Die Nutzung moderner Informations- und Telekommunikationstechniken ermöglicht eine Telerehabilitation neurologischer Defizite im häuslichen Umfeld der Patienten. Die aktuelle Studienlage der rehabilitativen Teletherapie zur Verbesserung von Funktions- und Mobilitätseinbußen aufgrund motorischer Defizite nach einem Schlaganfall wird wiedergegeben. Zwei neurolinguistische „Proof-of-concept“-Studien zur rehabilitativen Effizienzüberprüfung der virtuellen („Screen-to-screen“-)Telesprachtherapie von Aphasie nach einem Schlaganfall und Dysarthrophonie bei der Parkinson-Krankheit im Vergleich zur konventionellen, realen („Face-to-face“-)Therapie werden dargestellt. Die Untersuchungen weisen darauf hin, dass die neurologische Telerehabilitation motorischer und kommunikativer Defizite im häuslichen Umfeld des Patienten mittels Teletherapie ebenso effizient ist wie die konventionelle Rehabilitation. Telerehabilitation ermöglicht unter Erübrigung eines Patiententransfers den posthospitalen, flächendeckenden und wohnortnahen Abschluss einer medizinischen Versorgungskette („stroke chain“) nach einem Schlaganfall.
Die Stroke-Angel-Initiative untersucht den Einsatz von Telemedizin zur Verbesserung der präklinischen Kommunikation zwischen Rettungsdienst und Stroke-Unit beim akuten Schlaganfall.
The use of modern information and telecommunication technologies enables telerehabilitation of neurological deficits in the domestic environment. The current state of studies on rehabilitative teletherapy for improvement of motor function and mobility deficits due to stroke is reviewed. Two neurolinguistic proof of concept studies investigating the efficacy of online interactive telespeech therapy are reported, which compared virtual screen to screen interactive telerehabilitation of aphasia after stroke and dysarthrophonia in Parkinson's disease to conventional face to face rehabilitation. The results of the studies indicate that the neurological rehabilitation of motor and communicative deficits in the domestic environment of patients by means of teletherapy is just as efficient as conventional rehabilitation. Under home-based telerehabilitation patient transfer becomes unnecessary. Rehabilitative Teletherapy is a posthospital component of a cross-sector supply chain for patients with handicaps or impairments due to stroke and other neurological diseases.
The Stroke Angel initiative investigates the implementation of telemedicine for improvement of preclinical communication between emergency medical services (EMS) and stroke units in cases of acute stroke. Stroke Angel is a technical system for the telemedical prenotification of patients in cases of suspected stroke at a stroke unit by the EMS. Within the framework of an observational study, the team has been investigating the effects of the system on door-to-computed tomography (CT) and door-to-needle times as well as the lysis rate in the neighboring regions of Rhon-Grabfeld and Bad Kissingen since 2005. The system supports the acute treatment of neurological emergencies and functions as a catalyst for the interlinking of medical institutions in the region as well as for communication between emergency physicians/EMS and hospital physicians. The use of a computer-based data collection enables a continuous improvement process leading to an acceleration of internal clinical procedures and an increase of the lysis rate with the mortality rate staying constant. Telemedicine is applicable in the preclinical care of acute stroke and, thanks to the computer-based data collection, leads to an increase in process transparency, which helps to improve the internal clinical processes in and around a stroke unit.
ZusammenfassungSchlafstörungen gehören zu den häufigsten nicht motorischen Symptomen bei der Parkinson- Krankheit. In zahlreichen Fällen ist auch die Vigilanz am Tag mit verstärkter Einschlafneigung und Tageschläfrigkeit gestört. Pathophysiologisch liegen überwiegend nondopaminerg vermittelte Störungen und Fehlsteuerungen von im Hirnstamm lokalisierten Neurotransmitterneuronen und den nachgeschalteten Regelkreisen vor. Die Ursachen sind multifaktoriell und überlappen sich nicht selten. Im Vordergrund stehen primäre krankheitsimmanente Ursachen einschließlich psychiatrischer und urologischer nicht motorischer Symptome, hypo- oder hyperdopaminerge Phasen meist im Rahmen der Medikation, nächtliche Schmerzen unterschiedlicher Genese sowie Komorbiditäten wie Restlesslegs- Syndrom, REM-Schlaf-Verhaltensstörung oder ein Schlafapnoe-Syndrom. Entsprechend den vielfältigen und häufig gegenseitig überlappenden Ursachen ist die Therapie auszurichten. Die ursachenbezogene Differenzialdiagnostik und damit die richtige Therapie stellen für den Arzt eine besondere Herausforderung dar. Idealerweise kann die Abklärung durch Patientenselbstbeurteilungsbögen und durch apparative Messverfahren wie Schlafapnoe- Screening oder Polysomnografie ergänzt werden. Schlafstörungen nachts oder ausgeprägte Tageschläfrigkeit beeinflussen die Lebensqualität von Patient und/oder Betreuer in zum Teil beträchtlichem Ausmaß.
