BACKGROUND:The risk of recurrence after a first episode of an anterior shoulder dislocation (ASD) is high with nonoperative treatment in younger patients. In a previous study, arthroscopic labral repair (Bankart repair) reduced the risk of secondary shoulder dislocations and improved functional outcomes versus nonoperative treatment at 2-year follow-up. PURPOSE/HYPOTHESIS:The aim of this study was to compare the results of arthroscopic Bankart repair and nonoperative treatment at a minimum of 6 years' follow-up in patients aged ≤25 years. The hypothesis was that acute surgery would decrease the risk of recurrence and improve functional outcomes. STUDY DESIGN:Randomized controlled trial; Level of evidence, 1. METHODS:We included patients aged between 18 and 25 years after a first episode of an ASD and divided them into 2 groups. The first group was treated surgically with arthroscopic Bankart repair in the initial 2 weeks after the dislocation, and the second group was treated nonoperatively. Both groups were immobilized for 3 weeks in internal rotation and followed the same physical therapy protocol. Follow-up was performed at a minimum of 6 years. The primary outcome measure was the recurrence of instability, defined as another ASD requiring closed reduction or a subluxation. Secondary outcome measures included the need for stabilization surgery; return to sport; and functional outcomes according to the quick version of the Disabilities of the Arm, Shoulder and Hand (QuickDASH), Walch-Duplay score, and Western Ontario Shoulder Instability Index (WOSI). RESULTS:There were 20 patients included in each group. The mean age at the time of inclusion was 21.4 ± 1.8 years. A total of 37 patients were evaluated at a mean follow-up of 81.4 months (6.8 years). In the surgical group, the recurrence of instability (dislocation or subluxation) was significantly lower compared with the nonoperative group (n = 5 [27.8%] vs 17 [89.5%], respectively; P < .0005), and the rate increased in both groups compared with 2-year results (n = 2 [10%] vs 13 [65%], respectively; P = .003). Fewer patients had another episode of a dislocation in the surgical group compared with the nonoperative group (n = 4 [22.2%] vs 15 [79.0%], respectively; P < .003). In the surgical group, all dislocations occurred after 2 years' follow-up, while 6 patients in the nonoperative group already had dislocations at 2 years. The Walch-Duplay score (93.24 vs 76.05 points, respectively; P = .0004), WOSI score (12.12 vs 20.95 points, respectively; P = .009), and QuickDASH score (4.84 vs 16.14 points, respectively; P = .0088) were significantly better in the surgical group than in the nonoperative group. The rate of return to the same or better level of sport was 82% in the surgical group compared with 21% in the nonoperative group (P < .0009). Additionally, 11 patients (29.7%) required primary or secondary shoulder stabilization surgery: 2 (11.1%) in the surgical group and 9 (47.4%) in the nonoperative group (P < .04). CONCLUSION:Arthroscopic labral repair (Bankart repair) reduced the risk of secondary shoulder dislocations and improved functional outcomes versus nonoperative treatment at 6-year follow-up. Surgical treatment after a first episode of a shoulder dislocation could be offered as a primary treatment option in a younger population. REGISTRATION:NCT03315819 (ClinicalTrials.gov).
Introduction La tendinopathie corporéale du tendon calcanéen est une pathologie fréquente dont le traitement endoscopique prend de plus en plus de place dans l’arsenal thérapeutique. L’objectif de notre étude était de comparer les résultats fonctionnels et anatomiques du traitement chirurgical à ciel ouvert et sous endoscopie des tendinopathies achilléennes corporéales. Hypothèse La chirurgie endoscopique donnerait des résultats fonctionnels supérieurs et une récupération plus rapide en comparaison à la chirurgie à ciel ouvert. Méthode Une étude prospective multicentrique, comparative non randomisée incluant tous les patients majeurs opérés d’une tendinopathie achilléenne corporéale résistante au traitement médical a été réalisée. L’évaluation clinique reposait sur les scores EFAS et VISA-A et la reprise du sport. Résultats Soixante patients ont été inclus dont 22 opérés sous endoscopie et 38 à ciel ouvert. Un retard de cicatrisation cutanée a été constaté à la suite d’une chirurgie endoscopique. Aucune lésion nerveuse n’a été rapportée. Aucune différence sur les scores fonctionnels n’était constatée entre ces deux approches. La reprise de la marche était précoce avant 6 semaines et la reprise du sport était progressive à partir du 3e mois postopératoire et possible dans 80 % des cas au 6e mois postopératoire, sans différence significative en fonction de la voie d’abord. Discussion Dans notre étude, la prise en charge chirurgicale des tendinopathies corporéales réfractaires au traitement médical, présentait une faible morbidité, permettait un retour précoce à la marche à 6 semaines et une reprise du sport dans 80 % des cas au 6e mois postopératoire, mais seulement dans un tiers des cas au même niveau. Ces résultats semblaient peu influencés par l’approche chirurgicale endoscopique ou à ciel ouvert. Niveau de preuve III.
