There are described an algorithm and methods for collecting the evidential base for causing hazard to the health of the population of Zakamensk (the Republic of Buryatia) in the zone of influence of the waste of the past activities of the Dzhidinsky tungsten-molybdenum combine. The source of danger is lying sands, containing a complex of toxic heavy metals - lead, cadmium, manganese, nickel, chromium, etc. The algorithm for the formation of the evidential base included: a general analysis of the sanitary and hygienic situation; assessment in the dynamics of the quality of atmospheric air, drinking water, soil, food; assessment of the risk to the health of the population; epidemiological studies; in-depth clinical surveys of a representative group of city residents (280 people). In the blood of the examinees, heavy metals were identified as biological markers of the exposure. There was evaluated a complex of the response (effect) adequate to effects of laboratory markers. By methods of mathematical statistics there was established the presence of reliable relationships in the system “source of hazard - the quality of the environment - exposure to risk factors - health status.” The hazard to the health of a citizen, due to the negative impact of the factor, was considered to be proven if it was possible to build a continuous step-by-step logical chain of relationships between the source of the harmful effect, the quality of the habitat and the establishment of the fact of the disease. The quality of the habitat of the population of Zakamensk was shown to give rise in unacceptable risks to the health of residents. The main influencing factor is the pollution of atmospheric air, local food products and drinking water from public and private wells. Risks are realized in terms of a level of the primary morbidity rate of the population of the respiratory diseases, endocrine system, digestive organs that is relatively high compared to the matched area. The relationship between health hazard and environmental factors has been fully proved in 14% of the surveyed persons. At the same time, the territory was shown not be characterized as a zone of “ecological disaster” or “an emergency environmental situation”.
On the basis of mathematical modeling of links in the system "environment – human health - control and supervisory actions of Rospotrebnadzor", were obtained data on reliable dependences of mortality and morbidity rates of the population in Russian regions on the frequency of violations of hygienic standards for the content of harmful chemical and biological substances in the air, drinking water and soils of urban and rural settlements. It is shown that in Russia in 2017 harmful influence of environment factors is probabilistically associated with almost 64.9 thousand deaths and more than 5.1 million cases of diseases of children and adults. Rospotrebnadzor prevented about 48.1 thousand deaths and more than 7 million cases of infectious diseases, respiratory diseases, digestion, tumors, skin and subcutaneous tissue diseases, etc., associated mainly with unsatisfactory quality of drinking water and atmospheric air. Over 43.27 million days of economically inactivity have been prevented. The transition to risk-based surveillance increases targeting of control measures and allows to minimize the risks of environmental pollution, to prevent the violation of hygienic requirements of standards by business entities and, accordingly, to substantially reduce the medical and demographic losses associated with the factors of the country's habitat.
The application of the method of spatial analysis in the system of evidence of harm to health when exposed to the waste of past economic activities of the DWMC on the health of the population made it possible to identify the presence of reliable dependencies both for the adult (p < 0.04) and for the children (p < 0.02) of the population of Zakamensk , between the prevalence of the primary incidence of the population in terms of turnover and the remoteness of the population's places of residence to the waste storage sites. The incidence of the population of Zakamensk, living at a distance of 50 to 500 m from the storage sites of DWMC waste, was 1.07–2.58 times higher than in the zone remote from the tailing dams at a distance of more than 500 m, according to the whole list of disease classes. As a result of the research, it was concluded that the methods of spatial analysis can be used to form the evidence base for harm to health.
The article presents the results of complex studies of dust emissions of the mining and processing complex. The main technological processes and the sources operation that produce intense dust emissions are determined. It has been proved that dust emissions of the investigated enterprise contain fine dust dangerous for human health. The results of the studies allowed us to detect precisely the dispersed composition of dust emissions, with the separation of PM10 and PM2.5 fractions, the chemical composition of the dust, and the shape of the particles. Thus, operating with obtained data on dispersed composition of the dust emissions and the specified sedimentation coefficient, we were able to calculate the dispersion of all solid particles and separately the PM10 and PM2.5 at the location of the enterprise. The dust exposure of the population at the targeted zones has been also determined. The obtained concentration values were used for assess health risk level to population living at the border of sanitary protection zone of the enterprise. Due to the obtained results, the enterprise was provided with recommendations on the inclusion of fine particles PM 10 and PM 2.5 in the production control program. In the case of increasing its production capacity was recommended to introduce the environmental measures for the reduction of emission of the finely dispersed fractions PM10 and PM 2.5.
The research results described prove the existence of health injury to the population, living in the areas of exposure to oil storage and transfer objects on the basis of in-depth biomedical research. It is shown that the presence in the blood of children living in the area of the objects' exposure to specific chemical contaminants (benzene, toluene, ethyl benzene, phenol, formaldehyde) was significantly associated with the development of processes connected with toxic metabolites' intoxication and accumulation, cellular immunity activation, damage to the liver cells' membranes, cell regulatory mechanisms' destabilization, regardless of age.
In article questions of estimation the risk to health of the population caused by influence of chemicals, arriving in a human body with emissions from motor transport are considered. The quantitative characteristic of not cancerogenic and cancerogenic risk for children's and adult population of the city formed by mobile polluters of free air is given. Actions for minimization of risk to health of the population in large industrial centre are proved.
In article questions of estimation of cancerogenic risk to population health caused by influence of chemical sub-stances, acting in an organism of the person with emissions from stationary sources, motor transport and with potable water, for problems of social-hygienic monitoring are examined. Generalization and analysis of data of free air and potable water quality are done. Procedure of estimation the risk of inhalation and water peroral ways of entering the harmful substances for a population health living in territory of Perm is lead. The quantity indicator of cancerogenic risk for nursery and adult population of city is given. For problems of optimization the system of social-hygienic monitoring the priority chemical substances forming the greatest contributions to risk of health breaking are allo-cated. Zoning the city territory on risk of cancerogenic inhalation and peroral influences of chemical substances of free air and potable water of the centralized economic-drinking water supply net is lead.
Questions of the estimation of risk to health of the population at impact of the chemical substances act-ing with potable water from network of centralized economic-drinking water supply are considered. Gen-eralization and analysis of potable water quality is executed. The quantitative characteristic of cancero-genic and not cancerogenic risk for children's and adult population of city is given. The priority chemical substances forming the greatest contributions to risk of health hazard are allocated. Zoning territory of city on risk of cancerogenic and not cancerogenic impacts of chemical substances in potable water from network of economic-drinking water supply is lead. It is shown, that all children's population (more than 137 thousand people) and almost all adult population (more than 806 thousand people) in large industrial Perm city lives in conditions of negative impact of water factor of inhabitancy which defines the raised level of chronic not cancerogenic and cancerogenic risk for health.
Using got in tube study dioxin level in lactating women breast milk (industrially developed city inhabitants) average daily dioxin dose for breast-fed only children was obtained. Besides dioxin concentration in slow metabolism biomedium was determined.
