Objective:To investigate the number and allocation of rehabilitation beds of medical institutions in Shanghai,to identify the problems and to put forward suggestions on the construction of rehabilitation medical service system.Methods:Questionnaire survey was conducted in medical institutions,and the results were compared with relevant policy and planning demands.Results:There were 2 687 rehabilitation beds set in the investigated institutions in total. The setting rates of rehabilitation beds in tertiary general hospitals,secondary general hospitals,rehabilitation hospitals and community health service centers were 27.28%,44.44%,100.00% and 8.82%,while the share of rehabilitation beds were 6.44%,50.91%,29.77% and 8.49%,respectively. The proportions of tertiary general hospitals,secondary general hospitals and rehabilitation hospitals whose rehabilitation physician to bed ratio,therapist to bed ratio and nurse to bed ratio met the policy and planning demands were 14.29%,8.57% and 22.86%.Conclusion:The rehabilitation beds were inadequate in quantity and mainly allocated in secondary general hospitals and rehabilitation hospitals. Therefore,it is suggested that the beds allocation be further improved. The rehabilitation human resources to bed ratios were lower than the policy and planning demands. It is suggested that the cultivation of rehabilitation human resources keep pace with the growth of rehabilitation beds.
Objective: To understand the rehabilitation service situation of community health service centers in Shanghai, analyze the prob-lems and main reasons and provide suggestions of its development .Methods: We investigated the rehabilitation resource such as human re-sources, beds, equipment, referral and services by taking exampling survey and interviewing key insiders.Results:There were 87.18% commu-nities offering rehabilitation services, 106 rehabilitation physicians, 81 rehabilitation therapists and 47 rehabilitation nurses.Only 18.31%of peo-ple who engaged in the rehabilitation work were practicing in rehabilitation.People own the Bachelor’s degree or above accounted for 52.54%. The rehabilitation equipment was equipped with a rate of 12.22%.Only 14.71% communities signed the rehabilitation referral agreement.Con-clusion: The basic facility of rehabilitation was weak in community health service centers and more investment of government was needed .Tradi-tional rehabilitation services took a large proportion in communities while modern rehabilitation services needed to be strengthened .The situation of rehabilitation referral was poor and the referral mechanism needed to be improved.
Objective:To explore the configuration and overall utilization of rehabilitation equipment in medical institutions in Shanghai and to find out the problems. Methods:A self-designed questionnaire survey was carried out in three types of medical institutions in Shanghai to analyze configuration,utilization and the fact of up to standard. Results:The configuration rates of rehabilitation equipment in general hospitals,rehabilitation hospitals and community health service centers were 50.79%,71.15%and 17.77%,respectively. The average configuration rates of different equipment ranged from 0.00%to 78.89%,and seven equipment were not equipped at all. The average scores of overall utilization were 8.22,8.86 and 6.83,respectively. Conclusion:The configuration rates of rehabilitation equipment were low in three types of medical institutions in Shanghai,so essential equipment configuration needs to be improved. The overall utilization of rehabilitation equipment was uneven. It is suggested that the utilization be enhanced and the configuration be combined with modern rehabilitation concept.
Objective:To understand the flow situation and problems of rehabilitation manpower from 2011 to 2014 in Shanghai and to pro ̄vide suggestions of Shanghai rehabilitation team construction. Methods:Questionnaires were surveyed based on sampling survey to investigate the situation of the flowing manpower,including age,education background and professional title. SPSS 17.0 was applied to analysis the data. Re ̄sults:The amounts of flow manpower were 1376 and inflow was more than outflow. 50.10% inflow person was from new graduates recruits, only 25.00% outflow person continued to engage in rehabilitation work. The inflow person was getting younger and had more highly educated, lower professional title and higher degree of specialization with an average age of 28.72. In which undergraduate education person accounted for 42.13%,no title person accounted for 20.87% and rehabilitation professional accounted for 50.79%. Conclusion:The rehabilitation manpower size had in ̄creased on the basis of keeping stable. The team of inflow person had been optimized in education background,professional title and specialization. The overall attractiveness of the rehabilitation industry needed to be strengthened.
Objective:To analyze the current situation of rehabilitation service provision,and to find out problems. Methods:A self-designed questionnaires survey was carried out to collect data of rehabilitation service provision in three types of medical institutions in Shanghai during 2011 and 2013. Rehabilitation departments setting,service items and services provided by rehabilitation departments to other clinical departments were analyzed. Results:There were 54.95%of general hospitals and 95.59%of community health service centers that provided outpatient rehabilitation services;all of the five specialized rehabilitation hospitals provided both outpatient and inpatient rehabilitation services. The average institution providing rates of outpatient and inpatient service items were 20.77%and 16.04%,respectively;the maximum providing rate was 94.12%;providing rates of three items were zero. The proportion of services provided by rehabilitation departments to internal medicine departments reduced from 92.92%to 76.41%,and that to other clinical departments increased from 7.08%to 23.59%during 2011 and 2013. Conclusion:Three types of medical institutions in Shanghai showed functional differences in rehabilitation service provision,the institution providing rates were low,and the service capacity needs to be further improved. The providing rates of rehabilitation service items were low;differences existed in providing rate among service items;service items provided showed a concentrative trend. The proportions of rehabilitation services provided by rehabilitation departments to other clinical departments in general hospitals showed a reasonable trend.
