The regulation of guest molecules on the self-assembly system of tetracarboxylic acid derivative (H4 BDETP) at the liquid/solid interface was studied by scanning tunneling microscopy (STM) and density functional theory (DFT) calculations. Coronene (COR) guest molecules induced H4 BDETP to transform from linear assembly structure to diversified nanoporous structures in which COR could be captured. And the regulation of pyridine guest molecules (DPE, BPYB, TYPY) on the mono-component assembly structure of H4 BDETP was achieved by forming O-H N hydrogen bonds with H4 BDETP. The introduction of both COR and pyridine derivatives could destroy the hydrogen-bonded dimers of H4 BDETP, and new O-H O hydrogen bonds were formed between H4 BDETP molecules. In addition, H4 BDETP/pyridine co-assembly structures depended on the central bridging units and the number of pyridine groups in pyridine derivatives. Furthermore, H4 BDETP/TYPY structure underwent structural transformation induced by COR and multi-component assembly structures with thermodynamic stability were constructed. Background: Phyllodes tumors (PTs) account for 0.3-1.0% of all breast tumors and often occur in women aged 35 to 55. They are similar to giant fibroadenomas. PTs are famous for local recurrence. No more than 10% of PTs grow larger than 10 cm. The National Comprehensive Cancer Network (NCCN) guidelines recommend extensive resection with a margin of >= 1 cm for PTs, which is much larger than that required for breast cancer. Positive resection margin is associated with recurrence. However, little is known about whether all subtypes really require radical tumor negative resection margins. Case Description: We report on a 49-year-old woman with a giant borderline PT in her left breast. The tumor was greater than 10.5 cm x 7.0 cm. She had a bilateral benign PT excision in January 2014 and a left benign PT excision in December 2018. A chest computerized tomography (CT) scan and abdomen ultrasound did not reveal distant metastasis. Therefore, left breast mastectomy was performed. Wound healing was satisfactory. Pathological and immunohistochemistry findings showed a borderline PT. Conclusions: As the rare tumor of the breast, PTs pose a great challenge for surgeons. The initial evaluation of PTs relies on a triple evaluation of clinical, radiological, and histological examination. Local recurrence of PTs is more common than distant metastasis. The histology of recurrent tumors is usually identical to that of the primary tumor, or has a tendency to malignancy. Although most surgeons are uncomfortable with PTs with a positive margin, it is reasonable to adopt a "watchful waiting" strategy for benign PTs. The current recommendation that PTs should be extensively resected regardless of tumor size might be revised.
The self-assembled behavior of an unsymmetric molecule(BCDTDA)with one imidazole group as center and benzoic acid group as functional group is studied,and the regulatory behaviors of coronene(COR)and three bipyridine derivatives(named BP,PEBP-C4 and PEBP-C8)on BCDTDA self-assembly structures are also investigated.Based on highly oriented pyrolytic graphite(HOPG)substrate,scanning tunneling microscopy(STM)is used to observe the variation of assembled behaviors at the solid-liquid interface.Because of the concentration effect,BCDTDA molecules can assemble into grids and Kagomés structures in the form of N-H…O hydrogen bonded dimers.BCDTDA molecules still maintain dimeric structures in the regulation of COR and BP molecules to BCDTDA self-assembly.However,PEBP-C4 and PEBP-C8 de-stroy the structure of the dimers,and form a variety of co-assembled structures with BCDTDA.Different guest molecules coordinate the host molecules differently,which makes the experiment more meaning-ful.Combined with density functional theory(DFT)calculation,the discovery of molecular interactions provides a promising strategy for the construction of functional nanostructures and devices.
The supramolecular self-assembly behavior of a pair of low-symmetry tetracarboxylic acid molecules (H4OBDB and H4ADDI) and their co-assembly behavior with TMA as a bridging molecule were studied at the liquid-solid interface. Scanning tunneling microscope (STM) observations revealed that H4OBDB and H4ADDI molecules both tend to form O-shaped dimers but end up forming different types of self-assembly structures. We also investigated the construction of two-component co-assembly structures by mixing H4OBDB or H4ADDI molecules with bridging molecules such as TMA. The two formed co-assembly structures are similar. Based on the analysis of the STM results and the density functional theory (DFT) calculations, the formation mechanism of the assembled structures was revealed.
In this work, the two-dimensional self-assembly and co-assembly behaviors of two tetracarboxylic acid derivatives (H4BDETP and H4BTB) were investigated by scanning tunneling microscopy (STM). H4BDETP molecules self-assembled into linear nanostructures, and H4BTB molecules formed lamellar and tetragonal nanostructures. The formation of a H4BDETP/H4BTB co-assembly nanostructure was closely related to the deposition sequence of H4BDETP and H4BTB on highly oriented pyrolytic graphite (HOPG). The introduction of H4BTB into the self-assembly system of H4BDETP resulted in the emergence of the H4BDETP/H4BTB nanostructure, while the addition of H4BDETP had no effect on the self-assembly system of H4BTB and a H4BDETP/H4BTB co-assembly nanostructure was not obtained.
