Schilddrüsenkarzinome stellen die häufigsten malignen Tumore des endokrinen Systems dar. In jüngeren Studien wurde gezeigt, dass die MR-Diffusionsbildgebung (DWI) zwischen malignen und benignen Noduli differenzieren kann. Obgleich diese Unterscheidung zweifellos hilfreich ist, wäre eine weitere Abgrenzbarkeit von undifferenzierten und differenzierten Tumoren wichtig, um den optimalen Behandlungsalgorithmus zu identifizieren. Ziel der Studie war es daher zu untersuchen, ob DWI mittels RESOLVE Sequenz differenzierte von undifferenzierten Schilddrüsenkarzinomen unterscheiden kann.
Die Bestimmung der Häufigkeit und des Schweregrads von LAE in einer konsekutiven Gruppe von onkologischen Patienten mit symptomatischen Ereignissen sowie Zufallsbefunden bei zuvor nicht beschriebenen Ereignissen.
In both conditions, post-transplant lymphoproliferative disorder (PTLD) and hemophagocytic lymphohistiocytosis (HLH), infection with Epstein-Barr virus (EBV) is a key mechanism: almost all PTLD in allogeneic stem cell transplantation (alloSCT) is caused by EBV-related neoplastic lymphoproliferation, and secondary HLH is most frequently triggered by EBV infection. Therefore, concomitant EBV-driven PTLD and HLH early after alloSCT require an approach to eliminate EBV and balance immune activation simultaneously. We report on a patient who developed simultaneous PTLD and signs of HLH on day 64 after alloSCT. Treatment was comprised of stopping cyclosporine, short-course dexamethasone, and 3 courses of rituximab. The patient showed full recovery and complete remission of lymphadenopathy. This result indicates that immediate reduction in EBV-carrying B cells by rituximab, suppression of general inflammation, and parallel support of reconstitution of long-term T-cell function, might be an appropriate therapeutic approach in this rare situation.
Portsysteme (PS) haben sich als permanenter venöser Zugang für die Langzeitchemotherapie bewährt und als effizient erwiesen. Die Benutzung von PS ist oft mit diversen mechanischen Komplikationen vergesellschaftet.
BACKGROUND:Totally implantable venous access devices (TIVAD) may be associated with different complications. Certain mechanical port disorders can easily be diagnosed on chest radiographs if the implanted systems are radiopaque and well visible. There are no reports regarding the visibility of TIVAD on chest X-rays.PURPOSE:To assess the radio opacity of TIVAD implanted in the chest as well as type and frequency of mechanical complications of ports on chest X-ray images.MATERIAL AND METHODS:Chest X-rays of 985 patients from the time period 2007-2009 were analyzed retrospectively. In these patients 1190 TIVAD were inserted. All parts of the TIVAD, i.e. port chamber, connection, and port catheter, were checked for their visibility on chest radiographs. An opacity score was used here as follows: ++ well visible; + visible; - partly or completely invisible. Mechanical complications of TIVAD incidentally detected on chest X-ray were also analyzed retrospectively.RESULTS:Nineteen TIVAD models with diverse configuration and visibility of port chambers, connections, and catheters were identified in our study. Eighty-eight percent of the analyzed port systems were well visible or visible on chest radiographs. Twelve percent of the port chambers and catheters were partly visible or completely invisible. In 9% of the TIVAD, different mechanical complications were diagnosed on chest X-ray images.CONCLUSION:TIVADs should be evaluated carefully on every chest X-ray. Ideally, they should be radio-opaque and well visible on thoracic X-ray images. Unfortunately, this is not always the case. Therefore, manufacturers of TIVAD should take into consideration to use exclusively radio-opaque materials that allow sufficient visibility of each port component on chest radiographs.
Ziele: Bei Staging-Untersuchungen von Patienten mit Non-Hodgkin Lymphomen (NHL) ist in seltenen Fällen auch die Niere von der Erkrankung betroffen. Ein Nierenbefall entspricht einem extralymphatischer Organbefall (Ann-Arbor Stadium 4), und kann daher entscheidend das Staging bestimmen.