Medializing displacement calcaneal osteotomies are part of conservative surgical treatments and represent a reliable option in valgus flatfoot deformities. Favorable short-term results of this procedure is well-known. However, there are few series with follow-up beyond five years. This study reports the clinical outcomes of calcaneal medialization osteotomy with a minimum follow-up of five years. The primary objective was to compare functional scores and radiographic measurements at the preoperative stage, immediately post operative, and at the final follow-up. This was a retrospective, single-centre, multi-operator study of 32 patients, who underwent a medialization calcaneal osteotomy for type II flatfoot. Clinical evaluation of the patients was conducted using the American Orthopaedic Foot and Ankle Surgery (AOFAS) score and the European Foot and Ankle Society (EFAS) score. Radiographic evaluation used the plantar arch angle angle, the talus–first metatarsal axis (T-M1) on lateral weight-bearing radiographs, and calcaneal valgus on Meary’s angle in a hindfoot alignment view. Mean follow-up was seven years. AOFAS score improved from 46 to 87 and EFAS score from 11 to 20 (p < 0.05). Each radiographic parameter was significantly modified between the preoperative and immediate postoperative periods. We observed a significant and lasting improvement in functional scores at a mean follow-up of seven years. The correction of the evaluated radiographic parameters was significant and remained stable over time.
La neuropathie inter-capito-métatarsienne, nommée improprement névrome de Morton, est une cause fréquente des douleurs de l’avant-pied, s’accompagnant d’un retentissement fonctionnel variable. Le diagnostic en est relativement facile, lorsque la symptomatologie est caractéristique, intéressant en particulier le troisième espace inter-capito-métatarsienne, faite de sensations de brûlures, de picotements, de décharges électriques avec ou sans dysesthésie digitale et irradiation proximale ou distale. La sémiologie est assez riche, visant à déclencher les douleurs par différents manœuvres d’irritation du névrome. Enfin, l’échographie et l’IRM sont très performantes dans la mise en évidence du névrome. Le traitement chirurgical ne sera proposé qu’après l’échec d’un traitement conservateur de quelques mois. L’adaptation du chaussage, privilégiant un chaussage large, souple, à talons modérés avec une semelle épaisse est la première étape. Un étirement des chaînes postérieures doit être proposé. Des semelles réduisant les contraintes mécaniques sur l’avant-pied, soulageant l’espace incriminé, sont également à envisager. Le traitement infiltratif parfois échoguidé par dérivés corticoïdes, l’utilisation de l’éthanol, du phénol connaissent un succès variable. Il en est de même pour les ondes de choc, la radiofréquence. Le traitement chirurgical comporte deux options : la neurolyse ou la neurectomie. La neurolyse de proposition récente, libère le nerf en sectionnant le ligament inter-métatarsien et en supprimant les facteurs compressifs. Cette technique peut se faire à ciel ouvert, ou par endoscopie. La neurectomie reste la technique la plus répandue et peut être réalisée par voie dorsale ou plantaire mais la voie interdigitale est la moins agressive pour la peau et n’a pas d’inconvénient secondaire. La neurolyse comme la neurectomie donnent globalement de bons résultats dans 70 à 90 % des cas, stables à long terme. La persistance de douleurs est quelques fois liée à la voie d’abord, en particulier dorsale. Les douleurs dites d’amputation après neurectomie restent très limitées. La neurolyse a l’inconvénient d’une reprise par neurectomie dans 5 à 10 % des cas. Les suites de ces interventions, relevant le plus souvent de l’ambulatoire, sont simples. L’appui complet sur le talon est immédiat. L’appui sur l’avant-pied est variable selon la voie d’abord. Le diagnostic différentiel avec des métatarsalgies statiques est parfois difficile conduisant certains auteurs à réaliser des ostéotomies de recul des métatarsiens selon Weil en association avec le traitement du névrome. INTRODUCTION:Non-insertional Achilles tendinopathy is a common condition, and endoscopic treatment is becoming an increasingly important therapeutic option. The aim of our study was to compare functional and anatomical results between open and endoscopic surgical treatment of non-insertional Achilles tendinopathy. HYPOTHESIS:Endoscopic surgery provides better functional results and faster recovery than open surgery. METHOD:A multicenter non-randomized prospective study included all adult patients operated on for non-insertional Achilles tendinopathy resistant to medical treatment. Clinical evaluation was based on EFAS and VISA-A scores and return to sport. RESULTS:Sixty patients were included, 22 of whom underwent endoscopic surgery and 38 open surgery. Skin healing was later following endoscopic surgery. No nerve damage was reported. There were no differences in functional scores between the two approaches. Walking was resumed before 6 weeks. Return to sport was progressive as of month 3 and achieved in 80% of cases by month 6, without significant difference according to approach. DISCUSSION:Surgical management of non-insertional tendinopathy refractory to medical treatment presented low morbidity, allowed early resumption of walking by 6 weeks and return to sport in 80% of cases by month 6, but only a third of cases at the previous level. Results were comparable between endoscopic and open approaches. LEVEL OF EVIDENCE:III.