Objective This study analyzed health supervision agency funds in the process of health supervision system construction in different areas and levels from 2007 to 2012,including the funding of income , expenses, income and expen?diture proportion , as well as regional funding gap. Methods Questionnaires were surveyed based on Systematic Sampling. Excel 2007 and SPSS 18.0 software were applied to analysis the data. Results (1) The total funding increased significantly, compared with 3.14million yuan in 2007, the total funding of health supervision agency in2012 is 4.02million yuan, which increased28.27%. (2) Compared with2.64 million yuan in 2007, the financial provision in 2012 is 3.79 million yuan, which increased 43.91%. (3) The government attaches great importance to the investment funds of western region, in the aspects of total funding and financial provision ,the western region is close to the central region;(4)Compared with the actual work demand and spending of health supervision agency, the government’s investment was still not enough, leading to funding gap, especially the western region, which needs to be solved recently;(5) The health supervision agency funds were unrea?sonable, because there were still 7.34%of service revenues to supply agency funding.
This article provides an overview of key findings in the project“The investigation and analysis of present situation for health inspection industry”:Firstly,Human resources:personnel management tended to be reasonable and aca?demic education was partly enhanced ,but the number of staff was insufficient.Sencondly,Funding status:health inspection of financial provision increased significantly,its sources became more reasonable and its fairness was improving year by year.Thridly,Housing construction:health inspection infrastructure has been improved.Fourthly,Equipment configuration:equipment configuration was improved;the rate of general equipment configuration was higher.Fifthly, Assist in supervised service status: assist in supervised services of health supervision were generally unfolded and the team has been estab?lished.Sixthly, Technical support status:improvements have been acquired in the health inspection and technical support for capacity-building.Finally,Functioning implement status:in comparison with 2006, improvements were made in the 11 functions. Overall, in addition to manpower reduction trend, financing, housing, equipment configuration and Functioning implement were improved, but with health inspection requirements there was still a certain gap in the health inspection agencies in China.
Objective To investigate the status of rehabilitation personnel in Shanghai. Methods Shanghai medical institutions were in-vestigated through self-questionnaire including rehabilitation personnel quantity, basic characteristics of sociology and training from 2011 to 2013. Results There are 2262 rehabilitation personnel on duty in the medical institutions, with 321 rehabilitation doctors, 860 rehabilitation therapists and 666 rehabilitation nurses, 415 people without identity;female rehabilitation doctors, female rehabilitation therapists and fe-male rehabilitation nurses account for 48.91%, 54.77%, 98.80% respectively; 25-34 years old group accounts for 44.24%, 38.84% and 39.79%respectively;working less than 5 years accounts for 30.84%, 65.00%and 60.81%respectively. Rehabilitation doctors with bachelor degree and clinical profession account for 56.70%and 75.07%respectively;rehabilitation therapists with college degree and rehabilitation profession account for 48.37% and 89.19% respectively; rehabilitation nurses with college degree and nursing profession account for 52.25%and 99.55%respectively;rehabilitation physicians, therapists and rehabilitation nurses who didn't accept the professional training of rehabilitation account for 4.36%, 4.07%and 28.68%respectively. Conclusion Shanghai rehabilitation personnel are deficient and some staff has not yet been identified, rehabilitation personnel structure and functional reeducation also need to be reinforced.
Objective To investigate Shanghai rehabilitation resources and services, define the status and problems and provide sugges-tions for making Shanghai rehabilitation health care system planning. Methods Based on the"Structure-Process-Result"theoretical frame-work, the questionnaire survey and field investigation were conducted on medical institutions in term of the rehabilitation departments set-ting, manpower, beds, equipment, housing, income and expenditure, and referral services. Results There were 321 rehabilitation doctors, 860 rehabilitation therapists and 666 rehabilitation nurses, 2199 preparation beds and 2573 real beds in all kinds of rehabilitation medical in-stitutions. The rates of carrying out rehabilitation services were 94.79%in general hospitals, 100.00%in rehabilitation hospitals, 75.56%in community health service centers, rehabilitation equipment rates were 50.79%, 71.15%, 17.77%, and 27.73%, 30.00%, 13.21%of them provided outpatient rehabilitation services. Conclusion Shanghai tertiary rehabilitation medical services architecture has initially formed, however, there are still many problems in the rehabilitation function. The rehabilitation health care system planning should focus on rehabili-tation services capacity, rehabilitation staff team, rehabilitation resources and services referrals.
Objective To explicate the operation of the health inspection function and items of health inspection agen?cies at the county level.Methods 438 agencies at the county level were investigated by stratified cluster sampling to provide the information about the operation of the health inspection function and items of health inspection agencies at the county level.Results (1) of all the items identified and supposed to be carried out by each agency, items confirmed by the investigat?ed institutes accounted 47.77%; (2) the average operational proportion of the investigated institutes was 88.1%, among which east areas91.73%, middle areas 89.21%, west areas 84.83%;(3)among all the 11 functions investigated,9 of which have operational proportions higher than 85%, the operational proportion of case investigation function was the highest, which was 97.55%,while that of health license,the operafiona Lproperfion of health monitoring&evaluation function were 83.56%&70.75%respectively.Conclusion The health inspection function and items were well carried out by health inspec?tion agencies at the county level in China,but more efforts are still needed for functions unconfirmed.