Ziele: Portsysteme sind integraler Bestandteil in der Behandlung von Tumorpatienten mit Chemotherapeutika. Im Rahmen der Betreuung von Patienten mit Portsystemen verlaufen Komplikationen wie Dislokation, Leckage oder Embolie meist klinisch stumm, können aber bereits durch die konventionelle Röntgen-Thorax-Übersicht diagnostiziert werden. Bei der heutigen Vielzahl an Portmodellen sind bisher Arbeiten ausstehend, die die variable Beurteilbarkeit von Portsystemen in der radiologischen Diagnostik einschließlich der Komplikationen erörtern. Methode: Eine retrospektiven Analyse von Röntgen-Thorax-Aufnahmen an unserem Haus der Maximalversorgung wies für den Zeitraum 2007–2009 überwiegend in onkologische Patienten 1190 eingebrachte Portsysteme nach. Die einzelnen Bestandteile eines Portsystem Kammer, Konnektion und Katheter wurden bezüglich ihrer Sichtbarkeit in den konventionellen Aufnahmen kategorisiert: sehr gut sichtbar; sichtbar; schwach sichtbar oder vollständig unsichtbar. Ergebnis: Es konnten insgesamt 19 unterschiedliche Modelle ausgemacht werden. In 12% der eingebrachten Portsysteme zeigte sich, dass die Portkammer und der Katheter materialbedingt nur schwach sichtbar waren oder gänzlich dieser radiologischen Diagnostik entgingen. In 104 (9%) Portsystemen wurden Komplikationen identifiziert: häufig Kammerdrehung (n=36) oder Pinch off Syndrom (n=35). In drei Fällen (zwei Diskonnektionen und eine Embolie) bestand eine erhebliche, zeitliche Latenz in der radiologischen Diagnosestellung, weil diese Portsysteme und deren Komplikationen in vorhergehenden Aufnahmen nur schwach bzw. nicht sichtbar waren. Schlussfolgerung: Die Ergebnisse zeigen, dass verschiedene Portsysteme der konventionellen Röntgendiagnostik entgehen und damit meist auch die Portsystemkomplikationen längere Zeit undiagnostiziert bleiben. Dahingehend sollte beachtet werden, in der Therapie von Tumorpatienten Portsysteme mit ausschließlich radiopaken Material zu verwenden, die eine suffiziente Beurteilung in Röntgen-Thorax-Aufnahmen ermöglichen.
Kopf-Hals-Paragangliome („head and neck paraganglioma“, HNP) sind seltene endokrine Tumoren, bei denen die Therapieentscheidung vor allem durch die Genetik, Tumorgröße, Lokalisation, Dignität, Patientenalter und Symptomatik bestimmt wird. Die Hauptgründe für ein alternatives operatives oder strahlentherapeutisches Vorgehen der sich in enger Lagebeziehung zu wichtigen Gefäß-/Nervenstrukturen befindlichen Tumoren sind bei beiden Verfahren vergleichbare Langzeitergebnisse bei relativ hoher Morbidität der Operation.
Mutations in C19orf12 have been recently identified as the molecular genetic cause of a subtype of neurodegeneration with brain iron accumulation (NBIA). Given the mitochondrial localization of the gene product the new NBIA subtype was designated mitochondrial membrane protein-associated neurodegeneration. Frequent features in the patients described so far included extrapyramidal signs and pyramidal tract involvement. Here, we report three C19orf12-mutant patients from two families presenting with predominant upper and lower motor neuron dysfunction mimicking amyotrophic lateral sclerosis with juvenile onset. While extrapyramidal signs were absent, all patients showed neuropsychological abnormalities with disinhibited or impulsive behavior. Optic atrophy was present in the simplex case. T2-weighted cranial MRI showed hypointensities suggestive of iron accumulation in the globi pallidi and the midbrain in all patients. Sequence analysis of C19orf12 revealed a novel mutation, p.Gly66del, compound heterozygous with known mutations in all patients. These patients highlight that C19orf12 defects should be considered as a differential diagnosis in patients with juvenile onset motor neuron diseases. Patients have to be examined carefully for neuropsychological abnormalities, optic neuropathy, and signs of brain iron accumulation in MRI.