Durch Verbesserung der präklinisch-klinischen Schnittstelle bei der akuten Schlaganfallversorgung soll die Zeit bis zum Eintreffen der Patienten in die Stroke Unit möglichst kurz gehalten werden. Eine frühzeitige Information der Klinik ermöglicht es, innerklinische Prozesse zu verbessern. Es werden Vorschläge erarbeitet, wie die Prozesse an der Schnittstelle zu messen, zu bewerten und darauf aufbauend zu verbessern sind. Aus den Erfahrungen im Stroke-Angel-Projekt werden präklinische und klinische Prozesse der Schlaganfallversorgung unter QM-Aspekten analysiert und Definitionen für Messzeitpunkte gegeben. Darauf basierend werden Empfehlungen für eine Integration der präklinischen Prozesse in ein QM-Konzept der Stroke Unit formuliert. Es werden Bewertungskriterien und Kennzahlen für die präklinisch-klinische Schnittstelle definiert. Diese lassen sich unterteilen in den Wissensstand zum Krankheitsbild Schlaganfall, in Prozess- und in Ergebniskriterien (Outcome). Die Prozesskriterien bestehen im Wesentlichen aus den präklinisch und klinisch zu erfassenden Zeiten vom Symptombeginn bis zum CT bzw. dem Beginn der Lysetherapie. Die Ergebniskriterien beziehen sich auf das Outcome, gemessen an der Morbidität (NIHSS), der Mortalitätsrate und der modified Rankin Scale (mRS) nach 3 Monaten. Eine präzise Datenerfassung und -auswertung präklinischer und innerklinischer Prozesszeiten und Outcomeparameter führt zur Evaluationsmöglichkeit und Verbesserung der Schlaganfallversorgung. Eine Integration dieser Prozesse und Analysen sollte in das Qualitätsmanagement einer Stroke Unit und des Rettungsdienstes erfolgen.
Introduction: Since the introduction of hospital payment according to fixed rates on a case-by-case basis (introduced 2004), treatment of patients has changed. From a purely economic perspective, it is advisable to release patients earlier than the average duration of stay (German: mittlere Verweildauer, mVD). This contribution aims to describe the development of the mVD and assessment relations (German: Bewertungsrelationen, BWR) for neurology from 2008 to 2012 by using relevant neurologic base-DRGs. Method: The data are based on the case rate catalogues (German: Fallpauschalenkatalog; FPK) from 2008 to 2012. According to anonymized data from Rhoen-Klinikum AG (RKA)'s clinics, the top 15 base-DRGs as well as the top 40 DRGs were determined. The DRGs were compared according to the DRG texts without migration tables. The top 15 base DRGs as well as the top 40 DRGs were then analyzed in order to compare the case mix index (CMI) and the average duration of stay (mVD) for each case. Results: The top 15 DRGs are made up of 21 452 cases (80.77%). Using 3 base DRGs (B70, B68, B77), the most important developments are described. These are a parallel decline in mVD and BWR, the deprecation of "uncomplicated" DRG splits and a relative upgrade of "complicated" DRG splits as well as increasing BWR and falling mVD in DRGs with already short durations of stay. The anonymized top 40 individual DRGs include 21 319 cases (78%). The 5-year results are: the mVD decreased from 7.7 to 6.6 days (-15.4%), while the CMI fell from 1,071 to 1,000 (-6.7%). Discussion The shorter duration of stay, felt in clinical day-to-day routine, is also found in the FPK. At the same time, the DRG proceeds are decreasing, showing a correlation between duration of stay and proceeds. If the actual duration of stay permanently remains below the average of the FPK, the BWR will further fall. Thus, with increasing costs, clinics will have to treat more and more patients to achieve the same profits. Furthermore, in-patient neurology is running the risk of having individual indications falling out of the in-patient sector. This is likely to be a nation-wide development. It has to be noted that the FPK is a learning system. The case data of each year are considered in the calculation of the case rates of the year after the following. Thus, it is our opinion that measures have to be taken by the neurological professional society to move the correlation of the duration of stay and the financial situation into a medically justifiable development.