Based on previous research, a new coassembly formed by a porphyrin derivative (IPETPP), which contains a flexible substituent of m-phthalic acid, is observed with coronene (COR) molecules at a higher concentration. Besides, a fresh IPETPP self-assembly formed at a lower concentration and another new coassembly with COR molecules are obtained. Moreover, the addition of a series of bipyridines alters the diamond arrangement of IPETPP, which enhances the stability of the two-component structures. It is unprecedented that bipyridine derivatives break intermolecular hydrogen bonds containing m-phthalic acid substituents. All the coassemblies are investigated by scanning tunneling microscopy on a highly oriented pyrolytic graphite. Combined with density functional theory, the formation mechanism of the assembled structures is revealed. These results would contribute to understanding the interfacial crystal behaviors and probably provide an efficient pathway to regulate the binary structures.

α-Ketoglutarate (α-KG), a pivotal metabolite in energy metabolism, has been implicated in nonalcoholic fatty liver disease (NAFLD) and several cancers. It is recently proposed that plasma α-KG is a surrogate biomarker of NAFLD. Here, we report the development of a novel "turn-on" chemosensor for α-KG that contains a coumarin moiety as a fluorophore. Using benzothiazole-coumarin (BTC) as inspiration, we designed a probe for calcium ion recognition that possesses a unique fluorophore compared with previously reported probes for α-KG measurement. This chemosensor is based on the specific Schiff base reaction and the calcium ion recognition property of the widely used calcium indicator BTC. The probe was synthesized, and a series of parallel experiments were conducted to optimize the chemical recognition process. Compared to the initial weak fluorescence, a remarkable 7.6-fold enhancement in fluorescence intensity (I/I0 at 495 nm) was observed for the conditions in which the probe (1 μmol/L), α-KG (50 μmol/L), and Ca2+ (100 μmol/L) were incubated at 30 °C in EtOH. The probe displayed good selectivity for α-KG even in an environment with an abundance of amino acids and other interfering species such as glutaric acid. We determined that the quantitative detection range of α-KG in EtOH was between 5 and 50 μmol/L. Finally, probe in serum loaded with α-KG (10 mmol/L) showed a 7.4-fold fluorescence enhancement. In summary, a novel probe for detecting the biomarker α-KG through a typical Schiff base reaction has been discovered. With further optimization, this probe may be a good alternative for detecting the physiological metabolite α-KG.
目的 探讨在甲状腺癌手术中,行Ⅵ区淋巴结清扫时甲状旁腺的保护方法及效果.方法 对172例行甲状腺全切/近全切和甲状腺腺叶及峡部切除术的患者,在术中采用精细化梯次解剖甲状腺被膜的方法,以保护甲状旁腺及其血供.结果 术后有18例患者出现暂时性甲状旁腺功能低下,但所有患者均未发生永久性甲状旁腺功能低下.结论 精细化梯次解剖被膜、原位保护甲状旁腺及其血供、必要的甲状旁腺移植是预防术后甲状旁腺功能减退的关键.
甲状腺结节是甲状腺外科面临的主要问题之一,近年来,随着影像学技术的发展,其发病率有所增加,手术是针对部分甲状腺疾病,尤其是分化型甲状腺癌的有效治疗手段.喉返神经损伤是甲状腺手术常见和严重的并发症之一,尽管目前甲状腺外科已进入“甲状旁腺”时代,但有关喉返神经损伤的报道及医疗纠纷仍是不绝于耳.总结我科256例至少行一侧喉返神经暴露的甲状腺手术患者资料,总结术中显露喉返神经的方法,进一步探讨显露的意义.

Through the questionnaire investigation, we compared the self-worth between high-level athlete and college students of Physical Education major. The results indicated that the scores of masculine high-level athletes were higher than masculine college students of Physical Education major on the total self-worth, psychological and familial special self-worth of social tropism; the scores of feminine high-level athletes were significantly higher than feminine college students of Physical Education major on general self-worth of social tropism, interpersonal familial special self-worth of individual tropism, and psychological and familial special self-worth of social tropism.
Objective:To analyze the therapeutic efficacy and adverse reaction induced by Kanglaite injection(KLT) combined with CEF regimen (cyclophosphamide+epirubicin+5-fluorouracil) in the neoadjuvant chemotherapy of breast cancer. Methods: Seventy seven patients who had stages Ⅰ to Ⅱ breast cancer were divided into 2 groups randomly: the Kanglaite injection combined with neoadjuvant chemotherapy group (the KLT group, n=37) and the conventional neoadjuvant chemotherapy group (the control group, n=40). The chemotherapy cycle was equally 21 d. Evaluations about therapeutic effects were made after three cycles of neo-adjuvant chemotherapy and consequent operation. Results:The effective powers (complete response+partial response) of the KLT group and the control group were 81.08% (30/37 cases) and 47.5% (19/40 cases), respectively. The difference was statistically significant (P0.05). The main adverse effects were grade Ⅱ to Ⅲ gastrointestinal tract indisposition and myelosuppression. The WBC decrease and the incidence rate of nausea and vomiting were obviously lower in the KLT group than those in the control group (P0.05). The life quality of patients in the KLT group was greatly improved than that in the control group (P0.05). Conclusion: The application of KLT combined with the neoadjuvant chemotherapy can improve the therapeutic efficacy, reduce the poisonous side effects of chemotherapy, and therefore improving the life quality of the patients.