INTRODUCTION:Neurologic complications after limb schwannoma resection are not unusual, but there is no consensus on risk factors for neurologic deficit or poor functional results. We therefore conducted a retrospective study, to screen for factors predicting, firstly, postoperative neurologic deficit and, secondly, poor functional results. HYPOTHESIS:Certain pre- and intraoperative features predict risk of failure, poor results or aggravation. PATIENTS AND METHODS:A single-center retrospective study was conducted in the University Hospital of Lille, France, for the period January 2004 to March 2020, including 71 patients. Preoperative variables (gender, age, symptoms, progression, tumor location and size) and operative data (type of surgery) were collected as possible risk factors for postoperative sensory deficit (Weber) and/or motor deficit [Medical Research Council (MRC)] and poor functional result [Quick Disabilities of the Arm, Shoulder and Hand (QuickDASH); Lower Extremity Functional Scale (LEFS) and douleur neuropathique (neuropathic pain) 4 (DN4)]. RESULTS:Results were assessed a mean 69.4±38.5 months' follow-up (range, 6-180 months). In total, 21 patients (29.6%) had deficits (21 sensory, 1 motor) preoperatively and 25 patients (35.2%) postoperatively (20 sensory, 9 motor) (p=0.689). Fourteen patients (19.7%) showed functional aggravation. Fascicular resection was associated with risk of postoperative deficit [OR = 4.65 (95% CI: 1.485-15.543); p=0.004] and functional deterioration [OR = 3.9 (95% CI: 1.143-13.311); p=0.042]. Thirteen patients (18.3%) showed no improvement on DN4. Preoperative pain was a factor for improvement on DN4 [OR = 3.667 (95% CI: 1.055-12.738); p=0.0409]. DISCUSSION:The study identified fascicular resection as a risk factor for postoperative deficit and functional deterioration after limb schwannoma resection. Patients with preoperative neuropathic pain showed alleviation. Resection should be precise, under magnification, avoiding fascicular resection. Preoperative patient information is essential. IntroductionLa mesure de la force de flexion plantaire de cheville après rupture du tendon d’Achille est rarement rapportée chez le sportif de loisir, devant l’absence de méthode de mesure accessible et rapide. Elle permettrait d’optimiser leur récupération musculaire et sportive. L’objectif principal de cette étude était de mesurer la force isométrique du triceps sural après réparation d’une rupture du tendon d’Achille et de la comparer au côté controlatéral à l’aide d’un dispositif de mesure portatif. Les objectifs secondaires étaient de comparer la trophicité du tendon d’Achille, du mollet, la flexion dorsale et le single heel-rise test par rapport au côté sain ainsi que d’analyser des scores fonctionnels au dernier recul.Hypothèses(1) Il n’y avait pas de différence de force de flexion plantaire entre le côté opéré et le côté sain ; (2) Il n’y avait pas de différence de trophicité du mollet, de la flexion dorsale et au single heel-rise test.Patients et méthodesCette étude rétrospective monocentrique a inclus des patients entre 18 et 65ans ayant eu une réparation chirurgicale à ciel ouvert d’une rupture unilatérale aiguë du tendon d’Achille au recul minimum d’un an. La force de flexion plantaire, la trophicité du mollet, la flexion dorsale de cheville et la distance talon-sol ont été comparées. Les scores fonctionnels EFAS, ATRS et VISA ont été évalués. Toutes les complications ont été recherchées.RésultatsTrente patients ont été revus au recul moyen de 20,3 mois (12–28). Une diminution significative de la force du triceps sural opéré, de la circonférence du mollet et de la distance talon-sol étaient retrouvées, comparées au côté controlatéral (p<0,0001). Le tendon était plus large du côté opéré (p<0,0001) mais sans différence de flexion dorsale (p=0,106). Les scores fonctionnels ATRS, VISA-A et EFAS étaient en moyenne de 91,5/100 (±6), 85/100 (±12) et 19,5/24 (±4) respectivement.DiscussionÀ un an de recul minimum, la mesure de la force du triceps sural par dispositif portatif retrouvait une diminution significative persistante après réparation du tendon d’Achille comparée au côté controlatéral. Une diminution de la circonférence du mollet et de la distance talon-sol a également été retrouvée, soulignant l’intérêt de mieux suivre la récupération musculaire du triceps sural pour optimiser la rééducation et la reprise sportive.Niveau de preuveIV, étude de cohorte rétrospective comparative.
