文章以军队院校教学档案室为例,对档案资源利用现状作了问卷调查,从档案资源利用、资源建设宣传、信息服务需求等方面对调查结果进行分析,以此为依据为教学档案室加强档案资源建设、拓宽服务领域提供思路建议.
随着我国文化事业的不断发展,编辑与出版也迎来了深层次的变革.其中,以期刊编辑为改革的重点,传统的期刊编辑已经不能满足现代社会的需求.期刊编辑要提升专业素质与能力,立足时代发展,将工作内容与社会现实需要有机结合,提高工作能力.期刊编辑对于编辑、出版工作的重要性不言而喻,一名优秀的期刊编辑或一支优秀的期刊编辑队伍能够简化编辑工作内容,明确工作主体,将所有的工作环节巧妙整合,运用最高效的手段完成工作任务.鉴于此,期刊编辑只有不断提升素质与能力,才能不被社会淘汰,屹立在时代发展的潮流中.
《白求恩医学杂志》是经国家新闻出版总署批准,由白求恩医务士官学校主办出版,面向国内外公开发行的综合性医药科技类学术刊物.该刊创办于2003年,属于双月刊.自创刊以来,该刊立足于军队,面向社会,坚持传播医药信息、交流学术成果,推广教学、医疗经验,促进军内外医学学术交流.经过多年努力,该刊已被中国核心期刊(遴选)数据库等多家权威数据库收录,为中国学术期刊综合评价数据库统计源期刊.该刊物论文刊载情况在一定程度上反映了军队及地方医疗机构医务工作者的科研水平及研究现状.研究组曾做过"2003~2006 年载文分析"[ 1 ] ,现对2014~2018 年《白求恩医学杂志》5年的载文情况统计分析,以获取期刊论文刊载的基本状况及趋势,为期刊更好发展提供支持.
腹股沟疝及阑尾炎均是临床常见疾病,临床上同时患有上述两种疾病者并不少见.我院近期1患者在行右侧腹股沟斜疝腹腔镜修补术时发现其阑尾粘连于疝环内口并构成疝囊一部分,遂在腹腔镜下同时行右侧腹股沟斜疝修补术和阑尾切除术.现结合本病例手术经验,就腹腔镜同时治疗腹股沟疝及阑尾炎的几个问题作一分析探讨.
卫生士官学校担负着培养军队卫生士官的重要任务,其目标是培养大批"政治坚定信得过、技术优良用得上、献身国防留得住"的优秀卫生士官人才,使其走向全军一线岗位,为基层部队卫勤保障提供人才支撑.卫生士官学员培养过程中,教学、训练和管理三者相互促进、相辅相成.如果说教学、训练是卫生士官学员培养工作的核心任务,那么管理就是这一核心任务的基础,学员队管理是保证卫生士官培养任务顺利完成的前提保障[1].因此,作为学员队管理者,需要结合卫生士官学员的新特点、新变化,不断探索科学有效又贴近岗位需求的学员队管理模式.
军队医学期刊是我国学术期刊的重要组成部分.伴随着军队编制调整和国家报刊体制改革的深化,我们清醒认识到,要以服务改革强军实践、服务军队医学科技创新为目标,大力推进以特色化、规范化为主要内容的精品期刊工程建设.鉴于此,如何提升军队医学期刊的学术质量和影响力,既是现阶段期刊面临的挑战,也是今后努力的方向.
Objective To investigate the protective effects and action mechanism of Salvia miltiorrhiza monomer -IH764-3 on aristolochic acid(AA) induced renal tubular epithelial cell injury .Mtehods The human renal tubular epithelial cell line-HK-C cells were cultured in DMEM medium with 10%fetal calf serum for 72 hours,which were divided into three large groups ( eight small groups ):control group , the culture solution without AA;AA group , AA was added into culture solution with final concentration of 20mg/L;observation group,AA and IH764-3 (final concentration of 20mg/L)and different concentrations of Salvia miltiorrhiza monomer-IH764-3.After cultured for 24h, 48h, 72h, the expression levels of transforming growth factor 1 (TGF-β1),tissue inhibitor of matrix metalloproteinase 1 (TIMP-1), plasminogen activator inhibitor type 1 (PAI-1), matrix metalloproteinase 2 ( MMP-2)in culture medium were detected by enzyme-linked immunosorbent assay ( ELISA) The activity of HK-C cells was measured by MTT assay,and the expression levels of Caspase-3 mRNA were detected by Real-time quantitative PCR.Results After AA stimulation, the expression levels of TGF-β1, TIMP-1,PAI-1 were increased, however, the expression levels of MMP-2 were obviously decreased ( P <0.05).After intervention by Salvia miltiorrhiza monomer -IH764-3, the expression levels of TGF-β1, TIMP-1 and PAI-1 were decreased, however, the expression levels of MMP-2 were increased, in which the group with IH764-3 80mg/L had the best effects ( P <0.05).As compared with that in control group ,the activity of HK-C cells in AA group and observation group was decreased , however , cell apoptosis rates were increased ,moreover, the expression levels of Caspase-3 mRNA were significantly increased ( P <0.05).As compared with that in AA group , the activity of HK-C cells in observation group was obviously increased ,however, cell apoptosis rates were decreased , furthermore, the expression levels of Caspase-3 mRNA were significantly decreased ( P <0.05),with the best effects in IH764-3 80mg/L group ( P <0.05).Conclusion Salvia miltiorrhiza monomer-IH764-3 can obviously enhance the ability of HK-C cells against AA that results in cell damage ,thus,which can protect HK-C cells from injury, moreover, its action mechanism may be related with decreasing the expression levels of TGF-β1, TIMP-1, PAI-1 Caspase-3 and increasing the expressions of MMP-2.