Bei einer 26-jährigen Patientin wurde bei Vorliegen eines Long-QT-Syndroms im November 2009 ein automatic implantable cardioverter defibrillator (AICD) mit epikardialen Elektroden installiert. Im Rahmen der postoperativen Untersuchung wurde eine erhöhte Reizschwelle bzw. ein schlechtes Sensing festgestellt.
Ziele: Zufallsbefunde in der Brust finden sich selten in computertomographischen Untersuchungen des Thorax. Ziel dieser Studie war es, Zufallsbefunde der Brust retrospektiv zu beurteilen und radiologische Merkmale benigner und maligner Läsionen miteinander zu vergleichen. Methode: Von Januar 2006 bis Dezember 2009 wurden in unserer Einrichtung insgesamt 7711 computertomographische Untersuchungen durchgeführt. Bei 78 Patienten fanden sich Zufallsbefunde der Brust. 49 der identifizierten Patienten wurden daraufhin klinisch, radiologisch und histopathologisch untersucht. Alle Patienten waren weiblich mit einem medianen Alter von 62 Jahren. Ergebnis: 82 Brustläsionen wurden bei den 49 Patienten gefunden. Die mittlere Größe betrug 12.8±10.1mm. Ein primäres Mammakarzinom wurde bei 6, Brustmetastasen in 23 und gutartige Befunde bei 20 Patienten diagnostiziert. Es war kein signifikanter Unterschied zwischen den Brustkrebspatienten und den Patienten mit gutartigen Befunden bezüglich der Charakteristik der Läsionen nachweisbar. Schlussfolgerung: Die Brust als sehr wichtige Region sollte im CT des Thorax sorgfältig beurteilt werden. Da es keine sicheren radiologischen Kriterien zur Unterscheidung zwischen malignen und benignen Brustläsionen im CT gibt, sollten alle Zufallsbefunde durch weitere radiologische/histologische Untersuchungen abgeklärt werden.
Aquired diverticula of the small bowel (not Meckel?s diverticulum) have a prevalence of 1 to 5 % and are relatively common. They are usually asymptomatic. However, in rare cases they can cause unspecific abdominal symptoms and even critical complications that require surgical intervention. Patients with diverticula of the small bowel were selected from all patients treated at an university hospital of maximal care in a retrospective study covering nine years. In 72 patients with diverticula, 47 % were asymptomatic, 47 % had unspecific abdominal symptoms and 6 % presented acute complications. In 83 % diverticula were localised in the duodenum and proximal jejunum. More distal parts of the small intestine were affected with decreasing frequency. A disseminated affection of the entire small bowel was found in 4 % of all patients. Diagnosis was made by endoscopic techniques in most cases (87 %) and to a smaller extent by CT- and MR-imaging and by other methods. The pattern of complications found is similar to earlier studies. Endoscopic techniques have replaced enteroclysis as the most widespread diagnostic technique in diagnosis of small bowel diverticula.