Schlaganfälle sind in Industrienationen die dritthäufigste Todesursache. Hauptursache ist Arteriosklerose. Regelmäßige körperliche Aktivität beeinflusst verschiedene zerebrovaskuläre Risikofaktoren günstig (arterielle Hypertonie, Diabetes mellitus, Dyslipidämien, Adipositas) und beugt damit der Schlaganfallentstehung vor. Nach wie vor nicht generell akzeptiert ist ein direkter präventiver Effekt körperlicher Aktivität unabhängig von diesen atherogenen Faktoren. Die vorliegende Übersicht kommt anhand einer Meta-Analyse von prospektiven randomisierten Kohortenstudien zu dem Ergebnis, dass Hirninfarkte unabhängig von anderen Risikofaktoren bei körperlich aktiven Männern um 27%, Hirnblutungen 40% seltener auftreten als bei inaktiven. Bei Frauen sind die Unterschiede geringer und erreichen das Signifikanzniveau nicht. Körperliche Aktivität ist daher zumindest bei Männern eine effektive, nebenwirkungsfreie und kostenarme Primärprävention von Schlaganfällen.
Introduction: The time that passes before a patient arrives at the stroke unit shall be kept at a minimum by improving the preclinical/clinical interface for the acute treatment of strokes. Informing the clinic at an early point makes it possible to improve internal processes in the hospital. Suggestions regarding the measurement and evaluation of processes at the interface and their improvement based on these results are to be developed. Method: Experiences from the Stroke Angel project serve to analyse the preclinical and clinical processes of stroke treatment under consideration of aspects of quality management and to give definitions for measuring points. Based on these results, recommendations are given for the integration of preclinical processes into a quality management concept for stroke units, with the target of achieving constant evaluation and improvement of the preclinical/clinical interface. Results: Evaluation criteria and key figures for the preclinical/clinical interface have been determined. They can be divided into the knowledge of the clinical picture of apoplexy, process, and outcome criteria. Process criteria are mainly the preclinically and clinically acquired times between the onset of symptoms and the performance of a CT or the start of lysis therapy. Outcome criteria are related to the outcome of the disease measured by morbidity (NIHSS), mortality rate and modified Rankin scale (mRS) after 3 months. Conclusion: Precise collection and evaluation of data concerning the preclinical and intraclinical process times and outcome parameters lead to the possibility of evaluation and improvement of the treatment of apoplexy in Germany. These processes and analyses should be integrated into the quality management of a stroke unit and the rescue services.