IntroductionLes complications neurologiques après exérèse des schwannomes des membres ne sont pas rares, pourtant il n’existe pas de consensus concernant les facteurs de risque de déficit neurologique ou de mauvais résultat fonctionnel postopératoire. La présence d’une douleur préopératoire était un facteur d’amélioration du score DN4 en postopératoire (OR=3,667 [95 %IC : 1,055–12,738] (p=0,0409)).DiscussionCe travail identifiait qu’une résection fasciculaire lors de l’exérèse chirurgicale des schwannomes des membres augmentait le risque de déficit postopératoire et d’aggravation des scores fonctionnels. Les patients ayant une douleur neuropathique préopératoire présentaient une amélioration postopératoire de leurs symptômes. Une exérèse chirurgicale précise, sous moyen grossissant et en évitant au maximum la résection fasciculaire devrait être réalisée. Objective: The results of surgical versus conservative treatment of acute Achilles tendon ruptures are still controversial. The objective of this study was to compare surgical and conservative treatment at a minimum follow-up of 1 year in terms of the complications, functional outcomes and clinical results.Hypotheses: There is no difference in the complications, clinical results and functional outcomes between the two treatment groups. There is no difference in the occurrence of complications or the clinical results due to the immobilization or rehabilitation protocols.Methods: This was a retrospective comparative, multicenter, non-randomized study of acute Achilles tendon ruptures treated between 01/01/2018 and 31/12/2019 at 21 study sites in France. All patients who received surgical or conservative treatment were included. The demographics, sports participation, nature of treatment, immobilization parameters (type, duration, position) and rehabilitation protocol were collected. Rerupture, general and specific complications, clinical results (heel-rise test, single leg hop, calf circumference, ankle dorsiflexion) and the functional outcomes (ATRS, VISA-A, EFAS, SF-12) were collected at the final review.Results: Four hundred five patients were reviewed at a mean follow-up of 24 (± 7) months. Surgical treatment was done in 372 patients (92%) and conservative treatment in 33 patients (8%), with these two sets of patients having comparable preoperative characteristics. There was a similar number of reruptures in the conservative group (3 cases, 9%) as in the surgical group (15 cases, 4%) (p=0.176). There were more general complications in the conservative group (24%) than in the surgical group (11%) (p=0.04). There was a 9% rate of surgery-related complications (infection, nerve damage, anesthesia after-effects). The ATRS (p=0.017), EFAS Total (p=0.013), EFAS daily living (p=0.008), and SF-12 physical (p=0.01) were better in the surgical group. Strict then relative immobilization provided the best balance between functional recovery (EFAS total of 33, p<0.01) and tendon lengthening (0°, p=0.01) without increasing the occurrence of rerupture (2%, p=0.18). Early weightbearing accompanied by immobilization and rehabilitation within 30 days did not lead to more reruptures than if it was started beyond 30 days (p=0.082 and p=0.07).Conclusions: This study found no differences in the number of reruptures between surgical treatment and conservative treatment of acute Achilles tendon ruptures. Surgical treatment led to better clinical results but had a variable effect on improving the functional scores. No matter which treatment is used, in the ideal case, 3 weeks of strict immobilization in equinus should be followed by progressive reduction over the next 3 weeks. Early weightbearing and mobilization within 30 days did not increase the risk of rerupture; it actually optimized the clinical and functional outcomes.Level of Evidence: III; retrospective comparative, non-randomized.
Background: The main aim of this study was to analyse the 6-month survival rates in peri-acetabular metastasis patients undergoing total hip arthroplasty (THA) with an acetabular cage and without curettage. The secondary objectives were to analyse the global survival rates, the factors influencing patient survival and to evaluate mechanical complication rates after THA. Methods: This study was carried out on a cohort of 93 consecutive patients who underwent THA with an acetabular cage without curettage for acetabular metastasis or multiple myeloma lesions between 2010 and 2020. The National Death Registry was consulted to obtain the exact date of death of the patients; the minimum follow-up time was 2 years. Results: The 6-month survival rate for all types of cancer was 78 % [68 - 85], the 1-year survival rate was 66 % [55 - 74], and the 5-year survival rate was 26 % [17 - 36]. The median overall survival for the cohort was 24.37 months [16.10 - 32.63]. The mean overall survival was 46.02 months [32.89 - 59.16]. At last contact, 86 % of the operated patients were walking again. No patient died from surgery. The ECOG performance status score, the number of bone metastatic sites, the presence of visceral metastases and the number of lines of systemic therapy undertaken prior to surgery were negative survival factors. Three patients (3.2 %) had early prosthetic dislocation, 2 patients (2.2 %) showed aseptic loosening of her partial hip implant after 10 and 11 years respectively and 4 patients (4.3 %) had an early infection treated by debridement, antibiotics and implant retention to control the infection. During the follow-up period, no new femoral metastases were detected in any patient. Conclusion: Surgery without curettage is an effective treatment for periacetabular metastasis. It gives reliable results, regardless of the type of acetabular lesion, allowing most patients to walk again and does not modify the patient's survival.
