评分均差异有统计学意义,其中躯体疼痛、生理机能和总体健康状况领域电针和西药差异有统计学意义.结论:电针与SSRIs均能改善抑郁症患者抑郁程度,提高抑郁症患者的总体生存质量,其中电针对躯体疼痛改善作用明显,SSRIs对生理机能和总体健康状况改善效果明显.
Objective To evaluate the clinical effect of the external validity and security of electro acupuncture in the real treatment environment with the real world study method ( RWS ) . Methods Patients diagnosed with depression by the ICD-10 (n=60) were divided into two groups:(1) electro acu-puncture(EA) group(n=35);(2) western medicine group (n=25). The Hamilton Depression (HAMD) scales, CGI scales, MYMOP scales were used to assess the main curative indexes during each period of the treatment. Results There was no significant difference between these two groups at baseline level (P>0. 05). (1) HAMD:Compared with the western medicine group, the clinical control rate, markedly effective rate, cure rate, effective rate of EA group were significantly better than western medicine group (P<0. 05). In both of the two groups, there were statistically significant differences between baseline score of HAMD and the score in every treatment period(P<0. 05). From the beginning of the fourth week, the HAMD score of EA group was significantly better than that of the western medicine group(P<0. 05). And four weeks after treatment, the score of EA group was still better than the western medicine group; ( 2 ) CGI:After 8 weeks of treatment, Compare with the western medicine group, score of CGI scale showed that severity of the disease, effective evaluation and effective index of EA group was significant differences better than those of western medicine group (P<0. 05);(3) MYMOP:After 8 weeks of treatment, compare with the western medicine group, the score of MYMOP scale, of EA group on symptom 1, symptom 2, activity and general health condition has significant differences ( P<0. 05 ). Conclusion In real clinical conditions, EA treatment could improve the scores of HAMD, CGI and MYMOP significantly in patients with depression. Generally, the effect of EA treatment for depression is better than western medicine.
Objective:To observe the clinical effectiveness of electro-acupuncture( EA) on depression and the correlation between Hamilton Depression Scale ( HAMD) and Traditional Chinese Medicine Dialectical Scales ( TCMDS) .Methods:55 depressive patients were divided into two groups according to patients’ wishes, 32 ca-ses in the treatment group and 23 cases in the control group.HAMD and TCMDS were used to evaluate the re-sults.Results:After 8 weeks'treatment, chi-square test showed no significant difference in the total efficiency (P>0.05).The curative rate of the treatment group was higher than that of the control group.There was a sig-nificant difference between treatment group and control group in TCMDS scores ( P<0.0001 ) .HAMD total score had correlation with TCMDS score.Conclusion:EA treatment is effective for depression and can improve some clinical symptoms of TCM.
Purpose: we will prove the clinical effect of acupunctural treatment to anti-depression. Methods: According to the principle of informed consent and the wishes of patients, the participants were divided into two groups, 35 for acupuncture group(acupuncture treatment), 25 for western medicine group(SSRI). Hamilton scales have been evaluated at the 0,1,2,4,6,8,12 weeks period. Results: 1. The significant difference of the effects between the two groups was detected(P 0.05). 2. There was significant difference of the cure rate between two groups(P<0.05),acupuncture group was significantly better than the western medicine group. 3. On HAMD scores of the two groups after treatment in 4,6,8 weeks, significant differences were presented (P<0.05). From the beginning of the fourth week, the HAMD score of acupuncture group was significantly better than the western medicine group. In both of the two groups, there were statistically significant differences between baseline score of HAMD and the score in every treatment period(P<0.05), 4. Significant difference was shown between the two groups in HAMD score of the followup period. Acupuncture group were better than the western medicine group score. Therefore, persistent effect of acupuncture was better than western medicine group. Objective To study the effect of acupuncture on alleviating side effect of paroxetine and improving quality of life in treating mild or moderate depression.Methods A randomized controlled clinical trial was performed.Inpatients and outpatients with depression (n =72) were randomly divided into electro-acupuncture (EA) and paroxetine group (group A,n =23),manual acupuncture and paroxetine group (group B,n=32) and paroxetine alone group (group C,n=17).Outcome was assessed with Hamilton depression scale(HAMD) at baseline and weeks 1,2,4,and 6.Side effects were assessed with rating scale for side effects(SERS) at baseline and weeks 2,4,and 6.The world health organization quality of life(WHOQOL-BREF) scale on quality of life was given at baseline and week 6.Results There were no differences among the three groups at baseline (P>0.05).Accorling to the HAMD scores,significant difference was seen between group A and C (Z =3.2242,P<0.05),and also between group B and C(Z =L6224,P<0.05).No significant difference was found between the two acupuncture groups (P>0.05).SERS scores in the acupuncture groups were significantly lower than those for paroxetine alone at weeks 2(F=5.48,P<0.05),week 4(F=7.70,P<0.05),and week 6 (F=7.73,P<0.05).WHOQOL-BREF scores were significantly higher in the two acupuncture groups than those in the paroxetine only group in five periods,which were total subjective feelings regarding quality of life (F=4.19,P<0.05),health sensation(F=4.49,P<0.05),physical condition(F=12.46,P<0.05),psychology condition (F=10.52,P<0.05) and circumstances (F=5.36,P<0.05).Conclusion Acupuncture can alleviate side effect of paroxetine and improve quality of life in treating mild or moderate depression.