Seit Einführung der Krankenhausvergütung nach den Fallpauschalen (2004) hat sich die Patientenversorgung verändert. Aus rein ökonomischer Sicht empfiehlt es sich, die Patienten möglichst unterhalb der mittleren Verweildauer (mVD) zu entlassen. Ziel dieser Arbeit ist, anhand relevanter neurologischer Basis-DRGs die Entwicklung der mVD und Bewertungsrelationen (BWR) für die Neurologie von 2008 bis 2012 darzustellen. Datengrundlage sind die Fallpauschalenkataloge 2008–2012. Anhand der anonymisierten Daten der Konzernkliniken der Rhön-Klinikum AG (RKA) wurden die Top 15 Basis-DRGs sowie die Top 40 DRGs festgestellt und ein Abgleich der DRGs anhand der DRG-Texte ohne Migrationstabelle vorgenommen. Es folgten Analysen der Top 15 Basis-DRGs sowie der Top 40 DRGs zum Vergleich des Case Mix-Index (CMI) und der durchschnittlichen mittleren Verweildauer (mVD)/Fall. Die Top 15 DRG bestehen aus 21 452 Fällen (80,77%). Beispielhaft an 3 Basis-DRG (B70, B68, B77) werden die wichtigsten Entwicklungen dargestellt. Hierbei handelt es sich um eine parallele Abnahme von mVD und BWR, die Abwertung „unkomplizierter“ DRG-Splits und eine relative Aufwertung der „komplizierten“ DRG-Splits sowie bei DRGs mit schon kurzer Verweildauer eine Zunahme der BWR bei fallender mVD. Die anonymisierten Top 40 Einzel-DRG bestehen aus 21 319 Fällen (78%). Das Ergebnis aus den 5 Jahren ist ein Rückgang der mVD von 7,7 auf 6,6 Tage ( − 15,4%) und ein Absinken des CMI von 1,071 auf 1,000 ( − 6,7%). Die im klinischen Alltag gefühlte kürzere Verweildauer findet sich auch im FPK wieder. Parallel hierzu nehmen die DRG-Erlöse ab, sodass sich eine Korrelation zwischen Verweildauer und Erlös zeigt. Liegen permanent die Ist-VD unterhalb der mVD des FPK, sinken die BWR weiter ab. Damit müssen die Kliniken bei steigenden Kosten immer mehr Patienten behandeln, um die gleichen Erlöse zu erwirtschaften. Zusätzlich läuft die stationäre Neurologie Gefahr, dass einzelne Indikationen aus dem stationären Bereich herausfallen. Hierbei dürfte es sich um einen bundesweiten Trend handeln. Zu beachten ist, dass der FPK ein lernendes System ist. Die Falldaten des jeweiligen Jahres gehen in die Berechnung der Fallpauschalen des übernächsten Jahres ein. Somit müssen unserer Meinung nach aus der neurologischen Fachgesellschaft heraus Maßnahmen getroffen werden, die die Korrelation von Verweildauer und Erlössituation in eine medizinisch vertretbare Entwicklung führen.

Regular physical activity has beneficial impacts on several cerebrovascular risks, e.g., arterial hypertension, diabetes mellitus, dyslipidaemia, and obesity, and thus may prevent strokes. A direct preventive effect of regular physical activity on the stroke-incidence, independently from other risk factors, however is still not generally accepted. This review is based on a meta-analysis of prospective, randomized cohort studies and proves that physically active men have a 27% reduced risk of suffering from cerebral infarcts and a 40% reduced risk of cerebral bleedings. In women the differences between active and inactive individuals are less clear and do not reach statistical significance. Physical activity thus serves as an effective and inexpensive primary prevention of strokes, at least in males.
Tri-axial accelerometers are becoming increasingly important and provide new opportunities for medical care. Using the example of multiple sclerosis (MS), this work presents the current utilities and potentials of accelerometers for the diagnosis of and therapy for chronic diseases. By measuring the daily activity of MS patients, accelerometers could help us to observe the course of the disease more objectively. With the measured data, indicators like "active speed " can be gained to objectify the patient's disease status at home. In this study over a period of 2 years, patients with MS wore accelerometers, embedded in the buckle of a belt. The results of the study show that patients were willing to wear the accelerometer devices and that the obtained data reveal differences between healthy people and patients with MS. Future studies are necessary to continue the development of data analysis and to develop indicators for the early recognition of gradual increases of illness when patients are at home.
Objective: This is an overview of the projects in prehospital application of telemedicine in a neurological emergency care. Material and methods: Using the example of telemedical projects in emergency care with main focus on the acute care of stroke, different attempts and possibilities for improving medical care are indicated and first results are reported. Results: Prehospital telemedical projects support the acute care of neurological emergencies and act preclinically as a catalyst for an interlinking (of medical care facilities) in the region and for a better communication between emergency doctors/rescue service and clinic teams. By increasing data analysis based on electronical data programmes (EDP) a continuous process optimisation is possible so that, e.g. in stroke, internal-clinical processes can be accelerated and the rate of lyses under a steady rate of mortality can be raised. Conclusions: Applications of telemedicine are applicable within the prehospital stroke care by the compilation of data based on EDP, a higher transparency of the processes can be achieved and internal clinical pathway can be optimised within the Stroke Unit concept.