ObjectifLes résultats du traitement chirurgical ou orthopédique des ruptures aiguës du tendon calcanéen (TC) restent débattus. Notre étude avait pour objectif de comparer les techniques chirurgicales et orthopédiques à un an de recul minimum concernant les complications, les résultats cliniques et fonctionnels.HypothèsesIl n’y avait pas de différence en termes de complications, de différences cliniques et fonctionnelles entre les 2 groupes. Il n’y avait pas de différence entre les modalités d’immobilisation et de rééducation et le taux de complications et les résultats cliniques et fonctionnels.MéthodesIl s’agissait d’une étude rétrospective comparative, multicentrique, non randomisée colligeant les ruptures aiguës du TC entre le 01/01/2018 au 31/12/2019 dans 21 centres investigateurs. Tous les patients ayant eu un traitement chirurgical ou orthopédique étaient inclus. Les données démographiques et sportives, le traitement (chirurgical ou orthopédique), les modalités d’immobilisation (type, position, durée) et de rééducation étaient collectés. Les ruptures itératives, les complications générales et spécifiques, les résultats cliniques (distance talon-sol, saut monopodal, circonférence du mollet, flexion dorsale de cheville) et les scores fonctionnels (ATRS, VISA-A, EFAS, SF-12) étaient collectés au dernier recul.RésultatsQuatre cent cinq patients ont été évalués au recul moyen de 24 (±7) mois. Le traitement était chirurgical pour 372 patients (91,9 %), orthopédique pour 33 patients (8,1 %), avec deux populations comparables. Il n’y avait pas plus de rupture itérative dans le groupe orthopédique (3 cas, 9 %) que dans le groupe chirurgical (15 cas, 4 %) (p=0,176). Le groupe orthopédique présentait plus de complications générales (24 %) que la chirurgie (11 %) (p=0,04). Le groupe chirurgical présentait un taux de complications spécifiques de 9 % (infectieuses, nerveuses, anesthésiques). Les scores ATRS (p=0,017), EFAS Total (p=0,013) et Vie Quotidienne (p=0,008), et SF-12 physique (p=0,01) étaient meilleurs pour le groupe chirurgical. Une immobilisation stricte puis relative permettait le meilleur équilibre entre la récupération fonctionnelle (EFAS total à 33, p<0,01) et l’allongement tendineux (0°, p=0,01) sans augmenter la survenue d’une rupture itérative (2 %, p=0,18). Une reprise d’appui précoce sous couvert d’immobilisation et rééducation avant 30jours n’entraînait pas plus de rupture itérative que si elles débutaient après 30jours (p=0,082 et p=0,07).ConclusionsCette étude ne retrouvait pas de différences du nombre de ruptures itératives entre traitement chirurgical versus orthopédique d’une rupture aiguë du tendon calcanéen. La chirurgie assurait de meilleurs scores cliniques mais une supériorité variable des scores fonctionnels. Quel que soit le traitement proposé, l’immobilisation devrait être idéalement stricte en équin 3 semaines, progressivement réduit les 3 semaines suivantes. Une remise en charge et une mobilisation précoce avant 30jours n’augmentaient pas le risque de rupture itérative et optimisait les résultats cliniques et fonctionnels.Niveau de preuveIII ; étude rétrospective comparative non randomisée.
The shoulder is the most common site for upper extremity tumors. The aim of the study was to analyze the outcomes and the complications of modular reverse shoulder arthroplasty (RSA) after proximal humerus resection. We retrospectively included 15 consecutive patients who underwent a modular MUTARS™ RSA reconstruction after proximal humerus tumour resection between 2017 and 2020. The mean age was 52 years. Their clinical outcomes were assessed using the Constant-Murley score and the MSTS shoulder. Radiological outcomes were assessed based on the presence of loosening, osteolysis, and scapular notching. Complications such as dislocation, oncological recurrence, and infection were assessed. Mean follow-up time was 32.9 months (24 to 45). The mean adjusted Constant score was 50.7
INTRODUCTION:Development and use of specific anatomic prosthesis for shoulder's fracture aimed to reach the best rate of consolidation of the greater tubercle, which means a cuff restitution to improve functional results. The lack of bone healing of the tubercles suggested the use of a fenestrated humeral implant with interposition of a bone graft in the metaphyseal part of the prosthesis. This characteristic of specific implant, have few reports in the literature, leading us to the current retrospective study aiming: 1) to evaluate the healing rate of the greater tubercle after implantation of fracture dedicated anatomic implant, which includes a fenestration in the prosthetic metaphysis for the addition of a cortico cancellous graft, 2) to asses on patients with proximal humerus fractures, the bone healing of the greater tubercle close to the intraprosthetic bone graft. HYPOTHESIS:The specific "implant-fracture" can achieve a high rate of bone healing of the humeral greater tubercle. PATIENTS AND METHODS:Between January 2001 and December 2020, fifty-one patients were operated on by implantation of fracture dedicated implant for proximal humerus fracture. Six were excluded (2 operated for revision, 2 operated after 3 weeks, 1 died, 1 without follow-up). In total 45 patients were included in radiological analysis, clinical analysis had been performed on 23/45 patients at the longest follow up, the other 22/45 were only analyzed on radiographies. Mean-aged 66 years (range, 47 years -88 years), 25/45 (56%) of women, with 3-4-parts fractures according to Neer's classification. The techniques of tubercle fixation were isolated