Objective To analyze correlations between personality traits,life events and depressive symptoms based on structural equation model and to identify risky factors of depressive symptoms.Methods Using stratified random cluster sampling method,industrial workers recruited from Shougang Qianan corporation received questionnaire survey consisted of LES,NEO-FFI and CES-D.Maximum likelihood method was applied to calculate parameters of structural equation model,then the final model was confirmed based on modification indices.Results Depressive symptom(CES-D ≥ 16) prevalence was 41.05% among industrial workers in current study.Neuroticism,extraversion,agreeableness and negative life events were significantly predictive to depressive symptoms (r=0.356,-0.304,-0.135,0.227).Positive and negative life events,neuroticism and extraversion,extraversion and conscientiousness,neuroticism and agreeableness presented high correlation,with coefficients 0.614,-0.469,0.432 and 0.402 respectively.The regression weight of affective symptoms,somatic symptoms and interpersonal relationship in measurement model were 0.779,0.678,0.464.Conclusion The onset of depressive symptoms among industrial workers is remarkably related to neuroticism,extraversion,agreeableness and negative life events.Certain measures are supposed to be conducted for mental nursing and intervention in order to strengthen depression prevention.
目的 通过采用基于患者报告临床结局的方式观察针刺联合西药抗抑郁的疗效.方法 将88例轻、中度抑郁症患者随机分为电针组28例,手针组25例,药物组35例.药物组患者给予盐酸帕罗西汀片,治疗第1、2天每日10 mg,每日1次,从第3天开始剂量增加到每日20 mg,每日1次,共服用6周.手针组在药物组基础上配合针刺治疗,主穴:百会、印堂,基本配穴:风府、风池(双侧)、大椎、内关(双侧)、三阴交(双侧);电针组在药物组基础上配合电针治疗,选穴同手针组,以百会和印堂、双侧风池连接于电针仪正负极,刺激频率选2/15Hz疏密交替波.手针组和电针组均隔天治疗1次,每次针刺30 min,1周治疗3次,疗程6周.观察各组治疗前后汉密尔顿抑郁量表17项(HAMD-17)、基于患者报告的症状量表(MYMOP)评分变化情况,治疗后评价临床疗效.结果 电针组临床疗效总有效率为89.28%、手针组为92.00%、药物组为85.71%,电针组和手针组优于药物组(P<0.05).各组患者治疗后MYMOP各项评分与本组治疗前比较差异均有统计学意义(P<0.05),并且电针组和手针组各项评分明显低于药物组(P<0.05),而电针组和手针组各项评分比较差异无统计学意义(P>0.05).结论 针刺联合西药可以显著降低轻、中度抑郁症患者HAMD-17及MYMOP评分,改善主要症状、总体健康状况和活动情况,优于单纯西药治疗.