目的通过与不保留肋间臂神经(ICBN)的病例患肢前臂内侧皮肤感觉障碍、肩关节活动范围及心理创伤的严重程度进行比较,评价乳癌手术中保留ICBN的疗效。方法2005年共行乳癌手术113例,其中保留ICBN的有21例,均为Ⅰ~Ⅱ期乳癌,在同期中67例Ⅰ~Ⅱ期不保留ICBN的乳癌病例中随机抽取21例,比较2组患肢前臂内侧皮肤感觉障碍范围、肩关节活动范围及心理创伤程度。结果保留ICBN的病例皮肤感觉障碍发生率及发生范围明显低于不保留ICBN者,两者比较有显著差异性(P<0.05);患肢肩关节活动稍好于不保留ICBN者,但无统计学差异(P>0.05),抑郁发生率低于不保留者(P<0.05)。结论乳癌手术保留ICBN的疗效优于不保留ICBN者。
Objective To explore the clinical results and operation experiences for benign breast mass by ultrasound guided mammotome minimally invasive biopsy system (MMIBS). Methods 212 benign breast masses in 186 patients were resected by ultrasound guided MMIBS. Clinical data of 186 patients were retrospectively analyzed. Results Needle position in 186 patients was visualized. Lesions were completely removed in 134 cases of 186 (72%) patients. The complete resection rate for tumors on major pectoral muscle or near areola were 31.5% (6/19) and 33.3% (4/12) respectively. Identified by postoperative ultrasound, 118 out of 134 patients (88.0%) with tumor sizes 0.5 to 2.5 cm and 16 out of 38 patients (42.1% ) with sizes 2.5 to 3.0 cm were completely removed. No lesions larger than 3.0 cm were completely removed. All 52 cases in which the tumors were not completely removed by MMIBS were converted to open surgery. Ultrasound follow-up after 4 weeks showed that all the 134 cases that had had masses completely removed had no residual masses, whereas 6 months after operation, 16 out of the 112 cases proved tumor recurrent necessitating open reoperation in 6 cases and second MMIBS operation in 10 cases, among them one case recurred after six months and received open operation. Conclusions For small benign breast mass, MMIBS has therapeutic effect with significantly minimal invasion.
目的正确认识乳腺患者抑郁症的发生率,评价舍曲林治疗抑郁症的疗效。方法选择2002年5月至2003年4月因乳腺疾病入院的患者,计算其抑郁症的发病率,同时,对符合CCMD-3抑郁发作诊断标准的65例患者服用舍曲林及安慰剂进行随机对照研究,采用HAMD、CGI-SI量表评价抑郁症的症状和疗效。结论219例乳腺患者中,抑郁症的发病率约为30%,舍曲林对乳腺患者的抑郁症治疗有效,可作为一线用药。
肿瘤患者因化疗引起的恶心呕吐被认为是最痛苦的不良反应,有许多患者甚至因其难以忍受而放弃化疗.因此迅速控制此症状对于改善患者生活质量,提高化疗依从性至关重要[1].我们自2004年4月~2005年4月,采用随机分组的方法,对72例接受乳癌辅助化疗的患者,应用奈西雅(雷莫司琼)与恩丹西酮进行止吐疗效对比观察,结果报告如下:
Objective: To discuss the method of decreasing the risk of the injury on laryngeal nerve in the thyroid gland operation.Methods:We could get the conclusion from the analysis of the reason why the patients' recurrent laryngeal nerve get injured. In the analysis the 375 cases were picked among the patients undergone the thyroid gland operation from Sep.2005 to Jan.2007. Results:Among the 375 cases there were 5 patients whose recurrent laryngeal nerve was injured,among which there are three thyroidectomies,one Hashimoto thyroiditis and one thyrocarcinoma. In all these five cases,the injuries were all on the site of abduction when they were observed seen through the laryngoscope. After fifteen days to three months' hormonal and neurotrophic treatment,the patients could recover completely and the vocal cord could also got over which could be examined through the laryngoscope. Conclusions:The majority of the recurrent laryngeal nerve’s injuries can be avoided,it is important to know the nerves anatomical structure and its variation well for the operators,the operators should firstly understand the visual field and the histiostructure of the operation region especially of the recurrent laryngeal nerve.
Objective:To explore the effects and method of modified radical mastectomy by reserving nipple with endoscope-assistant for breast cancer.Methods:from Sept.2004 to Apr.2006,24 cases of breast cancer were performed by modified radical mastectomy by reserving nipple with endoscope-assistant.Results:All patients were treated with no obvious complication.Nether neither local nor distant recurrence had been found for a longest follow-up period of 18.6 months and a median follow-up period of 9.2 months.Conclusion:modified radical mastectomy by reserving nipple with endoscope-assistant is recommended for early breast cancer patients.