Objective It is to observe the clinical effect of combination of Danshen injection on stage Ⅳdiabetic nephrop-athy.Methods Five hundred and fifty-two patients with stage Ⅳ diabetic nephropathy were randomly divided into 3 groups:control group, Benazepril group, Benazepril+Danshen injection group (combination group), each group had 184 cases.All the patients were treated with insulin with or without hypoglycemic agents and hydrochlorothiazide by oral administration . Benazepril group was given Renazepril 10 mg/d orally, the combination group was added with Danshen injection 400 mgd/by intravenous drip additionally on the basis of treatment in Benazepril group .All the groups were treated for 4weeks.Blood pressure, plasma glucose ,HbAlc, D-dimer, fibrinogen (FIB), homocysteine (Hcy), total cholesterol(TC), Triglyceride (TG), urine AQP2, serum creatinine(SCr),urine protein and glomerular filtration rate (GFR) were measured respectively before and after treatment .Results There was no significant difference in the indexes above before treatment among every group.After treatment, the levels of blood pressure,SCr and urine protein, D-dime,FIB,Hcy,TC,TG were obviously de-creased and the levels of GFR , urine AQP2 were increased significantly in Benazepril group and combination group ( P <0.05 ) ,and the improvements were all better than those in control group .The levels of blood pressure , urine protein , D-dime, FIB, Hcy, TC, TG and SCr were much lower and the level of GFR ,urine AQP2 after treatment was higher in combina-tion group than those in Benazepril group .Conclusion Combination of Benazepril and Danshen injection had good curative effect on diabetic nephropathy , which could enhance GFR and urine AQP 2, decrease the levels of urine protein , D-dime, FIB, Hcy,TC,TG and SCr in the patients with diabetic nephropathy .
丹参具有“活血化淤,养血宁心,宽胸止痛,安神除烦”等功效.现代药理研究发现丹参含有很多成分,主要有两大类:脂溶性的丹参酮类化合物和水溶性的丹参酚酸类化合物.丹参在临床上应用广泛,其药理学研究表明,丹参对泌尿系统、呼吸系统、消化系统、中枢神经系统等均有有益作用[1].丹参单体IH764-3是中国医学科学院血液病研究所从中药丹参中提取的一种单体(分子量158),它在保护肾脏、阻止肺纤维化、抑制胶原蛋白合成、沉积及抗肝纤维化、保护脑细胞、抗动脉粥样硬化及降压等方面有着显著的药理活性.作者就丹参单体IH764-3近年来药理学研究进展及其临床应用前景进行分析,为进一步开发和利用其药理活性提供理论依据.
Objective It is to observe the effect of expression of Salvia miltiorrhiza monomer IH 764-3 on the expression of Aquaporin 2 (AQP2), AQP4 in kidney and AQP1 in lung in rats with diabetic nephropathy , so as to explore the the regula-ting mechanism of Salvia miltiorrhiza monomer IH 764-3 for body fluid metabolism abnormality .Methods 60 healthy SD rats were selected , among which ten as control group , and the other 50 ones were used to establish diabetes model by high glucose and lipoids feeder and low -dose STZ.They the successful models ( n=30 ) were randomly divided into 3 groups: model group, control group and study group, with 10 rats in each group.The control group was given captopril by gavage , study group was given captopril by gabage combined with IH 764-3 by intraperitoneal injection , normal group and control group were given distilled water at the same dose .After 16 weeks'treatment, the rats were killed to get their lung and renal tissue for pathological observation .The expression of protein and mRNA of AQP 2,AQP4 in renal tissue and AQP1 in lung tissue were detected by immunohistochemistry method , Western blot method, RT-PCR method.Results The expression of protein and mRNA of AQP2 and AQP 4 in renal tissue in model group was higher than that in normal group (P<0.05), and that in con-trol group and study group was lower compared with the model group (P<0.05), in study group was lower compared with control group (P<0.05).The expression of protein and mRNA of AQP1 in lung tissue in model group was lower than that in normal group (P<0.05),there was no significant difference in the indexes between study group and normal group (P>0. 05), control group and model group (P>00.5).Conclusion The increase of AQP2 expression in renal tissue and decrease of AQP1 expression in pulmonary tissue may be one of the important mechanism which could induce body fluid metabolism abnor -mality.Salvia miltiorrhiza monomer IH764-3 had regulatory effect on AQP expression , which may be the mechanism it regu-lates body fluid metabolism as well as the basis for preventing and treating diabetes .