目的 观察电针对慢性应激抑郁模型大鼠海马神经干细胞巢蛋白(Nestin)的影响.方法 将雄性SD大鼠按照数字表法随机分为健康组、模型组、电针组和氟西汀组,每组10只.采用慢性不可预知温和应激结合孤养的方法制备模型.电针组选取"印堂穴"和"百会穴",接HANS LH202H型电针仪,频率2 Hz,电流强度0.6 mA,治疗30 min,每天1次,共21 d.氟西汀组大鼠按3.3 mg/kg腹腔注射氟西汀,每天1次,共治疗21 d.用游泳不动时间测行为学,用免疫组化检测海马神经干细胞Nestin的表达.结果 健康组、模型组、电针组、氟西汀组大鼠游泳不动时间分别为(14.40±2.17)s、(37.50±7.69)s、(30.10±4.53)s、(32.10±4.98)s.与健康组比较,模型组大鼠游泳不动时间显著延长(t=7.53,P<0.01),海马神经干细胞Nestin阳性表达降低(P<0.01).与模型组比较,电针组、氟西汀组大鼠游泳不动时间明显减少(F=4.89,P<0.05),海马神经干细胞Nestin阳性表达增强(χ2=12.057,P<0.01),电针组与氟西汀组之间差异无统计学意义(χ2=3.899,P>0.05).结论 应激引起大鼠海马神经干细胞巢蛋白表达降低,而电针刺激可以促进抑郁模型大鼠海马神经干细胞增殖,改善慢性应激大鼠的行为表现.这可能是电针对慢性应激抑郁模型大鼠海马损伤的保护作用机制之一.

Immunological reactions induced by proinflammatory cytokines have been involved in the pathogenesis of depressive disorders. Recent studies showed that Electroacupuncture (EA) was able to reduce depressive symptoms; however, the underlying mechanism and its potential targets remain unknown. In the present study, we used a 21-day chronic restraint stress rats as a model to investigate how EA could alleviate depression. Open field test was carried out to evaluate the depressive symptoms at selected time points. At the end of study, immunohistochemistry (IHC) was performed to detect the expressions of IL-1beta, IL-6, and TGF-beta in hippocampal CA3 region. We found that chronic restraint stress significantly decreased behavioral activities, whereas EA stimulation at points Baihui (GV 20) and Yintang (GV 29) showed protective effect during the test period. In addition, the IL-1beta, IL-6, and TGF-beta increased in rats exposed to chronic restraint stress, while EA downregulated the levels of IL-1beta and IL-6. These findings implied that EA pretreatment could alleviate depression through modulating IL-1beta and IL-6 expression levels in hippocampal CA3 region.
Objective To study the correlation between life event scale (LES) and center for epidemiologic studies depression scale (CES-D) of community residents in Beijing.Methods LES and CES-D were measured in 15 communities in Beijing and the score of CES-D and LES were analyzed by correlation analysis.Results The average score of LES was 4.38±6.08,and the average score of CES-D was(8.76±9.43).18.47% of investigated community residents in Beijing were in depressive state to some extent.The score of LES had significant difference between depressive and non-depressive state (P<0.001).There was a positive correlation between the CES-D and LES scores (r=0.534,P<0.001).Conclusion LES and CES-D can be used urgently to screen and evaluate the depressive state so that to enhance the early identification and intervention of depression.
Objective To observe the therapeutic effect of early intervention with electro-acupuncture(EA) and cognitive behavioral therapy(CBT) on mild depressi on.Methods Ninety mild depression patients were random ized into CBT group,EA group and combined treatment group,with 30 in each. CBT group was intervented with cognitive behavioral therapy,once a week.EA group was given electro-acupuncture at Baihui(GV 20),Yintang(EX-HN3),twice a week.Combined treatment group was given both treatment of CBT group and EA group.The treatm ent course lasted for 8 weeks.The therapeutic effect was observed and compared before and after the treatment according to Hamilton Depression Rating Scale for Depression(HAMD) and Center for Epidemiological Studies Depression Sca le(CES-D).Results The total effective rate of the combined treatment group was 93.33%,which was superior to 68.97% of CBT group and 79.31% of EA group(P0.05).After treatment,the average scores of HAMD and CES-D decreased significantly in the three groups( P0.01),especially in the combined treatment group(P0.05).Conclusion The early intervention with EA and CBT in mild depression is more effective.