cerclages of combined horizontal cerclages and vertical sutures. Position and healing of the greater tubercle was controlled through antero-posterior and lateral X-Rays views at the longest follow-up (mean 50 months, range 3-193 months). Peroperative techniques of fixation, clinical and functional outcomes were noted and correlated to the radiologic position of the greater tubercle and the graft evolution. Factors associated with healing and anatomic position had been investigated. RESULTS:The rate of greater tubercle healing was 32/45 (73%). Factors significantly associated with greater tuberosity consolidation were higher age (p = 0.04) and the addition of a vertical osteosuture to the horizontal suture of the greater tubercle (p = 0.01). The rate of anatomic position of the greater tubercle was 15/45 (33%) of cases. When the fixation of the tuberosity was made with vertical suture, good position of the tuberosity was observed in 68% (17/24) at the longest follow-up. DISCUSSION:Our results were in accordance with the literature, but the current study underlined there were two types of factors influencing tubercle healing in the literature: the technique of fixation of the tubercle and the patient's age. L’intervention de Latarjet donne des résultats fonctionnels satisfaisants et un faible taux de récidive. Certains auteurs lui attribuent un caractère arthrogène lié au positionnement de la butée et au matériel. L’objectif principal était d’évaluer la prévalence de l’omarthrose à long terme, de déterminer les facteurs de risque d’omarthrose et le retentissement clinique de celle-ci. Notre hypothèse était que le risque d’arthrose pouvait être limité par le bon positionnement de la butée. L’objectif secondaire était d’évaluer le taux de récidive d’instabilité à long terme. Sur une cohorte de 102 patients (106 épaules), opérés entre 1984 et 1998, 77 avaient été revus avec un dossier radiologique complet en 2003 pour une précédente étude. Quarante-quatre de ces patients ont à nouveau été revus cliniquement et radiologiquement au recul moyen de 22 ans. L’évolutivité clinique et radiologique était comparée aux résultats observés au recul de 8 ans. Les patients étaient évalués cliniquement en relevant les mobilités, les scores de Constant et Walch-Duplay. Des radiographies étaient systématiquement réalisées pour déterminer le stade d’arthrose selon Samilson et un scanner pour 31 patients. Sur les 44 patients revus, le taux d’arthrose était de 34,1 %. Les patients étaient satisfaits dans 90,9 % des cas. Le score de Constant moyen était de 83,5 (±14) et le score de Duplay moyen de 71,6 (±23,4) points. L’âge élevé lors de l’intervention et à la révision, l’importance du recul et un âge supérieur à 30 ans lors du premier épisode d’instabilité étaient les principaux facteurs liés à la survenue d’une omarthrose et indépendants du geste chirurgical. Le débord latéral de la butée était lié à l’apparition d’une omarthrose (71,4 % vs 16,7 % en cas de butée médiale ou affleurante, p = 0,0004). La présence d’une encoche humérale ou d’une perte de substance glénoïdale et le nombre d’épisodes d’instabilité préopératoires étaient sans influence sur l’évolution arthrosique. Le débord latéral de la butée était le seul facteur de risque lié à la technique chirurgicale. Le retentissement clinique de cette omarthrose n’était significatif que pour les stades avancés (Samilson 3 et 4). Le débord latéral de la butée semblant être le seul paramètre lié à la technique chirurgicale pourvoyeur d’omarthrose à long terme, ce travail plaide pour un strict respect du positionnement coracoïdien auquel pourraient participer les ancillaires spécifiques ou les techniques arthroscopiques. Background: While modular reconstruction implants can be used to replace the bone lost after bone tumor resection, tumor excision from the neighboring soft tissues can lead to loss of strength and joint range of motion (ROM), which results in worse knee function. Functional recovery after total knee arthroplasty for osteoarthritis has been extensively documented. But few studies have evaluated the recovery after total knee reconstruction following tumor excision despite the fact that most of these patients are young and have high functional demands. We did a prospective cross-sectional study to: 1) compare muscle strength recovery around the knee with an isokinetic dynamometer after tumor excision and reconstruction with a modular implant to the healthy contralateral knee; 2) determine if the differences in peak torque (PT) in the knee extensors and flexors had a clinical impact.Hypothesis: Resection of soft tissues during tumor excision around the knee causes strength loss that cannot be fully recovered. Methods: The 36 patients who underwent extra-or intra-articular resection of a primary or secondary bone tumor in the knee area followed by reconstruction with a rotating hinge knee system between 2009 and 2021 were eligible for this study. The primary outcome was the ability to actively lock the operated knee. The secondary outcomes were the concentric PT during isokinetic testing at slow (90 degrees /sec) and fast (180 degrees /sec) speeds, flexion-extension ROM, Musculoskeletal Tumor Society (MSTS) score, the IKS, Oxford Knee Score (OKS) and KOOS.Results: Nine patients agreed to participate in the study, all of whom had regained the ability to lock their knee postoperatively. PT in flexion and extension on the operated knee was less than the healthy knee. The PT ratio for the operated/healthy knee at 60 degrees /sec and 180 degrees /sec in flexion was 56.3% +/- 