Case ReportIn December 2007, a 54-year-old man was diagnosed with a diffusely growing, poorly differentiated (G3) gastric adenocarcinoma that was staged as T3Nϩ on the basis of an endoscopic ultrasound (EUS), computed tomography (CT) scan, and histologic evaluation of biopsies.Liver metastases and a bone lesion at the T9 level that were suggested by the CT scan were not confirmed by magnetic resonance imaging.The patient's medical history included recurrent gastric ulcers (Helicobacter pylori positive).The patient was enrolled onto a prospective phase II study (Multicenter, Open-Label Phase II Study to Evaluate the Safety and Efficacy of the Trifunctional Bispecific Antibody Catumaxomab [Anti-EpCAM ϫ Anti-CD3] in Patients With Gastric Adenocarcinoma After Neoadjuvant Chemotherapy and Intended Curative Resection) that was designed to evaluate the safety and efficacy of adding intra-and postoperative intraperitoneal (IP) catumaxomab (given at the end of surgery and on postoperative days 7, 10, 13, and 16) to a perioperative standard chemotherapy regimen, followed by gastrectomy.From January 2008 on, and within the preoperative period of the trial, the patient underwent three cycles of chemotherapy beginning at weeks 1, 4, and 7. Epirubicin (50 mg/m 2 ) and cisplatin (60 mg/m 2 ) were given intravenously and oral capecitabine (625 mg/m 2 ) was administered in two daily doses on days 1 through 21.In April 2008, the patient was readmitted for restaging, and a CT scan and gastroscopy/EUS were performed.The CT scan did not show any visceral metastases or nodal manifestations.The gastroscopy demonstrated no evidence of the primary tumor at the aforementioned location macroscopically, but scattered cicatricial areas of mucosa in the previously described location of the tumor were found.Within the possible location of the malignancy, the EUS demonstrated thickening of the stomach wall (maximum diameter of 10 mm), with evidence of a hypoechoic mass with a maximum diameter of 22 mm

69-year-old man was admitted to our hospital withdyspnea, thoracic pain, and palpitations. Clinical exam-ination revealed mild peripheral cyanosis. Right bundlebranch block and atrial flutter were seen electrocardiograph-ically. There was no sign of myocardial infarction or ische-mia. Chest radiographs documented an ill-defined opacity atthe left lung together with a metal foreign body (Figure 1).On echocardiography, dilatation of the pulmonary trunk wassuspected (not shown). Computer tomography demonstrateda giant aneurysmatic dilatation of the pulmonary trunk andleft pulmonary artery (Figure 2).Further questioning revealed that the patient was woundedbyagrenadeexplosionin1945.Togetherwithboyfriends,hehad used the grenade as a toy during a “war game.”According to the patient, his status after injury was unevent-ful, and no trauma of internal organs had been suspected. Inthetimefromthisepisodetoadmission,hehadnoremarkablecomplaints. He was discharged in unchanged condition, as hedid not wish further investigations in our hospital.Traumatic aneurysms of the pulmonary arteries after gun-shot injuries are very rare and have been reported onlysporadically in the literature.
Ziele: Die Häufigkeit von intramuskulären Metastasen (IM) anhand eines großen onkologischen Patientenkollektivs zu beurteilen und ihre radiologischen Zeichen von zu identifizieren. Methode: Retrospektive Suche in der statistischen Datenbank der Martin-Luther-Universität vom Januar 2000 bis zum Dezember 2007 ergab 5.170 Patienten mit metastasierenden soliden Tumoren, die an unserer Einrichtung untersucht und behandelt wurden. Außerdem durchsuchten wir retrospektiv die radiologische Datenbank nach Muskelbefall bei onkologischen Erkrankungen. Ein Staging-CT war bei all diesen Patienten vorhanden und die CT-Bilder jedes Patienten wurden erneut analysiert. IM wurden in 61 (1,2%) Fällen diagnostiziert, darunter 27 Frauen und 34Männer mit einem medianen Alter von 65 Jahren. Ergebnis: 80 Metastasen wurden in den Skelettmuskeln von diesen 61 onkologischen Patienten gefunden. Die Skelettmuskelmetastasen stammten von folgenden malignen Tumoren: genitale Tumoren (24,6%), gastrointestinale Tumoren (21,3%), urologische Tumoren (16,4%) und malignes Melanom (13,1%). Andere Neoplasien waren selten. In 8,2% der Fälle konnte kein Primärtumor identifiziert werden.