临床实践能力是护理士官学员岗位任职基础,是临床护理教学的核心,通过临床教学,将所学理论知识与临床实践技能相结合,达到培养学员综合实践能力的目的,是护理士官能力素质培养的关键环节。护理士官肩负着平时保健康、战时保生命的使命。护理士官具备病情观察、急救等综合能力,可以挽救官兵生命,降低伤亡率,并可以鼓舞士气,提高部队战斗力,是战时抢救的骨干力量[1]。为有效提高护理士官学员临床实践能力,我们对护理士官临床能力培养模式进行了改革创新,构建了新型临床教学模式,取得了一定效果。
胸腰段椎体爆裂骨折后常有骨块突入椎管内而形成对椎管的占位,一般情况下,如果突入椎管内骨块的占有率在40%~50%以上时应给予手术直接干预(椎板减压后对骨块进行处理).本文对1例腰椎体爆裂骨折后骨块突入椎管并形成60%占有率,但没有神经受压的患者,采用术中对伤椎上下椎体适度撑开而使骨块大部分成功复位,从而避免了实施椎板减压手术处理骨块时直接对椎管的损伤.报告如下.
电击伤也称触电,是一定量的电流通过人体引起的机体损伤和功能障碍[1].据统计,在美国每年的总病死率中,因触电事故造成电击死的发生率约为0.5%,其中因低压电源致死者约占60%~ 70%,甚至有因汽车蓄电池受损短路而导致电击伤和电击死的情况[2].电击伤导致的心脏骤停较为常见,而在院前进行及时有效的心肺复苏对电击伤致心跳骤停患者的存活至关重要.我科成功抢救电击伤患者1例,呼吸停止40min,心跳停止50min,经胸外心脏按压、呼吸机辅助呼吸、药物和高压氧等综合抢救,住院114d患者生活基本能自理后出院.本文将抢救过程及治疗体会报道如下.
跟腱末端病是军事训练中的常见损伤,多发生在3000 m越野和跳跃训练中,在基层连队比较常见.作者下部队巡诊中遇到15例患者,将诊治情况报告如下. 1 临床资料 15例患者均为男性,年龄18~ 22岁,服役1年8例,2年3例,3、4年各2例.患病时间:服役1年8例均发生在新兵共同科目训练中;服役2年3例,1例发生在早操训练中,另2例说不清具体训练项目;服役3、4年的4例中,有3例发生在野外驻训中,1例不明原因.
军事训练伤是影响部队正常训练和部队官兵身体素质的重要因素,也是困扰从事军事训练人员的主要问题[1]。只有对其进行全面认识和了解,才能有效预防军事训练伤的发生。在教学中,军事训练伤防治相对于其他临床课而言,更具有抽象性和概念性。教员如单纯从理论到理论,概念到概念,学员就会形成从书本到书本,从记忆到记忆的单纯学习程序,待其毕业后的工作能力可想而知。本文尝试运用基层部队训练中的视频案例进行教学,取得较好效果。
19岁男性患者,膝关节伤后疼痛2 h就诊, X线示骨质无异常,按软组织伤处理。1周后无缓解再次就诊,行MRI检查,于胫骨上端内侧可见T1WI低信号影和短时反转恢复序列( STIR序列)高信号影,按骨挫伤诊治。
预选卫生士官作为基层部队的卫生保障人员,接触的是基层部队官兵,所从事的主要工作是在第一时间第一现场发现和处置相关损伤和疾病,当然也包括基层部队的卫生防疫、战救技术等方面的内容。通过多年来对基层部队损伤与疾病发生情况的调研,切身感受到连队卫生员的工作性质以及熟练掌握相应的理论和技能的重要性。因此我们认为,预选卫生士官的课程设置,除了已有的课程外,应以连队常见、多发伤病为基础,授以简明扼要的医学理论和常用的基本技能。为使学员在有限的时间内掌握所需的理论和技能,我们从以下几个方面进行探讨。
<正>肩锁关节是由肩胛骨的肩峰和锁骨的肩峰端共同构成的微动关节,肩锁关节脱位占肩部脱位的12%,占肩部损伤的3.25%[1],属较少见且诊断有一定难度的外伤改变。现总结我院2008年5月-2011年5月部分肩锁关节脱位病例的临床资料,报道如下。
目的观察脑苷肌肽注射液治疗脑干出血的临床疗效。方法 32例患者随机分为治疗组和对照组各16例,对照组给予常规综合治疗,治疗组在对照组治疗基础上应用脑苷肌肽注射液治疗,比较2组临床疗效。结果治疗组总有效率为81.25%高于对照组的43.28%,病死率为6.25%低于对照组的37.44%,差异均有统计学意义(P<0.05)。结论脑苷肌肽注射液治疗脑干出血效果显著,值得临床推广。