16.2 [23.2-80.1] and 57.8% +/- 12.3 [37.7-77.4], respectively, which corresponded to a slow-speed strength deficit of 43.7% in the knee flexors. The PT ratio for the operated/healthy knee at 60 degrees /sec and 180 degrees /sec in extension was 34.3% +/- 24.6 [8.6-76.5] and 43% +/- 27.2 [13.1-93.4], respectively, which corresponded to a slow-speed strength deficit of 65.7% in the knee extensors. The mean MSTS was 70% +/- 20 [63-86]. The OKS was 29.9/48 +/- 11 [15-45], the mean IKS knee was 149.6 +/- 36 [80-178] and the mean KOOS was 67.43 +/- 18.5 [35-88.7].Discussion: Despite all patients having the ability to lock out their knee, there was an imbalance in the strength between opposite muscle groups: 43.7% strength deficit at slow-speed and 42.2% at fast speed for the hamstring muscles, and 65.7% at slow-speed and 57% at fast speed for the quadriceps muscles. This difference is considered pathological with an increased risk of knee injury. Despite this strength deficit, this joint replacement technique, which is free of complications, can preserve good knee function with acceptable knee joint ROM and satisfactory quality of life. BACKGROUND:Plantar flexion force in recreational athletes after repair of Achilles tendon tears has rarely been reported, due to the lack of a widely available and fast measurement method. Knowledge of this parameter would allow optimisation of muscular and sports recovery. The main objective of this study was to measure the isometric force of the triceps surae at least 1 year after unilateral Achilles-tendon repair, comparatively to the unaffected side, using a portable device. The secondary objectives were to compare Achilles tendon and calf-muscle trophicity, dorsiflexion, and the single-leg heel-rise test versus the normal side and to assess functional scores at last follow-up. HYPOTHESES:(i) Plantar flexion force does not differ significantly between the operated and contralateral sides. (ii) Calf-muscle trophicity, dorsiflexion, and the single-leg heel-rise test do not differ significantly between the operated and contralateral sides. PATIENTS AND METHODS:This single-centre retrospective study included patients aged 18 to 65 years with a history of open repair of an acute unilateral Achilles tear at least 1 year earlier. Plantar flexion force, calf-muscle trophicity, dorsiflexion, and the heel-to-floor distance were measured on both sides and compared. The ATRS, VISA-A score, and EFAS score were determined. All complications were recorded. RESULTS:The study included 30 patients with a mean follow-up of 20.3 months (range, 12-28 months). The operated side had significantly lower values for isometric triceps surae force, calf circumference, and heel-to-floor distance compared to the contralateral side (p<0.0001). Tendon width was significantly greater on the operated side (p<0.0001). Dorsiflexion did not differ significantly between sides (p=0.106). Mean functional score values were 91.5±6 for the ATRS (maximum, 100), 85±12 for the VISA-A score (maximum, 100), and 19.5±4 for the EFAS score (maximum, 24). DISCUSSION:Isometric triceps-surae force measured using a portable device at least 1 year after open surgical Achilles-tendon repair was significantly decreased compared to the contralateral side. Calf circumference and heel-to-floor distance were also significantly lower. These data indicate a need for improved monitoring of triceps surae recovery to optimise rehabilitation and sports resumption. LEVEL OF EVIDENCE:IV, retrospective comparative cohort study.
Si les pertes de substances osseuses engendrées par la résection tumorale osseuse sont substituées par les prothèses de reconstruction modulaires, l’exérèse carcinologique des parties molles avoisinantes peut entraîner une perte de force et/ou d’amplitude articulaire et une réduction de la fonction du genou. La récupération fonctionnelle après arthroplastie totale de genou dans le cadre d’une gonarthrose a été largement évaluée, l’évaluation après prothèse de reconstruction après exérèse tumorale est plus rare malgré des sujets jeunes avec une demande fonctionnelle plus importante. Aussi nous avons réalisé une étude transversale prospective pour évaluer : (1) la récupération musculaire autour du genou par dynamomètre isocinétique après résection tumorale et reconstruction par une prothèse de reconstruction modulaire par rapport au genou sain controlatéral, (2) si une différence des moments de forces maximales (MFM) des fléchisseurs et extenseurs de genou avait un retentissement clinique. La résection des parties molles au cours de la résection tumorale autour du genou expose à une perte de force non récupérable à distance de l’intervention. Nous avons inclus 36 patients ayant bénéficié, entre 2009 à 2021 d’une résection extra ou intra-articulaire d’une tumeur osseuse primitive ou secondaire autour du genou avec reconstruction par une prothèse à charnière rotatoire. Le critère d’évaluation principal était le verrouillage actif du genou opéré. Les critères d’évaluation secondaire étaient un bilan isocinétique avec recueil des MFM à vitesse lente (90°/s) et vitesse rapide (180°/s) en mode concentrique et l’évaluation des amplitudes articulaires en flexion-extension, le score Musculoskeletal Tumor Society (MSTS), les scores fonctionnels IKS, Oxford-12 genou et Koos. Neuf patients ont accepté de participer à l’étude. Le verrouillage actif en postopératoire a été obtenu chez tous les patients. Le MFM en extension et en flexion du genou opéré était inférieur au genou sain. Le ratio du MFM genou opéré/sain à 60°/s et 180°/s en flexion était respectivement de 56,3 % ± 16,2 [23,2–80,1] et 57,8 % ± 12,3 [37,7–77,4] traduisant sur les fléchisseurs une perte de force de 43,7 %. Le ratio du MFM genou opéré/sain à 60°/s et 180°/s en extension était respectivement de 34,25 % ± 24,6 [8,6–76,5] et 43 % ± 27,2 [13,1–93,4] traduisant sur les extenseurs une perte de 65,7 % de force. Le score MSTS moyen, était de 70 % ± 20 [63–86]. Le score Oxford-12 Genou était de 29,9/48 ± 11 [15–45] et les scores IKS genou moyen et KOOS moyen étaient respectivement de 149,6 ± 36 [80–178] et 67,43 ± 18,5 [35–88,7]. Malgré l’obtention d’un verrouillage actif du genou, acquis dans tous les cas, nous avons observé une asymétrie de forces entre les groupes musculaires homologues, une perte de force de 43,7 % à vitesse lente et 42,2 % à vitesse rapide des ischiojambiers et une perte de force de 65,7 % à vitesse lente et 57 % à vitesse rapide du quadriceps. Cette différence est considérée comme pathologique avec un risque de lésion traumatique augmenté. Malgré ce déficit, cette technique, en l’absence de complications, peut préserver une bonne fonction du genou avec des amplitudes articulaires correctes et un niveau de vie jugé comme satisfaisant par les scores fonctionnels. INTRODUCTION:The Latarjet procedure provides satisfactory functional results, with low rates of recurrence. It is, however, sometimes claimed to induce osteoarthritis due to the positioning of the bone-block and hardware. The aims of the present study were to assess the long-term prevalence of osteoarthritis following the procedure, to determine risk factors and assess clinical impact. The study hypothesis was that osteoarthritis risk can be reduced by good bone-block positioning. The secondary objective was to assess long-term recurrence of instability. MATERIALS AND METHODS:Out of a cohort of 102 patients (106 shoulders) operated on between 1984 and 1998, 77 had complete radiology files for analysis in a previous study published in 2003. Forty-four of these patients were seen again for clinical and radiological assessment at a mean 22 years' follow-up, to be compared to results previously reported at 8 years. Clinical assessment comprised range of motion and Constant and Walch-Duplay scores. X-rays were taken systematically to determine Samilson osteoarthritis stage; 31 patients also underwent CT. RESULTS:In the 44 patients, the rate of osteoarthritis was 34.1%. 90.9% of patients were satisfied. Mean Constant score was 83.5±14 and mean Walch-Duplay score was 71.6±23.4. Advanced age at surgery and at assessment, length of follow-up and age >30 years at first episode of instability were the main factors for onset of osteoarthritis, independently of surgery. Lateral bone-block overhang correlated with onset of osteoarthritis: 71.4%, versus 16.7% in medial or flush bone-block (p=0.0004). Humeral notch, glenoid bone defect and number of preoperative episodes of instability were unrelated to onset of osteoarthritis. Lateral bone-block overhang was the only risk factor related to surgery. The clinical impact of the osteoarthritis was significant only in advanced stages (Samilson 3 and 4). CONCLUSION:Lateral bone-block overhang was the only surgical risk factor for osteoarthritis in the long-term. Strict coracoid positioning is therefore essential, and could be facilitated by specific instrumentation and by arthroscopy. LEVEL OF EVIDENCE:IV; retrospective study.
BACKGROUND:Hallux rigidus is the second most frequent pathology of the first ray. Surgical options for degenerative metatarsophalangeal joint disease are either joint destructive or conservative procedures. The hypothesis was that oblique distal shortening osteotomy of the first metatarsal is an effective conservative technique for the management of stage 1 to 3 hallux rigidus.METHODS:We conducted a retrospective cohort study of 87 feet with Coughlin and Shurnas's stage 1-3 hallux rigidus, operated between 2009 and 2019. The cohort consisted in 72 patients (87 feet) with an average age of 57±9 (30/79) years; 22 of 87 (25.3%) feet had the first metatarsal surgery performed in isolation; 65 of 87 (74.7%) had concomitant forefoot procedures, including 31 of 87 (35.6%) with Akin phalangeal osteotomies and 34 of 87 (39.1%) with Moberg phalangeal osteotomies.We evaluated the American Orthopaedic Foot & Ankle Society (AOFAS) Scale, subjective satisfaction, joint amplitudes, shortening rate, and occurrence of postoperative complications with a mean follow-up of 51 months (16/134).RESULTS:The AOFAS score increased from 54.2±11.3 (25/70) preoperatively to 92.2±7.8 (62/100) postoperatively (P < .001). Patients reported excellent or good outcome in 95.4% of cases. The 40-point self-reported pain subscale score improved from 19.6 (± 10.0) to 37.4 (± 5.4), P < .001.The overall range of motion increased from 61±21 (20/110) degrees to 69±17 (35/120) degrees (P < .001). The mean first metatarsal shortening rate (SRpo) was 9.6%. Neither the Coughlin grade, the metatarsal index, or the SRpo influenced the AOFAS score. At 6-month follow-up, 15 patients had transfer metatarsalgia compared with 5 at last follow-up without requiring another surgical procedure. The risk was not significantly different according to Coughlin's stage, preoperative metatarsal index, or SRpo.CONCLUSION:Oblique distal osteotomy of the first metatarsal for stage 1-3 hallux rigidus, often in combination with other first ray procedures, performed well during our follow-up time period, with a high subjective satisfaction rate and few complications.LEVEL OF EVIDENCE:Level III, retrospective cohort study.
